そこで、水中油型エマルション製剤を凍結乾燥して凍結乾燥製剤とし、使用時に分散溶媒を加えて水中油型エマルション製剤を再調製させる方法が検討され、いくつかの凍結乾燥製剤に関する報告がなされた(特許文献1及び2を参照)。
BCG−CWSを含む水中油型エマルション製剤を医薬品として供給するためには、(1)医薬品として求められる長期保存安定性を有すること、(2)使用前に簡便、迅速に水中油型エマルションを調製できること、そのためには凍結乾燥製剤が候補になるが、(3)凍結乾燥製剤については、注射用蒸留水等の分散溶媒を添加して得られるエマルションが、凍結乾燥前の水中油型エマルションと実質的に同等であること等が重要である。しかしながら、これまでに知られていた凍結乾燥製剤は、上記の3つの条件を満たすために、用時調製の手作業で少量作られていた水中油型エマルション製剤と対比して、BCG−CWSに対する油成分の比率を3〜7倍と高くせざるを得ないことなどの問題点があった。即ち、油成分の比率が高くなると刺激性が高くなり、副反応としての皮膚障害(潰瘍形成)が増大することが想定された。

なお、特許文献2や再表2005−102369などの凍結乾燥製剤では、BCG−CWSに対して重量比で油成分(スクワラン)が約13倍や約38倍になっている。一方、これまで用時調整の少量スケールで作製されていたBCG−CWSエマルション製剤では、BCG−CWSに対して油成分(ドレコール)の重量比が4〜8倍となっている。このように、特許文献2等に記載のエマルション凍結乾燥製剤では、BCG−CWSに対する油成分の重量比が増大して、3〜4倍になっている。これは凍結乾燥中に生じる水中油型エマルションの変性を抑制し、エマルションの構造を安定に維持するためにはBCG−CWSに対する油成分の重量比を増大させ、油成分の量を増加させることが必要になったと考えられる。

そこで、本発明者らは、油成分の比率を低減して副反応の皮膚障害を抑制すると共に、復水後のエマルションの変性が少ない、これまでにない水中油型エマルション製剤の製造方法の検討に着手した。
本発明者らは、鋭意検討の結果、製造方法を図1に示すように大きく変更した。即ち、オートクレーブ滅菌したBCG−CWSに対して、無菌濾過した油成分(Montanide ISA51、スクワラン、ドレコール6VRなど)を含む有機溶媒を加えて懸濁し、その懸濁有機溶媒をマンニトールの多孔質状基剤(無菌無塵のマンニトール水溶液を凍結乾燥して得られた多孔質状の基剤)に含浸させ、真空乾燥して溶媒留去することを行った。このことにより、BCG−CWSの油状ペーストがマンニトールの多孔質状基剤に吸着された無菌無塵の乾燥製剤を作製することができた。
上記の製造方法により得られたBCG−CWSの乾燥製剤は、極めて保存安定性が良好であった。そして、上記BCG−CWSの乾燥製剤に、界面活性剤を含有する水溶液を添加して混合することにより、再現性良くBCG−CWSの水中油型エマルションを無菌無塵で作製できることを見出した。
更に、上記エマルションの製造方法は、薬剤がBCG−CWS以外の薬剤にも応用でき、医薬品の水中油型エマルション製剤に有効に応用できることを見出した。即ち、本発明者らは、以上の知見により本発明を完成した。In order to provide an emulsion preparation of BCG-CWS as a pharmaceutical, long-term storage stability is required. (A porous base obtained by freeze-drying a sterile and dust-free mannitol aqueous solution), followed by vacuum drying and distilling off the solvent. As a result, it was possible to produce a sterile, dust-free dry preparation in which the BCG-CWS oily paste was adsorbed on the mannitol porous base material.
The dry preparation of BCG-CWS obtained by the above production method had extremely good storage stability. Then, they have found that an oil-in-water emulsion of BCG-CWS can be aseptically and dust-free with good reproducibility by adding and mixing an aqueous solution containing a surfactant to the dry preparation of BCG-CWS.
Furthermore, it has been found that the above-mentioned method for producing an emulsion can be applied to a drug other than BCG-CWS, and can be effectively applied to an oil-in-water emulsion drug product. That is, the present inventors have completed the present invention based on the above findings.

b)薬物と油成分の有機溶媒溶液を作製し、
c)上記有機溶媒溶液を上記多孔質状基剤に含浸させる、
d)含浸後、有機溶媒を減圧乾燥で留去して、
e)有機溶媒留去後に得られた薬物と油成分が含有された多孔質状基剤を得る、
f)界面活性剤を含有する水溶液を上記薬物含有の多孔質状基剤に添加し、
g)混合撹拌して、水中油型エマルション製剤を作製する
ことを特徴とする、水中油型エマルション製剤の製造方法。
(2)糖及び/又はアミノ酸が、マンニトールであることを特徴とする、上記(1)に記載の水中油型エマルション製剤の製造方法。

マンニトールのみからなる5%(W/V)水溶液を作製し、ミリポアフィルター(pore size0.22μm)で無菌無塵の濾過処理を行う。その濾過液の0.67mLを採取し、2mLガラスバイアルに分注する。分注後のバイアルを、ミリポアフィルター(pore size0.22μm)で外気と連結したステンレス製の滅菌缶に入れ、−20℃以下でマンニトール水溶液を凍結させ、その後、凍結乾燥処理を行った。
その結果、白色のマンニトール多孔質状(スポンジケーキ状)の基剤を得ることができた。
(2)BCG−CWSを含有した無菌無塵の有機溶媒溶液の製造
BCG−CWSを秤量してオートクレーブ滅菌(123℃、25分)をし、乾燥させる(真空乾燥器、60℃、60分間)。
別途、n−ヘプタンとエタノールの混合溶媒(9:1、v/v)を作製し、その混合溶媒にモンタナイド(Montanide)ISA51を添加し、40mg/mLになるように調整した。その混合溶媒を疎水性ミリポアフィルター(pore size0.2μm、マイレクス−FG、日本ミリポア株式会社)で濾過し、無菌無塵処理を行った。
濾過後の混合有機溶媒を、滅菌乾燥させたBCG−CWSに添加し、BCG−CWSの濃度が2.5mg/mLの懸濁溶液になるように作製した。その懸濁溶液を38℃で2分間超音波処理をしてBCG−CWSを充分懸濁させた。
(3)BCG−CWSを含有するマンニトール多孔質状基剤の製造
上記(1)で作製した2mLバイアル中のマンニトール多孔質状基剤(D−マンニトール50mg/バイアル)の上に、上記BCG−CWS懸濁溶液の0.4mL(BCG−CWS:1.0mg、モンタナイドISA51:16mg)を添加し、含浸させた。
添加後のバイアルを、ミリポアフィルター(pore size0.22μm)で外気と連結したステンレス製の滅菌缶に入れ、真空乾燥機中で40℃、30分間静置した後、40℃で徐々に減圧を行い、有機溶媒を留去した。更に、減圧度−30mHgで60℃、1時間、有機溶媒を留去した。
