Aに対する抗体は、抗M2抗体よりも治療上有効であるのが典型的である。それにもかかわらず、中和剤ではなく、依然としてHAを標的化する抗体は、IP投与した場合に中和抗体として匹敵するレベルの有効性を表し得、この送達経路は、IN投与した場合に利用することができる強力な効果を利用することができないことが示唆される。さらに、中和Fabを、IPではなくINにより送達すると、治療有効性がもたらされる。非中和FabはIN投与しても、治療有効性を表さない。まとめると、IN投与した中和抗体だけが、この有効性の増大を表す。この観察を拡張すると、この現象は、他のタンパク質(例えば、ノイラミニダーゼ)を標的化する中和抗体に、および他の呼吸器病原体に対する中和抗体(例えば、RSVに対するパリビズマブ)に生じる。INおよびIP/IVの両方の経路による抗体の送達は、それ自体様々な方法で有効であり得るため、本発明者らは、両方の経路を組み合わせて用いれば、中和抗体の最大の治療潜在性を利用するであろうと考えている。この取組みは、IN経路による中和活性の増大、およびIP/IV経路によるFc依存的活性の増大を利用することによって、最大の有効性を可能にするであろう。

中和抗体は鼻腔内実験試験で有効である
[000213] 中和抗体および非中和抗体の両方をIP経路により投与すると、致死的な感染を治療および防止する上で同様の効果を表すことから、全身送達される抗体の治療有効性は、中和能力にもっぱら依存するわけではない。中和抗体および非中和抗体10mg/kgを、感染24時間後(24hpi)にIP投与した場合、同様に有効であった(図1)。いくつかの非中和抗体(6P15、IP19、および1K17)は、中和抗体CA8020と同様の程度に致死的な誘発からマウスを保護することから、図1に示す結果は、全身送達されるHAに対する抗体は、エフェクター機能によって頑強な治療有効性を発揮することができることを実証する。中和抗体および非中和抗体は、ウイルスでの感染1時間前(−1hpi)にIP投与すると、同様に予防的に有効である(図2)。同様の結果が1hpiに、および異なるウイルスを用いて見られた(データは示さず)。これらの結果により、中和は、全身送達の間の治療有効性に著しく貢献するか否かという疑問がもたらされる。抗体のIVまたはIP経路による送達は著しい有効性の差をもたらさなかった(図3、データは示さず)。
中和Fabは鼻腔内で有効である
[000217] 本発明者らは、次に、Fcを除去することで、IPまたはIN投与した中和Fabおよび非中和Fabの治療有効性が抑止されるか否かを調べた。図5に見られる通り、10mg/kg以下のFab(CA6261抗体Fab)をIP投与しても、H1ウイルスに対する治療有効性をもたらさない。FabでIP治療したマウスは全て、PBS治療したマウス同様、感染で死亡した。対照的に、投与量10mg/kgおよび1mg/kgの中和FabでIN治療したマウスは、致死的な感染を生き延びることができた(図5)。IN投与した投与量は全て(0.1mg/kgまででも)、投与したいかなるIP投与よりも優れた有効性を示した。同じ実験において、CA6261 Fab INをIPまたはIVに対して比べて匹敵する結果が観察され、Fab CA6261をIPまたはIVいずれかで投与した場合は保護的でも、または効果的でもなかったが、同じ投与量(5mg/kg)をIN投与した場合は、著しい有効性を示した(動物は95%以上の体重を保持した)(データは示さず)。これらのデータは、Fabは、中和抗体に対して、鼻腔内でウイルス感染を阻止または治療するのに効果的であることを実証している。Fabが無効であったため、データはさらに、Mabの全身送達は、治療有効性にFcエフェクター機能を必要とすることを指摘している。

[000219] 本発明者らは、注目すべきことに、中和抗体の鼻腔内(IN)送達は、腹腔内(IP)送達よりも10〜100倍の間強力であることを発見した。マウスにPR8ウイルス(H1ウイルス)10×LD50を感染させ、24hpiに抗体で治療した(図7)。中和抗体CA6261を10倍段階希釈し、INまたはIPいずれかの経路によって投与した(図7A)。IN経路により治療したマウスは、体重減少によって指摘される通り、疾患の重症度をあまり表さず、全ての希釈で致死的な感染から100%保護された。比較上、最高投与量(10mg/kg)でIP治療したマウスだけが、一過性の体重減少および致死的な感染からの保護を表した。低希釈は全て、IP投与した場合、マウスを保護しなかった。対照的に、0.1mg/kgの投与量のIN治療は、全てのマウスに一過性の体重減少および生存をもたらした。投与量10mg/kgおよび1mg/kgのIN送達によって治療したマウスは、感染後全ての時間に、検出可能な体重減少から保護された。IP経路によって投与した抗体は、全投与量である程度の体重減少を表し、最高投与量の10mg/kgで治療したマウスだけが感染を生き延びた。
鼻腔内の有効性は感染後も維持される
[000222] IN送達した中和抗体の有効性の増強は、感染後、後々維持される。図8では、マウスを48hpiにINまたはIPのいずれかにより治療する。繰り返すが、中和抗体のIN送達は、IP送達よりも治療上有効である。IN経路によって投与した場合、致死的な誘発からの完全な保護が1mg/kgで実現されるが、IP送達は、投与量10mg/kgで致死的な誘発に対して完全な保護をもたらす。IN投薬は、72hpiに比較的低減した有効性を示したが、72hpiの同量のIP投薬に対して依然として大幅に改善されていた。72hpiにIN投与した場合のCA6261の有効性、および同投与量のIPに比べて非常に著しく増強された生存を実証する生存チャートを図18に提供する。
低投与量のIN投与は有効である
[000223] 中和抗体の鼻腔内送達は、感染後に投与した場合、非常に低い投与量で、致死的な誘発に対する完全な保護をもたらすことができる。図10に実証する通り、0.005mg/kgほどに低いIN投与量のCA8020抗体を8hpiに投与すると、10×LD50のウイルスに対して100%の生存をもたらす。これらの投与量は、感染後のIVまたはIP治療に対する標準の投与量よりも1000倍低い。これらの結果は、驚くほど低い投与量は、IN投与した場合に治療有効性を実現することができることを指摘する。
IN/IPの組合せプロトコールにより有効性の増強がもたらされる
[000226] モデルに基づき、図12に示す通り、本発明者らは、INおよびIP送達した抗体は別々の、非重複性の機能を表すという仮説を調査した。正しければ、INおよびIPの組合せ経路により投与した抗体は、単一の投与経路単独に比べて有効性の増大を表す。図13A〜13Dにおいて、本発明者らは、INおよびIPの組合せ送達は、様々な組合せ投薬レジメン下で(これらの試験における抗体の合計投与量は5mg/kgまたは2mg/kgであった)IP送達単独よりも有効であることを示す。IPまたはINの群に比べて、INおよびIPの組合せ投与を用いてもたらされる有効性の改善は、INおよびIPによるMab投与の作用機序および必要要件が明確であり、非重複であり得ることを確証するものである。INおよびIPの組合せ投与レジメンを用いて、INのための中和抗体、ならびに組合せIP投与様式のための中和抗体または非中和抗体を用いて、抗ウイルス有効性の増強がもたらされる。さらなる試験により、中和抗体(例示的なCA8020抗体)のIN投与を、同じ中和抗体または別個の非中和抗体(例示的な6P15)のIP投与と組み合わせると、IN 0.1mg/kgおよびIP 0.5mg/kgの低投与量を合計0.6mg/kgのみの抗体投与に用いても、同様に効果的であることが実証される(図19)。IN投与を実行することで、抗体IPの比較的低投与量と組み合わせても、有効性に必要とされる全体の抗体を低減することができる。IN投与した抗体の作用機序は、ウイルスの拡散を主に制限することができ、IP投与した抗体は、エフェクター機能によってウイルスを生成する感染細胞の数を主に低下させ得る。
インフルエンザBウイルスに対する抗体の有効性
[000228] 上記に示し、表1に記載し、先に報告されている通り(Dreyfus Cら(2012) Science 337(6100): 1343-1348)、抗体CR9114は、インフルエンザA型およびB型のウイルスに対しても結合し、有効である。キメラ抗体CA9114(マウスIgG2aに融合しているヒト可変領域を有すると先に記載されている)を、インフルエンザB/マレーシア株に対する有効性に対して、マウスモデルで試験した。B/マレーシア(B/Malと示す)10×LD50に対する有効性に対して、24hpiに、10mg/kg、1mg/kg、および0.1mg/kgのINおよびIP投薬を試験した。結果を図20に示す。試験した全投与量で、IN投与はIPよりも有効であった。インフルエンザBウイルス感染1日後に、CA9114抗体を鼻腔内送達すると動物を治療するのに有効であり、インフルエンザウイルスによる体重減少を排除した。
組合せ抗体試験
[000231] 現在のインフルエンザワクチンは、ヒト集団における循環性インフルエンザウイルス株に対する免疫を誘発するための抗原を含んでいる。4価のインフルエンザワクチンは、インフルエンザAウイルス、特に亜型H1ウイルスおよびH3ウイルス、およびインフルエンザBウイルスの山形系列およびビクトリア系列も網羅する。インフルエンザA亜型およびインフルエンザBにも対する鼻腔内投与した抗体に実証される上記の有効性、ならびに複数の抗体を組み合わせると効果的であることを考慮して、抗体を組み合わせて試験を行った。これらの組合せには、現在使用中の確立されている3価および4価のワクチンを擬し、これらに匹敵し得る組合せが含まれていた。したがって、インフルエンザを治療し、感染を防ぎ、または伝染を防ごうと、組合せの鼻腔内抗体組成物をin vivoで動物モデルにおいて評定し、インフルエンザAおよびインフルエンザBウイルスに対して評価した。
インフルエンザH1およびH3ウイルスに対する代替的な抗体の有効性
[000234] 気道に対する投与の有効性を、鼻腔内投与を用いて、さらなる代替のインフルエンザ抗体で評価した。インフルエンザAウイルスのグループ1およびグループ2の両方を中和し、これらに対する有効性を有するヒトモノクローナル抗体を、ヒト対象から直接単離した。ヒト抗体Mab53(TRL53とも示す)は、US2012/0020971およびWO2011/160083に記載されており、グループ1および2のH1、H9、H7、およびH5亜型を中和するのに有効である。抗体Mab579(TRL579とも示す)は、WO2013/086052に記載されており、H3およびH7を中和するのに有効である。Mab579および53抗体を、インフルエンザAウイルス感染に対する治療有効性に対してマウスモデルにおいて試験した。Mab579をH3インフルエンザに対して試験し、Mab53をH1インフルエンザに対して試験した。INおよびIPの投薬を比較し、IN投薬は1mg/kgであり、IP投薬は10倍高い10mg/kgであった。抗体Mab579を、H3インフルエンザウイルスVic11 10×LD50に対する治療の有効性に対して、感染24時間後(24hpi)に投与した(図30)。抗体Mab53を、H1インフルエンザウイルスCal09 10×LD50に対する治療の有効性に対して、感染24時間後(24hpi)に投与した(図31)。同実験においてIP投与は10倍高い投与量であっても、IN投与はIP投与よりも有効であった。
鼻腔内投与した抗体での予防試験
[000235] 感染後の中和抗体の鼻腔内投与の注目すべき有効性を考慮して、予防的な、およびウイルス感染前の、鼻腔内投与の有効性を評価する試験を行った。これらの試験は、個体がインフルエンザウイルスに暴露された場合の鼻腔内投与の適用性の評価および実証となり、また、感染もしくは疾患が全体的に大きな健康上の危険性となる患者では、暴露され、もしくは危険性のある集団内で、または臨床的に、伝染を防止または低減する有効な取組みとなる。マウス動物モデルにおけるインフルエンザウイルス感染の数日前に、グループ1(H1)の抗体CA6261を投与に対して評価した。感染誘発の3、4、5、6、および7日前にCA6261の投与を評価し、異なる投与量のINおよびIP投薬を直接比較した。
Claims (39)
- 異なる特異性を有する2つ以上のインフルエンザウイルス中和IgGモノクローナル抗体を、1mg/kg以下の単回単位投与量で含む、鼻腔内(IN)または吸入投与用に製剤化されており、かつ哺乳動物におけるインフルエンザウイルス感染の治療または予防に有効な組成物であって、
ここで、少なくとも1つの第一の抗体がインフルエンザAウイルスに対する抗体であり、かつ、
少なくとも1つの抗体が前記第一の抗体が認識するインフルエンザAウイルスとは異なる系統学的なグループのインフルエンザAウイルス若しくはインフルエンザBウイルスに対する抗体であるか、又は、少なくとも2つの前記抗体が前記第一の抗体が認識するインフルエンザAウイルスとは異なるグループのインフルエンザAウイルスに対する抗体及びインフルエンザBウイルスに対する抗体を含む、組成物。 - 前記一または複数のインフルエンザウイルス中和モノクローナル抗体を、各々0.5mg/kg未満の単回単位投与量で含む、請求項1に記載の組成物。
- 前記一または複数のインフルエンザウイルス中和モノクローナル抗体を、各々0.1mg/kg未満の単回単位投与量で含む、請求項1に記載の組成物。
- 前記一または複数のインフルエンザウイルス中和モノクローナル抗体を、各々0.05mg/kg未満の単回単位投与量で含む、請求項1に記載の組成物。
- 前記一または複数のインフルエンザウイルス中和モノクローナル抗体は、中和の能力があり、かつFab、Fab’、およびF(ab’)2から選択される抗体フラグメントである、請求項1〜請求項4のいずれか1項に記載の組成物。
- 薬学的に許容される賦形剤、担体もしくは希釈剤、経鼻もしくは肺送達用の添加剤、または免疫学的メディエータもしくは免疫応答刺激剤をさらに含む、請求項1〜請求項5のいずれか1項に記載の組成物。
- インフルエンザA H1ウイルスを中和する能力がある抗体、インフルエンザA H3ウイルスを中和する能力がある抗体、およびインフルエンザBウイルスを中和する能力がある抗体を含む、インフルエンザ中和抗体の組合せを含む、請求項1〜請求項6のいずれか1項に記載の組成物。
- インフルエンザウイルスに暴露された、もしくは暴露の危険性がある、または呼吸器ウイルス感染の臨床症状が出現している、哺乳動物におけるインフルエンザウイルスの治療または予防に用いられる、請求項1〜請求項7のいずれか1項に記載の組成物。
- 前記抗体は、感染後24時間までの期間に投与されるためのものである、請求項1〜請求項8のいずれか1項に記載の組成物。
- 前記抗体は、感染後48時間までの期間に投与されるためのものである、請求項1〜請求項8のいずれか1項に記載の組成物。
- 前記抗体は、感染後72時間までの期間に投与されるためのものである、請求項1〜請求項8のいずれか1項に記載の組成物。
- 前記抗体は、ウイルスへの感染もしくは暴露の前に、またはウイルス疾患もしくはウイルス疾患に付随する症状の臨床的な出現の前に、一または複数の投与で投与されるためのものである、請求項1〜請求項8のいずれか1項に記載の組成物。
- 前記抗体は、各々1mg/kg体重未満の単回投与量で投与される、請求項1〜請求項12のいずれか1項に記載の組成物。
- 前記抗体は、1投与量あたり1mg/kg未満の複数回投与で鼻腔内または吸入投与されるものであり、前記複数回投与は、少なくとも2時間の間をおいて行われるものであり、1回目の投与は、推定される感染、暴露、または臨床症状の出現の72時間後までに行われるものである、請求項1〜請求項13のいずれか1項に記載の組成物。
- 腹腔内(IP)または静脈内(IV)に投与されるための追加のインフルエンザウイルス特異的なモノクローナル抗体と同じ抗体をさらに含み、ここで、前記追加の抗体は、中和抗体または非中和抗体である、請求項1〜請求項14のいずれか1項に記載の組成物。
- 前記追加の抗体は、鼻腔内(IN)または吸入投与される抗体と同一である、請求項15に記載の組成物。
- 前記追加の抗体が、鼻腔内(IN)または吸入投与される抗体の前に、同時に、逐次的に、または後に投与される、請求項15又は請求項16に記載の組成物。
- 異なる特異性を有する2つ以上のインフルエンザウイルスに対する中和IgGモノクローナル抗体を含有する第一の組成物、ここで、前記第一の組成物は1mg/kg以下の投与量で投与され鼻腔内(IN)または吸入投与用である;及び
腹腔内(IP)または静脈内(IV)投与用の第二の組成物、ここで、前記第二の組成物は、1mg/kg以上の投与量で投与される1つ以上の腹腔内(IP)または静脈内(IV)投与用の抗インフルエンザ抗体を含有する、
を含む予防用又は治療用の組成物の組み合わせであって、
ここで、前記第二の組成物の抗体が前記第一の組成物の抗体と同一または異なる抗体である、予防用又は治療用の組成物の組み合わせ組成物。 - 前記第二の組成物の抗体は、非中和抗体である、請求項18に記載の組み合わせ組成物。
- 前記第二の組成物の投与量が、少なくとも5mg/kgである、請求項18又は請求項19に記載の組み合わせ組成物。
- 前記第二の組成物の投与量が、前記第一の組成物の投与量よりmg/kgにおいて少なくとも10倍高い、請求項18又は請求項19に記載の組み合わせ組成物。
- 前記第一の組成物の単回単位投与量は、0.5mg/kg未満である、請求項18〜請求項21のいずれか1項に記載の組み合わせ組成物。
- 前記第一の組成物が、推定される感染、暴露、または臨床症状出現の後24時間以内に投与されるためのものである、請求項18〜請求項22のいずれか1項に記載の組み合わせ組成物。
- 前記第一の組成物が、推定される感染、暴露、または臨床症状出現の後48時間以内に投与されるためのものである、請求項18〜請求項22のいずれか1項に記載の組み合わせ組成物。
- 前記ウイルス中和抗体は、推定される感染、暴露、または臨床症状出現の後48時間以内に投与されるためのものである、請求項18に記載の組み合わせ組成物。
- 前記ウイルス中和抗体は、推定される感染、暴露、または臨床症状出現の後24時間以内に投与されるためのものである、請求項18に記載の組み合わせ組成物。
- 前記ウイルス中和抗体は、推定される感染、暴露、または臨床症状出現の後24時間より後で72時間以内に投与されるためのものである、請求項18に記載の組み合わせ組成物。
- 前記ウイルス中和抗体は、暴露、または臨床症状出現の前に、一または複数の投与で投与されるためのものである、請求項18に記載の組み合わせ組成物。
- 前記第一の組成物は、0.1mg/kg未満の投与量で投与されるためのものである、請求項18〜請求項28のいずれか1項に記載の組み合わせ組成物。
- 鼻腔内(IN)または吸入投与用に製剤化されており、かつ哺乳動物におけるインフルエンザウイルス感染の治療または予防に有効な組成物であって、
インフルエンザA H1ウイルス及びインフルエンザA H3ウイルスのいずれか一方又は両方を中和する能力を有する抗体を少なくとも1つ含み、かつ、山形及び/又はビクトリア系列を含むインフルエンザBウイルスを中和する能力を有する抗体を1つ以上を含む二つ以上の抗体を単回単位投与量1mg/kgで含み、かつ、
薬学的に許容される賦形剤、担体もしくは希釈剤を含み、
ここで、前記二つ以上の抗体は以下の組み合わせのいずれか1つを含む:
インフルエンザA H1ウイルスを中和する能力を有する抗体、インフルエンザA H3ウイルスを中和する能力を有する抗体、及び山形及び/又はビクトリア系列のインフルエンザBウイルスを中和する能力を有する1つ以上の抗体;
インフルエンザA H1ウイルスを中和する能力を有する抗体、及び山形及び/又はビクトリア系列のインフルエンザBウイルスを中和する能力を有する1つ以上の抗体;
インフルエンザA H3ウイルスを中和する能力を有する抗体、及び山形及び/又はビクトリア系列のインフルエンザBウイルスを中和する能力を有する1つ以上の抗体;
インフルエンザA H1ウイルスを中和する能力を有する抗体、山形系列のインフルエンザBウイルスを中和する能力を有する抗体、及びビクトリア系列のインフルエンザBウイルスを中和する能力を有する抗体;並びに
インフルエンザA H3ウイルスを中和する能力を有する抗体、山形系列のインフルエンザBウイルスを中和する能力を有する抗体、及びビクトリア系列のインフルエンザBウイルスを中和する能力を有する抗体。 - 前記二つ以上の抗体がIgG抗体である、請求項30に記載の組成物。
- 推定される感染、暴露、または臨床症状出現の後48時間以内に投与されるためのものである、請求項30に記載の組成物。
- 推定される感染、暴露、または臨床症状出現の後24時間以内に投与されるためのものである、請求項30に記載の組成物。
- 推定される感染、暴露、または臨床症状出現の後24時間より後で72時間以内に投与されるためのものである、請求項30に記載の組成物。
- 暴露、または臨床症状出現の前に、一または複数の投与で投与されるためのものである、請求項30に記載の組成物。
- 該組成物が単回単位投与量0.5mg/kg未満で投与されるためのものである、請求項30に記載の組成物。
- 該組成物が単回単位投与量0.1mg/kg未満で投与されるためのものである、請求項30に記載の組成物。
- 前記単回単位投与量が25〜200μLの容量である、請求項30に記載の組成物。
- 前記インフルエンザAウイルスに対する第一の抗体と、前記第一の抗体が認識するインフルエンザAウイルスとは異なるグループのインフルエンザAウイルスに対する第二の抗体とが、少なくとも二つのHAサブタイプのインフルエンザAウイルスを中和可能である、請求項1に記載の組成物。
