Description
目的は、適切なバイパス箇所を正確に示し得る医用画像処理装置を提供することにある。
(1)左冠動脈主幹病変(50%以上の狭窄例)
(2)PCIの施行が困難(高度な三枝病変、慢性完全閉塞など)
(3)冠動脈末梢枝の血液の流れが良好(血管内径>1.5mm)であるもの(狭窄・不整なし)
(4)左心肺機能が次の状態であるもの(駆出率(EF)20%以上、左室拡張終期圧(LVEDP)20mmHG以下)
画像記憶部101は、CT装置20またはPACS50から制御部104の制御に基づいて送信されたボリュームデータを記憶する。処理画像としてのボリュームデータは、例えば、被検体の心臓を含む胸部領域に関する複数時相にわたる時系列の3次元造影CT画像データである。
通信インタフェース103はネットワーク40に接続されて、CT装置20あるいはPACS50と医用画像処理装置10との通信を可能にする。
心臓領域抽出部105は、心輪郭抽出処理などによりボリュームデータから心臓領域を抽出する。
さらに、心筋解析部106は、算出された血流量の空間分布から閾値処理により虚血領域を抽出する。
なお図8において、バイパス箇所よりも下流の部分は血管内径が基準を満たさない程度に細い(例えば1.5mm以下)のでバイパス不適箇所と判断され、カラーマッピング表示の対象から除外される。
図12は、リスク分布を含むバイパス箇所を示す3次元画像の一例を示す図である。実施形態に係る医用画像処理装置10によれば、CTボリュームデータを解析することによりFFR、血管内径、血管壁の状態、あるいは血管と狭窄箇所の位置関係などの種々のリスク要素を求めることができる。
また、本実施形態に係る医用画像処理装置10によれば、バイパス箇所を、他の指標と組み合わせて表示することも可能である。
Claims (7)
- 複数時相の画像に描出された血管の狭窄部位に対応するFFRを計算する計算部と、
前記画像に描出された虚血領域を抽出する抽出部と、
前記抽出された虚血領域と前記血管の支配領域とを照合して、前記虚血領域の責任血管を特定する第1特定部と、
前記FFRに基づいて前記責任血管内の責任狭窄を特定する第2特定部と、
前記責任狭窄を迂回するバイパス血管を接続するためのバイパス箇所を特定する第3特定部と、
前記特定されたバイパス箇所を表示部に表示させる表示制御部と、
を具備することを特徴とする医用画像処理装置。 - 前記第3特定部は、前記責任血管におけるFFRの降下度を用いて前記バイパス箇所を特定することを特徴とする請求項1に記載の医用画像処理装置。
- 前記第3特定部は、前記責任血管の内径を用いて前記バイパス箇所を特定することを特徴とする請求項2に記載の医用画像処理装置。
- 前記第3特定部は、前記責任血管の弾性値を用いて前記バイパス箇所を特定することを特徴とする請求項3に記載の医用画像処理装置。
- 前記表示制御部は、前記特定されたバイパス箇所に対応するFFRを示す第1グラフ及び前記特定されたバイパス箇所に対応する血管内径を示す第2グラフの少なくとも一方を前記表示部に表示させることを特徴とする請求項1に記載の医用画像処理装置。
- 前記表示制御部は、前記第1グラフ及び前記第2グラフの少なくとも一方を前記特定されたバイパス箇所が描出された画像と共に前記表示部に表示させることを特徴とする請求項5に記載の医用画像処理装置。
- 前記表示制御部は、前記特定されたバイパス箇所におけるリスク分布を前記表示部に表示させることを特徴とする請求項1に記載の医用画像処理装置。
