Description
図1は、本明細書に記載の医療用ドレッシング及び自己支持基材の一実施例を示す。医療用ドレッシング10は、自己支持基材12を包含する。裏材層14は、医療用ドレッシング10の外辺部15まで延びている。裏材層14は、典型的には、非常に薄く柔軟で、且つ透明又は半透明のどちらかであって、そこを通して創傷及び/又は周囲の皮膚を見ることができる。
本発明の創傷ドレッシングは、液体及び少なくともいくらかの気体の通路に十分に不透過性のバリアを提供する、任意の柔軟な裏材との関連において有用である。典型的な裏材としては、不織布及び織布繊維性ウェブ、ニット、フィルム、発泡体高分子フィルム、及び他の一般的な裏材材料が挙げられる。好ましい裏材材料としては、薄いエラストマー裏材を含む。これらの種類の裏材は柔軟性及び創傷部位周辺での高い接着性を確実にすることを助ける。好ましい裏材材料は、ポリウレタン(例えば、ESTANE)、ポリエーテルポリエステル(例えば、HHTREL)、ポリエーテルアミド(例えば、PEGAX)、並びに、ポリオレフィン(例えば、ENGAGE)を含む、半透明又は透明の高分子フィルムであり得る。
ほとんどの実施形態において、自己支持基材は熱可塑性ポリマーである。好ましいポリマーはポリエチレンである。
本発明の創傷ドレッシング物品で用いるのに好適な接着剤としては、皮膚に許容可能な接着を提供し、かつ、皮膚上で使用するのに許容可能な(例えば、接着剤は、好ましくは刺激が無く、増感作用を有さないべきである)任意の接着剤が挙げられる好ましい接着剤は、感圧接着剤であり、いくつかの実施形態では、水分の蒸発を可能にするために、比較的高い湿気透過速度を有する。好適な感圧接着剤としては、アクリル酸、ポリウレタン、KRATON、及び他のブロックコポリマーに基づくもの、シリコーン、ゴム系接着剤(天然ゴム、ポリイソプレン、ポリイソブチレン、ブチルゴムなど)、加えてこれらの接着剤の組み合わせが挙げられる。接着剤成分は、粘着付与剤、可塑剤、レオロジー変性剤、加えて、例えば抗菌剤を含む活性成分を含み得る。
本明細書に記載の医療用ドレッシングと共に、吸収性材料が使用されてもよい。吸収性材料は、創傷充填材(後述)と同一であってもよく、又は別個の要素であってもよい。吸収性材料は、織布若しくは不織布の木綿又はレーヨンを包含するがそれらに限定されない、様々な材料のいずれかで製造され得る。吸収性パッドは、任意に、抗菌剤、経皮的薬物送達のための薬物、患者の体内のホルモン又は他の物質をモニターする化学指示薬(chemical indicator)などを包含する、多数の物質を包含するのに有用である。
いくつかの例では、本発明の医療用ドレッシングで使用する裏材は、剥離ライナーが裏材から除去されるときに、裏材が患者の皮膚へのドレッシングの円滑で無菌の適用を阻害して、それ自体を褶曲し、かつそれに接着する傾向があり得るようなものであり、非常に可撓性及び柔軟であり得る。
ドレッシングを患者に取り付けて封止された空洞を作り出すために使用される感圧接着剤を保護するために、剥離ライナーを本発明の医療用ドレッシングに供給することもできる。本発明の医療用ドレッシングでの使用に適切であり得る剥離ライナーは、スーパーカレンダー仕上げのクラフト紙、グラシン紙、ポリエチレン、ポリプロピレン、ポリエステル、又は任意のこれらの材料の複合物からなり得る。
本発明の医療用ドレッシングとの関連において、弁は、好ましくは相対的に薄型であるか又は低い高さを有する。好ましくは、薄型の弁は、医療用ドレッシングが外力(例えば、寝具、衣類などからの外力)によって阻害される可能性を減じる。薄型の弁は、例えば、患者が座っている間、横になっている間、及び/又は立っている間に、患者の体重がかかるような位置にドレッシングを配置した場合の、患者の快適さを改善することもできる。
本発明の医療用ドレッシングに含ませることができる他の所望の要素は、医療用ドレッシングの弁近傍に配置させることができるバリア要素である。バリア要素は、好ましくは、創傷滲出物から、弁を別段に汚染させ得る物質を濾過するために機能させることができ、このような弁は、医療用ドレッシングによって画定された封止環境から流体を取り除いた後に、再閉止又は封止しない。バリア要素は医療用ドレッシング(例えば、自己支持基材の空洞内)に取り付けて提供することができ、又はバリア要素は、患者に医療用ドレッシングを送達する間に配置させることができるように取り付けられていない形態で、医療用ドレッシングと共に提供することができる。バリア要素によって濾過され得る、幾種類かの物質の例としては、例えば凝固した血液、遊離した組織、組織充填物などを挙げることができる。
更に他の実施形態では、封止環境への医療用ドレッシングを通しての再封止可能なアクセスを提供するために、隔膜要素(例えば、隔膜)を医療用ドレッシングの裏材に取り付けることができる。医療用ドレッシングは1つ以上の隔膜要素を含むことができる。いくつかの実施形態では、隔膜要素は、単に裏材の片面だけに又は裏材の両面に見出すことができる。更に他の実施形態では、1対以上の隔膜要素は、互いに反対向きに、直接、裏材の内側表面と外側表面の両面に配置されてもよい。
本発明の医療用ドレッシングは、可能性としては1つ以上の所望の要素を備えるキットの形態で供給することもできる。キットは好ましくは、封止されたパッケージ(例えば、バッグ、パウチ、トレイなど)で提供することができる。キットは、本明細書に記載のような1つ以上の医療用ドレッシングを含む。
医療用ドレッシングは、任意に、例えば、薬剤、抗感染剤、抗菌剤、防腐剤(例えば、ポリヘキサメチレンビグアニド(以降「PHMB」)、クロルヘキシジン、銀、ヨード、ヨードフォア、塩化ベンザルコニウム、過酸化水素、並びに同時係属出願である米国特許第2005/0089539号、同第2006/0051385号、同第2006/0052452号、及び同第2006/0051384号(これらは参照により本明細書に組み込まれる)に開示される防腐剤、抗生物質、鎮痛剤、局所麻酔、抗炎症剤、回復因子、ビタミン、成長因子、例えば、マトリックスメタロプロテイナーゼ(MMP)阻害剤などの酵素阻害剤、及び栄養素を含む多くの活性剤の少なくとも1つ、並びに/又はマイクロビーズ充填剤、及び/又は吸収性発泡体の1つを含み得る。このような活性剤は、裏材、接着剤、及び多孔質フィルターを含む創傷ドレッシングのいずれかの部分での溶出により、又は減圧環境による創傷空間への薬剤の制御された導入を可能にする別個の貯蔵チャンバから、組み込まれてもよい。あるいは、薬剤は米国特許第6,867,342号に教示されるように、又は直接ドレッシングを通して薬剤を注入することによって組み込まれてもよい。
本明細書で述べるように、本発明の医療用ドレッシングは、医療用ドレッシングによって画定された封止環境から、流体を中に通して取り除くことができる弁などの開口部又はポートを、医療用ドレッシングに設けることによって、負圧創傷治療のために使用されてもよい。流体は、好ましくは医療用ドレッシングに定期的に取り付けることができるポンプによって、封止環境から取り除かれる。ポンプが、医療用ドレッシングの裏材の外側表面を封止して液密な封止を提供することができるシートを含むことが好ましい場合がある。
いくつかの実施形態では、バリア要素の代わりに、又はバリア要素に加えて、医療用ドレッシングに創傷充填材を設けることもできる。いくつかの実施形態では、創傷充填材はまた、本明細書に記載のようなバリア要素としても機能し得る(この機能は必須ではない)。いくつかの実施形態では、創傷充填材は、本明細書に記載されるような封止環境内の負圧の維持を補助するために、所望により、創傷充填材をバラスト要素としても機能させることもできるように、弾性的に圧縮できるものであってもよい。例えば、真空が作用すると弾性の包装は圧縮される。真空が形成され、弁が閉じて創傷の空洞を封止する場合、包装は依然として、その非圧縮状態に戻るように膨張力を提供する。この膨張は、一定期間にわたって真空をつくり、維持することを助けるように機能する。
好ましい製造方法において、キャリア材料は、好ましい医療用ドレッシング上のキャリアの中心に位置する窓を形成するために、打ち抜きされる。好ましい実施形態では、打ち抜きは、当業者に周知である回転式打ち抜き装置を使用して達成される。窓がキャリア材料に打ち抜かれた後、それらは所望により除去される。窓の除去は、好ましいドレッシングのそれぞれの外辺部を囲んで位置するキャリア材料の内縁部を画定する。

- (a)裏材層であって、第1の主表面及び第2の主表面と、前記裏材層を貫通する開口部とを有する、柔軟性のある裏材層と、
(b)前記裏材層の前記第1の主表面の少なくとも一部にコーティングされた感圧性接着剤と、
(c)前記裏材層の前記第2の主表面の少なくとも一部に解放可能に取り付けられたキャリアと、
(d)弁を有する第1の平面と、前記第1の平面に対して0度〜180度の間で角度をなして形成される少なくとも1つの側壁と、フランジの形態で前記少なくとも側壁に接続される第2の平面と、を含む自己支持基材と、を含む接着性医療用物品であって、
前記自己支持基材が、100%〜500%の%伸長と、10,000〜400,000の弾性率とを備える半剛体であるとともに、前記裏材層に取り付けられて、前記裏材層を貫通して窓を形成する前記開口部を取り囲み、
(e)前記接着性医療用物品によって画定される封止環境内の負圧の維持を補助するために弾性的に圧縮可能な材料で形成され、前記自己支持基材によって形成される空洞内に配置されて前記弁の目詰りを制限するバリア要素をさらに含む、接着性医療用物品。
