Description
本願は、その出願の内容を引用をもってここに援用することとする、2009年12月14日出願の米国仮出願第61/286,127号、及び2010年4月1日出願の米国仮出願第61/320,044号に基づく優先権を主張するものである。
カテーテル先端の位置が低すぎると、心臓の穿孔、生命を脅かす不整脈及び血栓の形成などの合併症がある。カテーテルの先端が高すぎることに関する合併症もまた重篤であり、その中にはカテーテルの機能不全及び血管損傷がある。
血管内カテーテルは、放射線不透過性であってよいマーカーをその上に有していてもよい。該マーカーは、カテーテルの最も近位の開口部の位置にあっても、又は同開口部に隣接してもよく、そして画像上に見られるであろう。この形のマーキングにより、使用者は、カテーテルの「機能的な長さ」を、カテーテルの残り部分から区別することができ、適正な配置を確実にするのに役立てることができる。
血管内カテーテルには、撮像して目に見えるマーカーを含めることがある。マーカーは、カテーテルの最も近位の開口部に隣接して配置されるであろう。マーカーは、最も近位の開口部に配置することも、それに対して近位に配置することも、又はそれに対して遠位に配置することもできる。マーカーはいずれのカテーテル・デザイン上でも配置することができる。それが二重内腔又は先割れ型先端デザインであれば、複数のマーカーを配置することができ、一つ又は二つを各カテーテル上に配置することもできる。放射線不透過性のマーカーは、例えばタングステン、ヨウ素、鉛、バリウム又はいずれか他の物質など、いずれの放射線不透過性材料からも作製することができる。放射線不透過性マーカーは、カテーテル上の様々な位置に配置することができ、優先的には最も近位の開口部(即ち側壁の穴、側壁のスロット、孔、開窓、又は他の液体透過性連絡路)に対して近位、に配置することができる。マーカーの目的は、患者の身体内におけるカテーテルの位置を医師が精確に判断できるようにすることであろう。具体的には、操作者は、右心房/上大静脈接合部に、カテーテル上のマークを位置合わせすることができる。このカテーテルにより精確な配置が可能になり、従って、血管内カテーテルが低すぎる又は高すぎることで起きる合併症を減らすことができよう。
ワイヤ上に配置されたカテーテルを示す。精確なカテーテル先端位置は見えにくい。マーカーは点の形になっており、近位の穴及び先端に配置されている。マーカーにより、カテーテル位置が特定しやすい。
1. 放射線不透過性マーカーは、好ましくは、近位の開口部、そして、最も近位の開口部から最高1cmまでにあるか、あるいは、カテーテルの側壁の穴に位置していてもよい。
以下は、急性及び慢性カテーテルを配置する手法例である。
1. 静脈(1)に針(2)を到達させる(図23)。好ましくは超音波による案内があるとよい。好適な静脈は内頸静脈であるが、他の静脈も用いることができる。

Claims (10)
- 挿入のための遠位端と、近位端と、内部を通って延びる少なくとも一つのルーメンとを含む血管内カテーテルであって、
挿入のための前記遠位端が先端を末端に持ち;
前記カテーテルのルーメンと流体連絡した少なくとも二つの開口部であって、前記少なくとも二つの開口部が、前記カテーテル内の少なくとも一つの遠位の開口部と、前記カテーテル内の一つ以上の近位の側方開口部とを含み、前記一つ以上の近位の側方開口部が最も近位の側方開口部を含む、開口部を規定し、前記最も近位の側方開口部は、前記カテーテル内の遠位の開口部に近位に配置されると共に、前記カテーテルの全遠位端に最も近位の開口部であり;そして:
前記最も近位の側方開口部に近接して個別に配置されるか、又は前記最も近位の側方開口部にあると共に、遠位端の残りから前記最も近位の側方開口部に近接して配置されるか、又は前記最も近位の側方開口部にある放射線不透過性マーカーの位置を放射線画像上で区別する放射線不透過性マーカーであって、前記最も近位の側方開口部に近接して配置されるか、又は前記最も近位の側方開口部にある放射線不透過性マーカーに関連する開口部以外の開口部が、前記遠位端の残りから放射線で区別可能である、放射線不透過性マーカーと
を含み、
前記先端と前記放射線不透過性マーカーとの間の距離が、約1.5cm乃至約5cmの範囲の機能的カテーテル長を規定する、
血管内カテーテル。 - 前記放射線不透過性マーカーと前記先端との間の位置には他に放射線不透過性マーカーが配置されていない、請求項1に記載の血管内カテーテル。
- 前記遠位端が、前記放射線不透過性マーカー以外に、前記最も近位の側方開口部に近接して配置されるか、又は前記最も近位の側方開口部にある放射線不透過性マーカーを含まない、請求項1に記載の血管内カテーテル。
- 前記機能的カテーテル長が約3センチメートルである、請求項1に記載の血管内カテーテル。
- 前記カテーテルを患者に最適に配置したときに、前記最も近位の側方開口部に近接して配置されるか、又は前記最も近位の側方開口部にある前記放射線不透過性マーカーが患者の右心房と上大静脈との接合部に位置するように、前記放射線不透過性マーカーが前記遠位端に配置される、請求項1に記載の血管内カテーテル。
- 前記放射線不透過性マーカーが右心房と上大静脈との接合部に位置するように前記カテーテルを挿入したときに、前記先端が右心房の壁面にも又は床にも接触しないように、前記最も近位の側方開口部に近接して配置されるか、又は前記最も近位の側方開口部にある前記放射線不透過性マーカーが前記遠位端に配置される、請求項1に記載の血管内カテーテル。
- 挿入のための前記遠位端が複数の内腔に分割されている、請求項1に記載の血管内カテーテル。
- 前記最も近位の側方開口部に近接して配置されるか、又は前記最も近位の側方開口部にある前記放射線不透過性マーカーが、タングステン、ヨウ素、鉛、又はバリウムを含む、請求項1に記載の血管内カテーテル。
- 前記最も近位の側方開口部に近接して配置されるか、又は前記最も近位の側方開口部にある前記放射線不透過性マーカーが、点、線、又はZ型の形状である、請求項1に記載の血管内カテーテル。
- 前記先端が対称、食い違い型、尖頭型、ワイヤに向かって先細り型、又は先割れ型である、請求項1に記載の血管内カテーテル。
