Description
本出願は、いずれも全体として参照により本明細書に組み入れられる、2003年7月9日出願の米国特許仮出願第60/485,959号、および2003年10月14日出願の米国特許仮出願第60/511,244号の利益を主張する。

驚くべきことに、薬学的に許容される亜硝酸塩の投与が心臓血管系の調整に有用であるということが見出された。また、驚くべきことに、亜硝酸塩がインビボで一酸化窒素に還元され、それによって生成される一酸化窒素が有効な血管拡張剤であるということが見出された。これらの効果は、驚くべきことに、臨床的に有意なメトヘモグロビン血症を生じさせない用量で発揮される。これらの発見により、心臓血管系に関連する状態、たとえば高血圧、肺高血圧症、脳血管痙攣および組織虚血-再かん流傷害を予防し、処置する方法がここで可能となる。またこれらの発見により、組織、たとえば低酸素圧領域の組織への血流を増大させる方法が提供される。亜硝酸塩が心臓血管系を調整する効果を示すために、より具体的にはインビボで血管拡張剤として作用するために、酸性化状態で適用されなくてもよいということは、特に驚くべきことである。

亜硝酸アニオンは、血漿中で約150〜1000 nMの濃度、大動脈組織中で約10μMの濃度で存在する。亜硝酸塩をNOに還元するための生理学的機序が存在するとすると、これは一酸化窒素(NO)の最大血管プールに相当する。ヒト前腕における亜硝酸塩の血管拡張特性およびその生体活性化のために現存する機序が調査され、結果が本明細書で報告されている。亜硝酸ナトリウムを毎分約36μmolで健常なボランティア18名の前腕上腕動脈に注入すると、安静時の前腕血流中に約222μMの局所亜硝酸塩濃度および約175%の即座の増大が得られた。血流の増大は、安静時、NO合成酵素阻害中および運動時に認められ、静脈ヘモグロビン酸素飽和度、酸素分圧、およびpHの増大によって実証されるように、組織かん流の増大を生じさせた。亜硝酸塩の全身濃度は約16μMに増大し、平均動脈血圧を有意に低下させた。さらに6名の対象で、亜硝酸塩用量を約2対数減らし、毎分360 nmolで注入すると、約2μMの前腕亜硝酸塩濃度および約22%の血流増が生じた。

この実験でインビボで形成される鉄-ニトロシル-およびS-ニトロソ-ヘモグロビンのレベルは特筆すべきである。注入された亜硝酸塩(局所濃度200μM)は、運動時に前腕を循環する10秒未満の通過時間中、約750nMの鉄-ニトロシル-ヘモグロビンおよび200nMのSNO-Hbを生成した。両NO-ヘモグロビンアダクトの形成は、ヘモグロビン酸素飽和度と逆相関していた。ヘモグロビン酸素飽和度は、コオキシメトリー(co-oxymetry)による肘窩静脈からの計測によると、運動ストレス下で低下していた(鉄-ニトロシル-ヘモグロビンの場合、r=-0.7、P<0.0001、S-ニトロソ-ヘモグロビンの場合、r=-0.45、P=0.04、図4B)。異なる酸素圧(0〜100%)での全血への200μM亜硝酸塩の添加がインビボデータを再現し、低めの酸素圧で増大する濃度の鉄-ニトロシルヘモグロビンが形成され(鉄-ニトロシル-ヘモグロビンの場合、r=-0.968、P<0.0001、S-ニトロソ-ヘモグロビンの場合、r=-0.45、P=0.07)、NOおよびSNO形成が亜硝酸塩とデオキシヘモグロビンとの反応に依存するということを強く示唆した。

過去10年に、一酸化窒素(NO)が血管ホメオスタシスにおいて演じる決定的な役割について理解の深まりが見られた。NOの生成とNOの掃去との間のバランスがNOの生物学的利用能を決定し、このバランスは、正常な生理状態では注意深く維持されている。恒常性血管調整系は、局所強直性血管拡張に必要な十分な局所NOを考慮しながらも過剰なNOを掃去して総エンドクリン作用を制限するように微調整されていることは明白である。しかし、無細胞ヘモグロビンによる急速なNO掃去がこのバランスを乱す(Reiter et al., Nat Med 8, 1383-1389, 2002)。正常な生理学的状態では、ヘモグロビンは、ヘモグロビン掃去系によって速やかかつ効果的に除去される。しかし、鎌状赤血球病のような慢性的な溶血性状態は、結果的に、実質的な量のヘモグロビンを日々血管系中に放出させ、無細胞ヘモグロビンがNOの生物学的利用能に対して重大な全身性の影響を及ぼしうることが示唆される。現在の研究の焦点は、多くの慢性溶血性状態、たとえば肺高血圧症、皮膚潰瘍ならびに急性および慢性の腎不全に共通の血管性合併症を説明し、処置することを試みている。同様に、多数の臨床的疾患および治療、たとえば急性溶血発作、心肺バイパス処置中の溶血、高齢血の輸血、および筋梗塞後のミオグロビン尿症は、多くの場合、急性の肺および全身性高血圧症、急性腎不全、血管内血栓、虚血性中枢神経事象および/または死を併発する。
亜硝酸塩の治療用途は今や、対象に、特に対象における低酸素血性および虚血性組織に対して選択的血管拡張を提供するために使用することができ、溶血中に放出される遊離ヘモグロビンがNOを掃去し、NO依存性血管機能を乱す鎌状赤血球病のような溶血性状態を処置するのに有用であろう。亜硝酸塩は、遊離ヘモグロビンをメトヘモグロビンに酸化させることによってNOを掃去する遊離ヘモグロビンの能力を阻害することだけでなく、低酸素圧の組織床中でNOを生成することも予想される。したがって、適用された亜硝酸塩は、低酸素圧区域で優先的に一酸化窒素を放出し、局所的血管拡張および/または血流増大を提供する。
亜硝酸塩の血管拡張性およびその生体活性化の機序を、本明細書に記載するように調査した。亜硝酸ナトリウムを毎分36μmolで健常なボランティア18名の前腕上腕動脈に注入すると、222μMの局所亜硝酸塩濃度、驚くことに安静中の前腕血流の175%の増大が生じた。血流の増大は、安静時、NO合成酵素阻害中および運動中で認められた。亜硝酸塩注入はまた驚くことに、静脈ヘモグロビン酸素飽和度、酸素分圧、およびpHの増大によって実証されるように、組織かん流の増大を生じさせた。亜硝酸塩の増大した全身濃度(16μM)は、平均動脈血圧を有意に低下させた。
亜硝酸アニオン(NO2 -)は一酸化窒素(NO)酸化の結果として形成し、血漿中で0.3〜1.0μMの濃度、組織中で1〜20μMの濃度で存在する(Gladwin et al., Proc Natl Acad Sci USA 97: 11482-11487, 2000; Rodriguez et al., Proc Natl Acad Sci USA 100: 336-341, 2003; Rassaf et al., Nat Med 9:481-483, 2003; Bryan et al., Proc Natl Acad Sci USA., 2004; Gladwin et al., J Clin Invest 113:19-21, 2004)。亜硝酸塩は、歴史的に、固有の生物学的活性が限られた不活性代謝最終産物であると考えられてきた(Lauer et al., Proc Natl Acad Sci USA 98:12814-12819, 2001; McMahon, N Engl J Med 349:402-405; 著者返答402-405, 2003; Pawloski, N Engl J Med 349:402-405; 著者返答402-405, 2003)。我々および他のグループからの最近のデータにより、低酸素症およびアシドーシス中に亜硝酸塩がNOに還元されうるということが示唆される(Gladwin et al., Proc Natl Acad Sci USA 97:11482-11487, 2000; Bryan et al., Proc Natl Acad Sci USA., 2004; Cosby et al., Nat Med 9:1498-1505, 2003; Nagababu et al., J Biol Chem 278:46349-46356, 2003; Tiravanti et al., J Biol Chem 279:11065-11073, 2004)。きわめて低い組織pHおよびPO2で、亜硝酸塩は、不均化によって(酸性還元、Zweier et al., Nat Med 1:804-809, 1995)、またはキサンチンオキシドレダクターゼの酵素的作用によって(Millar et al., FEBS Lett 427:225-228, 1998; Zhang et al., Biochem Soc Trans 25:524S, 1997; Godber et al., J Biol Chem 275:7757-7763, 2000; Li et al., J Biol Chem 276:24482-24489, 2001)、NOに還元されうる。
新生児の持続性肺高血圧症は、出生児1,000人中0.43〜6.8人の発症率で起こり、死亡率は10〜20%である(Walsh-Sukys et al., Pediatrics 105, 14-20, 2000)。生存児は、神経発生および聴覚機能障害(46%)、認知遅延(30%)、聴力損失(19%)および高い率の再入院(22%)を発するおそれがある(Lipkin et al., J Pediatr 140, 306-10, 2002)。
さらに、亜硝酸塩注入を使用して動脈瘤出血後の脳動脈痙攣を予防できることがわかった(実施例4)。脳内動脈瘤の破裂によるクモ膜下出血(SAH)は毎年28,000人のアメリカ人に影響を与えている。動脈瘤性SAHの患者のほぼ70%は、SAH後7日目に脳動脈の重篤な痙攣を起こす。積極的な医学的治療にもかかわらず、血管痙攣の結果として生じる神経性欠損は、罹患率および死亡率の主要な原因であり続けている。脳血管痙攣の病因はほとんど解明されていないが、脳脊髄液中の赤血球溶血、および強力な血管拡張物質である一酸化窒素(NO)の利用能の低下が重要な役割を演じているという証拠が増大している。NOまたはNOプロドラッグによる血管痙攣の好転がいくつかの動物モデルで報告されている。
本明細書で提示する結果に基づき、亜硝酸イオン、特に本明細書で記載する薬学的に許容される亜硝酸塩を使用して、妊娠中の高血圧および子癇前症を処置できると考えられる。このような療法は、胎盤内の痙攣性および患部血管に対する亜硝酸塩の作用を含むであろう。
亜硝酸イオンは、その塩を含め、対象において血圧を下げたり、血管拡張を増したりするために、本明細書で提供する方法にしたがって対象に投与される。本開示による亜硝酸塩の投与は、たとえばレシピエントの生理学的状態、投与の目的が治療であるのか予防であるのか、および実務者に公知の他の要因に依存して、単一用量、多用量、および/または連続的もしくは断続的であってよい。亜硝酸塩の投与は本質的には、事前に選択された期間にわたって連続的であってもよいし、間隔を置いた一連の用量であってもよい。投与量は、処置される状態ならびに対象の体重、身体状態、健康状態、および年齢を含むがこれらに限定されない様々な要因に依存して異なる。このような要因は、臨床医により、動物モデルまたは当技術分野において利用可能な他の試験系を使用して決定することができる。
さらに亜硝酸塩はまた、記載の状態または他の状態に対し、他の治療剤、たとえば鎮痛剤、抗炎症剤、抗ヒスタミン剤などと組み合わせて使用することもできる。例として、さらなる薬剤は、ペニシリン、ヒドロキシ尿素、酪酸塩、クロトリマゾール、アルギニン、またはホスホジエステラーゼ阻害薬(たとえばシルデナフィル)からなるリストから選択される一つまたは複数である。
亜硝酸塩はインビボで血管拡張性を有する
この実施例は、亜硝酸塩が、注入によってヒト対象の前腕に投与された場合、有効な血管拡張剤であることを実証する。
ヒト対象プロトコル
このプロトコルは米国国立心臓・肺・血液研究所の施設内倫理委員会(the Institutional Review Board of the National Heart, Lung and Blood Institute)によって認可され、すべてのボランティア対象からインフォームドコンセントを得た。平均年齢33歳(21〜50歳)の男性9人および女性9人が実験に参加した。さらに10名の対象が、3〜6ヶ月後、低用量亜硝酸塩注入での第二の一連の実験のために戻ってきた。ボランティアは正常なヘモグロビン濃度を有し、全員、内皮機能不全の危険因子(空腹時血糖 >120 mg/dL、LDLコレステロール >130mg/dL、血圧 >145/95 mmHg、過去2年間の喫煙、心臓血管疾患、末梢血管疾患、凝固障害、または血管炎もしくはレイノー現象の素因となる他の疾患)をもたない優秀な健康状態であった。G6PD欠乏症、既知のチトクロームB5欠乏症またはベースラインメトヘモグロビンレベル >1%の対象は除外した(スクリーニングした対象のうち、これらの除外規準に該当したものはいなかった)。授乳中および妊娠中の女性は除外した(HCGレベル陽性の対象1名を除外した)。ボランティア対象には、実験前の少なくとも1ヶ月間、いかなる薬物(経口避妊薬は許可)、ビタミンサプリメント、生薬、栄養補助食品または他の「代替療法」をも摂取させず、実験前の1週間はアスピリンの摂取も許可しなかった。
上腕動脈および肘窩静脈カテーテルを腕に挿入し、動脈内カテーテルを血圧計測のための圧力変換器および生理食塩水を1mL/minで送達する注入ポンプに接続した。20分間の安静ののち、ベースライン動脈血および静脈血サンプルを採取し、以前に報告されているひずみゲージ静脈閉塞プレチスモグラフィー(Panza et al., Circulation, 87, 1468-74, 1993)によって前腕血流計測を実施した。血流測定ごとに7回の血流計測値を平均した。亜硝酸塩注入の間の前腕血流応答に対する時間の影響を最小限にするため、パートIおよびIIと呼ぶ一連の計測を無作為な順序で実施した。
パートI
0.9%NaCl(食塩水)溶液を1mL/minで20分間、上腕動脈に注入したのち、以下に記すアッセイのために動脈血および静脈血のサンプルを採取し、前腕血流を計測した。握力計(Technical Products社)を使用して、予め定めた最大握力の3分の1の力での反復把握動により、運動を実施した(Gladwin et al., Proc Natl Acad Sci USA, 97, 9943-8, 2000; Gladwin et al., Proc Natl Acad Sci USA, 97, 11482-11487, 2000; Cannon et al., J Clin Invest, 108, 279-87, 2001)。各収縮を10秒間継続させたのち、5秒間弛緩させた。5分間の運動ののち、運動の弛緩段階中に前腕血流計測値を得、動脈および静脈のサンプルを採取した。食塩水を上腕動脈に連続的に注入しながら20分の安静期間ののち、反復ベースライン血液サンプルおよび前腕血流計測値を得た。次に、L-NMMAを1mL/min(8μmol/min)の速度で上腕動脈に注入した。5分間のL-NMMA注入ののち、前腕血流を計測し、動脈血および静脈血のサンプルを得た。そしてその腕でL-NMMA注入を続けながら、前腕の運動を開始した。L-NMMA注入を続けながら5分間の運動ののち、前腕血流を計測し、血液サンプルを得た(図1)。
食塩水を注入しながら30分間の安静ののち、ベースライン計測値を得、そこで食塩水注入を止め、亜硝酸塩(NaNO2、0.9%食塩水中36μmol/ml)の注入を1 ml/minで開始した。ヒト用の亜硝酸ナトリウムは、Hope Pharmaceuticalsから得たものであり(水10ml中300 mg)、286 mgを、Pharmaceutical Development Serviceの0.9%食塩水100 ml中で最終濃度36μmol/mlまで希釈した。実験した最後の9名の対象の場合、0.01〜0.03 mM重炭酸ナトリウムを生理食塩水に加えてpHを7.0〜7.4に滴定した。亜硝酸塩溶液は遮光し、すべての実験ののち、溶液中の亜硝酸塩レベルおよび遊離NOガスを還元的化学発光によって計測した(Gladwin et al., J Biol Chem, 21, 21, 2002)。亜硝酸塩溶液中には50.5±40.5nMのNOしか存在せず、重炭酸緩衝による影響を受けていなかった。亜硝酸塩溶液中のNOレベルと亜硝酸塩の血流効果との間に相関関係は見られなかった(r=-0.23、P=0.55)。5分間の亜硝酸塩注入ののち、前腕血流計測値および血液サンプルを得るとともに、亜硝酸塩注入を短く中断して動脈サンプルを得た。亜硝酸塩注入を続けるとともに、前記のように運動を実施し、上記のように前腕血流計測値および血液サンプルを得た。亜硝酸塩注入を止め、その後30分の安静期間中、食塩水注入を再開した。2回目のベースライン計測ののち、L-NMMAを8μmol/minで注入しながら亜硝酸塩注入を再開した。5分後、前腕血流計測を実施し、血液サンプルを得たのち、亜硝酸塩およびL-NMMAの注入を続けながら5分間の運動を実施した。最終的な前腕血流計測値および血液サンプルを得た。パートII中のすべての時点で、メトヘモグロビンおよびNO修飾ヘモグロビンの全身レベルの測定のために、反対の腕の肘窩静脈から血液サンプルを得た(図2、3、および4)。亜硝酸ナトリウムの合計注入量は、36μmol/min×15分×注入2回 = 1.08 mmol = 75 mg(MW NaNO2 = 69)であった。
S-ニトロソ-ヘモグロビンは還元性赤血球環境では不安定であり、酸素圧には依存せずに温度および酸化還元依存的に急速に崩壊する(Gladwin et al., J Biol Chem, 21:21, 2002)。計測に備えてS-ニトロソ-ヘモグロビンを安定化するためには、赤血球をフェリシアン化物で速やかに酸化させなければならない。亜硝酸塩注入の前および最中、上腕動脈および肘窩静脈から血液を抜き取り、全血をただちに(処理時間を省くためにベッドサイドで)1%NP-40(膜を可溶化するため)、8 mM NEM(遊離チオールと結合し、人為的S-ニトロソ化を防ぐため)、0.1 mM DTPA(極微量の銅をキレート化するため)、ならびに4mMフェリシアン化物およびシアン化物(S-ニトロソヘモグロビンを安定化し、処理中の人為的なエクスビボ鉄-ニトロシル化を防ぐため)を含有するPBSのNO-ヘモグロビン「安定化溶液」中で1:10に溶解した。サンプルを総容積9.5 mLのSephadex G25カラムに通して脱塩して、亜硝酸塩および過剰な試薬を除去し、ヘモグロビンを部分的に精製した(99%ヘモグロビン調製物)。Drabkinの方法によってヘモグロビン画分を定量し、ヘモグロビン画分を、塩化第二水銀を用いた場合と用いない場合で反応させ(1:5のHgCl2:ヘム比、水銀不安定なS-ニトロソチオールと水銀安定な鉄-ニトロシルとを区別するために使用)、さらに0.1M HCL/0.5%スルファニルアミド中で反応させた(残留亜硝酸塩を除去するため。Marley et al., Free Radic Res, 32, 1-9, 2000)。そしてサンプルを、化学発光一酸化窒素分析装置(Sievers、モデル280 NO分析装置、Boulder, CO)と直列に配した三ヨウ化物(I3 -)の溶液に注入した。水銀安定なピークが鉄-ニトロシル-ヘモグロビンを表す。このアッセイは、全血中5nM(ヘムあたりS-NO 0.00005%)までのS-ニトロソ-ヘモグロビンおよび鉄-ニトロシル-ヘモグロビンの両方に対して感度および特異性を示す(Gladwin et al., J Biol Chem 21, 21, 2002)。
亜硝酸塩とデオキシヘモグロビンとの反応からフリーNOラジカルが形成しうるかどうかを判定するため、化学発光NO分析装置(Seivers, Boulder, CO)と直列に配した、ヘリウムまたは酸素(いずれも21%および100%)でパージした遮光反応容器中、100および200μM亜硝酸塩を660および1000μM脱酸素化赤血球5mLと混合した。5分間平衡させたのち亜硝酸塩を注入し、NO生成の速度を計測した。亜硝酸塩を、対照としてのPBSおよび100μMヘモグロビンに注入して、660および1000μM脱酸素化赤血球溶液中の溶血を制御した。すべての実験の最後に、上澄みおよび赤血球反応混合物の可視吸収スペクトルを解析し、最小二乗アルゴリズムを使用してヘモグロビン組成を逆重畳積分した。系中には100μM未満の溶血しかなく、ヘモグロビンの変性はなく、鉄-ニトロシル-ヘモグロビンの有意な形成があった。赤血球懸濁液からのNO生成は、溶血産物対照から生成されるものを超え、赤血球からのNO排出と合致していた。
前腕NO合成がL-NMMAによって阻害される場合、生理食塩水注入対照値と比較して亜硝酸塩注入中の前腕血流の25%改善を検出するための実験には18名の対象が必要であろうことが先験的サンプルサイズ計算により決定された(アルファ=0.05、パワー=0.80)。実験の比較しうる時点でのベースライン値とL-NMMA注入値との間、ベースライン値と運動値との間、および亜硝酸塩値と食塩水対照値との間の対単位の比較のために、対応t検定によって両側P値を計算した。ベース、L-NMMA注入、および運動条件の間のNO種の動脈から静脈への勾配に関して反復測定ANOVAを実施した。示す計測値は平均±標準誤差である。
亜硝酸塩が血管拡張剤であるかどうかを判定し、亜硝酸塩のインビボ化学作用を試験するため、健常な対象18名(男性9名、女性9名、年齢21〜50歳)を生理学的実験に参加させた。プロトコルのパートIは、運動およびNO合成阻害に対する前腕内での正常な血行動態的および代謝的応答を計測するために、亜硝酸塩注入中にこれらの介入を実施したプロトコルのパートIIの対照として設計されたものであった。初期ベースライン計測値は、85.6±3.7mmHgの平均血圧および組織100 mLあたり4.0±0.3 ml/minの前腕血流を含んだ(図1A)。反復把握前腕運動が血流を安静時の値に対して約600%増大させ、同側の静脈ヘモグロビン酸素飽和度、pO2、およびpHを有意に低下させ、これは酸素消費およびCO2生成の増大と合致するものであった。20分の安静期間ののち、初期ベースライン値と比較して、反復血行動態的計測は約10%高い前腕血流を示したが、全身血圧または前腕静脈ヘモグロビン酸素飽和度、pO2およびpH値に変化は見られなかった(図1B)。次に、NO合成酵素阻害薬L-NMMAを8μmol/minで5分間、上腕動脈に注入すると、前腕血流が有意に約30%低下し、静脈ヘモグロビン酸素飽和度、pO2およびpH値が有意に低下した。L-NMMA注入を継続しながらの反復前腕運動が血流を増大させたが、運動のみの場合に比べると有意に低いピーク値であった(P<0.001)。加えて、L-NMMAを注入しながらの運動中、ヘモグロビン酸素飽和度、pO2およびpHは、局所NO合成酵素阻害なしで運動した場合によりも有意に低かった(それぞれP<0.001、P<0.005およびP=0.027)。プロトコルのパートIのすべての構成部で平均動脈血圧に変化はなかった。
亜硝酸塩注入の前および最中、上腕動脈および肘窩静脈から血液を抜き取り、全血をただちに(処理時間を省くためにベッドサイドで)NO-ヘモグロビン「安定化溶液」中で1:10に溶解し、鉄-ニトロシル-ヘモグロビンおよびS-ニトロソ-ヘモグロビン含量を、方法の項で述べた三ヨウ化物ベースの還元的化学発光および電子常磁性共鳴分光法によって測定した。S-ニトロソ-ヘモグロビンおよび鉄-ニトロシル-ヘモグロビンのベースラインレベルは検出限界(<50 nMまたはヘムあたり0.0005% NO)にあり、動脈-静脈間の勾配はなかった。プロトコルのパートIIでの亜硝酸塩注入ののち、鉄-ニトロシル-ヘモグロビンおよびS-ニトロソ-ヘモグロビンの静脈レベルは両方とも著しく上昇した(図4A)。両NO-ヘモグロビンアダクトの形成は、血管床にかけて、10秒未満の半循環時間で起こった。鉄-ニトロシルおよびS-ニトロソ-ヘモグロビンとして計測され、静脈レベルから動脈レベルを差し引き、その差を血流で乗じることによって定量化したNO形成の速度は、局所亜硝酸塩濃度を希釈する血流の増大に続発して起こる亜硝酸塩の静脈濃度の有意な低下にもかかわらず、運動中に大幅に増大した(図4A;反復測定ANOVAによる。鉄-ニトロシル-ヘモグロビンの場合、P=0.006、およびS-ニトロソ-ヘモグロビンの場合、P=0.02)。
心臓および肝臓の虚血-再かん流中の亜硝酸塩の細胞保護効果
実施例1で実証したように、亜硝酸塩は、生理学的酸素勾配に沿ったデオキシヘモグロビンとの反応、すなわち、速度が酸素およびpH依存性であり、低酸素血管拡張に寄与する可能性のある化学作用によって、NOに還元される。この予想外の発見に基づき、我々は、虚血組織における亜硝酸塩からの低酸素症依存性NO生成が虚血-再かん流傷害を抑制する可能性を提唱した。この実施例は、亜硝酸ナトリウムの注入が心臓および肝臓の虚血-再かん流中に細胞保護を提供するのに効果的であることを示す。

薬品および試薬
亜硝酸ナトリウム(S-2252)および硝酸ナトリウム(S-8170)は、Sigma Chemical Co.(St. Louis, MO)から得た。亜硝酸ナトリウムおよび硝酸ナトリウムはリン酸緩衝食塩水に溶解し、pHを7.4に調節した。すべての実験において、最終量50μLの亜硝酸ナトリウムまたは硝酸ナトリウムをマウスに投与し、総循環血流量を2mLと仮定して0.6〜240μMの亜硝酸塩最終循環濃度を達成した。直接血管内NO掃去剤カルボキシ-PTIO[2-(4-カルボキシフェニル)-4,4,5,5-テトラメチルイミダゾリン-1-オキシル-3-オキシドカリウム塩]を使用して、肝I/R傷害後のNO依存効果を阻害した。カルボキシ-PTIO(Alexis Biochemicals)をリン酸緩衝食塩水に溶解し、肝臓虚血の30分前に50μL容量中1 mg/kgの用量で静脈内投与した。ヘムオキシゲナーゼ-1阻害薬である亜鉛(II)重水素化ポルフィリンIX-2,4-ビスエチレングリコール(Zinc(II) Deuteroporphyrin IX-2,4-bisethyleneglycol; ZnDBG)(Alexis Biochemicals)を肝臓虚血誘発の30分前に50μL容量中10mg/kgの用量で腹腔内投与した。
本研究で使用したすべてのマウスは、Jackson Laboratories(Bar Harbor, ME)から得た8〜10週齢のC57BL6/Jであった。肝I/R傷害のさらなる実験では、内皮一酸化窒素合成酵素(eNOS)を完全に欠損する(-/-)マウスを使用した。eNOS-/-マウスはもともと、Dr. Paul Huang(Mass. General Hospital)から寛大にも提供されたものであり、LSU-Health Sciences Centerにある我々の繁殖コロニーで繁殖させた。eNOS-/-マウスは、8〜10週齢のものを使用した。
肝I/Rプロトコルは、図6Aで示されており、以前に記載されている(Hines et al., Biochem Biophys Res Commun 284: 972-976, 2001; Hines et al., Am J Physiol Gastrointest Liver Physiol 284:G536-545, 2001)。マウスをケタミン(100 mg/kg)とザイラジン(8 mg/kg)との組み合わせで麻酔し、正中開腹術を実施して肝臓を露出させた。次に、血液凝固を防止するため、マウスにヘパリン(100μg/kg, i.p.)を注射した。マイクロ動脈瘤鉗子を使用して肝動脈および門脈を完全に締め付けることにより、肝臓の左葉および中葉を虚血状態にした。この実験モデルの結果、区域的(70%)肝虚血症が生じた。この部分虚血症モデルは、肝臓の右葉および尾状葉の門脈除圧を許すことにより、腸間膜静脈うっ血を防止する。次に、肝臓を腹腔内に戻して元の場所で45分間配置した。0.9%生理食塩水に浸漬したガーゼを使用して肝臓を湿らせておいた。加えて、加熱灯を使用し、直腸温度プローブで体温を監視することによって体温を37℃に維持した。擬似手術は、マイクロ動脈瘤鉗子によって肝臓血流を減らさなかったこと以外は同じであった。すべての実験で肝虚血の期間は45分であり、その後、マイクロ動脈瘤鉗子を取り外した。肝臓再かん流の期間は、血清肝トランスアミナーゼレベル(すなわちASTまたはALT)の実験では5時間であり、肝組織病理学(たとえば肝細胞梗塞)の実験では24時間であった。
アスパラギン酸アミノトランスフェラーゼ(AST)およびアラニンアミノトランスフェラーゼ(ALT)に関して、分光光度分析法を使用して血清サンプルを分析した(Sigma Chemical Co., St. Louis, MO)(Harada et al., Proc Natl Acad Sci USA 100:739-744, 2003)。これらの酵素は肝臓特異的であり、傷害時に肝臓から放出される(Hines et al., Biochem Biophys Res Commun 284:972-976, 2001; Hines et al., Am J Physiol Gastrointest Liver Physiol 284:G536-545, 2001)。
以前に報告されているようにして肝組織の組織病理学実験を実施した(Hines et al., Biochem Biophys Res Commun 284:972-976, 2001)。肝組織を10%緩衝ホルマリン中で24時間固定し、パラフィンに包埋し、10μM切片をヘマトキシリンおよびエオシンで染色した。以下の規準を使用して、無作為な高倍率視野で組織病理学的スコア付けを二重盲検的に実施した。
0―肝細胞損傷なし
1―細胞質液胞化および限局性核濃縮を特徴とする軽度の損傷
2―洞様毛細血管拡張、サイトゾル液胞化、および細胞内境界の不明瞭化を示す中程度の損傷
3―凝固性壊死、豊富な洞様毛細血管拡張、肝細胞索中へのRBC溢出、ならびに過好酸球増加および好中球の辺縁趨向を示す中程度ないし重度の損傷
4―肝臓構造の損失、肝細胞索の崩壊、出血、および好中球浸潤を示す重篤な壊死
以前に記載されている方法(Jones et al., Am J Physiol Heart Circ Physiol 286:H276-282, 2004)と同様にして、左主冠状動脈(LCA)の外科的結紮を実施した。簡潔にいうと、マウスをケタミン(50 mg/kg)およびペントバルビタールナトリウム(50 mg/kg)の腹腔内注射によって麻酔した。次に、マウスを手術台に腹側を上にして取り付けた。PE-240管に接続したPE-90ポリエチレン管をマウスに口から挿入したのち、モデル683齧歯類用人工呼吸器(Harvard Apparatus, Natick, MA)に接続した。一回換気量を2.2ミリリットルにセットし、呼吸速度を毎分122回にセットした。人工呼吸器サイドポートを介してマウスに100%酸素を与えた。電気メスを使用して正中胸骨切開術を実施し、近位左主冠状動脈を見えるようにし、先細り針に取り付けた7-0絹縫合糸(BV-1 ethicon)で完全に結紮した。心筋梗塞サイズの初期実験では、冠状動脈閉塞を30分間維持したのち、縫合糸を抜糸し、24時間再かん流した。心臓機能のさらなる実験では、近位LCAを45分間完全に閉塞したのち、縫合糸を抜糸し、48時間再かん流した。これらの実験では、二次元超音波心臓診断をベースラインおよび再度48時間の再かん流で実施した。
24時間の再かん流で、マウスを前記のように麻酔し、挿管し、齧歯類用人工呼吸器に接続した。カテーテル(PE-10管)をエバンスブルー染料注入に備えて総頸動脈に配置した。正中胸骨切開術を実施し、左主冠状動脈を前と同じ場所で再結紮した。エバンスブルー染料(2.0%溶液1.2 mL、Sigma Chemical Co.)を頸動脈カテーテルに通して心臓に注入して、虚血区域を非虚血区域から画定した。心臓を速やかに切除し、長軸に沿って厚さ1 mmの5枚の切片に連続的に切断したのち、それらの切片を1.0% 2,3,5-トリフェニルテトラゾリウムクロリド(Sigma Chemical Co.)中、37℃で5分間インキュベートして、危険区域内の生存可能心筋と非生存可能心筋とを区別した。5枚の厚さ1 mmの心筋スライスそれぞれを計量し、梗塞、危険、および非虚血性の左心室面積を、盲観察者がコンピュータ援用面積測定によって評価した(NIH Image 1.57)。危険左心室面積および梗塞サイズ測定のすべての手法は以前に記載されている(Jones et al., Am J Physiol Heart Circ Physiol 286: H276-282, 2004)。
Sequoia C256(Acuson)とインターフェスさせた15MHz線形アレイ変換器(15L8)を使用する左心室の経胸郭超音波心臓診断を、45分の心筋虚血および48時間の再かん流に供したさらなるマウスグループ(n=9溶媒、n=10亜硝酸塩)で実施した。以前に記載されているように(Jones et al., Am J Physiol Heart Circ Physiol 286:H276-282, 2004; Jones et al., Proc Natl Acad Sci USA 100:4891-4896. 2003)、二次元超音波心臓診断をベースラインおよび48時間の再かん流で実施した。最新の技術を使用して心室パラメータを計測した。中乳頭レベルで左心室(LV)の胸骨傍短軸および長軸2D図からM-モード(掃引速度=200 mm/sec)心エコー図を捕捉した。以下の式にしたがってLV部分短縮率(FS)を計算した。LV%FS=((LVEDD-LVESD)/LVEDD)×100。すべてのデータは、実験あたり10回の心周期から計算した。
血中亜硝酸塩測定のために、全血160μLを、80 mMフェリシアン化物、20 mM N-エチルマレイミド(NEM)、200μLジエチレントリアミン五酢酸(DTPA)、および0.2% NP-40を含有する亜硝酸塩安定化溶液40μLと混合した(前記濃度は全血と混合した後の濃度)。次に、全血中の亜硝酸塩を、以前に記載されている三ヨウ化物ベースの還元化学発光によって測定し(Gladwin et al., J Biol Chem, 21:21, 2002; Yang et al., Free Radic Res 37:1-10, 2003)、立証した。
データを、StatViewソフトウェアバージョン5.0(SAS Institute, Carey, North Carolina)を使用する事後(post hoc)ボンフェローニ解析により、二方向分散分析(ANOVA)によって解析した。データを平均値±平均値の標準誤差(SEM)として報告し、p<0.05の場合、差を有意とみなす。
腹腔内亜硝酸塩が肝虚血-再かん流(I/R)傷害を抑制する
肝虚血中の1.2〜480 nmolの亜硝酸ナトリウム(マウスの総血量2mL中0.6μM〜240μMの推定最終濃度)の腹腔内送達が、肝トランスアミナーゼ、アスパラギン酸アミノトランスフェラーゼ(AST)およびアラニンアミノトランスフェラーゼ(ALT)の血清中上昇を用量依存的に抑制し(図6Bおよび6C)、ピーク効果が24μM(添加亜硝酸塩48 nmol)の計算全身濃度で起こった。全く対照的に、食塩水または硝酸ナトリウム(48 nmol)での処置は、肝I/R傷害の状況で何ら保護効果を及ぼさなかった。インビボ肝臓虚血(45分間)およびより長い再かん流(24時間、図6D、6E、および6F)ののちのマウスにおける肝細胞傷害に対する亜硝酸塩処置の効果を評価するため、さらなる実験を実施した。最終血中濃度24μM(48 nmol)の亜硝酸塩の投与が、食塩水および硝酸塩で処置した動物と比較して、24時間の再かん流での肝細胞傷害を有意に減らした。加えて、亜硝酸塩治療はまた、45分間の肝臓虚血および24時間の再かん流ののち、肝細胞アポトーシスの程度を有意に(p<0.001)減衰した(図6F)。I/Rに供された亜硝酸塩処置動物における肝細胞アポトーシスの程度は、擬似処置された対照動物で観察された程度と同様であった(p=NS)。
次に、肝臓虚血-再かん流傷害に対する亜硝酸塩の強力な細胞保護効果が他の器官系にも一般化することができるかどうかを判定するため、冠動脈閉塞および再かん流の状況での急性亜硝酸塩療法の潜在的心臓保護効果を評価するための実験を次に実施した。心筋I/R研究の実験プロトコルが図7Aに示されている。再かん流5分前での左心室腔への亜硝酸塩(48 nmol)の投与が、48 nmol硝酸塩処置に比べて、心筋梗塞サイズを有意に(p<0.001)制限した(図7Bおよび7C)。同様な心筋危険区域(グループ間のp=NS)にもかかわらず、危険区域あたりおよび左心室あたりの心筋梗塞サイズはいずれも、硝酸塩の場合に比べて亜硝酸塩療法で67%減少した。
デオキシヘモグロビンおよび脱酸素化ヘムタンパク質との反応における亜硝酸塩からNOおよびS-ニトロソチオールへの以前に記された還元と合致して(Bryan et al., Proc Natl Acad Sci USA., 2004; Cosby et al., Nat Med 9:1498-1505, 2003; Nagababu et al., J Biol Chem 278:46349-46356, 2003; Doyle et al., J Biol Chem 256:12393-12398, 1981)、再かん流の1分後、虚血に供された食塩水(対照)処置マウスの肝臓中の亜硝酸塩のレベルは、1.75μMから検出不能なまでに低下し(p<0.001対擬似グループ)、水銀安定なNO修飾タンパク質(N-ニトロソアミンおよび鉄-ニトロシルタンパク質、RxNOと同様)のレベルは約750 nMまで増加した(図8A、p<0.001)。興味深いことに、亜硝酸塩処置により、亜硝酸処置マウスにおける亜硝酸塩(図8B)、S-ニトロソチオール(図8C)およびN-ニトロソアミン(図8D)の再かん流後の肝臓レベルの有意な増大があった(p<0.01対食塩水処置対照)。これらのデータは、低酸素ストレス中に亜硝酸塩が生体活性化されるという提唱と一致し、全身性無酸素傷害後の組織亜硝酸塩からRSNOおよびRxNOへの急性転換を実証するBryanらの最近の研究(Proc Natl Acad Sci USA., 2004)とも一致する。細胞保護性である亜硝酸塩の低いレベル(最低用量で1.2 nmol、図6Bおよび6C)および食塩水処置対照動物中の「生来の」肝臓亜硝酸塩の還元的分解(図8A)は、これが、低酸素NO生成および細胞保護の自然な機序である可能性を示唆する。与えられた亜硝酸塩の近生理学的量と合致して、最大有効量である48 nmolの亜硝酸塩で処置されたマウスにおける血中亜硝酸塩レベルは有意には上昇しなかった(594±83nMから727±40nM、n=3、p=0.16)。
亜硝酸塩からNOへの低酸素還元を含む機序をさらに裏付けるものとして、全要因設計実験でNO阻害薬PTIOが亜硝酸塩の保護効果を完全に阻害した(図9A)。対照的に、内皮NO合成酵素(eNOS)欠乏マウスでは有意な亜硝酸塩細胞保護が認められ(図9B、p<0.001)、虚血-再かん流中の亜硝酸塩からのNO生成がeNOS非依存性であることを示唆している。このモデルでは、虚血-再かん流後にヘムオキシゲナーゼ1タンパク質発現が有意に誘発され、保護を提供するようである(図9Cおよび9D)が、ZnDPBG(特異的かつ強力なヘムオキシゲナーゼ1阻害薬)で前処置されたマウスでは、亜硝酸塩は組織損傷を有意に抑制し、ヘムオキシゲナーゼ非依存性効果が示唆された(図9C、p<0.05)。
この実施例では、亜硝酸処置が、食塩水および硝酸塩処置対照に比較して、肝臓亜硝酸塩レベルおよびニトロソ化(ニトロシル化)種(RSNOおよびRXNO)を有意に増大させ、劇的な用量依存性細胞保護効果を提供して、壊死、アポトーシスを抑制し、器官機能を保存した。注目すべきことには、加えられた亜硝酸塩のレベルは生理学的レベルに近く、1.2 nmolの亜硝酸塩添加量でさえ保護効果が認められ(計算血中レベル600 nM)、これが、重度の代謝または病態生理学的ストレスを緩衝する内因性保護機序を表している可能性を示唆した。
噴霧吸入亜硝酸塩は低酸素感応性NO依存性選択的肺血管拡張剤である
この実施例は、新生児肺高血圧症を処置するための噴霧吸入される亜硝酸塩(特に亜硝酸ナトリウム)の使用の詳細な説明を提供する。
動物プロトコルは、Loma Linda UniversityのInstitutional Animal Research Committeeによって認可され、実験動物の使用に関する米国国立衛生研究所(National Institutes of Health)ガイドラインに準ずるものであった。
静脈内チオペンタールナトリウム(20mg/kg)で麻酔を誘導したのち、新生仔ヒツジに経口気管内挿管を実施し、カテーテルを外科的に配置するまで1%ハロタンで麻酔状態を維持した。その後、ハロタンを中止し、モルヒネ(0.1 mg/kg/hr)で麻酔状態を維持した。ベクロニウム(0.1 mg/kg/hr)で麻痺させたのち、肺を、圧力:22/6 cm H2O、頻度:毎分25呼吸、FiO2:0.21、および吸気時間:0.6秒の初期設定で機械的に呼吸させた(Sechristモデル100、Sechrist Industries, Anaheim, CA, USA)。初期、および酸素正常状態での実験全体を通じて、頻度、ピーク吸気圧、およびFiO2の人工呼吸器設定を調節して、SaO2>95%、PaO2 90〜150Torr、およびPaCO2 35〜45Torrを維持した。
胸郭中央レベルでゼロに合わせて較正した圧力変換器(COBE Laboratories, Lakewood, CO)を使用して、平均動脈圧、平均肺動脈圧、および中心静脈圧を連続的に計測し、肺毛細管楔入圧を断続的に計測した。Com-2温度希釈モジュール(Baxter Medical, Irvine, CA, USA)を使用する温度希釈により、実験を通じて心拍出量を15分間隔で計測した。氷冷食塩水の5 ml注入を使用した。測定は三重反復で実施し、サンプリング時点ごとに結果を平均した。標準式を使用して肺血管抵抗および全身血管抵抗を計算した。
実験中に間隔をおいて収集した血液サンプル(0.3 ml)中で動脈および混合静脈pH、PCO2、およびPO2を計測した。血液ガスを計測し(ABL3, Radiometer, Copenhagen, Denmark)、ヘモオキシメータ(OSM2 Hemoximeter, Radiometer, Copenhagen, Denmark)を使用してオキシヘモグロビン飽和度およびヘモグロビン濃度を計測した。OSM2ヘモオキシメータを用いる光覚計測により、血液ガス測定の場合と同じ動脈サンプルを使用して、動脈および混合静脈メトヘモグロビン濃度を分析した。
すべての実験で、亜硝酸ナトリウム水溶液(1 mM溶液)または食塩水のいずれか5ミリリットルをジェット噴霧器(Hudson RCI Micro Mist Nebulizer(Hudson Respiratory Care, Temucula, CA)に入れ、8 L/minの定流量で駆動した。亜硝酸ナトリウム溶液は270μmol/minの速度で噴霧した。エアロゾルを人工呼吸器の吸気ループに送達した。ジェット噴霧器を使用すると、噴霧した薬物の<10%しか肺の中に付着しないと一般に考えられている(Coates et al., Chest 119, 1123-30, 2001)。これは、噴霧器の無駄な容積および呼気段階中の薬物の損失の結果である。肺付着は、気流、充填容量、薬物溶液、および周囲温度の影響を受ける粒度分布に依存する(Flavin et al., Pediatr Pulmonol 2, 35-9, 1986; Suarez & Hickey, Respir Care 45, 652-66, 2000; Clay et al., Thorax 38, 755-9, 1983; Clay et al., Lancet 2, 592-4, 1983)。これは簡単で廉価で広く利用可能な臨床噴霧システムであるが、他のシステムを使用してもよい。
噴霧亜硝酸塩が低酸素新生仔ヒツジにおいて選択的肺血管拡張剤であることを実証するため、7頭のヒツジで実験した。麻酔および器具処置ののち、ヒツジを30〜90分間回復させ、その間に適切な血行動態パラメータをモニタした。ベースライン計測値を得たのち、吸気のFiO2を30分間0.12まで減らすことにより、30分間の肺高血圧症を誘発した。低酸素状態の開始から10分後、低酸素期間の残りの期間、食塩水または亜硝酸ナトリウムのエアロゾルを投与した。1時間の回復期間ののち、再び30分間の低酸素状態を誘導し、その最後の20分間に食塩水または亜硝酸ナトリウムのエアロゾルを投与した。血液ガスおよび分析アッセイのための動脈血サンプルを採取し、規則的な間隔で心拍出量計測を実施した。
酸素正常状態肺高血圧症に対する亜硝酸塩噴霧の効果を評価するため、さらに6頭のヒツジで実験した。トロンボキサンの安定なエンドペルオキシド類似体(U46619−9,11-ジデオキシ-11α-エポキシメタノ-プロスタグランジンF2α、Cayman Chemicals, Ann Arbor, MI)の注入により、安定な酸素正常状態肺高血圧症を誘発した。この薬物は食塩水に溶解し、2μg/kg/minの速度で30分間、大腿静脈カテーテル中に投与した。注入の最後の20分間、亜硝酸塩を噴霧して吸入させた(図11)。
このプロトコルは、亜硝酸塩の効能を臨床標準、すなわち20 ppm吸入NOガスと比較するために設計されたものである。このNOガス濃度は、原発性肺高血圧症の小児に与えられる治療量の上限であり(Kinsella & Abman, Semin Perinatol 24, 387-95, 2000; Kinsella et al., Lancet 340, 819-20, 1992)、また、新生仔ヒツジの低酸素血管狭窄を好転させるのに効果的であることが示されている(Frostell et al., Circulation 83, 2038-47, 1991)。第二の目的は、長期低酸素誘発肺血管狭窄における血行動態的および生理学的計測において短い亜硝酸塩噴霧の効果の持続期間をNOガス吸入に対して決定することであった。ベースライン計測を実施したのち、上記のようにして35分間ヒツジを低酸素状態にした。低酸素状態の発生から10分後、20分間のNOガス吸入(20 ppm)を開始し、NOガス送達を止めた後も5分間、低酸素状態を継続した。その後1時間ヒツジを回復させた。再び、ベースライン計測を実施し、2回目の90分間の低酸素状態を開始した。低酸素状態の発生から10分後、亜硝酸ナトリウムエアロゾルを20分間投与し、亜硝酸塩エアロゾル化を止めた後も60分間、低酸素状態を継続した(図13)。
化学発光NO分析装置(NOA 280, Sievers Instruments, Inc., Boulder, CO)を用いて呼気NO濃度を計測した。化学発光分析装置は、NOを含まない空気およびNOガス(45 ppm)を使用して、製造元が推奨するやり方にしたがって較正した。NOは、250 ml/minの流れが分析装置まで通過する気管内チューブの近位端でサンプリングポートに取り付けたTeflonサイドアームを通してサンプリングした。
上腕動脈および中心静脈カテーテルから血液を抜き取り、速やかに処理した。遠心分離後に血漿を分離し、すぐにドライアイスで凍結させたのち、以前に記載されたように(Cosby et al., Nat Med 9, 1498-505, 2003; Gladwin et al., J Biol Chem 277, 27818-28, 2002; Yang et al., Free Radic Res 37, 1-10, 2003)化学発光法(Sieversモデル280 NO分析装置)を使用して亜硝酸塩をアッセイするまで-70℃で貯蔵した。凍結した赤血球ペレットを解凍し、8 mM NEM、100μM DTPA、および4 mMフェリシアン化物中で反応させ、5分間インキュベートし、Sephadex G25カラムに通した(Yang et al., Free Radic Res 37, 1-10, 2003; Xu et al., Proc Natl Acad Sci USA 100, 11303-8, 2003)。G25カラムからのヘモグロビン画分をDrabkinの方法(J. Biol. Chem. 112, 51-65, 1935)によって定量し、0.1M HCl/0.5%スルファニルアミド中で反応させて残留亜硝酸塩を除去した。そして、サンプルを化学発光一酸化窒素分析装置(Sievers、モデル280 NO分析装置、Boulder, CO)と直列に配した三ヨウ化物(I3 -)の溶液に注入した。三ヨウ化物溶液中で鉄-ニトロシル-ヘモグロビンからNOガスを化学量論的に除いた(Yang et al., Free Radic Res 37:1-10, 2003)。
これは、9.4 GHz、10 mW、5G変調、0.08s時定数および600Gで84sの走査時間にセットしたEMX10/12EPR分光システム上、Bruker 4131VT温度制御装置を使用して110Kで実施した。各曲線が一つの84秒走査を表す。ピーク間高さを標準サンプルと比較することによって鉄-ニトロシル-ヘモグロビン濃度を計算した。
平均動脈圧、肺動脈圧、中心静脈圧、心拍数、呼気NO濃度、および中心体温を連続的に計測した。アナログ信号を100Hzでデジタル化し、アナログ・デジタル変換器(PowerLab SP, ADInstruments, Colorado Springs, CO)およびデータ取得ソフトウェア(Chart v 5.02 for Macintosh, ADInstruments, Colorado Springs, CO)を使用して記録した。実験ののち、動脈血圧、中心静脈圧、心拍数、および呼気NO計測値を60秒ブロックに平均化した。
生理学的変数の連続計測値を、反復測定をグループおよび時間とともに因数として用いる二方向ANOVAによって比較した。差の有意性をダネット事後試験で評価した。ベースライン期間からの有意差は、反復測定を個々の動物および時間とともに因数として用いる一方向ANOVAを使用して評価した。差の有意性をさらにNewman-Keulの事後試験で評価した。計算は、GraphPad Prism(GraphPad Software Inc., San Diego, CA, USA)を使用して実施した。統計的有意性をP<0.05で仮定した。データは平均±SEMとして提示する。
低酸素誘発肺血管狭窄におけるエアロゾル化亜硝酸塩の肺血管拡張性
低酸素肺高血圧症に対する噴霧亜硝酸塩の効果を判定するため、7頭の新生仔ヒツジ(2〜10日齢)を全身麻酔下で器具処置し、人工呼吸器およびモルヒネ注入で維持した。ベースライン安定化ののち、FiO2を0.12に減らすことによってヒツジを30分間の低酸素状態に供した。低酸素期間の最後の20分間、噴霧亜硝酸塩または食塩水を投与した。低酸素状態(動脈HbO2約55%)の開始は、平均肺動脈圧の急速な増大(21±1から34±2 mmHgへ、P<0.01)(図10A、10B)および肺血管抵抗の急速な増大(20% (P<0.01))、ならびに全身血管抵抗の低下(約20% (P<0.01))と関連していた。食塩水とは違って(図10A、10B)、噴霧亜硝酸塩の吸入は肺動脈圧を選択的に約60%低下させ(P<0.01)(図10A、10C)、肺動脈抵抗を約70%低下させたが(P<0.05)、対照動物と比較した場合、平均動脈血圧(図10A、10C)または全身血管抵抗に対する計測しうる効果はなかった。亜硝酸塩噴霧による肺動脈圧の低下は、呼気NOの3±1ppbから15±4ppbへの漸増を伴った(図10A、10C)。心拍出量、動脈オキシヘモグロビン飽和度、およびメトヘモグロビンレベルは、亜硝酸塩吸入ののち、直前の10分間の低酸素状態中の値と比較して、計測しうる変化はなかった(図10A)。動脈Po2は、実験的に鉗子で締め付けたため、我々のシステムでは認めうるほどの変化はなかった。
正常なデオキシヘモグロビンの存在下における肺動脈圧に対する噴霧亜硝酸塩の効果を、低下した酸素化ヘモグロビンの存在下における効果と対比させるため、酸素正常条件下で肺高血圧症に供したヒツジ6頭の別々のグループで噴霧亜硝酸塩の効果を実験した。トロンボキサンのエンドペルオキシド類似体(U46619)の注入により、安定な酸素正常(SaO2約98%)肺高血圧症を誘発した。2μg/kg/minで30分間のU46619の静脈内注入は、肺動脈圧の24±1 mmHgから51±4 mmHgへの急増を伴った(P<0.001)(図11)。注入を開始してから10分後、噴霧亜硝酸塩の吸入添加の結果、肺動脈圧の23±6%の選択的低下(注入ベースラインに比較してP<0.05)が生じたが、平均動脈血圧または全身血管抵抗に対する効果はなかった(図11)。亜硝酸塩噴霧による肺動脈圧の低下は、呼気NOの4.8±1.2 ppbから10.1±2.0 ppbへの漸増と関連していた(ベースラインに比較してP<0.05、図11)。図2は、低酸素誘発肺血管狭窄および薬物誘発酸素正常肺血管狭窄に対する20分後の亜硝酸塩吸入の効果の比較を示す。平均肺動脈圧および呼気NOの変化は、低酸素状態における亜硝酸塩処置の場合で有意に大きかった。総じて、酸素正常(トロンボキサン誘発)肺高血圧症に対する亜硝酸塩吸入の効果は、低酸素肺高血圧症で観察された効果よりも低く(図10、11、12A)、亜硝酸塩の血管活性の低酸素血、およびおそらくは酸血の増強作用のモデルと合致していた。
我々は、亜硝酸塩からNOへの生化学的転換がデオキシヘモグロビンおよびプロトン化の両方を要すると仮説を立てた。そして、酸素正常状態実験および低酸素状態実験の両方からのデータを使用して、亜硝酸塩からのNO生成に対するヘモグロビン飽和度およびpHの影響を研究した。呼気NOガスおよびNO修飾ヘモグロビン(鉄-ニトロシル-ヘモグロビン)の計測値を、NO生成の用量計として、また、NOを生成する亜硝酸塩とヘモグロビンとの亜硝酸還元酵素反応の直接的副生成物の尺度として使用した。図12は、三ヨウ化物ベースの還元化学発光(図12B)および電子常磁性共鳴(図12C)による計測で、鉄-ニトロシル-ヘモグロビンが、低酸素状態では亜硝酸塩吸入によって顕著に増大したが、薬物誘発酸素正常肺血管狭窄ではそうはならなかったことを示す。図12Dに示すように、噴霧亜硝酸ナトリウムの吸入後の低酸素状態における平均肺動脈圧の変化は血液pHと相関し、血管拡張の増大はpHの低下を伴った(r=0.57 P=0.055)。
次に、亜硝酸塩の効能を、現在の治療標準、すなわち吸入NOガスと比較した。低酸素状態の開始ののち、ヒツジを(20 ppm)吸入NOガスまたは噴霧亜硝酸塩に20分間供した。図13のデータは、低酸素状態における血行動態および代謝計測値に対するNOガス吸入(図13A)または亜硝酸塩噴霧(図13B、13C)の効果の持続期間および大きさを示す。両処置とも低酸素肺高血圧症の顕著な軽減を生じさせたが、吸入NOに対する反応のほうがわずかに速く、肺圧は、亜硝酸塩によって誘発された60〜70%の圧力修正と対比すると、ベースラインにより近くなった。全身的には、平均動脈血圧および抵抗は、低酸素状態における両方の処置で同程度に低下した。しかし、NOガスと比較した場合の亜硝酸アニオンの相対的な化学的安定性により、亜硝酸塩吸入を停止した後でも60分(低酸素負荷の期間)を超える持続的な血管拡張が得られたが、NOガス送達の停止は、数秒のうちに血管拡張効果を消滅させた(図13A、13B)。亜硝酸塩噴霧の相対的に持続性の効果は、βアドレナリン作用薬を計量吸入器によって用いるぜん息の治療に類似した断続的な治療を可能にすることにより、治療的に有利である可能性がある。亜硝酸塩吸入誘発肺血管拡張の時間経過および血漿亜硝酸塩レベルを示す(図13C、13D)。図10の場合よりも長い期間で生化学的変化を追跡したこの実験では、メトヘモグロビン(MetHb)濃度は、ベースライン時の2.1±0.1%から亜硝酸塩噴霧後の2.8±0.2%まで上昇した(P<0.05)。
この実施例の主要な発見は、噴霧亜硝酸ナトリウム溶液の短期間の吸入が、新生仔ヒツジにおける低酸素誘発肺高血圧症において急速かつ選択的な肺血管拡張を生じさせるということである。亜硝酸塩噴霧後の肺動脈圧の有意な低下は、亜硝酸塩噴霧の停止ののち低酸素状態が1時間を超えて続いた場合でも持続した。いずれの実験でも、亜硝酸塩吸入が全身性低血圧症を生じさせることはなく、メトヘモグロビン上昇は最小限に抑えられた。機序的観点から、亜硝酸塩投与は、呼気NOガスおよびNO修飾ヘモグロビンによって計測されるNO生成と関連し、応答は、ヘモグロビン酸素不飽和化のレベルおよび血液pHの低下と比例していた。これらのデータは、亜硝酸塩が、その生体活性化がヘモグロビン脱酸素化およびプロトン化と連結するNO依存性血管拡張剤であるというパラダイムを支持する。
クモ膜下出血後の脳動脈痙攣の予防のための亜硝酸塩注入の使用

0日目に、麻酔を施した14匹のカニクイザルの右中大脳動脈(RMCA)の周囲に自己由来動脈血餅を配置した。亜硝酸ナトリウムの0.9%食塩水溶液(NaNO2、1日135 mgおよび1日180 mg、およそ1日あたり45 mg/kgおよび60 mg/kgに相当)(n=6)または食塩水のみ(n=8)を、着装携行式MiniMed注入ポンプを介して、覚醒中の動物に14日間2μl/minで静脈内注入した。DCVの評価のため、血餅配置前ならびに7日目および14日目に脳動脈造影を実施した。脳動脈造影のAP投影で計測した場合のRMCA面積の近位14 mmでの25%以上の減少を動脈造影血管痙攣と定義した(盲評価)。0日目から毎日、平均動脈血圧を計測し、血液サンプルを採集した。脳脊髄液サンプルを0日目、7日目、および14日目に採集した。
対照動物では、脳脊髄液亜硝酸塩レベルは、3.1±1.5μMから、7日目に0.4±0.1μMまで減少し、14日目に0.4±0.4μMまで減少し(図14)、8匹の動物すべてがRMCAで有意な血管痙攣を発症し(図15および16)、1匹は脳卒中を併発し、死に至った。
亜急性亜硝酸ナトリウム注入は、SAHの霊長類モデルでDCVを予防し、毒性なしで予防する。これらのデータは、現在予防的療法が存在しない疾病であるDCVのための新規で安全で廉価で合理的に設計された療法を明示する。
Claims (16)
- 対象において血圧を低下および/または血管拡張を増大させることによって、該対象における
(a) 肝臓または心臓または脳の虚血-再かん流傷害、
(b) 肺高血圧症、または
(c) 脳動脈痙攣
から選択される状態を処置または改善するための治療に用いるための薬剤の製造における、酸性化されていない亜硝酸ナトリウムの使用であって、亜硝酸ナトリウムが0.6〜240μMの循環濃度まで投与される、使用。 - 治療が肝臓または心臓または脳の虚血-再かん流傷害を処置または改善するための治療である、請求項1記載の使用。
- 対象への亜硝酸ナトリウムの投与が静脈内投与である、請求項2記載の使用。
- 治療が肺高血圧症を処置または改善するための治療である、請求項1記載の使用。
- 肺高血圧症が新生児肺高血圧症である、請求項4記載の使用。
- 対象への亜硝酸ナトリウムの投与が吸入による投与である、請求項4または5記載の使用。
- 亜硝酸ナトリウムが噴霧される、請求項6記載の使用。
- 亜硝酸ナトリウムが270μmol/minの速度で投与される、請求項4〜7のいずれか一項記載の使用。
- 治療が脳動脈痙攣を処置または改善するための治療である、請求項1記載の使用。
- 対象への亜硝酸ナトリウムの投与が静脈内投与である、請求項9記載の使用。
- 亜硝酸ナトリウムが45〜60 mg/kgの割合で投与される、請求項9または10記載の使用。
- 亜硝酸ナトリウムが少なくとも一つのさらなる薬剤と組み合わせて投与される、請求項1〜11のいずれか一項記載の使用。
- 対象が哺乳動物である、請求項1〜12のいずれか一項記載の使用。
- 対象がヒトである、請求項13記載の使用。
- 亜硝酸ナトリウムが、50μM以下の循環濃度まで投与される、請求項1記載の使用。
- 亜硝酸ナトリウムが、16μM以下の循環濃度まで投与される、請求項1記載の使用。
