Description
本発明は、National Institute of Neurological
Disorders and Stroke(NIDS),National Institute of Healthによって与えられた1R01 N5 39097−01A1のもと、米国政府の支援でなされた。
本発明は、中枢神経系に対する外傷性損傷の治療法に関する。
毎年、150万から200万のアメリカ人が外傷性脳損傷(TBI)を受けている(Anonymous,“Traumatic Brain Injury,”Center
for Disease Control and Prevention,National Center for Injury Prevention and Control,2003,Vol.2003)。米国では、TBIは、毎年、5万件の死亡例および10万件の入院例の原因になっていると推定されている(Anonymous,“Traumatic Brain Injury,”Center for Disease Control and Prevention,National Center for Injury Prevention and Control,2003,Vol.2003)。毎年、8万を超える人々が不具となり、その内約1万7千の人が、生存のために特別なケアを必要としている(Kraus(1997)“Epidemiology of Head Injury,”in Head Injury,ed.Cooper(Williams & Wilkins Co.,Baltimore)pp1−19;Selecki et al.(1982)Australian &
New Zealand Journal of Surgery 52(1):93−102)。脳に対する突然の外傷によって形成される初期の病変に加えて、生体力学的に過剰な力がかかることによって、一連の二次的な有害事象が起き、これによって最初の損傷の数日から数ヵ月後に病変部のサイズ、病変の重度、および死亡率が急激に増加し得る(McIntosh et al.(1996)Lab Invest,74(2):315−42;Stambrook et al.(1990)Can J Surg33(2):115−8)。問題の重大性にも拘わらず、ヒトにおけるTBIに対しては有効な薬理学的治療は未だに特定されていない。
中枢神経系の外傷性損傷、より詳細には、外傷性脳損傷を有する被験体を治療する方法が提供される。本方法は、プロゲステロン、または合成プロゲスチンの一定レベル、または2段階レベル投与レジームを含む治療を含む。
例えば、本願発明は以下の項目を提供する。

ヒトにおけるプロゲステロン治療の適用性を評価する第1ステップとして、本発明者らは、静脈注入によって与えたPGの薬物動態における急性TBIおよび頭蓋外外傷の効果を調べた。17時間PGの投与を受けた11名の女性および21名の男性外傷患者、および同じくプラシーボの投与を受けた1名の女性および3名の男性患者から、多数の血液サンプルを得た。AUC(0−72)および注入後血液サンプルを用い、CSS、CL、t1/2、およびVdを得た。CSS値は、337±135ng/mlであり、これは、標的濃度の450±100ng/mlよりも有意に低かった。このようにCSSが低いのは、予想よりもCLが高かったせいである。さらに、t1/2およびVdも予想よりも高かった。これらのパラメータのいずれにも有意な性差は見られなかった。これらの変化は、致死的損傷と関連する、速度論の適用を超える変化と一致する。本発明者らの結果から、TBI後速やかに安定なPG濃度が達成されることがわかる。
患者選択
本研究は、Emory大学学内検閲委員会、薬剤安全監視委員会(NINDS)、およびFDA(IND#58,986)によって認可された。インフォームドコンセントの受領後、下記に概略する参加基準を満たした36名の患者について調べた。治療は、4:1プロゲステロン:プラシーボ比を用いてランダム化した。参加基準の要求は、各患者は≧18歳であること、鈍的外傷による、頭部の閉鎖損傷を持つこと、中等度から重度の脳損傷であること(グラスゴー昏睡スコア(GCS)が4−12)、損傷の11時間未満に救急専門科に到着し、インフォームドコンセントを提出(近親者から)したことである。排除基準は下記の通り。生存不能の損傷;神経学的活性が見られない(GCS 3);軽度のTBI(GCS 13−15);損傷の時期が不明;重度の酩酊(ETOH≧50mg%);神経欠損を伴う脊髄損傷;心肺停止;到着時癲癇状態;到着時、または、実験参加前≧5分に亘って、血圧、収縮期圧が<90;到着時、または、実験参加前≧5分に亘って、低酸素状態pO2<90;妊娠している女性;活動性乳癌または生殖器癌;または、プロゲステロン、またはIntralipid(登録商標)成分(卵黄または大豆油)に対してアレルギーの既往を持つ人である。
実験薬剤の溶液は、Emory保険部の実験薬剤調剤科によって下記のように調製された。プロゲステロンは、95%エタノールに溶解し、0.2μフィルターを用いてろ過し、滅菌バイアルに封入した。各溶液の分液については、最終濃度と滅菌性について定量した。プロゲステロン/プラシーボの保存液を、ランダム化治療割り当てに合致するようにキット(A、B、C、D、またはE)に包装した。各キットは6個のバイアルを含むものとした。バイアル1は、15mlのプロゲステロンまたはプラシーボを含み、これは、初回ボーラス投与量および最初の注入投与を調製するのに用いられた。残りの5個のバイアルは、残りの注入用として12mlのプロゲステロンまたはプラシーボを含むものとした。プロゲステロンは、アルコールにのみ溶けるので、プロゲステロン液と複合される希釈剤は95%エタノールとした。プラシーボキットも95%エタノールによって処方された。アルコール濃度が高いので、注入の直前に各用量の実験薬剤をイントラリピッドと混合した。各注入用量を12時間に亘って投与し、12時間おきに繰り返し、合計72時間とした。患者を無作為化した後、体重およびプロゲステロン最終濃度に基づく用量計算シートを用い、標準的負荷注入速度(14cc/時)、および維持注入(10cc/時)の最初の11時間を達成するために、イントラリピッドで希釈しなければならないバイアル#1の用量を決めた。この用量計算シートは、10cc/時の標準速度において残りの注入容器それぞれからの注入を行うためにイントラリピッドで希釈しなければならない試験薬剤の用量を計算するのにも用いた。
全ての安定性試験のために、Segall,et al.の方法を些少の修正を加えて用いた(Segall,et al.(1999)Journal of Pharmaceutical & Biomedical Analysis,19(5):803−8)。この方法は、元々酢酸メドロキシプロゲステロンおよび吉草酸エストラジオールの錠剤の安定性を評価するために、その有効性が確かめられたものであった。これは、外部標準化を利用する、イソクラティックHPLC−UV法である。5ミクロン、4.6×250mm BDS−Hypersil C−18カラム(Keystone Scientific)を用い、分析は、フォトダイオードアレイ検出器を備えたAgilent 1100モデルHPLCシステムにおいて行った。移動相は、40% 0.07M酢酸アンモニウムバッファー、pH7.2および60%アセトニトリルから成るものとした。検出器は247nMに設定した。システムの適性をチェックしたところ、プロゲステロンピークに基づきメートル当たり2769プレート(最低要求>2500)、および0.80%の相対標準偏差(最低要求1.0%以下)が得られた。プロゲステロンピークの減衰係数は0.5であった。複数の異なる日々に実行した、同じ調製物を10回注入することによって評価した再現性は、常に10%未満であった。
プロゲステロン試験薬剤液の注入は、14mL/時(0.71mg/kg/時)の負荷速度で1時間行い、次いで、残りの71時間は、注入速度を10mL/時(0.5mg/kg/時)に下げた。プロゲステロンを含むイントラリピッド溶液は、最低24時間安定であることが判明したが、試験薬の貯留分を調製し、生物学的汚染の危険度を最小にするために注入期間の間12時間置きに変えた。薬剤投与に中断があった場合、または、その他プロトコールからの逸脱があった場合、それがどのようなものであれ薬剤投与フローシートに記録し、各患者に対して実際に投与されたプロゲステロンの合計ミリグラム数を計算する際に考慮に入れた。
注入中下記の時点で9(5ml)サンプルが得られた。すなわち、注入前(0)、4、6、12、24、36、48、60、および72時間である。さらに5サンプルが、注入停止後下記の時点で得られた。すなわち、0.5、1、2、4、および8時間である。サンプルは凝固させ、次に遠心した。その後、血清を取り出し、分析時まで−70℃に保存した。
血清プロゲステロン濃度は、Emory大学病院、病理学教室、免疫学研究室によるImmulite(登録商標)プロゲステロン化学発光イムノアッセイを用いて測定した。アッセイ変動の、日内および日間変動係数は共に<10%であった。本発明者らは、彼らの実験室で定量した0.5から700ng/mLの範囲に亘る9サンプルの結果を、参照実験室(The Nichols,San Juan Capistrano,カリフォルニア州)によって定量されたものと比較することによって彼らのアッセイの正確であることを確かめた。
薬物動態の一次パラメータCLは、用量の、血清濃度−時間曲線下面積(AUC)に対する比として推定される。AUCは、直線台形則を用いて計算した(Veng−Pedersen(1989)Clin Pharmacokinet,17(6):424−40)。排除相の速度定数keは、注入終了後の血清濃度−時間データから計算した(WinNonin(登録商標)、Pharsight Corporation,Mountatin View,カリフォルニア州)。分布容量は、CLとkeの比として推定した。Cssは、用量とCLの比として推定した。実際のCssは、3個以上の連続サンプルの血清濃度−時間曲線の勾配がゼロではない時に達成される濃度と定義した。
本発明者らの実験室によって測定されたプロゲステロン濃度を、Nichols研究所によって測定されたデータと比較するために、反復測定値用‘t’検定およびスペアマンの順序相関係数を用いた。Css予想濃度を、注入速度/クリアランスの比として計算した。Css予測値とCss測定値の差は、反復測定値用‘t’検定を用いて検定した。本法に関連して何らかの偏倚があった場合、その大きさを評価するためにBland−Altman分析を行った(Bland and Altman(1986)Lancet,1(8476):307−10)。男性および女性間の薬物動態比較は、独立手段に対する‘t’検定を用いて行った。0.05未満のp値を、帰無仮説の拒絶のための最低レベルと見なした。
36名の患者について実験した。32名(男性21名および女性11名)にはプロゲステロンを与え、4名(男性3名および女性1名)にはプラシーボ注入を行った。注入前のプロゲステロン濃度において、女性(2.86±1.37ng/mL)と男性(2.53±1.73ng/mL)(p<0.5)の間に有意差は無かった。プラシーボを与えられた患者の注入前プロゲステロン濃度は、2.1±0.8ng/mLであり、プロゲステロンを与えられた患者と有意に異なっていなかった。さらに、これらの注入前測定値は、実験の84時間過程の間有意に変動しなかった。図1は、プロゲステロンを与えられた一人の患者、および、プラシーボ注入を受けた一人の患者で、共に完全なサンプリングパラダイムが可能であった患者から得た、代表的な血清濃度−時間プロフィールである。プラシーボ注入を受けた患者のプロゲステロン濃度は、実験期間を通じて一定であった。プロゲステロンを受容した患者ではCss濃度に速やかに達せられ、一旦達せられると、注入期間中安定であった。注入開始周辺時、および注入後における完全なサンプリングは、女性の7/11、男性の10/21においてのみ可能であった。これは、実験患者の受けた損傷が生命を脅かす性質のものであったためである。本研究におけるCss値は、癌患者におけるプロゲステロン注入について報告されているデータ(Christen et al.(1993)Journal of Clinical Oncology 11(12):2417−2426)に基づいて予想されたものよりも低かった。
臨床家はこれまで、外傷性脳損傷後早期に患者に与えるべき、効果的な神経保護剤を長らく求めてきた。脳損傷の病理生理学は十分に理解されてはいるものの、研究者達は、細胞死、脳浮腫、従って頭蓋内圧の上昇をもたらす、病理生理的因果関係で結ばれる一連の有害作用を、高い信頼度で変調することが可能な薬剤をこれまで特定することができなかった(Chesnut,et al.(1993)Journal of Trauma−Injury Infection & Critical Care,34(2):216−22;Povlishock and Jenkins(1995)Brain
Pathology,5(4):415−26)。外傷性脳損傷の治療は主に支援性のものであり、脳の還流圧および十分な酸素補給の維持を図る努力を中心とする(Brain Trauma Foundation(1996)“Guidelines for

外傷性脳損傷の治療に関する第II相、ランダム化、二重盲検、コントロール設定治験が実行された。実行された投与プロトコールは、実施例1に記載した通りである。

本発明者らは、急性TBIに対する治療薬としてのプロゲステロンの安全性を評価するために臨床治験を行った。このII相、ランダム化、二重盲検、プラシーボコントロール設定臨床治験は、都市部のレベルI外傷センターで行った。グラスゴー昏睡スケールのスコア4−12を持つ、鈍的TBI後11時間以内に搬送された100名の成人を代理人の同意の下に登録した。被験体は、4:1割合でプロゲステロン対プラシーボにランダムに割り当てた。投薬内容を知らされていない観察者は、有害事象の有無について患者を綿密に監視し、損傷後30日に最初の機能的結果を評価した。一次的安全性結果は、死亡率を含む有害事象率の差である。一次的活性尺度は、損傷30日後における、分岐型グラスゴー結果スケール拡張版(GOSE)である。77名の患者がプロゲステロンの投与を受け、23名がプラシーボの投与を受けた。両グループは、きわめて近似した身体計測および臨床特徴を有していた。死亡率を除き、有害事象の発生率は両グループでほぼ同様であった。実験室数値および生理学的パラメータもほぼ同様であった。プロゲステロンが原因の重大な有害事象は見られなかった。両グループの間で、GOSEおよび、その他の神経学的結果に有意差はなかったが、プロゲステロン治療被験体は、コントロールに比べ全理由を含めた30日死亡率がより低かった(発生率比0.43;95%信頼区間0.18−0.99)。この試験的実験では、プロゲステロンは、目立った害毒を起こすことはなく、TBI治療用として有望な活動兆候を示した。

実験設計:この第II相、ランダム化、二重盲検、プラシーボコントロール設定治験の主要目的は、男女の、急性脳損傷患者に対しプロゲステロンを静脈投与した時に生じる可能性のある有害事象を評価することである。本発明者らはまた、活性シグナルを検出することも期待した。
スクリーニングおよび登録−合計281名の患者をスクリーニングした。参加資格の可能性を持つ3名の患者を見逃し、かつ、18名の患者は、身元不明のため、または、損傷の11時間以内に代理人と接触できなかったために登録できなかった。三つの手続き「保留」時の一つに現れた6名の参加資格可能な患者については登録することができなかった。同意後、ランダム化前に1名の患者が排除された。治療チームが、彼の損傷は生存不能と結論したためである。参加資格があるのに11名の患者は登録されなかった。代理人が同意することを拒否したためである(図1)。非参加者は、性別、人種、および損傷の原因に関して参加者と類似していた。

急性外傷性脳損傷のためにプロゲステロンを使用することはこれまで無かったので、本発明者らは、毒性の可能性を評価するために、試験的な、II相実験を行った。到着する患者達を、その試験参加資格について入念にスクリーニングした。参加資格の可能性を持つ患者の99パーセントをスクリーニングし、参加基準を満たした患者の内90%が、代理人の合意を得て登録された。治療される患者およびコントロール患者は、損傷の重度、治療までの時間、および、結果に関する他の独立予測因子に関してよく一致した。
Claims (18)
- ヒト被験体における外傷性脳損傷の治療において使用するための組成物であって、該組成物は、治療有効量のプロゲステロンを含み、該組成物は、100ng/ml〜1000ng/mlの最終プロゲステロン血清レベルが達成されるように、プロゲステロンの一定の投与レジームまたは2段階レベルの投与レジームにしたがって静脈内注入により投与されることを特徴とする、組成物。
- 少なくとも200ng/mlの最終プロゲステロン血清レベルが達成されるように投与されることを特徴とする、請求項1に記載の組成物。
- 950ng/ml以下の最終プロゲステロン血清レベルが達成されるように投与されることを特徴とする、請求項1または2に記載の組成物。
- 850ng/ml以下の最終プロゲステロン血清レベルが達成されるように投与されることを特徴とする、請求項3に記載の組成物。
- 少なくとも0.1mg/kg/時の時間当たりの用量で静脈内注入により送達されることを特徴とする、請求項1〜4のいずれか一項に記載の組成物。
- 少なくとも0.5mg/kg/時の速度で注入により送達されることを特徴とする、請求項5に記載の組成物。
- 少なくとも0.71mg/kg/時の速度で注入により送達されることを特徴とする、請求項6に記載の組成物。
- 少なくとも0.8mg/kg/時の速度で注入により送達されることを特徴とする、請求項7に記載の組成物。
- 7.1mg/kg/時以下の速度で注入により送達されることを特徴とする、請求項1〜8のいずれか一項に記載の組成物。
- 治療の継続期間は1〜7日である、請求項1〜9のいずれか一項に記載の組成物。
- 損傷後、0時間〜1時間に投与されることを特徴とする、請求項1〜10のいずれか一項に記載の組成物。
- 損傷後、1時間〜120時間に投与されることを特徴とする、請求項1〜10のいずれか一項に記載の組成物。
- 1時間〜120時間の期間の間、注入により投与されることを特徴とする、請求項1〜12のいずれか一項に記載の組成物。
- プロゲステロンの一定の投与レジームにしたがって投与されることを特徴とする、請求項1〜13のいずれか一項に記載の組成物。
- 時間当たりの一定の合計注入投与レジームにしたがって投与されることを特徴とする、請求項14に記載の組成物。
- 1〜136時間の継続期間を有する一定の投与レジームを使用して、注入により投与されることを特徴とする、請求項1〜15のいずれか一項に記載の組成物。
- 前記治療は、複数の維持サイクルを含み、各維持サイクルは、完了した一定のプロゲステロン投与レジームを含み、該維持サイクルの間の継続期間は、1時間から1日の間である、請求項1〜16のいずれか一項に記載の組成物。
- エチルアルコールをさらに含むことを特徴とする、請求項1〜17のいずれか一項に記載の組成物。
