Description
Jeppesenら、Gastroenterology 2001;120:806-815.
調査対象は、PNを受けている患者を介護する米国とデンマークのセンターから募集された。全患者は、胃、十二指腸又は膵臓のいずれの外科的切除もされることなく、小腸の広範囲に及ぶ切除を受けていた。
これは、短腸症候群(SBS)を伴う患者におけるテドグルタイドの安全性と効果を測定するための非盲検な試験的研究である。前記患者の病歴を再検討し、また本調査包含前に身体検査を行って適格性を決定した。推定残存小腸及び大腸の長さは、手術報告書の見直し、また使用可能な放射線学的検査によって決定された。適任患者を18日間隔で分離した3時点、ベースライン期間の終りの3泊4日の間、並びに処理期間及びフォローアップ期間の終了時に病棟又は総合臨床研究センター(GCRC)に入院患者として収容した。治療薬は、L-ヒスチジン、マンニトール、リン酸1ナトリウム、及びリン酸2ナトリウムを含む凍結乾燥粉末(Lot 8502901)として調剤された組換えテドグルタイド(カナダ、オンタリオ、ミシソーガ、NPS Allelix社提供)から成る。腹部又は大腿部の皮下(s.c.)注射による投与のために水を加えて薬剤を再調製した。10人の空腸瘻造設を伴うSBS患者に0.03mg/kg/日、0.10mg/kg/日、又は0.15mg/kg/日で1日1回21日間投与した。5人の≧50%の大腸連続性を伴うSBS患者には、0.10mg/kg/日で1日1回21日間投与した。
内視鏡検査を、ベースライン期、処理期、及びフォローアップ期の完了時にそれぞれの被験者で行った。絨毛高及び腺窩深度測定用の全組織サンプルについては、同一病理学者が盲検方式で調製し、解析した。絨毛高及び腺窩深度については、十分に配向させた絨毛及びクリプト細胞10個の平均として光学顕微鏡(接眼レンズマイクロメータ)を用いて測定した。100クリプト上皮細胞あたりの有糸***像数を算出した。
SAS(バージョン8.2;SAS Institute社、ノースカロライナ、ケーリー)統計プログラムを使用して、スチューデント対応t検定をベースライン値に対する処理、及びベースライン値に対するフォローアップを比較するのに用いた。本研究では患者数が限られているため投与量の異なる、又は構造の異なる患者間では比較をしていない。データを平均±SD (標準偏差)で示した。p<0.05の値を有意とみなした。
(患者)
16人のSBS患者(女性9人、男性7人)を、本非盲検複数センター、投与量決定予備試験に登録し、テドグルタイドを投与した。米国施設4箇所で6人の患者が採用され、またデンマーク施設1箇所で10人の患者が採用された。処置されたこれら16人の患者のベースライン特性を表1に示す。該患者は、10人の末端空腸瘻造設グループ(グループ2)と5人の≧50%の大腸連続性グループ(グループ3)に細分された。当初11人の空腸瘻造設患者がいたが、患者03は生検で大腸の残存部を有することがわかり、後に30%あると判断された。本個体については、どのサブグループ解析にも含めなかった。断られた食物に合わせて調節した複製食の分析から判断されるように、一部の患者は他者よりもより良い食餌一貫性を有していた。治療において食物湿重量及びエネルギー含量が、ベースライン値と多くて10%の差である経口食物摂取を有する患者を「高食餌コンプライアンス」と呼んだ。末端空腸瘻造設を有する5人と≧50%の大腸連続性を伴う5人からなる10人が高食餌コンプライアンスであった(ベースライン値から<10%の変動性)(グループ4)。

連続した大腸部分を有することがわかっている空腸瘻造設患者03、及び大腸連続性を伴うことが知られている患者20は、フォローアップ期間を完了しなかった。服薬遵守(回収瓶の数をカウント)は、栄養バランス期間、参加した全患者で徹底された。15日間は総量で、2日間は半量で投与し、そして4日間は投薬なしであった大腸連続性を伴う患者の一人を除いて21日の治療期間中、投薬遵守は徹底された。
安全性について全患者をモニターした。死亡及び有害事象(AE)による中止はなかった。評価可能なデータを有する2人の患者が、投与後にカテーテル関連感染又はカテーテル関連合併症の重大な有害事象を報告した。該AEはテドグルタイドとは関連しないと判断された。AEの発生は、グループ間で類似していた。最も多く見られたAEは、下肢の浮腫、及び空腸瘻造設ニップルの限局性腫脹であった。他のAEとしては頭痛、腹痛があった。3人の患者に軽微な導入部位副次反応(痛み、硬化、発疹の単独症状、紅斑の4症状)が見られた。臨床的に有意な異常検査値は、テドグルタイド処理に関連するとは同定されなかった。バイタルサインやECG(心電図)から、安全性に対する懸念はみられなかった。テドグルタイドに対する抗体は、検出されなかった。患者の1人は処理後、1:1624の大腸菌タンパク質相対抗体力価を有し、6人の患者は1:107と1:228間の相対力価を有していた。
≧50%の大腸連続性を伴う患者について、内在性GLP-2の空腹時濃度をテドグルタイド投与前に測定した。5人の対象者のうち4人について該濃度は、健康なボランティアの空腹時平均レベルと摂食時平均レベル(空腹時:15±2pmol/L、摂食時:61±9pmol/L)の間にあった。濃度については、HartmannらによるPeptides 21 2000;73-80(本願に引用により援用する)に記載されたように測定した。5人の対象者の前記レベルは、16、27、37、41、及び73pmol/Lであった。
湿重量
図1は、グループ2及び3における個々の患者のベースライン時、処理期間、及びフォローアップ時の便湿重量、腸湿重量吸収、及び尿重量を図示している。テドグルタイドでの処理21日目に、各サブグループ(グループ2及び3;表2)において便湿重量がベースラインと比較して有意に減少した。前記減少は、末端空腸瘻造設を有する患者(グループ2、682±911g/日, p=0.040)、及び大腸連続性を伴う患者(グループ3、822±341g/日, P=0.006)において同様であった。絶対湿重量吸収は、食物湿重量から便湿重量を差し引くことで得られた。テドグルタイドは、15人のSBS患者全てで絶対湿重量吸収をベースラインと比較して増加した。これらの増加は、全グループで有意であり、また便***の減少を直接反映した。前記絶対湿重量吸収は、15人のSBS患者のうち13人で摂取のパーセントとして増加した。相対吸収における増加は、末端空腸瘻造設を有するSBS患者(グループ2、20±18%, p=0.007)、及び≧50%の大腸連続性を伴う該患者(グループ3、26±16%, p=0.023)で類似していた。腸湿重量吸収の増加から予想されたように、テドグルタイドは15人のSBS患者のうち13人で尿重量を増加した。同様の増加が、末端空腸瘻造設を有する該患者(グループ2、680±535g/日, p=0.003)、及び≧50%の大腸連続性を伴う該患者(グループ3、461±200g/日, p=0.007)において見られた。
図2は、個々の患者のベースライン時、処理期間、及びフォローアップ時の便エネルギー***、及び腸エネルギー吸収を図示している。ベースラインと比較すると、15人のSBS患者のうち12人に、テドグルタイドでエネルギー***における有意な減少が見られた。本減少は、末端空腸瘻造設を有する該患者(グループ2、532±1703kJ/日, p>0.05)よりも大腸連続性を伴う該患者(グループ3、1343±916kJ/日, p=0.031)、及び高食餌コンプライアンスの該患者(グループ4、1060±1083kJ/日, p=0.013)において一層明らかであった。有意な効果は、改善されたエネルギー吸収(グループ3、1027±798kJ/日, p=0.045、グループ4、963±1290kJ/日, p=0.043)、及び相対エネルギー吸収(グループ3、10±7%, p=0.030、グループ4、8±11kJ/d, p=0.040)でも見られた。
テドグルタイドに関する組織学的に有意な変化が、生検を行ったグループ2の患者8名中7名の空腸で見られた。すなわち、絨毛高(38±45%, p=0.030)、腺窩深度(22±18%, p=0.010)、及び***指数(115±108%, p=0.010)の増加があった。これらの結果は、観察された吸収の改善と一致する構造的変化を示していた。小腸の生検は、大腸連続性を伴う患者(グループ3)からは得なかった。しかし、大腸腺窩深度測定用の大腸生検を該患者から得た。腺窩深度は、テドグルタイド処理後、大腸生検5セット中4セットで増加したが、13±22%の平均増加は統計学的有意性に達しておらず(p=0.330)、また***指数における増加は有意ではなかった(76±112%, p=0.170)。テドグルタイドに関連する腸管吸収及び組織学での変化は殆どが、フォローアップ時に逆転した。
本研究における主な発見は、SBS患者の腸湿重量排出の減少と湿重量吸収の改善に対するGLP-2レセプターアゴニスト、テドグルタイド[Gly2]hGLP-2の注目すべき効果であった。これらの効果は、ほとんどで内在性GLP-2濃度が増加している大腸連続性を伴うSBS患者でも見られた。末端空腸瘻造設SBS患者10人では、有意に増加した絶対湿重量吸収(788±551g/日, p<0.01)及び有意に増加した相対湿重量吸収(20±18%, p<0.01)、並びに680±535g/日(p<0.01)の増加した尿生成及び682±911g/日(p<0.05)の減少した便湿重量***が見られた。同程度の効果が、≧50%の大腸連続性を伴うSBS患者で見られた。すなわち、有意に増加した絶対湿重量吸収(751±328g/日, p<0.001)及び増加した相対湿重量吸収(26±16%, p<0.05)、並びに461±200g/日(p<0.01)の増加した尿生成、並びに822±341g/日(p<0.01)の有意に減少した便湿重量***が見られた。これらの著しい効果は、従来の抗分泌薬及び止瀉薬で最適に処理した定常状態のSBS患者で見られた。
Claims (12)
- GLP-2レセプターアゴニストが大腸連続性を呈する短腸症候群患者による腸管吸収を増強するのに効果的な量で存在する、前記患者を治療するための薬剤調製におけるGLP-2レセプターアゴニストの使用であって、前記患者が、ヒト患者であり、かつ残存小腸と繋がった少なくとも50%の大腸を有し、前記GLP-2レセプターアゴニストが[Gly2]hGLP-2である、前記使用。
- 前記患者は、残存小腸と繋がった少なくとも75%の大腸を有する、請求項1に記載の使用。
- 前記患者は、摂食状態で少なくとも5pmol/Lの内在性GLP-2血漿中濃度を生じるのに十分な大腸を有する、請求項1に記載の使用。
- 前記患者は、摂食状態で少なくとも10pmol/Lの内在性GLP-2血漿中濃度を生じるのに十分な大腸を有する、請求項3に記載の使用。
- 前記残存小腸は、少なくとも25cmの長さを有する、請求項1〜4のいずれか1項に記載の使用。
- 前記残存小腸は、少なくとも50cmの長さを有する、請求項5に記載の使用。
- 前記腸管吸収の増強は、便湿重量の減少である、請求項1〜6のいずれか1項に記載の使用。

- 前記GLP-2レセプターアゴニストを腸管吸収の増強のために効果的な投与計画で使用する、請求項1〜8のいずれか1項に記載の使用。
- 前記投与計画は、前記GLP-2レセプターアゴニストの連日投与を含む、請求項9に記載の使用。
- 前記投与計画は、少なくとも21日の期間にわたる前記GLP-2レセプターアゴニストの投与を含む、請求項10に記載の使用。
