Description
しかしながら、尿素呼気検査に関して現在ゴールドスタンダードであるプロトコルでは、クエン酸溶液の摂取が必要とされる(特許文献1)。加えて、このプロトコルでは、残留尿素と口腔内に定着している微生物のウレアーゼ活性により偽陰性となる場合がある。尿素呼気検査でのクエン酸溶液の摂取は、この溶液によって胃内容の排出に遅れが生じることから、ある程度容認されている。しかしながら、胃内容排出の遅れはクエン酸溶液の摂取と関係ないことが判明している(非特許文献1)。更に、尿素呼気検査中にクエン酸溶液を摂取する患者は、検査前には制酸薬を飲むのを一時的にやめなくてはいけないことや、酸性のものを飲むと症状が一層悪化するとの無理からぬ考えから、胃十二指腸の症状の悪化を恐れてクエン酸溶液の摂取を嫌がる。更に、多量のクエン酸の摂取は、特に小児の場合、悪心や嘔吐を引き起こす場合がある。クエン酸溶液の量の多さや味の悪さも患者に不評である。更に、クエン酸アレルギーの患者は、現在のゴールドスタンダードである尿素呼気検査を受けることができない。
A.個体の第1基礎呼気試料を採取し、
B.第1基礎呼気試料を採取したらすぐに、個体に炭素で標識した尿素溶液を経口で与え、
C.炭素で標識した尿素溶液を与えたらすぐに、個体に十分な量の水を経口で与え、
D.十分な量の水を与えてから所定の時間が経過した後に、第2呼気試料を採取し、
E.第1呼気試料及び第2呼気試料内に存在する炭素標識の量を測定及び分析することを含み、
検出または診断対象とする上記の病原微生物にヘリコバクター・ピロリ菌を用いることを特徴とする。
(a)水を入れた容器(1)(図1)と、
(b)炭素標識尿素を入れた容器(2)と、
(c)呼気検査開始時の炭素標識尿素を摂取する前に第1呼気試料を採取するための第1容器(3)と、
(d)呼気検査の最後の十分な量の水を摂取した後に第2呼気試料を採取するための第2容器(4)とを備え、
第1容器及び第2容器は、針状のセンサの導入が可能な機構を備えたキャップ(3A及び4A)を有し、検出または診断対象の病原微生物にヘリコバクター・ピロリ菌を用いることを特徴とする。
患者:調査集団は、胃腸に症候が見られない70人の健康な志願者から構成された。ヘルシンキ宣言及びコロンビア保健省の1993年の決議8430に従って、研究は、生物学的、生理学的、心理学的または社会的リスクがない研究と分類される。
プロトコル1:少なくとも8時間の絶食後、第1基礎呼気試料を採取した(t0 )。その直後に、患者に、15mlの水に溶解させた50mgの13C標識尿素を1杯の飲料として経口投与した。その直後に、患者に、200mlの水を経口投与した。次に、10分後(t10)、20分後(t20)、30分後(t30)に、呼気試料を採取した。

Claims (5)
- 呼気検査に基づき病原微生物を検出する方法であって、
A.個体の第1基礎呼気試料を採取し、
B.第1基礎呼気試料を採取したらすぐに、個体に炭素で標識した尿素溶液を経口で与え、
C.炭素で標識した尿素溶液を与えたらすぐに、個体に十分な量の水を経口で与え、
D.水を与えてから所定の時間が経過した後に、第2呼気試料を採取し、
E.第1呼気試料及び第2呼気試料内に存在する炭素標識の量を測定及び分析することを含む
ことを特徴とする病原微生物の検出方法。 - 十分な量の水が100〜250mlである
請求項1に記載の方法。 - 炭素で標識した尿素溶液が15〜50mlであり、炭素で標識した尿素が15〜50mgであり、尿素を標識する炭素が炭素13である
請求項1に記載の方法。 - 第2呼気試料が、個体に十分な量の水を与えてから4分59秒を越える時間が経過してから採取されるものである
請求項1に記載の方法。 - 病原微生物がヘリコバクター・ピロリ菌である
