Description
臨床医は、内腔壁からのもろい材料がステントの内部に侵入し、細胞増殖により内腔を狭める結果となる可能性、或いは損傷又は虚血を引き起こす下流への塞栓を生じさせる可能性を最小限にするために、比較的小さな開口部を備えたステントの使用を推奨している。Palmaz他に付与された米国特許第6,537,310号では、最小寸法が17ミクロン以下の開口部を有する多孔性薄膜でステントを覆い、ステントの内腔への塞栓性破片及びプラークの移動を最小限にするのが有利であることを教示している。しかしながら、Palmazは、被覆薄膜又は「ウェブ」に極めて小さな開口部が多数あるので、製造が極めて困難なステントの使用を教示している。
Mathis他(以下「Mathis」)に付与された米国特許第6,129,755号では、斜め長手方向ブリッジによって接合されたストラットの円周フープを備えた自己拡張ステントの改良を教示している。この明細書では、超弾性材料内の歪みを最小限にするため、及び塞栓がステントの壁を透過するのを防ぐために、フープ当たりのストラット数(円周周りで数えたストラットの数)が多数あり、且つ最小のストラット長を有することが重要であると記載されている。Mathisは、長手方向に測定したストラットの長さ(インチ単位)に対する円周周りのストラット数の比率である良度指数を定義している。逆インチ単位を有するこの比率は、発明者らがMathisとDuerigであったので、本明細書ではM−D比と呼ぶことにする。Mathisは、従来技術のステントが約200の比率を有し、彼らの改良ステントは400を超えるM−D比を有すると記載している。「スマートステント」と呼ばれるMathis−Duerig発明による、Cordis Corporationによって製造された代表的なステントは、円周当たりに32ストラット及び約0.077インチのストラット長を有し、結果として約416のM−D比が得られる。
本発明の特性としてみなされる他の特徴は、添付の請求項に記載されている。
ステント上にX線不透過性マーカを備え、X線を使用してマーカの配置及び展開を助けることによって該マーカを容易に見ることができるようにするのが一般的である。本発明は、これらのマーカを、特にストラットの螺旋状パターンの端部を越えて配置する好都合な区域を提供する。マーカ12がパドル形(すなわちほぼディスク様の拡大部分を有し、これをステントの構造体に接合する狭いエクステンションを備えている)である場合、マーカは、180度撓みセグメント7の端部に(或いは撓みセグメント7又は直線部分6上の他の場所に)取り付けることができる。短いエクステンションを有するパドルが螺旋の端部近く(螺旋状パターンの末端部)に位置し、より長いコネクタ上のパドルが円周部の周りの他の場所に位置するようにマーカを配置するのが有利である。このような構成では、パドルの末端部は平らであり、ステント1に比較的平面の端部を提供する。しかしながら、マーカ部分12は、パドル形である必要はない。マーカは、ステント1の遠位及び近位端2、3のいずれか又は両方から離れて延びるように単にロッド状とすることができる。これらのロッドは、血管内でより良好に固定させるために拡張することができ、パドル形マーカに比べて表面積がより小さい場合でも、依然としてより良好なイメージングを可能にするインジケータを受け取るのに十分な区域を提供することができる。
本発明によれば、ストラット4の隣接転回部又はカラムを連結して望ましいステント全体の可撓性及び構造一体性を提供する連結ブリッジ8のアレイが存在する。図7に示すようにこれらのブリッジ8をほぼ円周方向で形成するのが有利である。2つの有利な特徴は、連結ブリッジ8をこのように形成することによって明らかになる。第1に、ブリッジ8によって生じた垂直方向(円周方向)のオフセットは、拡張後に隣接180度撓みセグメント(拡張ストラットペアの頂点)が互いにオフセットされることで交互に噛合することを確保し、ステント1が容易に撓むことができるようにする。第2に、これらの円周方向ブリッジ8は、ステント1の軸10に垂直な平面内で鋭く湾曲され、その曲率は、小径管体から形成されているステント1により生じる。拡張プロセスを綿密に制御することによって、これらのブリッジ8の曲率のほぼ全てを保持しながらステント1を拡張することができる。その後、結果として得られる拡張ステント1においては、これらのブリッジ8は、ステント1の円筒状表面から離れて半径方向に延び、ステント1の軸10に垂直な縁部となる。従って、移植中及び移植後に、これらの特徴部は、血管又は体腔壁を係合し、ステント1の移動を防ぐ。図8のブリッジ8の拡大は、このようにしてこれらの構造体がステント1の壁を越えて突出している様子を示している。
通常は、X線による可視化を可能にするために、金、タンタル、酸化ジルコニウム、バリウム及びビスマス塩、ハフニウム、モリブデン、その他のようなX線不透過性材料がステントに取り付けられる。本発明は、上述のように特にパドル12、13の位置でこのようなマーカを組み込むのに好適である。
機械加工による管状金属ステントを製造するための標準的な方法は、通常、バルーン拡張ステント用としてはステンレス鋼、プラチナ合金、又はクロミウム−コバルト合金、自己拡張ステント用としてはニッケル−チタニウム合金の小径金属管体から始める。この管体は、レーザー機械加工システムに取り付けられ、該システムが固定軸の周りの部分を回転させてレーザビームの焦点が管体の表面に衝突するようにする。レーザー出力が、気体(空気、酸素、又はアルゴン等の不活性ガスのいずれか)の同軸ジェットと共に付加されると、材料は、レーザーエネルギーによって穿孔される(場合によっては、空気又は酸素との化学反応によって助けることもできる)。管体は、回転と長手方向の少なくとも2つの軸でレーザビームを受けて移動され、レーザーエネルギーが加えられている間に連続的な切断(又は切り口)が行われるようになる。レーザビームは、長手方向及び回転方向の運動と協働してコンピュータ制御によりオンオフされ、切断の不連続なパターンが管体に施工されるようにする。
本発明で記載されているように微細構造を有する自己拡張ステントで克服すべき1つの製造上の問題は、最終の拡張ステントに対して切断したままの管体の熱機械的拡張中に生じる不均一な開口部である。標準製造プロセスは、次第に大きくなるテーパ付き端部円筒状マンドレル全体にわたってレーザー切断ステントを延伸する段階と、これらのマンドレルによって支持される間に幾つかのステージでマンドレルを熱処理する段階とを含む。ステントは、低温(柔らかい、マルテンサイト状態で)又は大気温度(弾力のある、オーステナイト状態で)のいずれかで、連続拡張マンドレル上でステントを延伸することによって拡張することができる。ステントは、マンドレル上で拡張されると、典型的には摂氏450度から摂氏500度の範囲内の高温で短時間(数秒から数分)の間露出され、拡張の当該レベルでステントを「形状設定」又はアニールする。
本発明は、この局部過度歪みを防止するためのプロセスを提供する。本発明では、未完成ステント内のストラットペアの隣接カラム(又は螺旋状転回部)の間に当初から存在するブリッジ8、9の最初の数に比較すると、わずか数ブリッジ8だけが完成ステント内に存在し、残りのブリッジ8は望ましい可撓性及び疲労に対する耐性を与える。切断したままの状態及び拡張段階中に、付加的犠牲ブリッジ9は、隣接転回部又はカラム内のストラットペアを接合する撓みセグメントを連結する。従って、本発明のステント1が拡張されると、ステントは堅牢性が大きく向上し、ストラットの各ペアは、最大数のポイントで拡張ステントの隣接部分に対して連結される。本明細書で犠牲ブリッジ9と呼ばれるものにより、これらブリッジの付加的連結が可能になり、ステントの全要素によって拡張歪みが遙かに均一に共有され、この共有により拡張中に歪みの均一性が有意に増大することになる。その結果、ばらつきが遙かに少ない頂点開口角度を有する拡張ステントになる。
犠牲ブリッジ9の除去を容易にするために、切断したままの構造内に特別な特徴部を設計し、犠牲ブリッジ9を切断又は破壊するための規定の場所を設けることができる。これらの特徴部は、例えば、隣接転回部4のストラットに連結された犠牲ブリッジ9の端部の1つ又は両方において形成されるノッチ14として図9に示される。ノッチ14を設けることは、切断/破壊場所を形成するための1つの実施例に過ぎず、犠牲ブリッジを除去する代替の例示的な方法は、化学エッチング、ブラスト、研削、電気化学エッチング又はポリシング、剪断、或いはレーザー切断を含む。
通常は、ステントは、ブラスト、ガラスビーズホーニング、化学エッチング、機械的ポリシング、及び電気化学ポリシングの組合せによって仕上げられる。これらのプロセスの全ては、犠牲ブリッジ9の除去によって残されたあらゆる残りのバリの除去に役立つ。加えて、研削、剪断、機械的ポリシング、及び切断等の他の手段を用いて、犠牲ブリッジ9によって残されたバリを局部的に平滑にして除去することができる。
本発明によるステント1の製造、特に、レーザー切断及び拡張段階は、ストラット5のサイズが減少し、ストラット5の数が増大すると大幅に困難になることが分かった。例えば、通常のレーザー切断プロセスは、約25ミクロンから40ミクロンの仕上げ切り口幅を生じる(所望のポリシング仕上げを有するステントを提供するのに必要な材料除去プロセス後)ことが分かっている。例えば、全46ストラットが円周部の周りに配置されている場合、切り口の全円周幅は、少なくとも46×25ミクロン、すなわち1150ミクロン(1.15ミリメートル)になる。この切り口スペースについて、切り口の半分はストラットの端部を接合する180度撓み部の内側にあるので、半分はステントの圧縮中に折り畳めない。従って、従来の製造プロセスによって作られた現在の構成のステントは、180度撓み部における切り口から生じる少なくとも0.57ミリメートルの非圧縮性円周部(0.18ミリメートルの直径減少に相当)を有する。しかしながら、本発明に従って全切り口を25ミクロンから18ミクロンまで減少させることによって、圧縮後の直径は、完全に折り畳まれた直径での有意差である0.05ミリメートルだけ減少する。更に切り口を従来の25ミクロンから18ミクロンに減少させることによって、別の利点が得られ、すなわち、より少ない金属が除去されることに起因して、残りのストラット幅が増大する。本実施例では、2ミリメートルの事前切断管径及び46ストラットを想定すると、全切り口の減少を25ミクロンから18ミクロンに低減し、結果として得られるストラット幅は112ミクロンから119ミクロンに増大し、剛性は幅の三乗に比例するので(119/112)3すなわち120%の相対剛性が得られることになる。
拡張ステントの壁を通過できる最大塞栓サイズは、直線部分6と撓みセグメント7との間の開口部のサイズによって決定付けられる。より正確には、最大塞栓サイズは、その開口構成内の特定のステントの開口部内に内接することができる最大円で記述される。従って、患者の塞栓の悪影響を防ぐために、最大塞栓サイズを最小にするのが望ましい。
本発明に従って形成されたステント1は、従来技術によって形成されたステントと比べると、比較的多数の特徴部を有し、更に、局部可撓性をもたらす直線部分6及び180度ループ5、並びに構造全体に対する構造一体性をもたらすストラット4の隣接転回部又はカラムを接合するブリッジ8を含む、多数のこうした特徴部があるので、実質的に少数の特徴部を有する従来技術のステントよりも、はるかに微細な範囲まで可撓性及び圧縮/拡張特性を微調整することが可能である。例えば、Cordis 8mm×50mm SmartStentのような同じサイズの典型的な従来技術のステントは、約700ストラットを有する。これに比べて、例えば本発明による8ミリメートル直径で50ミリメートル長さのステント1は、100%増加を超える約1500ストラットを有する。
同様に、より短いストラット5が螺旋ピッチの減少をもたらすので、ストラット5の長さが短いことは、ステントの所定の円周に対してより大きな螺旋角度(又は、長手方向軸に対して垂直方向により近接した螺旋軸)をもたらす。こうした螺旋角度の増大には幾つかの利点がある。まず、螺旋の端部が前の転回を接合する段階がより小さい(ストラット長にほぼ等しい)ので、ステントの遠位及び近位端の不均一性が軽減される。このような低減段階によって、より容易な手法でほぼスクエアの切断端部(通常医師によって所望される)を備えるステント1が提供される。
本発明の多数のストラット5及びブリッジ8、9は、使用中に多数の特徴部に対してもたらされる局部的な力及び偏位の拡散を生じ、その結果、これらの局部変形が、多数の変形要素全体にわたって広がる。結果として、各要素は比例的に変形が少ない。1500ストラットを備えたステントは、半分の数のストラットを備えたステントよりも、ステントが置かれる血管又は他の体腔に対する局部荷重及び変形の低減に伴って、より容易に変形並びに屈曲及び捻れを吸収することになる点は理解することができる。
捻り可撓性及び疲労抵抗が屈曲要素の数を増大することによって改良される同じ方法で、屈曲(又は撓み)可撓性及び疲労抵抗も改良される。ステント1の撓みにより、ストラット4の隣接転回部又はカラムは、互いに向かって(撓みの内側に)又は離れて広がって(撓みの外側に)押し出される。連結ブリッジ8は、隣接転回部又はカラム4を接合するので、ステント撓みによって引き起こされる局部変形は、連結ブリッジ8間のストラット5及び撓みセグメント7(ストラットの端部を接合する180度ループ)全体にわたって広がる。従って、連結ブリッジ8間にある要素(ストラット5及び撓みセグメント7)が多くなるほど、ステント撓みによって引き起こされる変形を吸収する要素の数が多くなる。同様に、より短いストラット5を有する構成においては、ステント1を撓ませることによって影響を受けるより多くの転回部又はカラム4が存在し、そのため、ステント1の撓みによって変形される要素の総数が更に増加し、結果として要素の各々に対し遙かに小さな変形を生じさせる。変形が小さくなり、ストラット幅が減少すると、ステント材料内の有効歪みが大幅に軽減され、結果として疲労抵抗が大きく改善されることになる。
本発明の教示に従って形成されたステント1のより短い長さの多数のストラット5は、より大きな表面積を有する。例えば、本発明の教示によるステント1は、円周周りに半数のストラットを有する他の同様の従来技術のステントよりも、2倍を超える切り口長さを有することになる。自己拡張ステントでは、切り口面積(ステント要素の切断半径面の面積)は、内面及び外面の面積がストラットの高アスペクト比(厚さ対幅)に起因して比較的小さいので、全表面積に寄与する主因となる。従って、本発明の教示により形成されたステント1の全表面積は、従来技術の構成に従って形成されたステントの全表面積よりも大幅に大きく、すなわちその全表面積は、薬用コーティングを塗布するための大きな表面積もたらす。この大きな表面積によって、実質的にステントのカバレージ領域内の全組織が薬剤溶出領域内にあるようにすることができる。詳細には、ステントは、壁組織のどの要素も最も近いストラットから350ミクロンから400ミクロンよりも離れないような組織カバレージを提供する。このような構成によって、組織のあらゆる部分に対して薬剤溶出剤で覆われたストラットからの短い拡散経路が保証される。

Claims (35)
- 円周部と、
転回部において前記円周部の周りに螺旋状に配置され、少なくとも2つがそれぞれストラット端部を有するストラットと、
前記ストラット端部のそれぞれから離れて延びる少なくとも2つのパドル形マーカと、
を有するステント本体を備え、
前記マーカがそれぞれ、マーカ末端部と、前記ステント本体の単一の円周部にほぼ沿って前記マーカ末端部を実質的に整列させる異なる長手方向全長とを有することを特徴とするステント。 - 前記ステント本体が長手方向軸を有し、
前記単一の円周部が、前記軸に対してほぼ直角である、
ことを特徴とする請求項1に記載のステント。 - 前記ステント本体が長手方向軸を有し、
前記マーカが、前記長手方向軸に対してほぼ平行な前記ストラット端部のそれぞれから離れて延びる、
ことを特徴とする請求項1に記載のステント。 - 前記マーカが第1のイメージング特性を備えた本体部を有し、前記本体部が前記第1のイメージング特性とは異なる第2のイメージング特性を備えた少なくとも1つの部分を有し、
前記第1及び第2のイメージング特性が、超音波イメージング特性、蛍光透視イメージング特性、X線イメージング特性、及び磁気共鳴イメージング特性からなるグループから選択されることを特徴とする請求項1に記載のステント。 - 前記部分が、凹部、孔、陥凹部、ノッチ、スロット、シリンダ、コーティング、充填剤、球体、テクスチャ、穿孔、前記マーカに取り付ける二次材料、及び粒子からなるグループの少なくとも1つから選択された構造体であることを特徴とする請求項4に記載のステント。
- 前記マーカ末端部が、円周方向にほぼ平坦であり、展開カテーテルを受けるように成形されていることを特徴とする請求項1に記載のステント。
- 前記ストラットが、前記螺旋の近位端に1つの開始点と、前記螺旋の遠位端に1つの端部とを有する単一の螺旋に配置されることを特徴とする請求項1に記載のステント。
- 前記ステント本体が、近位端と遠位端とを有し、
前記ストラットが、前記近位端に1つの開始点と前記遠位端に1つの端部とを各々が有する少なくとも2つの螺旋を備えた複数螺旋で配置されることを特徴とする請求項1に記載のステント。 - 前記複数螺旋が、前記近位端に1つの開始点と前記遠位端に1つの端部とを各々が有する4つの螺旋を備えることを特徴とする請求項8に記載のステント。
- 前記ストラットがS字型ストラットであり、前記螺旋が前記螺旋の長さ全体にわたって前記S字型ストラットの連続反復を有することを特徴とする請求項7に記載のステント。
- 前記ストラットがS字型ストラットであり、
前記螺旋の各々が、前記螺旋の長さ全体にわたって前記S字型ストラットの連続反復を有することを特徴とする請求項8に記載のステント。 - 前記ストラットが湾曲部分を有し、前記マーカが前記湾曲部分から離れて延びることを特徴とする請求項1に記載のステント。
- 前記マーカが、X線不透過性マーカであることを特徴とする請求項1に記載のステント。
- 前記X線不透過性マーカが、タングステン、タンタル、モリブデン、プラチナ、金、酸化ジルコニウム、バリウム塩、ビスマス塩、ハフニウム、及び次炭酸ビスマスからなるグループの少なくとも1つから選択された材料からなることを特徴とする請求項13に記載のステント。
- 前記マーカが、超音波マーカであることを特徴とする請求項1に記載のステント。
- 前記超音波マーカが、様々な音波特性を有する摩擦、孔、空隙、多孔性材料、多孔性コーティング、中空バルーン、及び層状材料からなるグループの少なくとも1つから選択された特徴部であることを特徴とする請求項15に記載のステント。
- 前記超音波マーカが、エポキシマトリックス中に懸濁されたガラスマイクロバルーン及びタングステン粉体の複合材料で充填された直径0.50ミリメートルの孔であることを特徴とする請求項15に記載のステント。
- 前記マーカが、磁気共鳴イメージングマーカであるであることを特徴とする請求項1に記載のステント。
- 前記磁気共鳴イメージングマーカが、常磁性、反磁性、及び強磁性からなるグループの少なくとも1つから選択された材料からなることを特徴とする請求項18に記載のステント。
- 前記磁気共鳴イメージングマーカが、ガドリニウム、ガドリニウム塩、ガドリニウム箔、ガドリニウム粉体、ヘマタイト、酸化物、ナノ結晶酸化鉄、及び鉄粉からなるグループの少なくとも1つからなることを特徴とする請求項18に記載のステント。
- 前記マーカが、超音波マーカであることを特徴とする請求項1に記載のステント。
- 前記超音波マーカが、ガラス又はセラミックマイクロバルーンからなることを特徴とする請求項21に記載のステント。
- 前記マーカが、X線不透過性、超音波、及び磁気共鳴イメージングマーカの組合せであることを特徴とする請求項1に記載のステント。
- 前記マーカの1つが、前記螺旋状転回部の端部に隣接する比較的短い拡張部分を有し、前記マーカの他のものが、前記比較的短い拡張部分から離れる方向に前記螺旋状転回部に沿ってサイズが増大する比較的大きな拡張部分を有することを特徴とする請求項1に記載のステント。
- 前記マーカが、整列端部を備えたパドル部分を有することを特徴とする請求項24に記載のステント。
- 前記ステント本体が長手方向軸を有し、
前記マーカが、前記長手方向軸に対してほぼ直角な円周平面を形成する前記拡張部分と反対側に端部を備えたパドル部分を有する、
ことを特徴とする請求項24に記載のステント。 - 前記拡張部分が、前記ステント本体の円周方向の所与の幅を有し、前記パドル部分が、前記所与の幅よりも大きな前記円周方向の幅を有することを特徴とする請求項24に記載のステント。
- 前記拡張部分が、前記ステント本体の円周方向の所与の幅を有し、前記パドル部分が、前記所与の幅に等しい前記円周方向の幅を有することを特徴とする請求項24に記載のステント。
- 前記パドル部分が、非円形であることを特徴とする請求項25に記載のステント。
- 前記マーカの隣接部分の前記末端部が、18ミクロンよりも大きくない距離だけ離隔されることを特徴とする請求項1に記載のステント。
- 前記ストラットが、低減状態と、第1の円周部を備えた外側円周シリンダを形成する第1の拡張状態とを有し、
前記ストラットの少なくとも2つが、それぞれストラット端部を有し、
前記パドル形マーカの少なくとも2つが、前記ストラット端部のそれぞれから離れて延びており、それぞれのマーカ端部を有し、
前記マーカは、前記マーカ末端部によって形成される第2の円周部が前記第1の円周部よりも大きい第2の拡張状態を有する、
ことを特徴とする請求項1に記載のステント。 - 円周部と、
転回部において前記円周部の周りに螺旋状に配置され、少なくとも2つがそれぞれストラット端部を有するストラットと、
前記ストラット端部のそれぞれから離れて延びる少なくとも2つのパドル形マーカと、
を有するステント本体を備え、
前記マーカがそれぞれ、円周方向に平坦な末端部と、前記ステント本体の単一の円周部にほぼ沿って前記マーカの平坦な末端部を実質的に整列させる異なる長手方向全長とを有することを特徴とするステント。 - 前記ステント本体が、長手方向軸を有し、
前記単一の円周部が、前記軸に対してほぼ直角である、
ことを特徴とする請求項32に記載のステント。 - 前記ステント本体が、長手方向軸を有し、
前記単一の円周部が、前記軸に対してある角度をなしている、

転回部において前記円周部の周りに螺旋状に配置され、低減状態と、第1の円周部を備えた外側円周シリンダを形成する第1の拡張状態とを有し、少なくとも2つがそれぞれストラット端部を有するストラットと、
前記ストラット端部のそれぞれから離れて延び、それぞれマーカ末端部を有する少なくとも2つのパドル形マーカと、
を有するステント本体を備え、
前記マーカが前記マーカ末端部によって形成される第2の円周部が前記第1の円周部よりも大きい第2の拡張状態を有し、
前記マーカは、前記ステント本体の単一の円周部にほぼ沿って前記マーカ末端部を実質的に整列させる異なる長手方向全長を有することを特徴とするステント。
