Description
本発明は、体の管腔の開存性の修復に使用されるステントグラフトに関し、特に、このようなステントグラフトの接続構造に関する。
血管などの人間または動物の体内の管が、疾病または外傷による損傷を受けた場合、血管の損傷を受けた領域の開存性を修復する管腔内技術によって、血管にステントグラフトを挿入することが可能である。このような損傷領域は、血管枝を含むことが多い。血流を確実に血管枝内に流すことができるようにするために、二股状または側枝ステントグラフトが使用される。
米国特許第6,015,431号は、管状部材と、管状部材の外面に固定された封止部材とを有する、植え込み可能な医療装置を開示している。管状部材は管腔内壁と係合するように拡張可能であり、この壁によって定められる管腔内空間を通した流れのための人工導管を与える管腔を有する。封止部材は、管状部材の周りにおいて外面と管腔内壁との間の流れを閉塞する。この発明で使用するのに適した1つの管状部材として、ステントグラフトがあり、これは、管内の圧力および流れから、管の分岐部に隣接する管壁の奇形を保護するために、二股状でもよい。封止部材は、流れに抗する1方向バルブとして、管状部材の一端の外面上において方向付けられたフランジを含んでいてもよい。複数の封止部材を、管状部材の対向する端部で、または、管状部材の一端で直列状態で用いてもよい。管状部材を管腔内壁に固定するのにアンカーを使用する場合、封止部材は、アンカーに隣接する管壁の変形の局所領域で形成される漏れ経路を通る流れからの保護を提供する。1つまたは2つ以上の封止部材を用いても良く、この場合、2つ以上の管状部材と共に用いて、並列する複数の管状部材間に隙間があるために流れに対して開放されたままである管腔内空間を通した流れを防止する。
国際特許公開公報第01/82833号は、患者の管内に植え込むためのステントグラフトを開示している。この発明は2つの局面を有する。第1の局面は、拡張可能な金属スケルトン構造で形成され、ステントを延在する高分子スリーブの形態のグラフトを有するとともに、グラフトの各端部を対応するステント端部領域に固定する拡張可能セキュリティリングを有する、ステントグラフトを提供する。第2の局面は、複数の別々の拡張可能金属リングおよび可撓性バンドを囲む内スリーブおよび外スリーブを提供する。
好ましくは、第1および第2のリングは、形状記憶ワイヤ、ステンレス鋼またはプラスチック材料から選択された材料を含む。

または、大動脈分岐内に配置するようにした二股状ステントグラフトの脚部もしくは単腸骨動脈(aorta-uni iliac)ステントグラフトの末端用でもよい。
ワイヤを少なくとも2回、好ましくは3回巻いたもので形成され、このニチノールワイヤの端部はループ81で終端をなす。ループ81を使用することにより、ニチノールワイヤのとがった端部が、ステントグラフトが中に配置される脈管構造の中に食い込むことを防止する。ループ81によって、リングを形成するワイヤの端部を、グラフトの織物に縫い付けることができ、これにより、使用中にリングが拡張することを防止する助けにもなる。第1のリング79と第2のリング80との間には、弾性材料から形成されたステント82がある。弾性ステント82は、たとえばニチノールワイヤから形成され、静止時に直径がサイドアーム74の直径よりもわずかに小さい大きさになるように作られ、したがって、サイドアーム74の外側に縫い付けられたときにはサイドアーム74に対し直径を減じるという効果を与える。

Claims (10)
- 生体適合性材料からなる管状体(10、30;72、91)と、ステント(46)を受けるための接続ソケット(12;32;76;102)とを含むステントグラフトアセンブリであって、前記ソケットは、その一端の、前記管状体の周りの第1の弾性リング(14;34;79;113)と、前記第1のリングから前記管状体に沿って間隔を置いて設けられた、前記管状体の周りの第2の弾性リング(16、36;80;114)とを含み、前記第1および第2のリングは、ワイヤを少なくとも2回巻いたものを含み、ワイヤの各端部はループ(18、38)で終端をなし、拡張制限構造を与える、ステントグラフトアセンブリ。
- 前記第1の弾性リング(14;34;79;113)および前記第2の弾性リング(16;36;80;112)の直径は、前記管状体(10;30;72;91)の直径より小さい、請求項1に記載のステントグラフトアセンブリ。
