【発明の属する技術分野】
本発明は、超音波診断装置および超音計測方法に関し、特に生体内の組織の性状診断を行うための超音波診断装置および超音計測方法に関する。

【従来の技術】
近年、心筋梗塞や脳梗塞などの循環器系疾病を患う国民が増加してきており、このような疾病の予防および治療を行うことが大きな課題となっている。

心筋梗塞や脳梗塞の発病には、動脈の硬化が深く関係している。具体的には、動脈壁に粥腫が形成されたり、高血圧等の種々の要因によって動脈の新しい細胞が作られなくなったりすると、動脈は弾力性を失い、硬く、脆くなる。そして、粥腫が形成された部分において血管が閉塞したり、粥腫を覆う血管組織が破裂することにより粥腫が血管内へ流出し、別の部分において動脈を閉塞させたり、動脈が硬化した部分が破裂したりすることによって、これらの疾病が引き起こされる。このため、動脈硬化を診断することがこれらの疾病予防や治療には重要となる。

従来より、動脈が硬化しているかどうかは、血管カテーテルを用いて血管内部の様子を直接観察することによって診断されていた。しかし、この診断には、血管カテーテルを血管に挿入する必要があるため、患者への負担が大きいという問題があった。このため、血管カテーテルによる観察は、動脈が硬化していることが確かである患者に対して、その場所を特定するために用いられ、例えば、健康管理のための検査として、この方法が用いられることはなかった。

動脈硬化の一因であるコレステロール値を測定したり、血圧値を測定したりすることは、患者への負担が少なく、容易に行うことのできる検査である。しかし、これらの値は、動脈が硬化している度合いを直接示すものではない。

また、動脈硬化の治療薬を動脈の硬化が余り進行していない患者に対して投与することができれば、動脈硬化の治療に効果を発揮する。しかし、動脈硬化が進行してしまうと、治療薬によって動脈の硬化を抑制することはできても、硬化した動脈を完全に回復させることは難しいと言われている。

こうした理由から、患者への負担が少なく、動脈の硬化が進行する前に動脈の硬化の度合いを診断する診断方法あるいは診断装置が求められている。

一方、患者への負担が少ない医療診断装置として、超音波診断装置が従来より用いられている。超音波診断装置を用いて超音波を体外から照射することによって、患者に苦痛を与えることなく、体内の形状情報、運動情報あるいは質情報を得ることができる。

特に超音波による計測を行えば、測定対象物の運動情報が得られるため、変位量から、測定対象物の弾性率を求めることができる。つまり、生体内の血管の弾性率を求めることができ、動脈の硬化の度合いを直接知ることが可能となる。また、患者に超音波プローブをあてるだけで測定できるため、患者への負担も少ない。このため、超音波診断装置を用いれば、動脈硬化の正確な診断も可能であるし、予防のための検診を被験者に対して負担を与える場合がなく行うことが期待される。

しかし、従来より用いられている超音波診断装置は、例えば胎児の形状を観察したり、胎児の心音を聴診したりする超音波診断装置に代表されるように、形状情報や運動情報の分解能はそれほど高くない。このため、従来の超音波診断装置を用いて、心拍に合わせて収縮する動脈の弾性率を求めることは不可能であった。例えば、特許文献1に示されているもののように、測定対象の変位量計測が十分ではないものが多かった。

近年、エレクトロニクス技術の進歩によって、超音波診断装置の測定精度を飛躍的に向上させることも可能になってきた。これに伴って、生体組織の微小運動を計測する超音波診断装置の開発が進んでいる。例えば、特許文献2は、検波信号の振幅および位相の両方を用い、制約付最小二乗法によって対象の瞬時の位置を決定し、高精度な位相のトラッキングを実現する超音波振動装置を開示している。この装置は、拍動により大きく動いている組織上の微小振動を計測することができる。この公報によれば、振幅が10mm以上ある拍動に伴う大振幅変位運動上の数百Hzまでの微小振動を拍動が10回程度繰り返されても十分再現よく計測することができる。

この公報の装置は、数百Hzまでの高い周波数成分を再現性よく計測でき、超音波ビームを収束させることによって心筋や動脈壁上の直径1〜2mm程度の領域の弾性特性を得ることができる。また、一心拍中、あらゆる時相の成分の超音波信号が得られ、その信号の周波数スペクトル解析が可能である等の優れた特徴を備えていると報告されている。

したがって、この公報の技術を用いた超音診断装置によれば、たとえば、健康診断などにおいて、被験者に負担を与えることなく、経時的に動脈硬化の進行度合いを調べ、動脈硬化による疾病を予防することが可能となると期待される。また、動脈の微小領域における弾性特性を測定することによって、血管破裂が生じ易い部位を特定し、その部位を治療することが可能になると期待される。

【発明が解決しようとする課題】
超音波診断装置による画像には、しばしば、スペックルと呼ばれる干渉波による特有の模様が現れることが知られている。スペックルは、使用する超音波の波長以下の大きさを有し、ランダムに配置された反射体や散乱体からの反射波が重なり合って発生する現象であり、観測している対象の形状などの情報とは無関係な情報を含む。スペックルが見られる場合、反射波の振幅が、対象の実態とは無関係に小さくなったり、大きくなったりする。超音波エコー画像では、スペックルによる模様が、包絡線の振幅が大きい箇所において明るく、振幅の小さい箇所において暗く表示される。

スペックルは、従来より用いられている超音波診断装置においても発生していた。この場合、スペックルの模様と同程度以下の分解能で対象物の形状情報や運動情報を得ることは困難となる。しかし、従来の超音波診断装置では、スペックルの影響が問題となるような高い分解能が必要とされることは少なかったため、実用上、スペックルの影響は問題とならないことが多かった。

これに対して、特許文献2の技術を用いた超音波診断装置は、特に運動速度をサブミクロンのオーダで測定することを目的としているため、スペックルによる影響は大きな問題となる。具体的には、観測している対象から得られるべき必要な情報が正確に得られなかったり、反射波の位相が不連続に変化してしまい、微小運動の計測を正確に行うことができないという問題が生じる。

本発明はこのような問題を解決し、スペックルによる影響を少なくし、測定対象物の各部の運動速度や各微小な領域の伸縮量や弾性率を計測することのできる超音波診断装置および超音波計測方法を提供する。

【課題を解決するための手段】
本発明の超音波診断装置は、超音波プローブから測定対象物へ超音波送信波を送信するための駆動パルス信号を出力する超音波プローブ駆動部と、前記測定対象物からの超音波反射波を受信する受信部と、前記超音波送信波および前記超音波反射波の超音波ビームの音響線の方向を制御するための遅延時間制御部と、前記超音波反射波を位相検波する位相検波部と、前記測定対象物の前記音響線上において順番に配列している複数の測定対象位置P1、P2、・・・Pn(nは3以上の自然数)における前記測定対象物の運動速度を前記位相検波された信号からそれぞれ求める運動速度演算部と、前記それぞれの運動速度から隣接する一対の前記測定対象位置に挟まれる前記測定対象物の各部の伸縮量を求める伸縮演算部とを備える。前記測定対象物の1または連続した複数の測定対象位置Pi、・・・・Pj(i、jは2≦i≦j≦n−1を満たす自然数)における運動速度が正常な値ではなく、測定対象位置Pi-1およびPj+1における運動速度が正常な値である場合、測定対象位置Pi-1およびPj+1における正常な運動速度を用いて測定対象位置Pi-1およびPj+1に挟まれる領域全体における測定対象物の伸縮量を求める。

【発明の実施の形態】
本発明の超音波診断装置は測定対象物の各部の運動速度や各微小な領域における伸縮量および弾性率を測定する。物体自体は移動しないが、超音波を反射し、各位置における運動速度が異なっている種々の物体を好適に本発明の超音波診断装置の測定対象物とすることができる。特に、生体の各部の弾性率を計測するのに適している。以下では、生体の各部を「組織」と称する場合がある。

まず、本発明による超音波診断装置の一実施形態を概略的に説明する。図1は、超音波診断装置50の構成を示すブロック図である。超音波診断装置50は、超音波診断装置本体1と、超音波プローブ2と、モニタ13とを備えている。また、血圧計14とECG(心電計)15が超音波診断装置本体1へ接続されている。

次に、超音波診断装置50の動作を概略的に説明する。遅延時間制御部5により遅延時間が制御された複数の駆動パルス信号が超音波送受信部3から出力され、超音波プローブ2は、各駆動パルス信号を超音波送信波に変換し、生体へ送信する。生体組織によって反射して得られる超音波反射波は、超音波プローブ2で受信され、電気信号に変換される。超音波送受信部3において受信した受信反射波信号は、遅延時間制御部5を経て位相検波部7に入力される。遅延データ記憶部6には予め設定した超音波送信波および受信波の音響線の偏向角度や焦点深度に基づいた遅延時間データが複数記憶されており、駆動パルス信号ごとに遅延時間制御部5が異なる遅延時間データを読み出すことによって、駆動パルス信号ごとに異なる偏向角度で超音波の送受信を行うことができる。

位相検波部7は、受信反射波信号を位相検波し、実部信号および虚部信号に分離する。実部信号および虚部信号は、フィルタ8によって組織の運動速度以外の反射波成分が除去され、演算部9に入力される。

次に、演算部9における組織の位置変位量の演算について、図2を用いて説明する。ここでは、生体組織として血管壁組織の位置変位量を求める。図2は、生体21および血管壁23を伝播する超音波ビーム24を模式的に示している。生体21の表面に配置された超音波プローブ2から発信した超音波送信波は、生体21中を進行する。超音波送信波は、ある有限の幅を持つ超音波ビーム24として生体21中を伝播し、その過程において生体21によって反射または散乱した超音波の一部が超音波プローブ2へ戻り、超音波反射波として受信される。超音波反射波は時系列信号r(t)として検出され、超音波プローブ2に近い組織から得られる反射の時系列信号ほど、時間軸上で原点近くに位置する。超音波ビーム24の幅(ビーム径)は、遅延時間を変化させることにより制御することができる。

超音波ビームの中心軸である音響線22上に位置する血管壁23中の複数の測定対象位置Pn(P1、P2、P3、Pk・・・Pn、nは3以上の自然数)は、ある一定間隔Hで超音波プローブ2に近い順にP1、P2、P3、Pk・・・Pnと配列している。生体21の表面を原点とする深さ方向の座標をZ1、Z2、Z3、Zk、・・・Znとすると、測定対象位置Pkからの反射は、時間軸上でtk=2Zk/cに位置することになる。ここでcは生体組織内での超音波の音速を示す。反射波信号r(t)を位相検波部7において位相検波し、検波した信号を実部信号および虚部信号に分離してフィルタ8を通過させる。演算部9では、反射波信号r(t+Δt)と微小時間Δt後の反射波信号r(t)において振幅は変化せず、位相および反射位置のみが変化するという制約のもとで、反射波信号r(t)とr(t+Δt)との波形の整合誤差が最小となるよう最小二乗法によって位相差を求める。この位相差から、測定対象位置Pnの運動速度Vn(t)が求められ、さらにこれを積分することにより、位置変位量dn(t)を求めることができる。

次に、これらの測定演算結果を用いた弾性率の演算・表示について、図3を用いて説明する。図3は、測定対象位置Pnと弾性率演算の対象組織Tnとの関係を示す図である。対象組織Tkは、隣接する測定対象位置PkとPk+1とに挟まれた範囲に厚みHを有して位置している。n個の測定対象位置P1・・・・Pnからは(n−1)個の対象組織T1・・・・Tn-1を設けることができる。

対象組織Tkの伸縮量である厚み変化量Dk(t)は、測定対象位置PkとPk+1の位置変位量dk(t)とdk+1(t)とから、Dk(t)=dk+1(t)−dk(t)として求められる。血管壁23の組織Tkの厚みの変化は、心拍による血圧の変化に応じて生じる。よって、対象組織Tkの厚みH(最低血圧時の値)、対象組織の厚み変化量Dk(t)の最大値Dkmax、および最低血圧値と最高血圧値との差である脈圧Bを用い、対象組織Tkの歪み率である弾性率χkをχk=(B×H)/Dkmaxにより求めることができる。ここで、弾性率χkは血管半径方向の弾性率である。そして、遅延時間を制御し、送受信する超音波の偏向角度を走査させることにより、血管壁23の任意の断面における弾性率を二次元マッピング表示することができる。二次元マッピング表示では、弾性率の大きさにしたがって、輝度を分布させたり色に濃淡をつけたりすることができる。たとえば弾性率の大きい箇所および小さい箇所をそれぞれ青色および赤色で表示し、弾性率がその中間の値である箇所は青色と赤色の中間色で表示することができる。色相の組み合わせは、ユーザが自由に選択できるようにしてもよい。

超音波反射波画像にスペックルが発生している場合、スペックルは、超音波反射波信号r(t)上では振幅の大小として現れる。このような信号を用いて上述した位置変位量や弾性率を求める場合、スペックルによりある測定対象位置Pkからの反射波信号r(t)の振幅が小さくなり、運動速度や位置変位量を正しく求めることが困難となる。これは、信号の振幅が小さい箇所ではS/Nが悪化し、ノイズ成分の影響を大きく受けてしまうためである。測定対象位置Pkの運動速度Vk(t)および位置変位量dk(t)が正しい値を示さないと、この値を用いて演算する厚み変化量Dk-1(t)およびDk(t)の二つの値も正しい値を示さなくなる。さらには、厚み変化量Dk-1(t)およびDk(t)から求める弾性率χk-1およびχkも正しい値を示さなくなる。
