Description
本願は、Hanspeter R.Bayerにより2001年6月26日に出願された米国仮特許出願番号60/301,059(表題「CONDUIT HARVESTING INSTRUMENT AND METHOD」)の利益および優先権を主張する。この出願の全内容は、本明細書中で参考として援用される。
(開示の分野)
本開示は、最小侵襲性の、腹腔鏡手術手順または内視鏡手術手順を実施するための器具および方法に関する。より詳細には、本開示は、解剖学的導管または血管を採取するために、それらに対して最小侵襲性のアクセスを必要とするかまたはそれから利益を受ける手順に特に適する器具および方法に関する。この器具は、バイパス手順(冠状動脈バイパス移植(CABG)または逆もしくはインサイチュでの大腿−膝窩もしくは大腿−脛骨末梢バイパス移植が挙げられるがこれらに限定されない)において使用するために、周囲の組織から血管を採取するのに適する。
冠状動脈疾患は、しばしば、代表的に、狭心症、心臓組織の壊死(心筋梗塞)、および突然死のような合併症の原因となる心筋への不十分な血流、すなわち、心筋虚血、を生じ得る、冠状動脈における損傷もしくは閉塞により特徴付けられる。いくつかの場合、冠状動脈疾患は、薬物の使用ならびに/または行動および食事の変更により処置され得る。他の場合、冠状動脈の拡張が、血管形成術、レーザー剥離、アテレクトミー、カテーテル法、および血管内ステント術のような手順により達成され得る。冠状バイパス手術は、これらの処置方法が、シールされた動脈を開けるために使用できないかまたはそれに失敗した場合に必要とされる。
本開示は、血管を採取するための手術器具に関する。この器具は、遠位端および近位端を有する細長シャフト、ならびにこのシャフトを通って配置された複数の管腔を備える。このシャフトは、好ましくは、シャフトの遠位端に配置された平滑解剖部分を有する先端部、および平滑解剖部分とシャフトの遠位端との間に配置されたクレードル部分を備える。好ましくは、先端部は、先端部がシャフトの遠位端の近位にある第1解剖位置から、シャフトの遠位端からさらに遠位の拡張位置に選択的に移動可能であり、クレードル部分を露出する。この器具はまた、複数の管腔の1つに配置された内視鏡、および残りの管腔の1つに配置された少なくとも1つのさらなる手術器具を備える。
体内の切開中に上記器具を挿入する工程;
切開を通し、脈管に沿ってその器具を遠位に前進させ、その先端部は、引っ込められた位置にあり、クレードル部分が実質的に露出していない工程;
その脈管を見るために内視鏡を利用し、そしてその脈管から周囲組織を切開して手術腔を形成するように、平滑先端部を利用する工程;
その切開に向かって近位方向にその器具を引っ込める工程;
そのクレードル部分を露出するように平滑先端部を伸長する工程;
切開を通し、クレードル部分が露出している脈管に沿って遠位にその器具を再前進する、工程;
露出したクレードル部分を利用して、その切片中に個々の脈管支脈を選択的に位置付ける工程;ならびに
複数のさらなる外科器具のうちの1つの使用によって、引っ掛けられた連続的な脈管支脈を処置する工程。
ここで図1A〜図4を参照すると、(例えば、バイパス処置(特に、冠状動脈バイパス処置)において使用するための)脈管200を採取するために用いられ得る内視鏡脈管採取器具10の1実施形態が開示される(図5)。採取器具10は、近位端15、遠位端17およびこれらの間に配置された細長シャフト12を備えるが、このような末端15および末端17は、必ずしも直接連絡していない。本明細書中での目的のために、図1Aが、ここで開示される器具の1実施形態を開示することが理解される。以下でより詳細に記載されるように、この器具が、シャフトハウジングと一体化しているかまたはシャフトハウジングに取り付け可能なベースハウジング、およびこのシャフトハウジングと一体化しているかまたはこのシャフトハウジングに取り付け可能なシャフトを備え得ることが想定される。このような場合では、通常「近位端」と呼ばれるものは、単一の一体化ベースハウジングが利用されるか、またはベースハウジングとシャフトハウジングとの組合せが利用されるかに依存して変化し得る。
Claims (14)
- 脈管を採取するための外科用器具であって、以下:
細長シャフトであって、遠位端および近位端ならびに該細長シャフトを通して配置される複数の管腔を有する、細長シャフト;
該シャフトの遠位端に配置される先端部であって、該先端部は、該先端部の遠位端に配置される解剖部分および該先端部の近位端に配置される切り欠き部分を備え、該先端部は、クレードル部分を露出するために、該シャフトの該遠位端に近接する第1位置から、該シャフトの該遠位端に近接する該第1位置からさらに遠位の少なくとも1つのさらなる位置まで移動可能である、先端部;および
該複数の管腔のうちの1つに配置される内視鏡;
を備え、該複数個の管腔の残りのうちの少なくとも1つは、少なくとも1つのさらなる外科用器具を収容するためのものである、
外科用器具。 - 前記シャフトが、長手方向軸を備え、そして前記先端部が該シャフトの長手方向軸に対して一般的に偏心であるノーズを備える、請求項1に記載の外科用器具。
- 前記先端部が、実質的に透明である、請求項1に記載の外科用器具。
- 前記先端部が、前記クレードル部分を露出させるために前記シャフトの前記遠位端に対して、選択的にスライド可能であり、遠位に伸長可能である、請求項1に記載の外科用器具。
- 前記先端部が、前記少なくとも1つのさらなる位置から近位に収容可能である、請求項1に記載の外科用器具。
- 前記先端部および前記シャフトが、前記内視鏡に対して回転する、請求項1に記載の外科用器具。
- 前記先端部の前記遠位端が、円錐形であり、そして実質的に平滑である、請求項1に記載の外科用器具。
- 前記内視鏡が、前記器具と一体化されている、請求項1に記載の外科用器具。
- 前記切り欠き部分が、脈管支脈を作動可能に係合するよう適合される、請求項1に記載の外科用器具。
- 前記少なくとも1つのさらなる器具が、前記残りの複数の管腔のうちの少なくとも1つに収容され、そして以下:結紮器具、双極剪断、超音波剪断、クリップアプライア、凝固器具、注入器、切断器具、脈管シール器具、脈管把持器具、洗浄器具、吸引器具、およびこれらの組み合わせからなる群から選択される、請求項1に記載の外科用器具。
- 前記シャフトが、前記シャフトの外側周囲の周りに配置されるバルーンを備える、請求項1に記載の外科用器具。
- 前記バルーンが、前記シャフトの遠位端近くに配置される、請求項11に記載の外科用器具。
- 前記バルーンが、選択的に膨張可能である、請求項11に記載の外科用器具。
- 内視鏡脈管採取器具であって、以下:
近位端および遠位端を有するハウジング;
該ハウジングに取りつけられそしてスライド可能に取りつけられる先端部を有する細長シャフトであって、該シャフトは、それを通して配置される複数の管腔を備え、該先端部は、その中に配置される近位切り欠き部分を有し、該管腔の各々は、以下:内視鏡、結紮器具、双極剪断、超音波剪断、クリップアプライア、凝固器具、切断器具、脈管シール器具、脈管把持器具、注入器、洗浄器具、吸引器具、およびこれらの組み合わせからなる群から選択させる複数の外科用器具のうちの1つを収容する寸法にされる、細長シャフト;
該シャフトの外側周囲に取りつけられるバルーン;
該シャフトに対して該少なくとも1つの器具を選択的に操作するための該複数の器具のうちの少なくとも1つに係合される、少なくとも1つのアクチュエーター;
クレードル部分を露出するために、該シャフトに対して、該先端部を選択的に操作するために、該先端部に係合される、アクチュエーター;および
該バルーンを選択的に膨張させるための膨張ポート、
を備える、内視鏡脈管採取器具。
