本発明は、血管内手術、制ガン剤治療等の超選択的注入および血管造影等に用いられるカテーテルに関する。

【従来の技術】
近年、外科手術を行わず、経皮的に血管内にカテーテルを挿入して血管病変の治療を行う血管内手術が盛んになっている。このような手技においては、カテーテルを複雑に蛇行して分枝の多い細い血管の特定の部位に選択的に挿入しなければならない。

例えば、脳血管等に見られる動脈瘤や動静脈奇形腫瘍に対し施される塞栓術と呼ばれる血管内手術では、細いカテーテルの先端を脳内の患部またはその近傍まで選択的に挿入して、カテーテルの先端から、シアノアクリレート、エチレン−ビニルアルコール共重合体のジメチルスルホキシド溶液等の液状の塞栓物質や、ポリビニルアルコールの顆粒等の粒状の塞栓物質、またはコイルを注入する。このように細い血管への液状塞栓物質の注入や薬剤の投与および造影剤の注入には、それに応じた細径のカテーテルが用いられる。

従って、このような細径のカテーテルには、通常のカテーテルに要求される化学的および生物学的安全性に加えて、細く複雑なパターンの血管に迅速かつ確実な選択性をもって挿入し得る操作性が要求される。更には、前述した塞栓物質を注入するために、耐薬品性、特にジメチルスルホキシド等の溶剤に接触したときに変質しない耐溶剤性が要求される。

また、カテーテルの上記操作性について詳述すると、血管内を挿通させるために術者の押し込む力がカテーテルの基端側から先端側に確実に伝達され得る、いわゆる押し込み性と、カテーテルの基端側にて加えられた回転力が先端側に確実に伝達され得るトルク伝達性と、曲がった血管内を予め挿入されたガイドワイヤーに沿って円滑にかつ血管内壁を損傷することなく進み得る追従性と、目的部位までカテーテル先端が到達し、ガイドワイヤーを引き抜いた後でも、血管の湾曲や屈曲した部位でカテーテルに折れ曲がりが生じない耐キンク性が必要とされる。さらに、これらの性質をカテーテルに付与する最大のファクターの1つとして、カテーテルの外表面の潤滑性が必要とされる。

このようなカテーテルの要求性能に関して、押し込み性および追従性を付与するための従来技術としては、比較的硬質な内管と、この内管の外面を覆いかつ内管の先端より突出した部分を有する比較的柔軟な外管で構成された、本体部が二重管構造のカテーテルが開発され、提案されている。

例えば、実表昭60−500013号公報では、内管にポリアミドを外管にウレタンを用いて、かつ内管の先端部分はその内径が漸増するようにテーパ状に形成されたカテーテルが開示されている。しかしながらこのカテーテルは外管がウレタン製であるため耐溶剤性に乏しく、塞栓術に用いるカテーテルには適さない。また、この考案ではポリアミドとウレタンの二重管部分からウレタンの一重管部分に移行する境界部において、剛性が急激に変化するため、カテーテル本体に折れ曲がりを生じる可能性がある。特に、ウレタンの一重管部分においては、外径および内径が一定であり、剛性が先端に向かって漸減していないため、二重管から一重管に移行する境界部に応力が集中して、折れ曲がりの原因になっており、また追従性も劣っている。

米国特許第4636346号明細書では、三重管構造の主要部とそれより先端方向に延長された二重管構造の先端部を有するガイディングカテーテルが開示されている。このカテーテルは、主要部の三重管のうち内管と外管の間の管が比較的剛性を有するものであるが、当該剛性のある管が無い先端部において、トルク伝達性と耐キンク性が要求するレベルまで達していない。

さらに、特開昭57−173065号公報では、内側にらせん巻ワイヤを用いたカテーテルが開示されている。このカテーテルは、ほぼ全長にわたり延びているチューブと、そのチューブの途中で終端しているらせん巻ワイヤとからなる。しかし、このカテーテルはらせん巻ワイヤがカテーテル内腔に露出しているので、塞栓物質のつまりや引っ掛かりを起こす可能性を有しており、さらにガイドワイヤーの引っ掛かりを起こす可能性もないとは言えないので、使用時には慎重に操作する必要がある。

【発明が解決しようとする課題】
そこで、本発明の目的は新規なカテーテルを提供することにある。すなわち、操作性に優れたカテーテルを提供することにある。特に、耐キンク性に優れたカテーテルを提供することにある。また、トルク伝達性に優れたカテーテルを提供することにある。さらに、追従性に優れたカテーテルを提供することにある。さらに、押し込み性に優れたカテーテルを提供することにある。また、耐薬品性に優れたカテーテルを提供することにある。さらに、脳血管治療に優れたカテーテルを提供することにある。

【課題を解決するための手段】
これらの本発明の目的は、先端と基端を有するカテーテル本体と、該先端と該基端との間に延びるルーメンとを備える脳動脈用カテーテルであって、該カテーテル本体の先端部は、a)該カテーテルの軸方向に延び、巻かれた形状を有する金属平板コイルと、b)該金属平板コイルの外側に設けられ、該金属平板コイルよりも先端側に延長した、比較的柔軟な外層または外管と、c)該先端部の先端側に設けられ、少なくとも前記外層または外管の前記金属平板コイルよりも先端側に延長した部分で形成された最先端部と、d)該最先端部に設けられ、前記金属平板コイルより先端側に配置されたX線造影部材と、を有していることを特徴とする脳動脈用カテーテルによって達成される。

本発明の脳動脈用カテーテルの構成について説明すると、比較的柔軟な内層は、カテーテルのほぼ全体を通して延びていることが好ましいが、途中において別部材を接続してもかまわない。例えばカテーテルの先端部において、より一層柔軟であることが求められるの場合には、先端部内層を本体部内層よりも柔軟な材料にて形成して、接続してもよい。さらに、本体部を硬質な材料にてチューブ状に押出成形して、本体部と先端部の境界部分においては、徐々に軟質な材料の混合比を増加させながら押出成形しつづけ、先端部にあたる部分では軟質な材料のみで押し出しをすることによって得られるチューブ、すなわち本体部が硬質材料で先端部が軟質材料で境界部分においては先端方向に軟質材料の割合が増加するような一体成形されたチューブを用いることも可能である。

内層の内部にはルーメンとなっており、ガイドワイヤーや薬液、塞栓剤等が通ることが可能な空間を備えている。ルーメン径、すなわち内層の内径は本体部から先端部にかけて、ほぼ同一であることが好ましいが、先端部の内層の内径は、先端部の外層の外径と共に先端方向に徐々に小さくすることも可能である。また、内層の厚みは先端部と本体部でほぼ同一でかまわないが、先端部を薄くすることは柔軟性が増加するので好ましい。

内層の材質は、内部に通る薬剤に影響されない、いわゆる耐薬品性に優れていることが好ましい。特に、塞栓術に用いられるカテーテルでは、塞栓物質にDMSO、エタノール、シアノアクリレート等の溶剤が含有されている場合もあるので、耐溶剤性を備えていることが好ましい。また、金属弾性芯材の先端に設けられたX線造影性コイルと芯材表面に被覆されたフッ素系被覆材とからなるガイドワイヤーや、金属弾性芯材の表面に被覆されたプラスチック被覆材と、被覆材表面に固定された親水性潤滑層とからなるガイドワイヤーなどのガイドワイヤーが内層内部のルーメンに挿通される際の摺動性を向上させるたり、塞栓物質がコイルの場合にその通過を容易にするために、摺動性の良い平滑な材料にて形成することが好ましい。内層の材料としては、柔軟性とともに耐薬品性と摺動性を満足するものが好ましく、例えばフッ素系エラストマー等の軟質フッ素樹脂、線状低密度ポリエチレン(LLDPE)及び変成ポリオレフィンを用いることができる。内層の内表面に摺動性を向上させるために、親水性処理を施すことが好ましい。親水性処理は、水膨潤性化合物を内層に直接またはバインダーによって固定することが可能である。また、親水性処理は、親水性と疎水性のブロックコポリマーを内層に固定することも可能である。

中間層は、カテーテルの本体部においては剛性層とコイル層で構成され、先端部においてはコイル層で構成される。また、中間層は、カテーテルの本体部においては剛性層で構成され、剛性層の先端側より先端部にかけてコイル層が延びるように構成される。さらに中間層は、所定の剛性を保持したコイル層によって本体部から先端部にかけて設けられていても良い。

中間層は内層の外側に設けられており、さらに後述する外層の内側に設けられている。コイル層と剛性層はどちらが内側でも良い。中間層は、実用的にトルク伝達性や押し込み性が良好であれば内層や外層に接着される必要は必ずしもないが、先端と基端を接着するとトルク伝達性や押し込み性が良好になる。

剛性層は比較的高い剛性を有してなるもので、カテーテルの軸方向および横方向の剛性を向上させ、手元での押し込み力を先端部に伝える押し込み性を向上させるために設けられる。内層や外層に相応の剛性を有するものを用いる場合は、剛性層を省略することも可能である。剛性層の厚みは、軸方向でほぼ同一で、剛性層の先端にて先端方向に薄くなるようにしても良い。また、剛性層の先端にて螺旋状スリットを入れても良い。その螺旋状スリットは先端方向にスリットのピッチが小さくなるようにするのが好ましい。又、剛性層の先端にて軸方向に複数本スリットを入れても良い。

剛性層の材料としては、カテーテルの軸方向および横方向の剛性を向上させ、手元での押し込み力を先端部に伝える押し込み性を備えるものであれば、特に限定はされないが、プラスチック系ではポリイミド、液晶ポリマーなどが用いられる。ステンレス(SUS304)のパイプやメッシュ編組体等の金属層として構成しても良い。さらに、金属コイルの外表面を編組体で覆って該コイルの自由度をある程度抑えて剛性層としてもよい。

コイル層は、トルク伝達性と耐キンク性を備えるために設けられたものであり、さらに押し込み性も向上させる。コイル層は少なくとも前記剛性層の先端からカテーテルの先端部にまで延長されている。カテーテル本体部から先端部までコイル層が延在していてもかまわない。このように剛性層の先端(終端)と先の部分にかけてコイル層が設けられていることにより、剛性の落差を補強してキンクを防止できる。さらに、剛性層の先端部分が肉薄化されているとより一層耐キンク性が向上する。コイル層はカテーテルの最先端の手前で終端しており、その先はカテーテルで最も柔軟な部分とすることが好ましい。

コイル層は、互いに逆方向に巻かれた二層コイルであることが好ましいが、一層のコイルでもかまわない。コイルは断面長方形の平板コイルであることが好ましいが、断面円形のコイルでもかまわない。コイルの巻ピッチはカテーテルの本体部と先端部で同一でもよいが、一部分ピッチを変えることは可能である。ここで、ピッチと言うのはコイル巾(素線断面が円形の場合はその径、長方形の場合は長辺の距離)とコイル隙間を合わせた距離のことを指す。例えば、本体部を先端部よりも小さいコイル隙間のコイルを用いるか、大きいコイル巾のコイルを用いることにより本体部の方が高剛性になる。また先端方向において前記剛性層の終端を過ぎてからコイル隙間を大きくするか、コイル巾を小さくすることにより、本体部から先端部における柔軟性を3段階4段階と言うように段階的若しくは連続的変えることができる。

また、二層コイルの場合、内層のコイルと外層のコイルの柔軟性を変えることも可能である。柔軟性を変える手段としては、コイルの厚みを変えることやコイル自身の材質を変えることが挙げられる。内層のコイルと外層のコイルのを変えることも可能である。カテーテルの本体部において二層のコイルであるものが、先端部において内層若しくは外層のコイルが一方のコイルの途中で終端することも可能である。これは多段階の柔軟性を実現する。本発明に用いられるコイルは、ステンレス(SUS304)、超弾性等の金属製や、PET、ポリイミド、ポリアミド、ポリカーボネートのプラスチック製が好適である。 外層は、比較的柔軟な管状体から構成される。また、内層や中間層の表面に柔軟性な材料を被覆して形成して外層としてもかまわない。また、比較的高剛性樹脂製の本体部と比較的低剛性樹脂製の先端部と、高剛性樹脂と低剛性樹脂の混合比が連続的に変化する移行部を有して、本体部・移行部・先端部が一体成形されてなるチューブを用いても良い。また、高剛性チューブと低剛性チューブをその間の剛性のチューブにて接合したものでも良い。さらに、高剛性チューブと低剛性チューブを数mm〜十数mmにかけて重ね合わせて接合したものでもよい。この場合、本体部にあたる高剛性チューブの先端方向は徐々に薄くなり、それに対して先端部にあたる低剛性チューブは徐々に厚くなるように重ね合わせて接合することが、剛性の変化が緩やかでかつ接合面積が大きく取れる点で好ましい。外層は、内層や中間層の表面にポリオレフィン系等の収縮チューブにより収縮させて形成してもかまわない。外層は生体適合性の良好な、特に抗血栓性の良好な材料にて形成されることが好ましい。

外層の表面には、親水性潤滑層を設けることが好ましい。この潤滑層は単なる被覆でも良いが、実用に耐える程度に固定されていることが好ましい。外層の表面にはさらに高い生体適合性を備えるような処理を施すことが好ましい。生体適合性としては、抗血栓性が必要であり、ヘパリン溶液のコーテイングやヘパリンボンディングが好ましい。すなわち、外管表面にヘパリンが固定されていることが好ましい。さらに外管材料に血栓溶解剤を混合して押出成形して外管としてカテーテルを構成すると、カテーテル使用時に血栓溶解剤が徐放するので好ましい。また、外層の表面には、親水性潤滑層として親水性化合物ブロックと疎水性化合物ブロックのブロック共重合体を固定すると、潤滑性とともに抗血栓性も向上するので好ましい。この親水性化合物は水膨潤性化合物であることが好ましい。このような共重合体としては、ポリグリシジルメタクリレート(PGMA)ージメチルアクリルアミド(DMAA)のブロック共重合が好ましい。

前記先端部の先端側に最も柔軟な最先端部を設けることが好ましい。最先端部は、外層と内層の二層で形成されることが好ましいが、より柔軟な先端部材を融着、接着等の固定をしても良い。また、最先端部は外層のみの一層にて形成することでも可能である。最先端部は、外径が減少したくびれ部を備えることが好ましい。最先端部には、X線造影部材を設けることが好ましい。X線造影部材は金や白金の線材をコイル状に巻き付けるか埋め込むことが、最先端部の位置がカテーテル手技中に判明して望ましい。

前記剛性層の終端からその先端側にかけてのカテーテル自体の剛性の急激な変化を防止する別の手段を設けても良い。すなわち、キンク防止チューブを該当箇所に設けることによりキンク防止に効果が期待できる。

本発明の脳動脈用カテーテルを脳動脈瘤を塞栓する脳血管治療(脳動脈瘤塞栓術)について簡単に説明する。

まず、本発明のカテーテルが挿通可能なルーメンを有するガイディングカテーテルを、大腿動脈から大動脈を経て内頸、外頸あるいは椎骨動脈まで挿入して留置する。ガイディングカテーテルの内面は、親水性コーティングがされていている。次に、金属弾性芯材とその表面に被覆されたプラスチック被覆材と、該被覆材表面に固定された親水性潤滑層とからなるガイドワイヤーや、金属弾性芯材と該芯材の先端に設けられたX線造影性コイルと、芯材表面に被覆されたフッ素系被覆材とからなるガイドワイヤなどを挿入・セットした本発明のカテーテルを、ガイディングカテーテルのルーメンに沿わて、ガイディングカテーテルの先端から突出した後は、本発明のカテーテルと前記ガイドワイヤで目的血管まで挿入していく。途中、血管分岐に至った時には、目的血管に選択挿入するようガイドワイヤを先導させて、その後カテーテルを押し込む。ガイドワイヤに沿って挿入しにくい場合は、カテーテルの手元を回転させて、その回転をカテーテル先端部まで伝達させてながら押し込んで、目的血管まで進める。目的血管に存在する脳動脈瘤を上流側からカテーテルからの造影剤によってX線透視下で確認する。脳動脈の微細で複雑に入り組んだ血管を導入・通過するのに可能な柔軟性を本発明のカテーテルの先端部は備えている。湾曲・屈曲した血管でもコイルがカテーテル先端部に設けられているので、キンクせずにガイドワイヤに沿って容易に進むことができる。目的部位まで到達したら、脳動脈瘤内にカテーテルの先端部を挿入して、微小コイルなどの塞栓物質を導入する。導入が完了したらカテーテルを抜去する。

【実施例】
以下、本発明のカテーテルの実施例を図面を使って説明する。図1は本発明のカテーテルの第1の実施例の全体図である。本実施例のカテーテル1は、本体部2と先端部4からなるカテーテル本体1aとカテーテル本体1aの基端に設けられたハブ11からなる。先端部4の先端は開口部4aを有しており、開口部4aからハブ11の基端11aまで連通している。ガイドワイヤー50が開口部4aより突出している。カテーテル1は全長1500mmで、先端部4の長さは200mm、本体部2は1300mmである。

図2は、カテーテル1の部分断面図である。図2に示すとおり、カテーテル1には、比較的柔軟な内層6と、内層6の外側に設けられたコイル層8と比較的高い剛性を有する剛性層10とからなる中間層12と、中間層12の外側に設けられた比較的柔軟な外層14から構成された本体部2と、内層6と中間層12のコイル層8と外層14から構成された先端部4とからなる。

外層14の外表面には潤滑層16が設けられており、潤滑層16により親水性が付与されて血管との摺動性が向上する。さらに、潤滑層16にはヘパリン溶液が被覆されていることにより抗血栓性が向上している。

内層6は、本体部2から先端部4まで延びて設けられ、内部空間はルーメン20として開口部4aからハブ11の基端11aまで連通している。内層6は内管にて構成されて、ほぼ均一の内外径を備えており、ガイドワイヤーや薬液、塞栓剤等が通ることが可能としている。内層6の材質は、フッ素系エラストマーが用いられているので、耐薬品性、特に耐溶剤性に優れており、塞栓材料やガイドワイヤの挿通性や摺動性が良好となる。

中間層12は、カテーテル1の本体部2においては剛性層10とコイル層8で構成され、先端部4においてはコイル層8で構成される。中間層12は内層6の外側に設けられており、さらに後述する外層14の内側に設けられている。コイル層8の外側に剛性層10が設けられている。中間層12は、その先端と基端を外層14や内層6に接着するとトルク伝達性や押し込み性の他、耐圧性が良好になる。

剛性層10は比較的高い剛性を有してなるチューブであり、カテーテルの剛性を向上させ、手元での押し込み力を先端部4に伝える押し込み性を向上させるために設けられる。剛性層10の厚みは、軸方向でほぼ同一で、剛性層の先端10aにて先端方向に薄くなっている。このことによりキンクを防止できる。剛性層10の材料としてはポリイミドが用いられている。剛性層10が備わる本体部2は押し込み性(プッシャビリティー)が良好になる。

コイル層8は本体部2から先端部4まで延在している。コイル層8は、トルク伝達性と耐キンク性を備えるために設けられたものであり、さらに押し込み性も向上させる。剛性層10の先端(終端)10aとその先の部分にかけてコイル層8が設けられていることにより、剛性の落差を補強してキンクを防止できる。コイル層8は、互いに逆方向に巻かれた二層コイルである。この二層コイルは、内コイル8aと外コイル8bによる二重コイルからなる。内コイル8aが右巻きの場合、外コイル8bは左巻きに巻かれている。内コイル8aと外コイル8bは共に断面ほぼ長方形の平板コイルであり、ピッチは両コイルとも0.12mmで、コイル隙間は0.02mmである。内コイル8aと外コイル8bは、軸方向に対して約80度の方向に互いに逆方向に巻かれている。両コイルの厚みは約0.01mmである。内コイル8aと外コイル8bは、ステンレス(SUS304)によって形成されている。

外層14は、比較的柔軟な管状体から構成される。外層14は、内層6や中間層12の表面にポリオレフィン系等の収縮チューブにより収縮させて形成されているので、外層14を容易に形成することが可能となる。また、外コイル8bなどのコイルの膨れやたわみを押さえられて、中間層12が固定できるのでトルク伝達性や押し込み性が向上する。

外層14の表面には、親水性潤滑層16を設けられている。この潤滑層16は実用に耐える程度に固定されて、血管挿入の際に含水して血管内壁との摩擦抵抗を低減して、曲がりくねった末梢血管にまで比較的容易に導入可能となる。親水性潤滑層としては、外層の材料にもよるが、本実施例の外層14は変成ポリオレフィンで形成されており、ポリグリシジルメタクリレート(PGMA)ージメチルアクリルアミド(DMAA)のブロック共重合である親水性化合物ブロックと疎水性化合物ブロックのブロック共重合体が固定されている。該ブロック共重合体の親水性化合物は水膨潤性化合物であるので潤滑性に優れて、親水/疎水のミクロドメイン構造をなしているので抗血栓性も期待できる。 先端部4の先端側には最も柔軟な最先端部4Aが設けられている。最先端部4Aは、外層14と内層6の二層で形成される。最先端部4Aの軸方向の長さは約3.0mmである。最先端部4Aには白金の線材をコイル状に巻いたX線造影部材4cが設けられているので、カテーテル1の最先端部4Aの位置がX線透視下において明確に確認できる。最先端部4Aにはルーメン20が開口する開口部4aを有する。

カテーテル1の外径は、本体部2が最も太く、最先端部4Aが最も細く、先端部4がその中間の細さである。例えば、本体部2の外径は約0.92mm、先端部4の外径は約0.80mm、最先端部4Aの外径は約0.72mmである。
