【発明の属する技術分野】
この発明は、全血を検体としてラテックス免疫比濁法により免疫測定を行うためのフロー測光セルに関する。

【従来の技術】
全血を検体としてラテックス免疫比濁法により免疫測定を行う方法は、既に知られている。この方法では、検体が全血であるから、血清・血漿を検体とする場合のような血液の遠心分離の工程が不要になる反面、赤血球等による濁りが光学的測定の著しい妨げとなり、適当な溶血試薬の添加等、免疫反応に影響しない方法によって全血を強制的に溶血させた試料を用いることが必要とされている。

【発明が解決しようとする課題】
しかしながら、溶血させた試料中には、血球の残滓が存在し、これがブラウン運動に伴って、フロー測光セルに照射された光束を出入りするから、血球残滓のゆらぎによる測定信号への影響が大きく、測定精度を高めることが困難とされていた。

この発明は、全血を検体としてラテックス免疫比濁法により免疫測定を行うためのフロー測光セルにおいて、血球残滓のゆらぎによる測定信号への影響を減少し、測定精度を向上することを課題とする。

【課題を解決するための手段】
上記課題を解決するために、この発明では、全血を検体としてラテックス免疫比濁法により免疫測定を行うためのフロー測光セルであって、円筒状に形成され、両端に透光窓を有するセル本体が外部光から遮断した状態でホルダーに保持され、このセル本体の両端透光窓に対向する位置に光照射部及び光検出部が設けられており、前記セル本体の内径を1とし、前記ホルダーによって形成されて前記セル本体と光照射部との間に設けたスリットの径をD1 、前記ホルダーによって形成されて前記セル本体と光検出部との間に設けたスリットの径をD2 としたとき、1>D1 >0.75で、且つ、D2 >0.75となるように設定していることを特徴とする。

上記の構成によれば、スリット径D1 ,D2 がセル本体の内径1に対して何れも0.75以上であり、光照射部からスリットを経てフロー測光セルに照射され、反対側のスリットを経て光検出部へと入射する光束の断面(直径)が、セル本体の内径の割に、比較的広くなっているから、光束全体としての光量に占める血球残滓のゆらぎによる変動の割合が小さくなる(平均化される)。

また、光照射部側のスリット径D1 がセル本体の内径より大きいと、スリットを経て照射された光の一部がセル本体の肉厚内部を透過して誤差となるが、上記の構成によれば、光照射部側のスリット径D1 がセル本体の内径以下であるから、このような誤差も生じない。また、光照射部側のスリット径D1 がセル本体の内径と等しい場合、光照射部、スリット、セル本体の三者を正確に同芯状に配置しないと、光束の一部がセル本体の肉厚内部を透過したり、セル本体の内面で反射して、誤差となる可能性があるが、光照射部側のスリット径D1 がセル本体の内径以下(1>D1 )であるため、このような不都合も回避される。

従って、血球残滓のゆらぎによる測定信号への影響が減少し、光の一部がセル本体の肉厚内部を透過したり、セル本体の内面で反射することによる誤差も生ぜず、測定精度を向上することができるのである。

尚、光照射部側のスリット径D1 は、セル本体の内径以下であることが必要不可欠であるが、光検出部には、光照射部側のスリットで絞られた光束以上の断面で入射することがないから、光検出部側のスリット径D2 には原理的に上限がない。むしろ、フロー測光セルの各部位の製造公差や組立誤差を考慮すると、光検出部側のスリット径D2 はセル本体の内径より大きくするほうが、フロー測光セルの製造を容易ならしめる点で望ましい。

【発明の実施の形態】
発明の実施の形態を図面を参照しながら説明する。図1〜図9は、この発明に係るラテックス免疫比濁法による免疫測定用フロー測光セルが採用された全血血球免疫測定装置を示す。

まず、図2は、側面パネルを取り外した状態で示す全血血球免疫測定装置の斜視図であり、図3はこの全血血球免疫測定装置の全体の構成を概略的に示す図である。図4は全血血球免疫測定装置の要部を上方から見た図であり、図5は全血血球免疫測定装置の要部の構成を概略的に示す図である。

これらの図において、1は装置ケースで、その前面部2側には、検体としての全血3を収容した検体容器4をセットするための検体セット部5が内方に凹んだ状態で形成されている。6は、検体セット部5を開閉する扉7によって構成された測定キーであり、扉7を閉めることによりオン(スイッチON)となるように構成されている。前記扉7は、下端側を支点に前後方向へ揺動して開閉するように構成されており、扉7の内側には、サイズの異なる検体容器4を選択してセットできるように上面に複数の穴を形成した検体容器ホルダー8が上下軸芯周りで回転自在で且つ扉7と一体に前後方向へ揺動することが可能な状態に設けられている。

そして、装置ケース1の側面部9の下方には、前面部2に近い側から順に、免疫測定を行う免疫測定部10と血球測定を行う血球計数測定部11が前面側から見て一直線状に配置されているとともに、複数の電磁弁12aからなる電磁弁部12が設けられている。また、側面部9の上方には、検体セット部5と血球計数測定部11との間を、一直線上に配設された免疫測定部10と血球計数測定部11に沿うようにして直線的に移動するサンプリング機構としてのプローブユニット部13が設けられている。

図3において、14は定注器、15は希釈液容器、16は溶血試薬容器、17は液排出用のポンプであり、これら15〜17はいずれも電磁弁部12に接続されている。18はポンプ17に接続された廃液容器である。

前記免疫測定部10は、この実施の形態においては、ラテックス免疫比濁法によってCRP(急性期蛋白であるC−反応性蛋白)を測定するように構成されている。すなわち、図3、図5において、19は上面の開口した試薬受容セルであり、試料受容セル19の底部には、両側に発光ダイオードからなる光照射部20aとフォトダイオードからなる光検出部20bを備えたCRP測定用のフロー測光セル20が流路21を介して接続されている。当該フロー測光セル20の出口側の流路22には、三方電磁弁12bを介して液移動用の定注器23が接続されている。三方電磁弁12bの下流側は前記電磁弁部12を介して前記ポンプ17に接続されている。24,25,26はCRP測定に用いられる試薬を収容した試薬容器で、それぞれ、溶血試薬(以下、R1試薬という)、緩衝液(以下、R2試薬という)、抗ヒトCRP感作ラテックス免疫試薬(以下、R3試薬という)が収容されている。

前記試薬受容セル19および試薬容器24〜26は、検体セット部5における検体容器4のセット位置に対して一直線状に配置され、これら試薬受容セル19および試薬容器24〜26は、ソレノイド27によって上下方向に揺動する蓋28によって一括して開閉されるように構成されている。また、29は例えばペルチェ素子よりなる電子冷却器30を備えたクーラーボックスで、図示例では試薬R2,R3の入った試薬容器25,26が収容されている。

また、前記定注器23は、CRP測定時に、試料受容セル19内の反応液(免疫測定用の試薬および全血試料)をフロー測光セル20へ流通させる作用を司るだけでなく、試料受容セル19やフロー測光セル20等とで、次のような反応液攪拌機構を構成している。すなわち、図6に示すように、試料受容セル19に反応液が収容された状態で、定注器23が、CRP測定に先行して、数回、摺動することにより、試料受容セル19内の反応液を試料受容セル19とフロー測光セル20とにわたって前後に往復移動させるように構成してある。

より詳しく説明すると、定注器23は、その摺動ストロークが制御されるように構成されており、試料受容セル19内に、R1試薬、R2試薬、および検体(全血)3が収容された時点で、それらの総量L1 に対応して設定されたストロークaだけ、定注器23が数回(例えば、3回)、前記三方電磁弁12bがフロー測光セル20と定注器23を連通させた状態において、摺動することにより、反応液を前後に往復移動させて、一回目の反応液の攪拌を行い、反応液にR3試薬が加えられた時点で、それらの総量L2 に対応して設定されたストロークb端まで定注器23が数回(例えば、3回)、摺動することにより、反応液を前後に往復移動させて、二回目の攪拌を行うように構成してある。

尚、CRP測定時には、定注器23がストロークb端まで一回だけ摺動して、反応液をフロー測光セル20に流通させるようになっている。定注器23のストロークや流路21、22の長さは、定注器23がストロークb端まで摺動した際、流路21、22内にある反応液の最前端Pが、三方電磁弁12bよりも上流側(三方電磁弁12bとフロー測光セル20の間)に位置し、最後尾Qがフロー測光セル20よりも上流側(図示の例では、流路21内で且つ試料受容セル19の出口近く)に位置するように設定されている。そして、この状態で、前記三方電磁弁12bが流路の切り換えを行って、定注器23を遮断すると共に、三方電磁弁12b前後の流路(フロー測光セル20側の流路とポンプ17側の流路)を連通させると、前記ポンプ17が流路内の反応液を吸引して、廃液容器18に排出するように構成してある。

この反応液攪拌機構によれば、反応液が試料受容セル19とフロー測光セル20とにわたって前後に往復移動すると、試料受容セル19と流路21の接続部、フロー測光セル20の入口および出口で、夫々、流路断面が変化しているので、反応液には、図6に示すように、流路断面が変化するこれらの部位で乱流が生じ、乱流が生じる部位が多いため、攪拌が効率よく行われることになる。

次に、血球計数測定部11は、この実施の形態においては、電気抵抗法により、WBC(白血球数)、RBC(赤血球数)、PLT(血小板数)、MCV(赤血球容積)、Hct(ヘマトクリット値)を、また、シアンメトヘモグロビン法における吸光光度法によりHgb(ヘモグロビン濃度)などをそれぞれ測定するように構成されている。すなわち、図3において、31はWBC/Hgb血球計数測定セル(以下、単にWBCセルという)で、WBCを測定するための測定電極31a,31bおよびHgbを測定するための光照射部31c、受光部31dを備えている。32はRBC/PLT血球計数測定セル(以下、単にRBCセルという)で、RBCおよびPLTを測定するための測定電極32a,32bを備えている。これらのセル31,32は、図4に示すように、免疫測定部10における試薬受容セル19および試薬容器24〜26と一直線になるように配置されている。また、WBCセル31は、後述するサンプリングノズル40を洗浄するための廃液チャンバを兼ねている。

さらに、プローブユニット部13は、例えば次のように構成されている。すなわち、図2および図4において、33はノズルユニットで、このノズルユニット33は、垂直に立設されたベース部材34に沿うようにして水平方向に設けられたタイミングベルト35に対して適宜の連結部材36によって固定され、これによって水平方向に往復移動できるように構成されている。

より詳しくは、ノズルユニット33は、検体セット部5から血球測定を行う血球計数測定部11までの間で、検体容器4、血球計数測定部11と一直線上に配設された免疫測定部10の試薬容器24〜26、試薬受容セル19、WBCセル31、RBCセル32のほぼ真上を往復移動するように構成されている。37はタイミングベルト35を駆動するためのモータ、38はノズルユニット33に設けられた被ガイド部材39をガイドする一対のガイド部材で、これらはベース部材34に適宜の部材を介して取り付けられている。

40はサンプリングノズルで、ノズルユニット33内をタイミングベルト41によって上下方向に移動するノズル保持体42に取り付けられている。このサンプリングノズル40の先端側(下端側)は、ノズルユニット33内に設けられたサンプリングノズル洗浄器43を挿通し、先端部外周が洗浄されるように構成されている。44はタイミングベルト41を駆動するためのモータである。45はサンプリングノズル40がホームポジション位置(定位置)にあるか否かを検出するセンサである。

そして、図3において、46は装置の各部を総合的に制御するとともに免疫測定部10および血球計数測定部11からの出力を用いて各種の演算を行う制御・演算装置としてのマイクロコンピュータ(MPU)、47はMPU46からの指令に基づいて電磁弁部12、プローブユニット部13のモータ37,44などに駆動信号を送るドライバ、48は免疫測定部10および血球計数測定部11からの出力信号を処理してMPU46に送る信号処理部、49はMPU46において処理されて得られる結果などを表示する装置で、例えばカラーディスプレイであり、50は出力装置としてのプリンタである。

なお、図3において、点線は検体3や各種の試薬などの流れを示し、また、やや太い一点鎖線は制御信号を、細い一点鎖線は測定によって得られる信号の流れをそれぞれ示している。

この発明の実施の形態では、上記の全血血球免疫測定装置において、前記フロー測光セル20を、次のように構成しているのである。すなわち、図1に示すように、フロー測光セル20のセル本体20Aは、円筒状に形成されており、両端に透光窓20B,20Bを有する。20C,20Cはセル本体20Aの両端に連設された側管である。セル本体20Aと光照射部20aとの間、セル本体と光検出部20bとの間には、夫々、セル本体20Aを外部光から遮断した状態に保持するホルダー20Dによって形成されたスリットa,bが設けられている。そして、セル本体20Aの内径を1とし、セル本体20Aと光照射部20aとの間に設けたスリットaの径をD1 、セル本体20Aと光検出部20bとの間に設けたスリットbの径をD2 としたとき、1>D1 >0.75で、且つ、D2 >0.75、好ましくは、D2 >1となるように設定してある。尚、セル本体20A、透光窓20B、側管20Cは、何れも、透明ガラス製である。

表1,2において、1欄〜10欄のデータは、10回の実験により得られた実測値、MEANは、それらの平均値、S.D.は、標準偏差を示す。これらの表1,2から、セル本体20Aの内径を1とし、セル本体20Aと光照射部20aとの間に設けたスリットaの径をD1 、セル本体20Aと光検出部20bとの間に設けたスリットbの径をD2 としたとき、D2 が1.43で、且つ、D1 が0.77や0.86である場合、標準偏差が小さいが、D1 が0.71や0.64になると、標準偏差が極端に大きくなり、血球残滓のゆらぎによる測定信号への影響が大きく、実用可能な精度範囲での再現性が得られないこ とがわかる。また、D1 が0.86であっても、D2 が0.71になると、標準偏差が極端に大きくなり、同様に、実用可能な精度範囲での再現性が得られないことがわかる。

上記構成の全血血球免疫測定装置の動作について、測定手順の一例を示した図7〜図9をも参照しながら説明する。

まず、測定キー6をオンする(ステップS1)と、定位置にあるサンプリングノズル40は、R2試薬の位置に移動し(ステップS2)、R2試薬を吸引する(ステップS3)。この試薬吸引の後、サンプリングノズル40は、上方に移動し、サンプリングノズル洗浄器43に供給される洗浄液としての希釈液によってその外面が洗浄される。その後、サンプリングノズル40は、R2試薬の位置に復帰する。

次いで、サンプリングノズル40は、R1試薬の位置に移動し(ステップS4)、R1試薬を吸引する(ステップS5)。この試薬吸引の後、サンプリングノズル40は、上方に移動し、サンプリングノズル洗浄器43に供給される洗浄液としての希釈液によってその外面が洗浄される。その後、サンプリングノズル40はR1試薬の位置に復帰する。

前記吐出を終わったサンプリングノズル40は、WBCセル31位置に移動し(ステップS11)、内部に残留している検体3、R1試薬、R2試薬を、ポンプ17により供給された希釈液とともにWBCセル31内に吐出する。そして、サンプリングノズル40は、サンプリングノズル洗浄器43に供給される洗浄液としての希釈液によってその外面が洗浄される。この洗浄における廃液は、WBCセル31に受け止められ、ポンプ17により廃液容器18に排出される。再度、サンプリングノズル洗浄器43より希釈液をWBCセル31に供給し、ポンプ17により廃液容器18に排出することにより、WBCセル31を洗浄する。なお、上記廃液の受け止めをRBCセル32によって行うようにしてもよい。

前記洗浄が終わったサンプリングノズル40は、検体セット位置に移動し(ステップS12)、検体容器4内の検体3をCBC測定のために吸引する(ステップS13)。この検体吸引の後、サンプリングノズル40は、上方に移動し、サンプリングノズル洗浄器43に供給される洗浄液としての希釈液によってその外面が洗浄される。

前記洗浄が終わったサンプリングノズル40は、WBCセル31内に検体3を吐出する一方、希釈液容器15内の希釈液が電磁弁部12を介してWBCセル31内に所定量注入され、CBC検体の一次希釈が行われる(ステップS14)。

WBCセル31位置にあるサンプリングノズル40は、前記一次希釈されたCBC検体を所定量吸引して、RBCセル32に移動し(ステップS15)、前記吸引した一次希釈されたCBC検体をこのセル32に吐出する(ステップS16)とともに、希釈液容器13内の希釈液が電磁弁部10を介してRBCセル32内に所定量注入され、CBC検体の二次希釈が行われる(ステップS17)。

上記一次希釈、二次希釈を終わった後、溶血剤容器16内の溶血剤が電磁弁部12を介してWBCセル31内に所定量注入され、WBCとHgbの測定が行われる一方、RBCセル32ではRBCとPLTの測定が行われ(ステップS18)、そのときのデータは信号処理部48を経てMPU46に取り込まれる。

前記測定が終わると、WBCセル31とRBCセル32は希釈液で洗浄される(ステップS19)。

上述したように、前記ステップS11〜S19は、血球計数測定部11においてCBC測定が行われているが、この期間中(約60秒間)は、試薬受容セル19内において、検体3、R1試薬、R2試薬の間で溶血反応が進行するとともに、妨害物質が除去される。

そして、CBC測定が終わると、RBCセル32の位置にいたサンプリングノズル40は、WBCセル31の位置に移動し、ポンプ17によって供給された希釈液でWBCセル31内面が洗い流すとともに、サンプリングノズル洗浄器43に供給される洗浄液としての希釈液によってその外面が洗浄される。このときの廃液は、WBCセル31に受け止められ、ポンプ17によって廃液容器18に排出される。そして、再度、サンプリングノズル洗浄器39より希釈液をWBCセル31に供給し、ポンプ17によって廃液容器18に排出することでWBCセル31を洗浄する。その後、サンプリングノズル40は、R3試薬の位置に移動し(ステップS20)、R3試薬を吸引する(ステップS21)。この試薬吸引の後、サンプリングノズル36は試薬受容セル19位置に移動し(ステップS22)、R3試薬を試薬受容セル19内に吐出し(ステップS23)、R3試薬が前記検体3、R1試薬、R2試薬の反応液内に混入される。

前記R3試薬の吐出後、サンプリングノズル40は、WBCセル31の位置に移動し、ポンプ17によって供給された希釈液でWBCセル31内面を洗い流すとともに、サンプリングノズル洗浄器43に供給される洗浄液としての希釈液によってその外面が洗浄される。このときの廃液は、WBCセル31に受け止められ、ポンプ17によって廃液容器18に排出される。そして、再度、サンプリングノズル洗浄器43より希釈液をWBCセル31に供給し、ポンプ17によって廃液容器18に排出することでWBCセル31を洗浄する。

そして、定注器23が、ストロークbだけ、数回、摺動して、二回目の反応液の攪拌を行い(ステップS24)、免疫反応が生じた時点で定注器23が、再度、ストロークb端まで摺動することにより、反応液をフロー測光セル20へと流通させて、CRP測定が行われ(ステップS25)、そのときのデータは信号処理部48を経てMPU46に取り込まれる。前記測定が終わると、試薬受容セル19は希釈液で洗浄され(ステップS26)、全ての測定が終わる(ステップS27)。

前記MPU46においては、血球計数測定部11において行われたCBC測定によって得られたデータに基づいてRBC(赤血球数)、赤血球容積(MCV)、などの測定値が得られる。また、MPU46においては、免疫測定部10において行われたCRP測定によって得られたデータに基づいて、所定時間当たりの吸光度変化を予め既知濃度の血清(または血漿)より求めておいた検量線から、全血中のCRP濃度が得られる。

この場合、CRP測定については、CBC測定と同様に検体3として抗凝固剤添加の全血を用いているため、この全血を用いることによって生ずる血漿成分容積誤差を補正する必要がある。そこで、CBC測定によって得られるRBC(赤血球数)と赤血球容積(MCV)とからヘマトクリット値(Hct)を求め、このヘマトクリット値を用いて、CRP測定によって得られる全血中のCRP濃度を、下記の補正式によって補正し、血漿中のCRP濃度を求めるのである。
