【発明の属する技術分野】
本発明は、生体内に注入されたラジオアイソトープ(以下RIという)から放出されるガンマ線を検出するためのガンマ線検出器及びそれを用いてガンマ線の体内分布を画像化する核医学診断装置に関する。

【従来の技術】
核医学診断装置は、シングルフォトン核種を用いてRIの崩壊時の一個のガンマ線の検出を行い、この検出データに基づいて2次元的なガンマ線蓄積画像をえることを特徴としたシンチレーションカメラ(ガンマカメラ)と、ポジトロン核種を用いて陽電子が消滅する際に反対方向に一対のガンマ線を放出することを利用し、放出場所を特定することにより2次元的なガンマ線の蓄積画像を得ることを特徴としたポジトロンカメラに分類される。

その内、シンチレーションカメラのガンマ線検出器としてアンガー型カメラが主流を占めているが、近年2検出器が対向するアンガー型のシンチレーションカメラを用い、ポジトロン核種の場所を検出するポジトロンカメラが台頭してきている。

アンガー型カメラは、NaIに代表されるシンチレータで発生した蛍光を、ライトガイドを介して稠密に配置された複数の光電子像倍管(PMT)で検出し、この検出した信号に対して位置計算処理、エネルギー値計算処理を行う。位置計算処理は、1つのガンマ線イベントに対する各光電子増倍管を重み付け加算することにより、ガンマ線の入射位置を求めるものである。エネルギー計算処理は、全光電子増倍管の出力を加算することにより、ガンマ線のエネルギー値を計算するものである。しかしながら、光電子増倍管は、光検出面上の位置による検出感度のバラツキ、感度の経時的な変化、温度によるドリフト等により検出精度が低下するという問題があった。また、この検出精度の低下を防ぐために、さまざまな補正回路を必要としていた。

また、従来のガンマ線検出器は光電子増倍管を使用していたため、ガンマ線検出器が厚くなるという問題があった。また、光電子増倍管を使用するため、ガンマ線検出器内のデットスペースが多くなり、ガンマ線検出器全体が大きくなり、さらにガンマ線検出器の重量が数百Kgと重くなるという問題があった。このような、大きさや重量が原因となり、様々な臨床目的に応じた検出器位置設定が難しく、収集目的が限定されたり、広範囲の収集が可能な場合でもガンマ線検出器を支持移動する装置が大規模かつ高価になるというという問題があった。また、検出器が大きいために、ガンマ線検出器の位置を合わせる際に患者に苦痛を伴う姿勢を要求する場合があった。

【発明が解決しようとする課題】
本発明の目的は、核種の生体内分布を高精度で得ることができ、かつ、様々な撮影対象や撮影方法に対する兼用性の高い核医学診断装置を提供することである。

【課題を解決するための手段】
本発明のガンマ線検出器は、ガンマ線を検出するための複数の半導体センサの2次元的の配列構造を有する単独で用いら得る矩形の第1のガンマ線検出部と、前記第1のガンマ線検出部と同じ構成及び構造を有する単独で用いら得る第2のガンマ線検出部と、前記第1のガンマ線検出部を前記第2のガンマ線検出部とともに単一の検出器として用いるに際して前記第1のガンマ線検出部と前記第2のガンマ線検出部とを長辺又は短辺同士を接するように分離可能に保持するフレームとを具備する。
本発明の核医学診断装置は、架台に回転自在に設けられる第1のアーム支持部と、前記第1のアーム支持部に取り付けられる第1のアームと、前記第1のアームの先端において自由関節機構を介して設けられ、ガンマ線を検出するための複数の半導体センサの2次元的の配列構造を有する単独で用いら得る矩形の第1のガンマ線検出部と、前記架台に前記第1のアーム支持部とは独立して回転自在に設けられる第2のアーム支持部と、前記第2のアーム支持部に取り付けられる第2のアームと、前記第2のアームの先端において自由関節機構を介して設けられ、前記第1のガンマ線検出部と同じ構成及び構造を有する単独で用いら得る第2のガンマ線検出部とを具備する。

【発明の実施の形態】
シングルフォトン核種を用いてRIの崩壊時の一個のガンマ線を検出して2次元画像として表示する核医学診断装置の内、SPECT(Single Photon Emission Computed Tomography) 収集、ホールボディー収集、通常のスタティック収集、マンモ収集、甲状腺収集等や、2検出器を対向状態に設置することによるPET収集が、一台のシステムで可能な例について説明する。

図1に本実施形態による核医学診断装置の構成を示す。図2に図1のガンマ線検出部の概略的構造を示す。核医学診断装置は、大きく、RIからのガンマ線を検出するためのガンマ線検出器と、ガンマ線検出器の出力に基づいてRIの生体分布を画像化するための信号処理系としての本体とから構成される。

ガンマ線検出器は、第1のガンマ線検出部1と、第2のガンマ線検出部2とを有している。第1のガンマ線検出部1と第2のガンマ線検出部2とは同じ構成及び構造を有している。そこで、第1のガンマ線検出部1の構成及び構造について説明し、第2のガンマ線検出部2に関する構成及び構造についての説明は省略する。

第1のガンマ線検出部1は、ガンマ線を検出し電気信号として出力するための複数の半導体センサ3を有する。半導体センサ3には例えばCdZnTeが採用される。複数の半導体センサ3は、例えば12.7cm×25.4cmの矩形平面内に稠密に2次元的に配列される。複数の半導体センサ3のガンマ線入射側には、ガンマ線入射方向を制限するためのコリメータ(COLLIMATOR)3が設けられる。1つの半導体センサ3毎にエネルギースペクトラムの収集を可能とするために、複数の半導体センサ3各々の出力にはそれぞれ個々にプリアンプ(PRE-AMP) 7、アナログディジタルコンバータ(ADC) 9、エネルギースペクトラム収集部(ESA) 11が設けられる。

エネルギースペクトラムとは一定時間内に入射したガンマ線の頻度(入射数に相当する)をエネルギー軸に関して分解した分布として定義される。実用的には、0.5〜2KeVのエネルギーバンドを1つのチャネルに対応させ、チャネル毎に頻度を計数することが行われている。

本体の波形処理部13は、エネルギースペクトラム収集部11各々で収集されたエネルギースペクトラムそれぞれに対して、統計上のノイズ成分を除去するためにスペクトラムの形状推定処理を行う。また、この形状推定処理により推定されたスペクトラム中の散乱性成分及び他核種からのクロストーク成分を除去するために、台形近似法等の散乱線成分及びクロストーク成分の除去処理を推定したスペクトラムに施す。波高分析部15は、波形処理部13で補正されたエネルギースペクトラムそれぞれに対して、対象核種に対応する関心ウインドウ内の計数値合計を求める。画像処理部17は、計数値合計及び半導体センサ3の位置情報に基づいて、ガンマ線の生体分布を画像データとして再構成すると共に、画像表示部23のマトリクスサイズに対応するように画像データを必要に応じて補間する。なお、画像処理部17は、第1のガンマ線検出部1及び第2のガンマ線検出器のの配置情報を入力ための配置入力手段が設けられている。画像処理部17は、この配置入力手段への入力に基づいて、半導体センサ3の位置情報を求める。この画像データは、画像メモリ19、データ処理部21を介して画像表示部23にガンマ線の生体分布として濃淡表示される。

第1のガンマ線検出部1と第2のガンマ線検出部2とはそれぞれ個々に用いられてもよいし、図3(a)に示すように第1のガンマ線検出部1と第2のガンマ線検出部2の長辺同士が接するようにして1つのガンマ線検出器としても良いし、図3(a)に示すように第1のガンマ線検出部1と第2のガンマ線検出部2の短辺同士が接するようにして1つのガンマ線検出器としても良い。

図3(a)のように長辺同士が接するように配置した場合(以下、横置きという)、第1のガンマ線検出部1と第2のガンマ線検出部2の長軸が並列し、第1のガンマ線検出部1と第2のガンマ線検出部2を合わせた検出面はほぼ正方形の形状となる。図3(b)のように短辺同士が接するように配置した場合(以下、縦置きいう)、第1のガンマ線検出部1と第2のガンマ線検出部2の短軸が並列し、第1のガンマ線検出部1と第2のガンマ線検出部2を合わせた検出面は長方形の形状となる。このように第1のガンマ線検出部1と第2のガンマ線検出部2とを個々に用いたり、縦又は横に並列して用いることは、第1のガンマ線検出部1と第2のガンマ線検出部2との個々の又は両者を合わせた全体としての検出面の形状や大きさを、撮影対象部位の種類や位置さらに大きさに応じて好ましく変形して使用できることを可能としている。

図4(a)は、第1のガンマ線検出部1及び第2のガンマ線検出部2を横置き用の配置に支持固定する縦置き用フレームを示している。横置き用フレーム35は、第1のガンマ線検出部1と第2のガンマ線検出部2の長辺同士が接触するように、第1のガンマ線検出部1と第2のガンマ線検出部2を支持固定するものである。第1のガンマ線検出部1と第2のガンマ線検出部2の隣辺関係にある2側面には、突起されたレール33が形成されている。フレーム35の内面には、レール33が挿入される溝37が形成されている。第1のガンマ線検出部1と第2のガンマ線検出部2はレール33が形成されてない長辺どうしを合わせられた状態で、溝37にそってフレーム35に挿入される。フレーム35の1辺を構成する蓋36は開閉自在に設けられており、第1のガンマ線検出部1と第2のガンマ線検出部2がフレーム35に挿入された状態で閉じられ、固定される。

これにより、第1のガンマ線検出部1と第2のガンマ線検出部2は横置きの状態で安定的に保持され、また第1のガンマ線検出部1と第2のガンマ線検出部2はレール33が形成されてない長辺どうしを合わせられるので両者の半導体センサ3の2次元アレイは連続的に繋がる。このような横置きの配置は、正方形に近い検出面を必要とする頭部の平面収集やSPECT収集,心臓の平面収集やSPECT収集等に特に有用である。

図4(b)は、第1のガンマ線検出部1及び第2のガンマ線検出部2を縦置き用の配置に支持固定する縦置き用フレームを示している。縦置き用フレーム39は、第1のガンマ線検出部1と第2のガンマ線検出部2の短辺同士が接触するように、第1のガンマ線検出部1と第2のガンマ線検出部2を支持固定するものである。フレーム39の内面には、レール33が挿入される溝40が形成されている。第1のガンマ線検出部1と第2のガンマ線検出部2はレール33が形成されてない短辺どうしを合わせられた状態で、溝40にそってフレーム39に挿入される。フレーム39の1辺を構成する蓋41は開閉自在に設けられており、第1のガンマ線検出部1と第2のガンマ線検出部2がフレーム39に挿入された状態で閉じられ、固定される。

これにより、第1のガンマ線検出部1と第2のガンマ線検出部2は縦置きの状態で安定的に保持され、また第1のガンマ線検出部1と第2のガンマ線検出部2はレール33が形成されてない短辺どうしを合わせられるので両者の半導体センサ3の2次元アレイは連続的に繋がる。
このような縦置きの配置は、この縦置きの配置のガンマ線検出器を移動可能に支持する支持機構と組み合わせることにより広い範囲のホールボディ収集、SPECT収集、平面収集を行うのに適している。尚、この時の移動方法は、収集と同時に移動を行うスキャン方式、収集と移動を交互に行うステップ移動方式のいずれで行っても良い。

ホールボディー収集を行う場合は、縦置きのガンマ線検出器の短軸が、患者の体軸(頭部と足部を結ぶ方向)と平行になるように、縦置きのガンマ線検出器を支持機構で支持する。次にこの縦置きのガンマ線検出器を体軸方向(頭部と足部を結ぶ方向)に順次移動させて、ガンマ線の収集を行う。この移動の際は、ガンマ線検出器と被検体と間の距離がほぼ一定となるように、ガンマ線検出器を被検体に対して近接・離反移動を行うようにしても良い。
この動作を患者の足部から頭部にかけて行うことにより全身の収集(ホールボディ収集)を行うことができる。

広い範囲のSPECT収集を行う場合は、縦置きのガンマ線検出器の長軸が、患者の体軸方向と平行になるように、縦置きのガンマ線検出器を支持機構で支持する。次にこの縦置きのガンマ線検出器を体軸を中心として順次回転移動させて、ガンマ線の収集を行う。この収集を例えば360度回転する間行うことにより広い範囲のSPECT収集を行うことができる。

上述したフレーム35,39は、周縁の半導体センサ3への側面及び背面からのガンマ線の入射を防止するために、側面部分及び背面部分をガンマ線遮蔽材で覆う構造になっている。このガンマ線遮蔽材は、剛性の強い材料に鉛等のガンマ線を遮蔽する材料を張り付けた2層構造となっている。ガンマ線遮蔽材は、鉛等のガンマ線を遮蔽する材料単体で形成する構造としても良い。

なお、上記では、第1のガンマ線検出部1と第2のガンマ線検出部2を組み合わせて1つのガンマ線検出器とするフレームについて述べたが、第1のガンマ線検出部1及び第2のガンマ線検出部2を単独で使用する場合はそれぞれを単独で固定支持するフレームを用いれば良い。なお、画像処理部17の入力手段には、この縦置き、横置き、単独の何れかを検出器の配置情報として入力すれば良い。この入力は、操作者が手動で入力しても良いし、また、ガンマ線検出部に配置を検出するための検出器を設けて、この検出器の出力を入力するようにしても良い。

第1のガンマ線検出部1は、アーム261 を介して回転リング251 に支持されている。第2のガンマ線検出部2は、アーム262 を介して回転リング252 に支持されている。回転リング251 と、回転リング252 とは、互いに独立して回転可能に架台に取付られている(C、C’)。架台は、床面に設置したレール上を移動できるように構成されている。架台は、このレール上を移動することにより被検体の体軸方向に沿って移動する。アーム261 ,262 は、上下左右に首振り運動が可能に回転リング252 、252 に設けられる((a、a’),(b、b’))。アーム261 ,262 は、そのアーム長が変化可能に、伸縮自在な構造が採用され、又はスライド可能に回転リング252 、252 に設けられる(d、d’)。第1のガンマ線検出部1は、アーム261 の軸方向に対して検出表面の向きを自由に変えられるように、ユニバーサルジョイント機構等の自由関節機構を介してアーム261 の先端に設けられている。第2のガンマ線検出部2も同様に、アーム261 の軸方向に対して検出表面の向きを自由に変えられるように、ユニバーサルジョイント機構等の自由関節機構を介してアーム262 の先端に設けられている。アーム261 及びアーム262 の先端には、着脱機構が設けられており、第1のガンマ線検出部1と第2のガンマ線検出部2を簡単に着脱できるように成っている。第1のガンマ線検出部1と第2のガンマ線検出部2とはそれぞれ支持機構から取り外してハンドキャリー収集(ガンマ線検出部を手で把持した状態で行う収集)やマンモ収集用として用いることが可能である。尚、この第1のガンマ線検出部1及び第2のガンマ線検出部2は、前述のフレームに収容した状態でアーム261 及びアーム262 に取り付けられる。

このような支持機構によれば次のように様々な部位、撮影方法に対応できる。ホールボディ、胸部、腹部等の平面又はSPECT撮影時には、図8(a)や図9(a),(b)に示すように、第1のガンマ線検出部1と第2のガンマ線検出部2とを、縦置きの状態にセッティングして用いることができる。この場合は、前述の縦置きフレーム39に第1のガンマ線検出部1と第2のガンマ線検出部2を収容した状態でアーム261 及びアーム262 に取り付ければ良い。また、頭部の平面又はSPECT撮影時には、図8(b)に示すように、第1のガンマ線検出部1と第2のガンマ線検出部2とを、横置きの状態にセッティングして用いることができる。この場合は、前述の横置きフレーム35に第1のガンマ線検出部1と第2のガンマ線検出部2を収容した状態でアーム261 及びアーム262 に取り付ければ良い。また、心臓の収集効率を向上したい時は、図7に示すように第1のガンマ線検出部1と第2の検出部2との間が角度90゜〜150゜の略L字型となるように配置して、収集を行えば良い。この場合は、第1のガンマ線検出部1と第2のガンマ線検出部2をそれぞれ単独でフレームに収容し、アーム261 及びアーム262 にそれぞれ取り付け、リング251 、252 の回転、アーム261 、262 の移動、ガンマ線検出部1、2の回転の複合動作により位置を設定する。

以下にマンモ収集用の支持機構について述べる。
図10は、マンモ収集用の支持機構の外観図である。台座42は、床面に設置される台である。支持柱43は、台座43に固定され、床面に対して垂直に延びる柱である。テーブル44は、支持柱43に固定され、ガンマ線検出部1(または、ガンマ線検出部2)を着脱できるようになっている。テーブル44は、患者の***を検出面に十分載せるために、ガンマ線検出部1の長辺が患者にできるだけ近づくようにガンマ線検出部1を固定するようになっている。また、テーブルはガンマ線検出部1の背面から不要なガンマ線が入射しないように鉛等の遮蔽剤を備えている。圧迫板45は、ガンマ線検出部1に載せられた***を押圧するものであり、ガンマ線検出部1に対して近接・離反移動できるように支持柱43に固定されている。

マンモ収集を行う場合は、患者の***をテーブル44に取り付けられたガンマ線検出部1上に載せた後、圧迫板45で圧迫する。この状態で収集されたデータを波形処理部13へ送ることにより、画像表示部23に***内の核種分布像を得ることができる。尚、画像処理部17の入力手段には、ガンマ線検出部1を単独で使用する旨を配置情報として入力すれば良い。また、圧迫板45の代わりに第2のガンマ線検出部2を取り付けて、2つのガンマ線検出部が対向するようにし、この2つのガンマ線検出部によりガンマ線の収集を行うようにしても良い。

このような本願発明によれば、次のような効果が得られる。
(1)ガンマ線の検出に半導体センサを用いているので、ガンマ線検出器を小型化・軽量化できる。ガンマ線検出器を小型化したことにより、患者への精神的圧迫感を軽減でき、また、安全性も向上する。また、ガンマ線検出器が軽量になったことにより、ガンマ線検出器の支持機構の構成を簡略にできる。
(2)半導体センサを用いているので、ガンマ線を高感度に検出でき、従来より収集時間を短くすることができる。これにより、従来、収集時間の長さが問題となり実現が困難だった全身SPECT収集が実現可能なる。
(3)ガンマ線検出部側部のシールドを短くすることができるので、頭部SPECT収集時の患者位置設定が容易になり、また、検出器の近接が容易になる。
(4)ガンマ線検出器の厚さを薄くできるので、心臓のSPECT収集の際に患者が腕を上にあげる必要がなくなり、患者の負担を減らすことができる。
(5)ガンマ線検出部側部のシールドを短くすることができるため、マンモ収集を行うことができる。
(6)ホールボディ収集時に、被検体の体軸方向のガンマ線検出器の長さを短くできるので、最接近軌道収集を行った時にガンマ線検出器が被検体により近づくことができ、これにより画像の分解能が向上する。
(7)支持機構にガンマ線検出部が着脱自在に取り付けられているので、支持機構を変えるだけで、頭部の収集、ホールボディ収集、マンモ収集、ハンドキャリー収集等の様々な診断に対応することができる。
