本発明は患者に電極を装着して患者の容体をモニタするための医用テレメータシステムに関する。

【従来の技術】
救急現場から搬送先の救急救命室などの施設まで、患者の容体をモニタする場合、従来は図9に示すように、各場所でモニタ装置などの医療機器が異なる毎にセンサや入力コードの変換が必要であった。

【発明が解決しようとする課題】
しかし、このように各場所でセンサやコードを変換すると、その都度患者に負担を強いることになり、また、一刻を争う救急救命の現場ではその交換に要する時間が問題である。

本発明は、このような従来の欠点に鑑みなされたもので、その目的は、センサからの信号を処理する機器が変わっても、患者に負担を強いることなく、迅速にかつ簡単な操作でその機器にその信号を入力することができるようにすることである。

【課題を解決するための手段】
請求項1に係るシステムは、患者に装着される電極からコードを介して生体信号を得るための生体信号入力部を有する表示装置と、生体信号を検出するセンサ部と、前記センサ部からの生体信号を無線信号にして送出する送信手段と、を備えたセンサ装置と、

【発明の実施の形態】
図2に本発明の医用テレメータシステムの全体構成を示す。この図に示すように、患者にはセンサ装置1が装着されている。このセンサ装置1は、生体信号を検出し、その信号を無線信号にして携帯型患者監視装置2に送出するものである。携帯型患者監視装置2は、表示装置3と受信機4とから成っている。受信機4は図3に示すようにコネクタ5Aを備え、表示装置3のコネクタ5Bに対し着脱自在となっている。これらコネクタ5A,5Bが接合したときには、図2に示したように表示装置3と受信機4は1つの箱型を形成する。図4にはベッドサイドモニタとして使用される表示装置7の外観が示されている。図2に示した受信機4のコネクタ5Aは表示装置7の生体信号入力部であるコネクタ8に対しても着脱自在となっている。また、コネクタAが他のベッドサイドモニタの入力部であるコネクタ8に対して嵌合できない場合は、変換コネクタ8aを使用して着脱できるようにしてもよい。

図3にはまた、センサ装置1が拡大されて示されている。センサ装置1は、送信機6とセンサ部9とから成り、センサ部9は、1対の電極9a,9bとから成っている。

センサ装置1と受信機4のそれぞれの内部構成を図1に示す。センサ装置1のセンサ部9は、上記の構成であり、送信機6は、直流増幅部36、AC増幅部11、電極はずれ検出部12、トーン発振部13、加算回路14、変調部15、送信部16、アンテナ17および電源部18とから成っている。

AC増幅部11は、センサ部9の1対の電極9a,9bの電位を増幅する回路であり、生体信号と見做される0.05〜100Hzの信号を増幅するものである。

電極はずれ検出部12は、直流増幅部36の出力信号から電極はずれの状態を検出する回路である。すなわち被験者に装着した電極がはずれたり、被験者と電極との接触抵抗が高くなったり、被験者と電極の間の分極電圧が過大となった時、これを検出するようにする。例えば特開平6−269417号に記載されたように、直流増幅部36の出力信号が設定値を超えた場合にその旨を示す信号を出力するようにする。また、例えば実公昭62−10875号に記載された手法によっても良い。この電極はずれを示す信号はトーン発振部13に所定の周波数320Hzの信号を出力させるための制御信号となっている。

ここで、電極はずれ検出部12およびトーン発振部13が、第1の検出手段を構成し、AC増幅部11、加算回路14、変調部15、送信部16およびアンテナ17が送信手段を構成する。

次に受信機4について説明する。受信機4は、アンテナ21と、アンテナ21を介して無線信号を受信する受信部22と、受信部22が受信した信号を復調する復調部23と、復調部23の出力信号のうち0.05〜100Hzの信号を抽出するBPF(バンドパスフィルタ)24と、BPF24の出力信号を減衰または増幅させる振幅調節部25と、復調部23の出力信号のうち320Hzの信号を抽出するBPF26と、BPF26の出力信号を整流検波する整流検波器27と、整流検波器27の出力信号が設定した閾値を超えたかを判断する比較器28と、比較器28がその信号が閾値を超えたと判断した場合に出力する切替え信号により同時に切替わる1対の切替え部29a,29bを備えたスイッチ29と、スイッチ29の各切替え部29a,29bの出力端子に接続され他の装置の接続部に接続される接続部30と、電源部31とを備える。電源部31は電池であり、上記各部に必要な電力を供給するものである。

スイッチ29の切替え部29aの一方の端子には振幅調節部25の出力信号が至るようにされている。接続部30は、3つのピンからなるコネクタ5Aを備えており、そのうち+側ピンはスイッチ29の切替え部29aの他方の端子に接続され、−側ピンはスイッチ29の切替え部29bの一方の端子に接続され、アースピンはスイッチ29の切替え部29bの他方の端子に接続されている。また、スイッチ29の1対の切替え部29a,29bは、比較器28からの切替え信号がないときは常時閉となるものである。また、切替え部29a,29bは、半導体を利用したアナログスイッチ、リレー、光を利用したフォトスイッチを利用してもよい。本説明では、1対の切替え部29a,29bとしたが、切替え部29aまたは切替え部29bのいずれか一つだけ利用した構成にしてもよい。

また、振幅調節部25は、上記のように復調部23で復調された信号を減衰または増幅させるものであるが、その減衰または増幅の程度は、生体電極から直接得られる微弱な生体信号振幅のレベルとなっている。

また、受信機4は、送信機6から送信された信号を受信部22が受信しているか否かを検出する電波切れ検出部22aと、この電波切れ検出部22aが電波切れを検出したときに警報を発するアラーム100aを備えている。更に受信機4は、復調部23の出力信号のうち540Hzの信号を抽出するBPF26aと、BPF26aの出力信号を整流検波する整流検波器27aと、整流検波器27aの出力信号が設定した閾値を超えたかを判断する比較器28aと、比較器28aがその信号が閾値を超えたと判断した場合に出力する信号により警報を発するアラーム100bを備えている。更に受信機4は、電源部18の電池切れを検出する電池切れ検出部31aと、電池切れ検出部31aが電池切れを検出したときに警報を発するアラーム100cを備えている。更に受信機4は、比較器28がスイッチ29に出力する切替え信号により警報を発するアラーム100dを備えている。

次にこのように構成されたテレメータシステムの動作を説明する。最初に、患者が自由に移動できる状態にあるときに、図2に示した携帯型患者監視装置2の表示装置3によって心電図信号をモニタし、次にベッドサイドモニタの表示装置7により心電図信号をモニタする場合について説明する。

尚、表示装置3のコネクタ5B、表示装置7のコネクタ8は、患者に装着される電極からの信号をコードを介して得るための接続部(生体信号入力部)として設けられているものである。さらに、表示装置3、表示装置7は、これらコネクタ5B、コネクタ8から入力される信号に基づいて、電極はずれを検出する手段およびそれが検出された場合に警報を出すなどの手段を備えている。

まず、患者にセンサ装置1を装着する。すなわち、センサ装置1の1対の電極9a,9bを患者に装着し、送信機6の電源スイッチ(図示せず)をオンにする。ここで、図3に示したようにコネクタ5Aとコネクタ5Bとは接続され、受信機4は表示装置3に装着されて図2に示した状態となっている。また、受信機4の電源スイッチ(図示せず)もオンにしておく。

この状態において、センサ装置1では電極9a,9bから得られる心電図信号が直流増幅部36と、AC増幅部11で増幅されて、加算回路14に至る。このとき、電極はずれ検出部12が電極はずれを検出していないので、加算回路14はAC増幅部11から与えられた信号をそのまま変調部15に出力する。変調部15はその信号に基づき搬送波信号を変調し、これを送信部16に出力する。送信部16は、この被変調波信号をアンテナ17を介して送出する。

一方受信機4では送信機6から送出された信号をアンテナ21を介して受信部22で受信し、これを復調部23で復調する。このとき復調された信号は、0.05〜100Hzの心電図信号であるからBPF24によりその信号が抽出され、振幅調節部25で減衰または増幅され、スイッチ29を介して接続部30に至る。表示装置3は、この信号に基づく表示を行う。ここで、BPF24により抽出された信号は、振幅調節部25で減衰または増幅されているので、表示装置3はセンサから直接コードを介して接続された場合に与えられる信号と同じレベルの信号を得ている。

次に、センサ装置1の電極9a,9bのいずれかまたは両者が患者からはずれた場合、送信機6の電極はずれ検出部12は、その状態を検出し、制御信号をトーン発振部13に送出し、トーン発振部13に320Hzの信号を出力させる。これにより、加算回路14はAC増幅部11から与えられる信号にトーン発振部13から出力された320Hzの信号を加算する。変調部15はその信号に基づき搬送波信号を変調し、これを送信部16に出力する。送信部16は、この被変調波信号をアンテナ17を介して送出する。

一方受信機4では送信機6から送出された信号をアンテナ21を介して受信部22で受信し、これを復調部23で復調する。このとき復調された信号には、320Hzのトーン信号が含まれでいるので、この信号はBPF26で抽出され、整流検波器27で整流検波される。そしてその整流検波器27の出力信号は、比較器28において設定された閾値と比較される。比較器28は、閾値より大きいと判断すると、スイッチ29の1対の切替え部29a,29bを開とする切替え信号を出力する。このため、接続部30は振幅調節部25との接続が断たれた状態となり、この接続部30に接続された表示装置3にとっては、患者に装着された電極をコードにより直接接続された場合にその電極がはずれたときと同じ状態となる。表示装置3は、受信機4からの信号に基づいて電極はずれを検出し、警報を出すなどの処理を行う。

電極9を再度装着すれば、電極はずれ検出部12は制御信号の送出を止めるので、トーン発振部13はそのトーン信号の送出を止め、加算回路14の出力はAC増幅部11の出力信号のみとなる。このため、受信機4では、BPF26の出力がなくなり、比較器28から切替え信号が送出されなくなるので、スイッチ29の切替え部29a,29bは閉となり、振幅調節部25の出力が接続部30に至るようになる。このため、表示装置3は再度、心電図信号を表示する。

次に患者がベッドサイドに戻り、ベッドサイドモニタである表示装置7に心電図信号をモニタさせるとする。このとき、表示器3から受信機4をはずし、このコネクタ5を表示装置7のコネクタ8に接合させる。この場合においても、表示器3と同様に、あたかもコード付きの電極から得られる信号を処理するのと同様の処理が行われる。

また本システムの各装置は種々のアラームを備えており、受信機4の電源部31の電圧低下が生じると電池切れ検出部31aはこれを検出し、アラーム100cによって電池交換の必要性を示す警報を発する。また、受信部22で、センサ装置1の送信機6から送信される電波が検出できないときは電波切れ検出部22aはその旨の信号をアラーム100aに出力し、アラーム100aはその旨を示す警報を発する。

また送信機6の電源部18の電圧が低下したならば、電池切れ検出部12aはこれを検出し、トーン発振部13aは540Hzの信号を出力する。この信号は、加算回路14、変調部15、送信部16を介して心電図信号と共に電波として伝送され、そして受信機4のBPF26a、整流検波器27a、比較器28aを介して検知され、アラーム100bによって電池交換の必要性を示す警報が発せられる。

また、前述したように送信機6の電極はずれ検出部12によって電極はずれが検出されると、受信機4では比較器28がスイッチ29を断とする信号を出力するが、この信号はアラーム100dにも至り、これによってアラーム100dは電極はずれを示す警報を発する。

アラーム100a〜100dの具体例としては、図10に示すように、視覚的に知らせるLEDや液晶であっても良いし、また聴覚的に知らせるブザーなどであっても良い。

本システムを救急現場で用いた例を図5を参照して説明する。この図に示すように、まず患者にセンサ装置1(センサZ)を装着して救急車に乗せる。ここで受信機4は救急車内のモニタ装置Aに取り付け、このモニタ装置Aに心電図を表示させる。次に病院に着き、受信機4をモニタ装置AからはずしてICU内のモニタ装置Bに取り付け、このモニタ装置Bに心電図を表示させる。次に、受信機4をモニタ装置Bからはずして検査室に行き、受信機4をその部屋のモニタ装置Cに取り付け、このモニタ装置Cに心電図を表示させる。次に、受信機4をモニタ装置Cからはずしてモニタ装置Dに取り付け、このモニタ装置Dに心電図を表示させた状態で、患者を搬送する。次に手術室に着き、受信機4をモニタ装置Dからはずして手術室内のモニタ装置Eに取り付け、このモニタ装置Eに心電図を表示させる。このように患者の移動先でモニタ装置がいかように変わっても、患者に取り付けたセンサ装置1は、一切取り外すことなく、また患者に何等の負担をかけることなく、各場所に設置されているモニタ装置に心電図を表示させる。また、センサ装置1の送信周波数に対応した受信機5を病院内の各モニタ装置にあらかじめ接続しておけば受信機5の付け替えを省略できる。

次に第2の実施の形態を説明する。本実施の形態が上記第1の実施の形態と異なるのは、図6に示すように送信機6Aの構成である。すなわち、第1の実施の形態の加算回路14の代わりに選択スイッチ35を設け、通常はAC増幅部11と変調部15を接続し、次に電極はずれ検出部12が電極はずれを検出し制御信号を出力すると、AC増幅部11と変調部15との接続を断ち、トーン発振部13と変調部15とが接続されるように構成したものである。ここで、AC増幅部11、選択スイッチ35、変調部15、送信部16およびアンテナ17が送信手段を構成する。他の構成は第1の実施の形態と同じである。但し、各種アラームを発生させるための手段は省略しているが、これらの手段を付加した構成としても良い(以下の実施の形態も同様である)。このような構成によっても、第1の実施の形態と同様の作用効果が得られる。

次に第3の実施の形態を説明する。本実施の形態が上記第1の実施の形態と異なるのは、図7に示すように送信機6Bおよび受信機4Bの構成である。すなわち、送信機6Bでは、第1の実施の形態のトーン発振部13を設ける代わりに電極はずれ検出部12Bが電極はずれを検出したときに所定のDC電圧を加算回路14に与えるようにしたものである。電極はずれ検出部12Bは、直流増幅部36によって増幅されたセンサ部9の出力信号から電極はずれを検出するものである。

一方、受信機4Bでは、第1の実施の形態のBPF26および整流検波器27が無く、比較器28は復調部23からの信号を直接受け、設定された閾値(上記電極はずれ検出部12が加算回路14に印加した電圧より低い値)との比較を行うようになっている。この比較結果に応じてスイッチ29の切替え部29a,29bをオン、オフすることは第1の実施の形態と同じである。ここで、比較器28およびスイッチ29が第2の検出手段を構成する。他の構成は第1の実施の形態と同じである。本実施の形態によれば全体の構成が簡単になるという効果の他、第1の実施の形態と同様の作用効果が得られる。

次に第4の実施の形態を説明する。本実施の形態は、検出した心電図信号をデジタル化して処理したものである。本実施の形態のセンサ装置1Cおよび受信機4Cの構成を図8に示す。この図に示すように、センサ装置1Cは、センサ部9Cと送信機6Cとから成る。センサ部9Cは上記の各実施の形態と同じである。送信機6Cは、センサ部9Cの出力信号を増幅する直流増幅部36とAC増幅部11と、直流増幅部36の出力信号から電極はずれを検出しその旨のデジタル信号を出力する電極はずれ検出部38と、増幅部37の出力信号をデジタル化するA/D変換部39と、電極はずれ検出部38およびA/D変換部39からの信号を時分割多重化する時分割多重化部40と、時分割多重化部40の信号により搬送波信号を変調する変調部41と、変調部41からの信号を無線信号として外部に送出する送信部42およびアンテナ43と、各部に必要な電力を供給する電源部44から成る。

上記の各実施の形態において、受信機4、4B,4Cは、種々の他の生体信号入力部に接続され、一定の位置に固定されないためどんな状況でも受信感度を確保する必要がある。そこで、より確実な受信を行うために偏波ダイバーシティ機能を有し、様々な他の機器に接続されても安定な受信を確保するようにする。例えば、図11に示すように、2種類のアンテナ21a,21bと、これらの切り替えるアンテナ切替え部21cを備える構成とし、電波切れ検出部22aが電波切れを検出すると、アンテナ切替え部21cによってアンテナ21a,21bを切り替えるようにする。

また図12の分解図、図13の外観図に示す受信機の例は、小型チップ誘電体アンテナ21a,21bを異なる方向、例えばお互いが直交する向きに実装し、アンテナ21aに近接する回路基板200aのグランド方向と、アンテナ21bに近接する回路基板200bのグランド方向がお互いに直交するように筐体4A(1対のケース4a,4bから成る)の上面と側面から突出するように配置し、お互いの指向性を補うようにしたものである。

ここで小型チップ誘電体アンテナ21a,21bは、送信周波数の波長に対して数十分の一程度の大きさであり、モノポールアンテナ(波長の四分の一)に対してかなり小型である。アンテナ21a,21bは、数ミリ〜数センチメートルのキャップで保護されて利便性を損なわないようにしている。このような受信機を図1に示した実施の形態に対応させると、回路基板200a,200bには、受信部22、復調部23、BPF26、整流検波器27、比較器28、振幅調節部25、スイッチ29、電波切れ検出部22a、BPF26a、整流検波器27a、比較器28a、電池切れ検出部31aが配置される。
