ベクター
操作されたヒトPGRN cDNAを、サイトメガロウイルス初期エンハンサーを有するニワトリベータアクチンプロモーター、キメライントロン、およびウサギβグロビンポリアデニル化配列を含む発現構築物にクローニングした(図1)。第2の操作されたヒトPGRN cDNAを、ヒトユビキチンCプロモーターを含む発現構築物にクローニングした。発現構築物は、AAV2逆位末端反復に隣接した。アデノ随伴ウイルス血清型1、5およびヒト68(AAVhu68)は、前述のようにHEK293細胞の三重トランスフェクションおよびイオジキサノール精製によって、この構築物から生成された(Lock M,et al.Hum Gene Ther.2010;21(10):1259-71)。
すべての動物プロトコルは、the Institutional Animal Care and Use Committee of the University of Pennsylvaniaにより認可された。つがいのGRNノックアウトマウスを、The Jackson laboratory(ストック番号013175)から購入し、コロニーをペンシルベニア大学で維持した。野生型C57BL/6(ストック番号000664)は、対照として機能した。最初の研究では、2ヶ月齢のマウスをイソフルランで麻酔し、1×1011ベクターゲノムコピー(GC)を5μLの量で側脳室(ICV)に注入した。注入60日後、マウスを、ケタミン/キシラジン麻酔下、失血により安楽死させ、頚椎脱臼により、死亡を確認した。第2の研究では、マウスは、7ヶ月齢で処置され、11ヶ月齢で犠牲にされた。剖検時に、血清を、心臓穿刺により採取し、CSFを、ポリエチレンチューブに連結された32ゲージ針を用いて、後頭下穿刺により採取した。血清およびCSF試料を、ドライアイス上で即時凍結し、分析時まで-80度で保存した。前頭皮質を、生化学用に回収し、ドライアイス上で即時凍結し、残りの脳は、組織学用に10%のホルマリンで固定した。

GRN-/-マウスにおける所見は、AAVベクターのCSFへの送達が、治療レベルのPGRNを産生し、PGRN欠乏に関連する生化学的および組織学的所見を予防または逆転させるのに十分な脳形質導入を達成できることを実証する。このアプローチをヒトに置き換えるために、臨床的に関連するベクター投与経路であるICM送達を使用して、非ヒト霊長類で研究を実施した。大槽内への注入による髄腔内AAV送達は、低侵襲性のアプローチであり、腰椎穿刺による投与よりも広範な脳形質導入をもたらす(Hinderer C,et al.Molecular therapy Methods&clinical development.2014;1:14051)。3~10歳のNHPを利用したのは、この年齢が意図される成人患者集団を表すためである。アカゲザル(1群あたりN=2)は、AAV1、AAV5、またはAAVhu68ベクターのイメージガイドによる単回ICM注入が投与され、これらのベクターは、ニワトリBAプロモーターおよびCMV IEエンハンサー(CB7プロモーターと称される)の制御下で、hPGRN(配列番号3)と称される導入遺伝子からヒトGRNを発現する。さらなる群が、ユビキチンCプロモーター(UbC)から発現される異なる操作導入遺伝子配列(hPGRN v2、配列番号4)を担持するAAVhu68ベクターで処置された。CSF中のヒトプログラニュリンのレベルは毎週測定され、注入の35日後、組織病理学の分析のために、動物が犠牲になった。予備的な安全性分析が実施され、毎日のケージぎわでの観察、一連の身体検査、全血球数、血清化学パネル、CSF化学検査および細胞診断、および脳および脊髄の顕微鏡評価を伴う完全な剖検が含まれた。以下の表に、処置群を要約する。

入の異なるパターン
本研究者らは、AAV1ベクターで処置されたNHPのCSFにおける著しく高いPGRN発現から、AAV1、AAV5、およびAAVhu68ベクターの形質導入パターンの差異をさらに探求することにした。GFPレポーター遺伝子を発現するAAV1、AAV5、またはAAVhu68ベクター(3×1013GC、ベクターあたりn=2)の単回ICM注入がNHPに投与された。脳形質導入の組織学的分析のために、注入の28日後に動物が犠牲になった。
rAAV1.PGRNは、1)AAV ITRに隣接する導入遺伝子カセットをコードするAAVシスプラスミド(pENN.AAV.CB7.CI.hPGRN.rBG.KanRと称される)、2)AAV2 repおよびAAV1 capの遺伝子をコードするAAVトランスプラスミド(pAAV2/1.KanRと称される)、および3)ヘルパーアデノウイルスプラスミド(pAdΔF6.KanRと称される)を用いたHEK293細胞の三重プラスミドトランスフェクションによって生成される。rAAV1.PGRNにパッケージングされたベクターゲノムのサイズは、4129塩基である。
シスプラスミド(pENN.AAV.CB7.CI.hPGRN.rBG.KanR(p4862)と称される)由来のベクターゲノムの線形マップは、図2を参照されたい。
1.逆位末端反復(ITR):ITRは、同一の逆相補配列であり、AAV2(130塩基対[bp]、GenBank:NC_001401)は、ベクターゲノムのすべての要素に隣接する。ITR配列は、AAVおよびアデノウイルスヘルパー機能がトランスで提供される場合、ベクターDNAの複製起点およびベクターゲノムのパッケージングシグナルの両方として機能する。したがって、ITR配列は、ベクターゲノムの複製およびパッケージングに必要とされるシス配列のみを表す。
2.ヒトサイトメガロウイルス最初期エンハンサー(CMV IE):このエンハンサー配列は、ヒト由来のCMV(382bp、GenBank:K03104.1)は、下流の導入遺伝子の発現を増加させる。
3.ニワトリβアクチンプロモーター(BA):この遍在性プロモーター(282bp、GenBank:X00182.1)を選択して、任意のCNS細胞型における導入遺伝子の発現を駆動させた。
4.キメライントロン(CI):ハイブリッドイントロンは、ニワトリβアクチンスプライスドナー(973bp、GenBank:X00182.1)およびウサギβグロビンスプライスアクセプターのエレメントを含む。イントロンは転写されるが、その両端のいずれかにある配列と合わせて、スプライシングによって成熟メッセンジャーRNA(mRNA)から除去される。発現カセットにおけるイントロンの存在は、核から細胞質へのmRNA輸送を促進し、それにより、翻訳のためのmRNAの一定レベルの蓄積を高めることが示されている。これは、遺伝子発現レベルの増加を意図した遺伝子ベクターにおいて共通の特徴である。
5.コード配列:ヒトGRN遺伝子の操作cDNAは、ヒトPGRN(hPGRN)タンパク質をコードし、リソソーム機能および他の神経系の役割に関与する(1785bp、GenBank:NM_002087.3、593アミノ酸[aa]、GenBank:NP_002078)。
6.ウサギβグロビンのポリアデニル化シグナル(rBG PolyA):rBG PolyAシグナル(127bp、GenBank:V00882.1)は、cisで導入遺伝子のmRNAの効率的なポリアデニル化を促進する。このエレメントは、転写終結、新生転写物の3’末端での特異的切断事象、および長いポリアデニル鎖付加のためのシグナルとして機能する。
AAV2/1トランスプラスミドは、pAAV2/1.KanR(p0069)である。pAAV2/1.KanRプラスミドは、鎖長が8113bpであり、AAVベクターゲノムの複製およびパッケージングに必要とされる4つの野生型AAV2レプリカーゼ(Rep)タンパク質をコードする。pAAV2/1.KanRはまた、3つの野生型AAV1ビリオンタンパク質カプシド(Cap)タンパク質をコードし、AAVベクターゲノムを収容するためにAAV血清型1(AAV1)のビリオンシェルに組み立てられる。pAAV2/1.KanRに含まれるAAV1 cap遺伝子は、サル源から単離された。
V p5プロモーター(通常rep発現を駆動する)を、repの5’末端からcapの3’末端へと移し、repの上流の切断型p5プロモーターを残す。この切断型プロモーターは、repの発現を下方制御し、結果として、ベクター産生を最大化する役割を果たす(Xiao et al.,(1999)Gene therapy vectors

プラスミドpAdDeltaF6(KanR)は、ペンシルベニア大学のJame M.Wilson博士および同僚の研究室内で構築され、サイズは15,774bpである。このプラスミドには、AAV複製に重要なアデノウイルスゲノムの領域、すなわちE2A、E4、およびVA RNAが含まれている(アデノウイルスE1の機能は、HEK293細胞によって提供される)。しかしながら、このプラスミドには、他のアデノウイルスの複製遺伝子または構造遺伝子は含まれていない。プラスミドは、アデノウイルスITRなどの複製に重要なシスエレメントを含んでおらず、したがって、感染性のアデノウイルスが産生されることは予想されない。プラスミドは、Ad5のE1、E3欠失分子クローン(pBHG10、pBR322ベースのプラスミド)に由来した。Ad5に欠失を導入して、不要なアデノウイルス遺伝子の発現を除去し、かつアデノウイルスDNAの量を削減した(32kbから12kbへ)。最後に、アンピシリン耐性遺伝子をカナマイシン耐性遺伝子によって置換して、pAdeltaF6(KanR)を作製した。このプラスミドに残るE2、E4、およびVAIのアデノウイルス遺伝子は、HEK293細胞に存在するE1と共に、AAVベクター産生に必要である。ベクターは、図13および図14に示される以下のフローチャートに従って生成される。
rAAV1.CB7.CI.hPGRN.rBGは、血清型AAV1からなる非複製組換えアデノ随伴(AAV)ベクターであり、これは、サイトメガロウイルスエンハンサーを有するニワトリベータアクチンプロモーターの制御下にある操作されたヒトプログラニュリン(PGRN)cDNAと、AAV血清型2逆位末端反復に隣接するウサギβグロビンポリアデニル化配列とを含み、髄腔内最終製剤緩衝液(ITFFB)中で製剤化された。ddPCR力価は、2.04×1013GC/mLであった。ITFFBのpHをpH7.4に調整して、試験物品生成物の溶解度を最大化した。対照物品は、投与日に調製した。希釈された物品は、同日の投薬まで、湿った氷上または2~8℃で保たれる。
動物は、ケタミン/デクスメデトミジンの組み合わせで鎮静された。鎮静化された動物を、横臥位または仰臥位で手術台に配置し、ヒートパックで体温を維持した。電子加温装置は、電気信号の取得に干渉する可能性があるため、推奨されなかった。
GRN遺伝子の変異によって引き起こされる成人発症性神経変性疾患を有する患者におけるrAAV1.hPGRNの単回投与のヒト初回(FIH)第1/2相用量漸増試験を実施する(以下の表を参照)。rAAV1.hPGRNは、GRN遺伝子を置き換えるように設計される。このFIH試験は、安全性および耐容性を評価し、有効性に関する予備データを収集する。最大12名の症候性ヘテロ接合性GRN変異保因患者をrAAV1.hPGRNで治療し、最初に2年(24ヶ月)の期間にわたって追跡し、投薬後5年間にわたって長期フォローアップを継続する。この試験は、登録用試験の開始を支持するデータを提供し、第1/2相最大耐容用量(MTD)を利用する。この登録用試験は、疾患に関連する臨床転帰およびバイオマーカーに対するrAAV1.hPGRNの効果を評価する。すべての試験は、GRN関連神経変性疾患を有する成人患者における単回ICM用量のrAAV1.hPGRNの投与を含む。
rAAV1.hPGRNは、単回用量として、1日目に、大槽内へのCTガイドによる後頭下注入を介して対象に投与される。1日目に、5.6mLの適切な力価でrAAV1.hPGRNを含むシリンジが、試験に関連する治験薬剤部(Investigational Pharmacy)によって調製され、処置室に送達される。
ヨード造影剤を髄腔内(IT)注入して、大槽の関連する解剖学的構造の可視化を補助する。静脈内(IV)造影剤は、IT造影剤の代替物として、針穿刺の前またはその間に投与され得る。IVまたはIT造影剤を使用するかどうかの決定は、介入医の判断に委ねられる。透視ガイド下、脊髄針(22~25G)を大槽内に進める。針の配置を補助するために、より大きな誘導針を使用してもよい。針の配置を確認した後、エクステンションセットを脊椎針に取り付け、CSFで充填する。介入医の裁量で、造影剤を含有するシリンジを、エクステンションセットに接続してもよく、少量注入することで、大槽内に針が配置されていることを確認することができる。針の配置を確認した後、rAAV1.hPGRNを含むシリンジをエクステンションセットに接続する。シリンジの内容物を1~2分かけてゆっくりと注入し、5.0mLの容量を送達する。
C)パネルを通してスクリーニングされ、患者は、5年間のフォローアップの時点で、脳および上位脊椎のガドリニウム造影剤を用いたMRIを介して、腫瘍についてモニタリングされる。
主に認知機能を測定する臨床的尺度
CDR-FTLD:CDR-FTLDは、ADスペクトル障害の重症度を評価するために歴史的に使用される古典的な臨床認知症評価(CDR)尺度の拡張版である。この評価には、CDRのオリジナルな6つのドメイン(記憶、見当識、判断力と問題解決、社会適応、家庭状況と趣味、介護状況)が含まれる。それはまた、より高い感度でFTLDスペクトル患者の減少の検出を可能にする、言語および行動の2つの追加のドメインを含む。0の評価スコアは、正常な行動または言語を示し、1、2、または3のスコアは、軽度から重度の障害を示す。CDR-FTLD評価項目合計(CDR-FTLD sb)は、個々のドメインの合計を表し、全体的な認知症の重症度を決定するために使用される。
UPDRS:UPDRSは、精神状態、行動および気分、ならびに日常生活での活動など、パーキンソニズムに関連するいくつかのドメインの42項目の4部構成の評価である。各項目には、0(障害なしを示す)から4(最も重度の障害を示す)の範囲の評価尺度が含む。各部のスコアは、疾患の重症度を提供するために集計され、高スコアの199点は、最悪/最も重度の障害を示す。
NPI:NPIは、脳の障害を有する患者における精神病理の存在を解明するために使用される。当初、これはAD集団で使用するために開発されたが、他の条件における行動変化を評価するのに役立つと考えられている。この評価は、10の行動ドメインと2つの自律神経領域で構成され、その中には、4つのスコア:頻度、重症度、総合的な負担、および介護者の負担がある。NPIの総スコアは、行動ドメインのドメインスコアを加算し、介護者負担のスコアを減算することによって得られる。
CGI-C:CGI-Cは、3部の簡潔で広く使用されている評価のうちの1つである。これは、3つの項目で構成され、臨床医によって評価される。CGI-Cは、改善が完全に治療によるものであるかどうかにかかわらず、研究への登録から始まり、1(非常に改善された)から7(非常に悪化した)の範囲の7段階で評価される。
