Abstract

本出願は、「脊髄性筋萎縮症のための併用療法」と題する2018年8月15日出願の米国仮出願第62/764,893号、及び「脊髄性筋萎縮症のための併用療法」と題する2018年12月20日出願の米国仮出願第62/783,189号の出願日の利益を米国特許法第119条(e)の下で主張する。それぞれの出願の内容は、その全体が参照によって本明細書に組み込まれる。
いくつかの態様では、SMAを治療するための併用療法は、SMN1をコードする組換え核酸(例えば、ウイルスベクター(rAAVなど)において投与されるもの)を含む。いくつかの態様では、SMN1をコードする組換え核酸(本明細書では、組換えSMN1遺伝子とも称される)は、プロモーター(例えば、運動ニューロン中で活性なプロモーター)に機能可能なように連結されたSMN1遺伝子を含む。いくつかの態様では、SMN1をコードする組換え核酸は、非ウイルスベクター(例えば、非ウイルスプラスミド)において提供される。一方で、いくつかの態様では、SMN1をコードする組換え核酸は、組換えウイルスベクター(例えば、ウイルスカプシド内にパッケージングされた組換えウイルスゲノム)において提供される。いくつかの態様では、組換えSMN1遺伝子は、組換えアデノ随伴ウイルス(rAAV)ゲノムにおいて提供され、AAVカプシド粒子内にパッケージングされる。

いくつかの態様では、発現コンストラクトは、SMN1遺伝子のコード配列の発現を促進する配列(例えば、コード配列に機能可能なように連結された発現制御配列)を含む1つ以上の領域を含む。発現制御配列の例としては、限定されないが、プロモーター、インスレーター、サイレンサー、応答エレメント、イントロン、エンハンサー、開始部位、終結シグナル、及びポリ(A)尾部が挙げられる。本明細書では、そのような制御配列の任意の組み合わせ(例えば、プロモーター及びエンハンサー)が企図される。

いくつかの態様では、SMAを治療するための併用療法は、SMN2をコードするpre−mRNAに相補的なASO(本出願ではSMN2 ASOとも称される)を含む。いくつかの態様では、ASOは、全長SMN2 mRNAを増加させる。いくつかの態様では、ASOは、SMN2 pre−mRNAのスプライシングを変化させる。いくつかの態様では、ASOは、SMN2 mRNA中にエクソン7が含まれることを促進する。SMN2のスプライシングを変化させる上で有用ないくつかの配列及び領域については、PCT/US06/024469(WO/2007/002390として公開されている)及びWO2018014041A2において見つけることができ、これらの文献は、それらの全体があらゆる目的のために参照によって本明細書に組み込まれる。

いくつかの態様では、アンチセンスオリゴヌクレオチドは、1つ以上の修飾ヌクレオシドを含む。そのような修飾ヌクレオシドは、修飾糖及び/または修飾核酸塩基を含み得る。いくつかの態様では、そのような修飾ヌクレオシドをオリゴヌクレオチド中に含めることで、限定されないが、ヌクレアーゼによる分解に対する抵抗性の向上、及びまたは修飾オリゴヌクレオチドの毒性及び/または取り込み特性の改善を含めて、標的核酸に対する親和性の増加及び/または安定性の向上が得られる。
ヌクレオシドの天然起源の塩基部分は、ヘテロ環式塩基であり、典型的には、プリン及びピリミジンである。「非修飾」核酸塩基または「天然」核酸塩基(プリン核酸塩基(アデニン(A)及びグアニン(G))、ならびにピリミジン核酸塩基(チミン(T)、シトシン(C)、及びウラシル(U)))に加えて、当業者に知られる修飾核酸塩基または核酸塩基模倣体の多くが、本明細書に記載の化合物への組み込みに適している。いくつかの態様では、修飾核酸塩基は、構造が親核酸塩基にかなり類似した核酸塩基であり、こうした核酸塩基は、例えば、7−デアザプリン、5−メチルシトシン、またはG−クランプなどである。いくつかの態様では、核酸塩基模倣体は、より複雑な構造を含む(例えば、三環式フェノキサジン核酸塩基模倣体など)。修飾核酸塩基を調製するための方法は当業者によく知られている。
本出願のアンチセンスオリゴヌクレオチドは、天然の糖と比較して糖部分が修飾されたヌクレオシドを任意選択で1つ以上含み得る。糖修飾ヌクレオシドを含むオリゴヌクレオチドは、ヌクレアーゼに対する安定性が増進しているか、結合親和性が増加しているか、または何らかの他の有益な生物学的特性を有し得る。そのような修飾には、限定されないが、置換基を付加するもの、非ジェミナル環原子を架橋して二環式核酸(BNA)を形成させるもの、リボシル環酸素原子をS、N(R)、またはC(R1)(R)2(R=H、C1−C12アルキル、または保護基)で置き換えるもの、及びこうした修飾の組み合わせが含まれ、こうした修飾の組み合わせは、例えば、2’−F−5’−メチル置換ヌクレオシド(他の開示の5’,2’−ビス置換ヌクレオシドについては2008年8月21日公開のPCT国際出願WO2008/101157を参照のこと)、またはリボシル環酸素原子をSで置き換えたものに対してさらに2’位の置換を行うもの(2005年6月16日公開の公開米国特許出願US20050130923を参照のこと)、あるいはBNAの5’位を置換するもの(2007年11月22日公開のPCT国際出願WO2007/134181(この文献では、LNAが、例えば、5’−メチル基または5’−ビニル基で置換されている)を参照のこと)などである。
アンチセンスオリゴヌクレオチドは、1つ以上の修飾ヌクレオシド間結合を任意選択で含み得る。連結基の2つの主要クラスは、リン原子の存在有無によって規定される。代表的なリン含有結合には、限定されないが、ホスホジエステル(P=O)、ホスホトリエステル、メチルホスホネート、ホスホロアミデート、及びホスホロチオエート(P=S)が含まれる。代表的な非リン含有連結基には、限定されないが、メチレンメチルイミノ(−CH2−N(CH3)−O−CH2)、チオジエステル(−O−C(O)−S−)、チオノカルバメート(−O−C(O)(NH)−S−)、シロキサン(−O−Si(H)2−O−)、及びN,N’−ジメチルヒドラジン(−CH2−N(CH3)−N(CH3)−)が含まれる。非リン連結基を有するオリゴヌクレオチドは、オリゴヌクレオシドと称される。天然のホスホジエステル結合と比較して改変されている結合は、オリゴヌクレオチドのヌクレアーゼ抵抗性を変化させるために使用され得る(典型的には、向上させるために使用され得る)。いくつかの態様では、キラル原子を有する結合がラセミ混合物(別々のエナンチオマー)として調製され得る。代表的なキラル結合には、限定されないが、アルキルホスホネート及びホスホロチオエートが含まれる。リン含有結合及び非リン含有結合の調製方法は、当業者によく知られている。

マイクロ浸透圧ポンプ(ALZET Osmotic Pumps,Cupertino,Calif.,USA)を使用することで、ヒトSMN2導入遺伝子を有する新生仔(P0〜P1)SMAマウスの脳脊髄液(CSF)に右側脳室を介してヌシネルセン及びAAV−SMN1が送達される。低用量または高用量のヌシネルセン(それぞれ1μg及び4μg)が、低用量または高用量のAAV−SMN1(それぞれ1×1010GCまたは8×1010GC)と共に出生時点(P0〜P1)のマウスに投与される。マウスの体重及び立ち直り反射が測定され、ヌシネルセンまたはAAV−SMN1のいずれかが単独投与されている同じ遺伝子型の対照マウスの体重及び立ち直り反射と比較される。

SMAマウス、SMAアカゲザル、及びSMAカニクイザルを使用することで、異なる用量及び投与経路でのヌシネルセン及びAAV−SMN1の組成物の分布が評価される。ヌシネルセン及びAAV−SMN1の組成物は、脳室内(ICV)注入またはくも膜下腔内(IT)注入によって約1mg/kgの用量で何匹かのマウス及び何匹かのサルに24時間にわたって投与される。注入時間の終了から96時間後に動物が屠殺され、組織が回収される。脊髄の頸部、胸部、及び腰部に由来する試料中のヌシネルセン及びAAV−SMN1の濃度が測定される。

ヒトSMN2導入遺伝子を有する新生仔(P0〜P1)SMAマウスにヌシネルセン及びAAV−SMN1を投与した。低用量または高用量のヌシネルセン(それぞれ1μg及び3μg)を、低用量または高用量のAAV−SMN1(それぞれ1×1010GCまたは3×1010GC)と共に、出生時点(P0〜P1)のマウスに投与した。マウスの体重及び立ち直り反射を測定し、ヌシネルセンまたはAAV−SMN1のいずれかが単独投与されている同じ遺伝子型の対照マウスの体重及び立ち直り反射と比較した。
