図1は、1つまたは複数の多能性幹細胞からの、巨核球前駆細胞(preMK)、巨核球(MK)、および血小板(PLT)のスケーラブルな分化に関する全体的な概略図を示す。しかし、注目すべきことに、本プロセスは多能性幹細胞に関連して記載されているが、様々な実施形態では、多能性幹細胞は、他のタイプの幹細胞で置換されてもよいし、またはそれが補充されてもよい。一部の実施形態では、2次元(2D)培養は、図2で示されるように、前駆体、巨核球、および血小板を生成するのに使用される。本明細書で開示された他の実施形態は、図3で示されるような、所望の細胞を産生するための3次元培養を利用するプロセスを記載する。
マトリクス依存性増大培養
マトリクス依存性増大培養に関しては、臨床グレード多能性幹細胞(PSC)を、多能性幹細胞培養培地中で、支持用マトリクス上でフィーダー細胞を伴わずに培養することによってコロニーとして増大させることができる。支持用マトリクスは、細胞の付着を可能にする2次元表面または3次元構造であってもよい。一部の実施形態では、臨床グレードヒト人工多能性幹細胞は、ヒト人工多能性幹細胞(iPSC)であってもよいが、他のタイプの多能性幹細胞、例えば胚性幹細胞、または他の幹細胞を使用することもできる。
図3および4は、3Dマトリクスを使用したpreMK細胞の定方向分化の概略図を提示する。マトリクス非依存性3D増大培養に関して、臨床グレードPSCは、自己凝集性スフェロイドとして増大させることができる。一部の実施形態では、これは、1ml当たり約10万から約150万個の密度で単一細胞を播種することによって達成することができる。例えば、一部の実施形態では、単一細胞は、1ml当たり50万個で播種することができる。
一部の実施形態では、分化の準備をするために、PSC凝集体は、マトリクス依存性2D培養物からPSCコロニーを部分的に解離させることによって、マトリクス非依存性3D培養物からPSCスフェロイドを部分的に解離させることによって、または当技術分野で公知のあらゆる方法によって生成した単一のPSCの自己凝集によって生成することができる。一部の実施形態では、分化を開始させる前に、これらの凝集体は、多能性幹細胞培養培地、例えば、これだけに限定されないが、Essential 8培地(ThermoFisher)、StemFlex培地(Thermofisher)、またはNutriStem培地(Biological Industries)中で生成することができる。一部の実施形態では、培地は、ROCK阻害剤、例えば、これだけに限定されないが、Y27632、H1152、またはそれらの組合せを含んでいてもよい。一部の実施形態では、培地は、可溶性ラミニン、例えば、組換えラミニン521を含んでいてもよい。一部の実施形態では、培地は、上皮増殖因子ファミリーメンバー、例えば、ヘレグリン−ベータ−1を含んでいてもよい。一部の実施形態では、ヘレグリン−ベータ−1培地は、24時間未満(例えば、18〜22時間)で使用される。一部の実施形態では、分化を開始させる前に、細胞は、37℃、5%CO2、20%O2で0から72時間の間培養することができる。
ステージ1において、調製されたPSC凝集体は、造血性内皮細胞に分化させることができる。簡単に述べると、多能性幹細胞培養培地の一部または全てを取り出し、ステージ1分化培地と置き換える。一部の実施形態では、ステージ1分化培地は、基本培地として、例えば、骨形成タンパク質4(BMP4)、塩基性線維芽細胞増殖因子(bFGF)、および血管内皮増殖因子(VEGF)などの1種または複数の増殖因子が補充された、StemSpan(商標)−ACF(STEMCELL Technologies、カタログ番号09855)を含む、動物構成成分不含の培地(ACF)であってもよい。一部の実施形態では、基本培地は、それぞれ1から200ng/mlの間の、BMP4(例えば、50ng/ml)、bFGF(例えば、50ng/ml)、およびVEGF(例えば、50ng/ml)のうちの1つまたは複数が補充されている。一部の実施形態では、ステージ1の培地は、WNTアゴニスト(例えばCHIR98014またはCHIR99021)、ラミニン(例えば組換えラミニン521)、またはそれらの任意の組合せがさらに補充されていてもよい。一部の実施形態では、細胞は、低酸素条件(例えば、37℃、5%CO2、5%O2)で2日から6日の間、続いて正常酸素下(37℃、5%CO2、20%O2)で2日から6日の間、インキュベートすることができる。一部の実施形態では、BMP4は、最初の6〜48時間にわたり(例えば、24時間)添加されてもよく、ステージ1の残りではなくてもよい(図28)。一部の実施形態では、VEGFおよびbFGFは、ステージ1の最初の6〜48時間(例えば、24時間)はなくてもよく、その後に添加されてもよい(図28)。一部の実施形態では、毎日の完全培地交換は、使用済みの培地の除去と新鮮なステージ1の培地での置き換えによって、ステージ1にわたり実行してもよい。一部の実施形態では、部分的な培地交換は、使用済みの培地の10〜99%を除去し、等体積の新鮮なステージ1の培地で置き換えることによって実行してもよい。一部の実施形態では、追加の体積の新鮮な培地を添加してもよく、この場合、培養物の総体積を増大させるという正味の影響が伴う。一部の実施形態では、新鮮なステージ1の培地の置き換えまたは添加の代わりに、具体的な培地構成成分が培養物にスパイクされる。
一部の実施形態では、巨核球前駆細胞(ステージ2)分化の開始は、4日から8日のステージ1の後に実行することができる。簡単に述べると、ステージ1の培地の一部または全てを取り出し、体積のステージ2の培地、例えば、STEMdiff(商標)APEL(商標)2基本培地(STEMCELL Technologies、カタログ番号05275)などと置き換える。このようなステージ2の培地は、それぞれ1および200ng/mlの、幹細胞因子(SCF)(例えば、25ng/ml)、トロンボポエチン(TPO)(例えば、25ng/ml)、Fms関連チロシンキナーゼ3リガンド(Flt3−L)(例えば、25ng/ml)、インターロイキン−3(IL−3)(例えば、10ng/ml)、インターロイキン−6(IL−6)(例えば、10ng/ml)、およびヘパリン(例えば、5単位/ml)のうちの1つまたは複数が補充されていてもよい。一部の実施形態では、ステージ2の培地は、UM171、UM729、SR−1、SU6656、ラミニン(例えば組換えラミニン521)、またはそれらの任意の組合せがさらに補充されていてもよい。
一部の実施形態では、成熟巨核球の分化は、上述の通りに生成したPSC由来preMKを使用して開始させることができる。新鮮な、または解凍した巨核球前駆細胞は、例えば、StemSpan(商標)−ACFを含むステージ3培地中の、非接着性表面上に播種することができる。非接着性表面は、大多数の細胞がこのような表面に張り付いたりまたはくっついたりせず、その代わりに大部分が懸濁物中に留まることが意図された表面を指す。例えば、このような表面は、表面への細胞の接着を防止または最小化するために、「超低粘着性プラスチック」製であってもよく、または細胞外マトリクスタンパク質でコーティングされていないものでもよい。一部の実施形態において、ステージ3の培地は、それぞれ0から200ng/mlの、TPO(例えば、25ng/ml)、SCF(例えば、25ng/ml)、IL−6(例えば、10ng/ml)、IL−9(例えば、10ng/ml)、ヘパリン(例えば、5単位/ml)、およびROCK阻害剤(例えば、5μMのY27632)のうちの1つまたは複数が補充されていてもよい。一部の実施形態では、ステージ3の培地はまた、UM171、UM729、SR−1、SU6656、またはそれらの任意の組合せが補充されていてもよい。
ステージ3の後、血小板は、成熟MKによって培養培地に産生される。これらの血小板は、回収し、定量化し、フローサイトメトリー、電子顕微鏡法、および蛍光顕微鏡法によって評価することができ、それにより、その真正な血小板としての正体が確認される(図44Aおよび44B)。一部の実施形態では、血小板は、4〜5日後に放出される。
充填層バイオリアクター
一部の実施形態では、3Dのスケーラブルな充填層バイオリアクターは、preMK、巨核球、血小板、または巨核球および血小板のうちの1つまたは複数の産生のために使用することができる。一部の実施形態では、充填層バイオリアクターは、ステージ1およびステージ2培養に使用することができる(図4)。例えば、充填層リアクターは、PSCの造血性内皮細胞への分化、続いてpreMKの産生に使用することができる。充填層リアクターキャリアは、マイクロサイズまたはマクロサイズのいずれであってもよく、生体適合性プラスチック、金属、ガラス、または天然材料、例えばアルギネートから形成することができる。一部の実施形態では、キャリアは、PTFEから、ラシヒリングの形状に、例えば1mmのラシヒリングに形成される。一部の実施形態では、キャリアは、上述したマトリクスでコーティングされていてもよい。一部の実施形態では、キャリアは、ラミニン、例えば組換えヒトタンパク質であるラミニン521でコーティングされていてもよい。一部の実施形態では、多能性細胞は、凝集塊としてキャリア上に播種することができる。一部の実施形態では、培地は、毎日の培地交換で、取り出して、ステージ1の培地で置き換えることができる。一部の実施形態では、ステージ1の間、多能性細胞は、充填層リアクター中のキャリアの内部に成長領域を示し得る。一部の実施形態では、初期における多能性細胞の造血性内皮への分化(すなわち定方向分化のステージ1)、加えて、preMKのさらなる分化および放出(すなわち定方向分化のステージ2)は、同じ容器で行うことができる。例えば、充填層バイオリアクターは、多能性細胞、例えばiPSCが播種された、ラミニン−521でコーティングされたマクロキャリアを含んでいてもよい。次いで充填層は、培地の連続流に曝露されてもよく、それにより造血性内皮へのステージ1分化が可能になる。培地は、充填層に浸透させた後、馴化チャンバーに循環させることができ、そこで新鮮な培地構成成分が添加され、酸素/CO2濃度は、スパージングまたは他の手段を介して調整されてもよく、その後、培地は、細胞に再循環することができる。
一部の実施形態では、ある特定のプロセスステップは、スケーラブルな3D解決策を使用して行ってもよく、このステップは、撹拌または振とうされた容器中で懸濁した自己凝集性スフェロイドを使用して分化を実行することを含み得る(図3)。一部の実施形態では、このような容器は、表面上の特異的および非特異的な細胞固定を阻害することで、細胞を懸濁状態にするために、親水性または中性電荷を有するコーティングで表面コーティングされた低接着性表面または非接着性表面を含んでいてもよい。多能性細胞は、単一細胞に解離し、多能性幹細胞培養培地、例えば、これだけに限定されないが、Essential 8培地(ThermoFisher)、StemFlex培地(Thermofisher)、またはNutriStem培地(Biological Industries)中に再懸濁することができる。一部の実施形態では、維持培地は、ROCK阻害剤、例えば、これだけに限定されないが、Y27632、H1152、またはそれらの組合せなどが補充されてもよい。一部の実施形態では、培地は、上皮増殖因子ファミリーメンバー、例えば、ヘレグリン−ベータ−1を含んでいてもよい。一部の実施形態では、培地は、可溶性ラミニン、例えば、組換えラミニン521を含んでいてもよい。一部の実施形態では、ヘレグリン−ベータ−1の使用は、24時間未満(例えば、18〜22時間)に制限される。次いで多能性細胞は、低接着性または非接着性容器中でインキュベートし、標準的な培養条件(例えば、37C、5%CO2、20%O2)で撹拌に供されてもよい。撹拌を提供するための一部の実施形態において、インキュベーション容器は、一定撹拌で、オービタルシェーカー、またはシェーカーフラスコまたはスピナーフラスコ上に設置されていてもよいし、または制御撹拌槽バイオリアクターを使用してもよい。24時間以内に、多能性細胞が自己凝集して、直径およそ50〜150umのスフェロイドを形成することができる。撹拌を中断すると、スフェロイドは、容器の底部に沈降することができる。
図5〜7を参照して、定義された細胞型(例えば、ステージ2からの巨核球前駆細胞、ステージ3からの成熟巨核球、ステージ4からのプレ血小板、およびステージ4からの血小板)は、治療用組成物を保持または発現するように改変することができる。一部の実施形態では、これらの細胞型は、治療用組成物の封入および/または表面コンジュゲーション(他の方法のなかでも)の目的のために回収される。一部の実施形態では、治療用組成物は、小分子、生物製剤、核酸、または後述するものを含めた他のあらゆる形態の治療物質を含む。細胞型のいずれかに、またはその上に負荷された目的の治療剤は、(これだけに限定されないが)がん、血液病、肝疾患、肺疾患などの様々な適応症に使用することができる。
一部の実施形態では、本開示は、in vitroにおいてPSC細胞または細胞株から引き出された巨核球前駆細胞、巨核球、プレ血小板、血小板前駆体または血小板を提供する。本開示の態様によると、PSC細胞または細胞株から引き出された巨核球前駆細胞、巨核球、プレ血小板、血小板前駆体または血小板は、それらの全体が参照により本明細書に組み込まれる、米国特許第9,763,984号の方法または国際出願第PCT/US2018/021354号に開示されているバイオリアクターを使用して産生される。
血小板は、血流を循環し、体内のあらゆる器官と接触することから、用途が多く、カスタマイズ可能であり、標的化可能な薬物送達系の一部として役立つ可能性が血小板にもたらされる。さらに、血小板は、その免疫調節および血管新生機能のために、腫瘍によって能動的に補充されて、免疫回避に役立ち、その成長と転移を支持する。本開示の一部の態様によれば、ex vivoの本明細書に記載されるPSC由来巨核球前駆細胞、巨核球、血小板前駆体、プレ血小板、または血小板は、治療用組成物、例えば薬物、小分子、生物製剤(例えばタンパク質)または同様の治療剤を送達するためのビヒクルとして使用することができる。このタイプの薬物送達の利益としては、これだけに限定されないが、薬物の透過性または保持により課せられた制限のために標的化が従来困難な組織に分子を送達する能力;標的化組織への薬物の局在化および濃縮;治療標的における選択的な放出まで巨核球/プレ血小板/血小板分泌顆粒中に薬物を隠すことによる、患者を全身処置する必要性の低減;および不要な毒性または免疫原性の回避が挙げられる。一部の実施形態では、このタイプの薬物送達はまた、所望の治療結果を達成して、全身毒性を減少させるのに必要な投薬量を低くすることができる。
図5および図6を参照すれば、一部の実施形態では、本開示に従って、iPSC由来巨核球前駆細胞、巨核球、血小板前駆体、プレ血小板または血小板に、またはその上に、治療用組成物を負荷することができる。
図7を参照すれば、本開示の一部の態様では、iPSC由来巨核球前駆細胞、巨核球、血小板前駆体、プレ血小板または血小板は、目的のタンパク質(目的のポリペプチドまたはペプチドを含む)を発現するように改変することができる。このような改変は、幹細胞レベルで、または本開示の血小板生成プロセス中の他のあらゆるレベルで行うことができる。例えば、本開示は、操作されたhPSC細胞または細胞株から分化した、改変された巨核球または巨核球前駆細胞であって、改変された巨核球または巨核球前駆細胞は、目的のタンパク質(目的のポリペプチドまたはペプチドを含む)を発現する、巨核球または巨核球前駆細胞を提供する。一部の実施形態では、改変された巨核球または巨核球前駆細胞由来の血小板前駆体、プレ血小板または血小板はまた、目的のタンパク質も発現することができ、標的部位に目的のタンパク質を送達するのに使用することができる。
一部の実施形態では、本開示の巨核球前駆細胞、巨核球、血小板前駆体、プレ血小板、または血小板に、in vivoにおける細胞性薬物製品を輸血するときに循環から遮蔽されると予想される治療剤が負荷される。血小板は、天然にがん病変、固形腫瘍、および循環腫瘍細胞に導かれ、これは、一部の例では、露出したコラーゲンおよび他の細胞外マトリクス構成成分との受容体相互作用によるものであり、他の例では、血小板活性化のときに露出した表面である受容体によるものである。血小板は、これらの相互作用の結果として腫瘍細胞に応答して凝集することが公知である(これは、腫瘍細胞により誘発される血小板凝集としても公知である)。血小板は、これらの相互作用によって「活性化」され、受動的な薬物負荷によって、またはiPSC、巨核球前駆細胞、巨核球、および「デザイナー」血小板を産生する他のあらゆる細胞の遺伝学的改変によってこれらの細胞に負荷された小分子および生物学的治療剤を含むその分泌顆粒の内容物を分泌することになる。これらはまた、微小胞を産生するエキソサイトーシスプロセスの一部としてそれらの膜を脱ぎ捨てることになる。一部の実施形態では、原形質膜に小分子および生物学的薬物が共有結合でコンジュゲートした血小板は、微小胞形成中に安定して結合したままになると予想され、腫瘍細胞により誘発される血小板凝集の結果として、がんに選択的に送達される。一部の実施形態では、遺伝子改変されたヒトiPSC、巨核球前駆細胞、または巨核球は、表面受容体からの膜アンカードメイン、一部の例においてCD3およびDAFに融合した組換え生物学的薬物を発現し、それを細胞表面に特異的に送達するように操作することができる。原形質膜に固定された組換え生物学的薬物はまた、血小板活性化の結果として微小胞に取り込ませることによって、疾患病理の部位に送達することもできる。原形質膜に固定された組換え生物学的薬物はまた、標的化薬物送達の別の例において、酵素、一部の例では、固形腫瘍、がん病変、または血管損傷もしくは血管新生部位などの疾患病理の部位において豊富なマトリクスメタロプロテアーゼが、組換え生物学的薬物を切断することが可能になるように、プロテアーゼ切断部位を含んでいてもよい。
上記で論じられたように、一部の実施形態では、本開示の巨核球前駆細胞、巨核球、血小板前駆体、プレ血小板、または血小板は、このような治療用組成物の標的化送達のための治療用組成物を含むように改変することができる。特定には、本開示の巨核球前駆細胞、巨核球、血小板前駆体、プレ血小板、または血小板は、それらの顆粒の表面上またはその中のいずれかに(例えば受動的な吸収または共有結合によるコンジュゲーションによって)治療用組成物が負荷されてもよいし、または治療用組成物を発現するように遺伝子操作されていてもよい。
本開示の態様は、本開示の実施形態による本巨核球前駆細胞、巨核球、血小板前駆体、プレ血小板、または血小板を含む医薬組成物に関する。一部の実施形態では、医薬組成物は、本開示の実施形態による本巨核球前駆細胞、巨核球、血小板前駆体、プレ血小板、または血小板を、薬学的に許容されるキャリアと共に含む。例えば、キャリアは、希釈剤、アジュバント、保存剤、抗酸化剤、可溶化剤、乳化剤、緩衝液、水、水溶液、油、賦形剤、補助剤もしくはビヒクルまたはそれらの組合せであってもよい。好適な薬学的キャリアは、"Remington's Pharmaceutical Sciences" by E. 本開示は、被験体における疾患または感染を処置または防止するための方法を特色とする。本発明はまた、創傷を処置するための方法も特色とする。本方法は、治療剤を含む人工多能性幹細胞(iPSC)由来血小板を含む組成物の治療有効量を、それを必要とする被験体に投与することを含む。実施形態において、組成物は、医薬組成物で使用される。
本開示は、本開示の巨核球または分化細胞を含むキットを提供する。一実施形態では、キットは、単離された巨核球を含む組成物を含む。特定の実施形態では、本開示は、本開示の巨核球またはその前駆体を分化させる、培養する、かつ/または単離するためのキットを提供する。ある特定の実施形態では、本開示は、血小板を産生するためのキットを提供する。
臨床グレードhiPSCの増大
分化の前に、hiPSCの増大は、適切なサイズにしたマスターおよび作業用細胞バンクでの使用のために多数の細胞を産生すること、加えて、臨床的な産生に適切なスケールで分化を開始させるために十分な細胞数を生成することを必要とする。臨床グレードhiPSC細胞株を、NINDS(National Institute of Neurological Disorders and Stroke)/NIH(National Institutes of Health)におけるNINDS Human Cell and Data Repository(NHCDR)保管所から得た。この細胞株(NINDS ID:LiPSC−Gr1.1)は、雄CD34+臍帯血(Lonza)から引き出されたものであり、組換えビトロネクチン(VTN)に加えてcGMP適合性試薬、例えばEssential 8、NutriStem、またはStemFlexを使用した2D培養で維持および増大させることができる(図8A〜8C)。全ての3つの成長条件で特徴的なコロニー成長および多能性マーカーの維持が観察された(図8A〜8C、図9A〜9C)。

マトリクス非依存性3D培養での定方向分化
巨核球および血小板の臨床生産に必要な収量を可能にするために、小規模組織培養プラスチックウェア(2Dマトリクス依存性)から3Dのスケーラブルな解決策に分化プロセス全体を移行させることが重要である。スケーラブルな3D解決策の例は、撹拌または振とうされている超低接着性容器中で懸濁される自己凝集性スフェロイドを使用して分化を実行することを含む(図3)。この実施例において、LiPSC−Gr1.1 hiPSCをTrypLEを使用して単一細胞に解離し、H1152または他のROCK阻害剤を加えた多能性維持培地(例えばEssential 8、Nutristem、StemFlex、他の同様の培地、またはそれらの組合せ)中に細胞50万〜100万個/mlで再懸濁し、90rpmのオービタルシェーカー上の6ウェルの超低接着性プレート中で、または一定撹拌を伴うスピナーフラスコ(125mlのスピナーフラスコにおいて50ml体積で90rpm)中で、37C、5%CO2、20%O2でインキュベートした。いずれの系でも24時間以内に、hiPSCは自己凝集して、直径およそ50〜150umのスフェロイドを形成した(図22A、異なる容器での同様の実施例については、図23Aおよび図11Aも参照されたい)。次いで撹拌を中断し、スフェロイドを容器の底部に沈降させた(およそ5分間)。次いで、培地の50%〜100%をステージ1分化培地と交換して、造血性内皮への分化を促進し、撹拌を低酸素条件(37C、5%CO2、5%O2)でのインキュベーションを伴い再開した。培地交換を同様に合計6日間にわたり(37℃、5%CO2、5%O2で4日間、続いて37C、5%CO2、20%O2で2日間)毎日実行し、その期間中、スフェロイドはより大きく成長し、6日目に特徴的な構造および形状を生じた(図22A)。6日目におけるこれらのスフェロイドの試料を解離し、フローサイトメトリーによって評価したところ、細胞の約44%が、造血性内皮のマーカーであるCD31およびCD34を発現することが見出され(図22B)、純度は、2Dマトリクス依存性培養物と好都合に匹敵していた(図16B)。ステージ2に移行するために、撹拌を中断し、スフェロイドを容器の底部に沈降させた(およそ5分間)。次いで培地の50〜100%をステージ2分化培地と交換して、懸濁細胞の分化および放出を促進させた(図24A)。その後毎日、懸濁細胞を収集し、部分的な培地交換を実行した。これを行うために、撹拌を中断し、造血性内皮スフェロイドを容器の底部に沈降させた(およそ5分間)。培地(懸濁細胞と共に)のおよそ80%を収集し、遠心分離した。新鮮なステージ2分化培地の作業体積の半分を、十分な体積の馴化培地(すなわち遠心分離後の上清)と共にスフェロイドに添加し、元の作業体積を回復させた。次いで残りの上清を捨て、細胞ペレットの一部をFACS分析のために使用し(図24B)、残りを凍結保存するか、または成熟MKへの成熟のためにステージ3に移行させた。懸濁細胞のフローサイトメトリー分析から、オービタルシェーカー上の6ウェルの超低接着性プレート中で分化した自己凝集したスフェロイドと、50mlのスピナーフラスコ中で分化させた自己凝集したスフェロイドとが、同様のpreMK産生動態、経時的な純度および収量の両方を示したことが解明された。2DのIV型コラーゲン分化培養と比較して、これらの3D培養物は、より高い純度で、ステージ2の初期において、より多くのpreMKを産生した(図24C、24D)。
iPSC凝集中またはステージ1から2の移行における可溶性ラミニン521の添加は2つの異なる3D分化様式でステージ2のpreMK収量を改善する
分化を開始させる前の24時間(−1日目)における初期のiPSC凝集ステップ中の、またはステージ1からステージ2(6日目)に移行するときのラミニン521の添加は、2つの異なる3D分化様式でpreMK収量を増大させた。可溶性組換えラミニン521含有または不含のStemFlexおよびROCK阻害剤であるH1152(対照培地)を含有する96ウェルのU底超低接着性プレートのウェル当たり、5000個の単一細胞の解離したiPSCを播種した。24時間後、培地をステージ1の培地で置き換え、培地交換を6日間実行した。次いで培地を、可溶性ラミニン521含有または不含のステージ2の培地と交換した。24時間後、追加で最大6日間にわたり、半分の培地交換を毎日実行した。ステージ2におけるpreMK収量を比較することにより、分化プロセスの−1日目または6日目におけるラミニン521の添加は、ラミニン521添加無しの対照培養物と比較してpreMK収量を増大させたことが解明された(図27A、27B)。ラミニン521の作用が撹拌した3D培養物でも観察できるかどうかを決定するために、iPSCを単一細胞として解離し、150万個の細胞を、可溶性組換えラミニン521含有または不含のStemFlexおよびROCK阻害剤であるH1152(対照培地)を含有する6ウェルの超低接着性プレートの各ウェルに播種し、オービタルシェーカー上に設置した。24時間後、培地をステージ1の培地で置き換え、培地交換を6日間実行した。次いで培地を、可溶性ラミニン521含有または不含のステージ2の培地と交換した。24時間後、追加で最大6日間にわたり、半分の培地交換を毎日実行した。ステージ2におけるpreMK収量を比較することにより、分化プロセスの−1日目または6日目におけるラミニン521の添加は、ラミニン521添加無しの対照培養物と比較してpreMK収量を増大させたことが解明された(図27C)。実施例1に記載される高効率単一細胞継代技術から引き出されたiPSCを、オービタルシェーカー上の6ウェルプレートで、NutriStem中で同様に自己凝集させた場合、ラミニン521作用が増幅した(図27D)。
ステージ1の間における増殖因子添加の順番およびタイミングの調整は分化効率を増大し、全体的な増殖因子の使用量を減少させる
分化のステージ1の間に起こる初期の特定化事象は、複雑であり、細胞シグナル伝達事象の固有の順番およびタイミングを必要とする。それゆえに、ステージ1の培地因子であるBMP4、bFGFおよびVEGFAの添加の順番およびタイミングを調整することは、ステージ1の全体にわたり3つ全ての増殖因子が含まれる標準的な完全St1培地条件と比較して、分化プロセスの効率を改善し、増殖因子の使用量を低減することができる。最初の実験(実験A)(図28A)は、添加の順番およびタイミングを試験した。最初の24時間にわたりBMP4単独を添加し、続いて次の24時間にわたりVEGFAおよびbFGF(BMP4不含)を添加し、続いて4日間の完全St1培地とした場合、ステージ2で産生されたpreMKの数は、ステージ1にわたり完全St1培地を受けた対照培養物より著しく多かった(図28B)。48時間にわたり同じ順番を実行することは、同じ作用を有さなかった。第2の実験(実験B)(図28C)は、この系においてBMP4は24時間を過ぎるとなくてもよいが、FGF2およびVEGFAは、分化を効果的に進行するために重要であることを実証した(図28D)。この実施例は、分化のステージ1が、分化のステージ2に移行する前にBMP4を単独で24時間、続いてbFGFおよびVEGFAを5日間使用して効果的に進行できることを実証する。
WNTモジュレーターはステージ1およびステージ2の分化効率に影響を与えることができる
WNTシグナル伝達は、発生中において重要である。GSK3キナーゼ阻害剤であるCHIR98014およびCHIR99021は、WNTアゴニストとして作用する。分化の最初の48時間のみ、本明細書に記載されるステージ1分化条件を、0.6μMのCHIR98014または6μMのCHIR99021で強化したところ、CD31およびCD34の免疫蛍光染色によって決定した場合、ステージ1の分化効率における劇的な増大が6日目に観察された(図29A〜29C)。次いで対照およびCHIR98014培養をステージ2に移行させ、そこでpreMKの産生および放出を、CD41およびCD43の免疫蛍光染色によって追跡した。CD41+細胞の数の視覚的な推定から、最初の48時間におけるWNTモジュレーターにより生じたより高いステージ1効率は、ステージ2の間のより高い産出に対応し得ることが示唆される(図30A〜30B)。それゆえに、WNTモジュレーターの短期間の添加は、後続の分化ステージにわたり分化効率に影響を与える可能性がある。
ラミニン521でコーティングされたマクロキャリアを用いた充填層バイオリアクター
ここで、1mmのラシヒリングの形状のラミニン521でコーティングされたPTFEマクロキャリアは、iPSCの分化のための支持体を提供できること、およびこのマクロキャリア材料は、図4に示される概略図で例示されるような充填層バイオリアクターでの使用に適すると予想されることを実証する証拠を提供する。まずPTFEリングを、ロッカー上で、4℃で、1.25ug/mlのラミニン−521と共に一晩インキュベートした。使用前に、PTFEリングを、6ウェルプレート中で、ROCK阻害剤であるH1152を加えたEssential 8培地で平衡化した。多能性iPSCを0.5mMのEDTAを使用して回収し、H1152を加えたEssential 8培地に再懸濁し、PTFEリング上に凝集塊として播種した。10分毎に、プレートを、オービタルシェーカー上で、75rpmで30秒回した。1時間後、プレートを75rpmで一晩連続的に振とうした。24時間後、培地の90%を取り出し、ステージ1の培地で置き換え、培地交換を毎日行った。ステージ1中、iPSCは、ラシヒリングの内部上に成長領域を示し(図31)、成長領域は、2D培養物で見られたものと類似した形態学的特徴を生じた(図22)。これらの細胞のフローサイトメトリー分析は、高い割合の造血性内皮細胞を示し、CD31を発現する細胞は約80%であり、これらの細胞の半分より多くが、CD34+について二重に陽性であった(図33A)。ステージ2の培地に切り換え、毎日の半分の培地交換を開始させたところ、全体的に平坦なコロニーから3Dスフェロイド型の構造に形態が変化したが、特筆すべきことに、それでもなおこれらの構造はリング形状のマクロキャリアの内部上のラミニン521コーティングに付着していた(図32)。ステージ2の間に放出された細胞は、6+2日目もの早期でも高いpreMK含量を有しており、細胞の約75%がCD43およびCD41を共発現し(図33B)、純度は2Dマトリクス依存性培養に有利に匹敵する程度であった(図18A)。6+3日目に放出された細胞を収集し、超低接着性プレート中で、ステージ3培地中で追加の3日間培養したところ、これらの細胞の約80%が、CD61およびCD42bを共発現したことから(図33C)、効率的なMK分化が起こったことが示される。このようなマクロキャリアは、iPSCの造血性内皮への初期の分化(すなわち定方向分化のステージ1)、加えてpreMKのさらなる分化および放出(すなわち定方向分化のステージ2)を同じ容器で行うことができる充填層バイオリアクターのための材料としての使用に適する(図4)。この設計において、充填層バイオリアクターは、新たに多能性iPSCを播種したラミニン−521でコーティングされたマクロキャリアを用いて構成される。次いで充填層は、培地の連続流に曝露されてもよく、それにより造血性内皮へのステージ1分化が可能になる。培地は、充填層に浸透させた後、馴化チャンバーに循環させることになり、そこで新鮮な培地構成成分が添加され、酸素/CO2濃度がスパージングまたは他の手段を介して調整されることになり、その後、培地は細胞に再循環されることになる。ステージ1の完了時に、培地は、ステージ2の分化および産生ならびにpreMKの放出が可能になるように切り替えることになる。適切なサイズおよび形状にしたマクロキャリア支持体、例えば1mmのラシヒリングは、放出された細胞の充填層を通る浸透および収集および凍結貯蔵のためのリアクターからの浸透を可能にするのに十分な培地の流れおよびチャネル幅を可能にすると予想される。この設計は、細胞が受けるせん断力を減少させ、その灌流に基づく設計のために、効率的な培地使用を可能にし、それらが放出される場合、preMKの連続的な収集を可能にする。
iPSC由来巨核球の詳細な特徴付け
本明細書に記載される方法を使用して生成した巨核球は、免疫蛍光顕微鏡法によって画像化した場合、MK特異的タンパク質であるベータ−1−チューブリン(図34)、加えてアルファ顆粒に関連するタンパク質(PF4およびVWF、図35A〜35F)および高密度顆粒に関連するタンパク質(LAMP1およびセロトニン、図36A〜36F)などに関する機能的な成熟MKに関連する多くの特色を実証する。iPSC由来MKの電子顕微鏡画像から、多小胞体、グリコーゲン顆粒、および陥入膜系を含む特徴的な超微細構造的特色が明らかになる(図37A〜37D)。遺伝子発現分析により、多能性遺伝子、例えばOCT4の下方制御(図38A)および巨核球系列遺伝子、例えばNFE2の上方制御(図38B)が明らかになった。同様の分析を関連遺伝子のパネル上で実行したところ、この分析の結果は、多能性幹細胞シグネチャーの喪失および巨核球シグネチャーの獲得と一致する(図39)。
ヒトiPSC由来血小板の詳細な特徴付け
血小板は、この出願書類に記載される培養方法および回収技術を使用してヒトiPSC由来成熟MKから生成される。血小板は、静置培養で収集してもよいし(図43A、43B)、または定方向分化プロセスのステージ3最高点で成熟MKをミリフルイディクスバイオリアクターに供給することによって産生してもよい(US9,795,965;US2017/0183616;US2018/0334652;WO2018165308での言及を参照)。定方向分化プロセスから引き出された血小板は、DNAインターカレート色素について陰性染色され、2〜5μmのサイズ分布の範囲内である;またプレ血小板も、この培養において5μmおよびそれより大きい直径で観察される(図43A)。ヒトiPSC由来血小板は、明確なβ1−チューブリンリング、および活性化マーカーであるCD62pの非存在を示す顕微鏡写真によって示されるように、休止表現型を有する(図44Bおよび45A)。これらは、糸状偽足および葉状仮足の形成および拡散を示す細胞骨格の変化を明らかにするガラス拡散技術を使用して実験的に活性化することができる(図44Aおよび44B)。
ヒトiPSC由来血小板は受動的な薬物負荷によって組換え生物学的薬物を取り込む
本明細書に記載される方法によって産生された血小板は、末梢血液全体から抽出される血小板に加えて、ヒトCD34+動員末梢血細胞源から分化した血小板にも類似した特徴を有する。
ヒトiPSC由来血小板における組換えタンパク質の共有結合によるコンジュゲーション
細胞膜上への組換えタンパク質の共有結合による連結を創出するための多くの戦略がある(図6)。本明細書に記載されるiPSC由来血小板の場合、2−イミノチオラン(トラウト試薬、Thermofisher #26101)を、様々な濃度で、1e6個の血小板と、500ulの緩衝液中、37セルシウス度で1時間インキュベートして、第一アミンをスルフヒドリルに変換した(図54Aに描写される)。それと同時に、スクシンイミジル4−(N−マレイミドメチル)シクロヘキサン−1−カルボキシレート(SMCC、Thermofisher #22360)を、ヒトIgGと共に4Cで2時間インキュベートした(図54Bに描写される)。次いでトラウト試薬で処置した血小板を、SMCCリンカーで連結されたIgGと共に37セルシウス度で1時間インキュベートした(図54C)。(SMCC無しのIgGに対して)SMCCリンカーで連結されたIgGの固定濃度を用いて、トラウトの用量滴定を洗浄されたドナー血小板で実行した。フローサイトメトリーによれば、シグナルは、SMCCリンカーで連結されたIgGで、トラウト試薬の用量が増大するにつれてより顕著に大きくなり、最大効率は、0.4mg/mlのトラウト試薬で観察された(図54D)。SMCCリンカーがないと、IgGシグナルは、フローサイトメトリーによれば増大しなかったことから(図54E)、洗浄された血小板の表面へのIgGのコンジュゲーションを促進することにおいて、コンジュゲーション反応は効率的であったことが示唆される。
ヒトiPSC由来preMKおよびMKへの組換え薬物である生物製剤の受動的な負荷
人工多能性幹細胞を、本明細書に記載される方法を使用して成熟巨核球に分化させた。細胞を、Dylight 488標識アテゾリズマブと共に30分間インキュベートし、4%パラホルムアルデヒドで固定し、調製されたポリ−l−リシンでコーティングされたガラスカバースリップ上に遠心分離した。培養物内の成熟巨核球を確認するために、細胞を追加でCD61について染色した。CD61を、予めコンジュゲートしたCD61−APC抗体を使用して可視化した。試料を洗浄し、Aqua−poly/mount(Fisher Scientific)を用いてガラスカバースリップ上にマウントした。Zeiss Meta 880共焦点走査顕微鏡を使用して画像を捕獲し、Fiji ImageJソフトウェアを使用して分析した(図56A〜56C)。これらのデータは、本開示の巨核球およびその生成された静止血小板が、アテゾリズマブを取り込むことが可能であることを示唆する。アテゾリズマブは、アルファ顆粒染色PF4と共局在化することが実証され(図56C)、さらに細胞表面上にあることも示される。これは、受動的な負荷(細胞膜への共有結合によるコンジュゲーションとは対照的に)は、血小板を産生するMKにおけるアルファ顆粒の局在化を容易にすることができ、血小板分化のときに顆粒中に保持され得るという証拠を提供する。
ヒトiPSC由来preMKおよびMKへの組換え薬物である生物製剤の共有結合によるコンジュゲーション
ヒトiPSC由来の巨核球前駆細胞(preMK)および成熟巨核球(MK)の細胞膜に組換え薬物である生物製剤をコンジュゲートする能力が本明細書において実証される。preMKをステージ2培養物から回収し、CD41およびCD43共発現について免疫表現型を決定した(図58Aおよび58B)。iPSC由来preMKを、500ulの緩衝液中の1e6個の細胞を用いて、0.4mg/mlのトラウト試薬で37℃で1時間処理して、第一アミンをスルフヒドリルに変換した。同時に、SMCCを、イピリムマブと共に4℃で2時間インキュベートした。次いでトラウト試薬で処置したpreMKを、100μg/mlのSMCCリンカーで連結したイピリムマブと共に37℃で1時間インキュベートした。AlexaFluor 647にコンジュゲートしたヒトIgGに対する二次抗体を使用して、コンジュゲートしたイピリムマブを検出した。薬物処置の非存在下で、CD41とCD43について二重陽性の細胞において検出可能な薬物はみられなかった(図58C)。薬物処置した試料について、全ての観察可能なCD41とCD43について二重陽性のpreMKは、検出可能なイピリムマブを有していた(図58D)。成熟巨核球を、ヒトiPSCからの定方向分化プロトコールのステージ3で回収し、CD61(図59A)およびCD42a(図59B)についての免疫表現型を決定した。MKを、500μlの緩衝液中の1e6個の細胞を用いて、0.4mg/mlのトラウト試薬で37℃で1時間処理して、第一アミンをスルフヒドリルに変換した。同時に、SMCCを、イピリムマブと共に4℃で2時間インキュベートした。SMCCリンカーで連結したイピリムマブを、トラウト処置したMKと37℃で1時間反応させた。AlexaFluor 647にコンジュゲートしたヒトIgGに対する二次抗体を使用して、コンジュゲートしたイピリムマブを検出した。薬物処置の非存在下で、CD61とCD42aについて二重陽性の細胞において検出可能な薬物はみられなかった(図59C)。薬物処置した試料について、全ての観察可能なCD61とCD42aについて二重陽性のMKは、検出可能なイピリムマブを有していた(図59D)。
受動拡散による洗浄したヒト血小板の小分子の負荷
ヒトiPSCからの血小板生成物における小分子の負荷および保持を実証するために、洗浄したヒト血小板を、DNAインターカレート化学療法剤である塩酸ドキソルビシン(Sigma #D1515)と共インキュベートした。ゲノム物質は、典型的には、血小板調製物との共インキュベーションおよび受動拡散による細胞への侵入の結果として細胞内にこの薬物を隔離すると予想されるが、血小板は、無核細胞型であるため、このようなゲノム物質を含有しない。100μMのドキソルビシンを、1mlの緩衝液中の1e7個の血小板の調製物と共に使用し、透析カセット(Thermofisher #88400)中で、オービタルシェーカーでの一定撹拌下で、周囲温度で30、120、240、および1440分インキュベートした。ドキソルビシンは、フローサイトメトリーによって検出することができる固有の蛍光特性を有し(Ex 427nm/Em 585nm)、血小板調製物に対して複数の洗浄ステップを実行することができ、非特異的に結合した分子がなくなった後でもなお薬物カーゴは保持されると予想されることが見出された(図60A)。洗浄された血小板へのドキソルビシン封入に必要な最小および最大時間を理解するために、動態研究が採用された。サンプリングした血小板における検出可能なドキソルビシン発現を見るには、30分で十分であり、シグナルは、1440分後に保持されていたことが観察された(図60B)。このデータは、小分子薬物が効率的に血小板中に捕獲され得ることを示唆する。
遺伝子改変された巨核球前駆体からの血小板の生成
治療的導入遺伝子を発現する血小板は、傷害、病気、および疾患の処置における著しい進歩を表すものになると予想される。導入遺伝子を発現する血小板を生成するために、巨核球前駆体を、レポータータンパク質をコードする核酸カセットを含むレンチウイルスベクターで形質導入した。具体的には、このカセットは、EF1アルファプロモーターおよびZsGreen蛍光タンパク質をコードしていた。レンチウイルスベクターでの感染の42時間後、形質導入された巨核球前駆体では蛍光が検出されたが、形質導入されていない(模擬)対照では検出されなかったことから(図61)、巨核球前駆体がうまく形質導入されたことが示される。
バイオリアクター系を使用した血小板の製造
治療剤の負荷または遺伝学的改変に好適なバイオリアクターおよび条件を使用して、血小板を産生した。血小板を産生するために、巨核球を血小板バイオリアクターに播種した(図63A)。バイオリアクターは、多孔質膜(5μmの孔)によって分離された2つの連続するチャネル(図63B、63C)からなっていた(図64)。巨核球は、多孔質膜上に高効率で固定され(フローサイトメトリー分析によって実証されるように、>99%の保持)、チャネル中を流動する培地は、巨核球上に生理的に適切なせん断力を与える(図64)。第1のチャネルおよび第2のチャネルにおける流速を独立して調整することで、膜上に巨核球が選択的に捕獲されるように、さらに、捕獲された巨核球上に第2のチャネルに沿って生理学的なせん断速度が作り出されるように構成されたチャネル間に差をもたらし、血小板が産生されるようする。

第2の培養培地中で、前記造血性内皮細胞を巨核球前駆細胞に分化させるステップ;
前記巨核球前駆細胞を成熟巨核球に分化させるステップ;
前記成熟巨核球を、血小板を産生するのに十分な条件下で分化させるステップ;および
前記血小板、前記巨核球、前記巨核球前駆細胞、またはそれらの組合せのうち1つに、治療剤を負荷するステップ
