一実施例において、免疫アッセイを、タンパク質レベルを測定するために使用することができる。上記のように、免疫アッセイは、検出部分(例えば、蛍光剤又は酵素)を有する抗体で行うことができる。生体サンプルに由来するタンパク質は、固相マトリクス(例えば、マルチウェルアッセイプレート、ニトロセルロース、アガロース、セファロース、コードされた粒子、若しくは磁気ビーズ)に直接的にコンジュゲートされてもよいし、又は、生体サンプルに由来するタンパク質は、特異的結合対の第2のメンバー(例えば、ストレプトアビジン若しくはビオチン)に結合すると固相マトリクスに付着する、特異的結合対の第1のメンバー(例えば、ビオチン若しくはストレプトアビジン)にコンジュゲートされてもよい。固相マトリクスへのこのような付着によって、タンパク質を、検出抗体との接触の前に生体サンプルの他の干渉成分又は無関係の成分から精製して取り出すことが可能となり、また、未結合の抗体をその後に洗浄することも可能となる。ここで、上記のように、結合した検出可能に標識された抗体の存在又は量は、生体サンプル中のタンパク質の存在又は量を示す。

血清の調製のための末梢血を42人の患者から回収し、その後、研究薬物(レンバチニブ及びペムブロリズマブの組み合わせ薬剤)を初回投与し、サイクル1の15日目及びサイクル2の1日目に投与し、そして、42人の患者のうち37人の患者の利用可能な血清サンプルをアッセイした。詳細には、アッセイに利用可能なサンプルの数は、37(ベースライン)、31(サイクル1の15日目)、及び35(サイクル2の1日目)であった。レンバチニブに関連する、又は文献において免疫チェックポイント阻害剤について報告されている、全部で41個の候補血清バイオマーカーが、Myriad RBM(Austin、TX、USA)での2回のQuanterix Simoaアッセイに加えて、18個の事前に構成されたCustomMAPイムノアッセイパネルを使用してアッセイされ、マルチプレックスフローサイトメトリーに基づくプラットフォームによって製造者(Multi−Analyte Profile(MAP))によって測定された。患者の>20%が範囲外の測定値を有していた血清バイオマーカーは、相関分析に含めなかった。サイクル2の1日目における、ベースラインからの処置後の血清バイオマーカーの倍率変化、及び、血清バイオマーカーの変化レベルと腫瘍奏効(CR/PR/uPR対SD/PD)との関係を試験するために設計された血清バイオマーカー分析を、血清バイオマーカーのデータ及び腫瘍奏効のデータが共に利用可能であった患者(29人の患者)について、ウィルコクソンの順位和検定によって分析した。不確定完全奏効(uCR)を伴う患者はいなかった。多重比較での偽発見率(FDR)がP<0.05である結果が、バイオマーカー分析について報告されている。統計解析は、SAS(SAS Institute,Inc.、Cary、NC、USA)のバージョン9以降を使用して行った。この臨床研究において、uCR/uPRは、臨床奏効が4週間も後に再び見られる場合には、CR及びPRとなっていることが確認されたと考えられる。言い換えると、CR/PRが一度だけ見られる患者はuCR/uPRと考えられるが、奏効が4週間もの期間をあけて少なくとも2回見られる患者は、CR/PRと考えられる。

(b)組み合わせ療法の開始後の対象から得られた生体サンプル中の1つ又は複数のタンパク質のレベル(処置後)を測定するステップ、
(c)測定された1つ又は複数のタンパク質のそれぞれについての処置前のレベルに対する処置後のレベルの比率を計算するステップ
を含み、
コントロールと比較した、測定された1つ又は複数のタンパク質の比率の増大が、対象が組み合わせ療法に奏効する可能性が高いことを予測し、
アンタゴニストがアテゾリズマブでもCS−1001でもない、方法。

Claims (33)
- レンバチニブ又はその薬学的に許容できる塩及びプログラム細胞死1タンパク質(PD−1)アンタゴニストを含む組み合わせ療法に対する、少なくとも1つのがんを有する又は有する疑いがあるヒト対象の奏効を予測する方法であって、
(a)組み合わせ療法の前の対象から得られた生体サンプルにおける、IFN−γ、IL−10、CXCL9、CXCL10、CXCL11、CXCL12、FGF−19、及びFGF−23からなる群から選択される1つ又は複数のタンパク質のレベル(処置前)を測定するステップ、
(b)組み合わせ療法の開始後の対象から得られた生体サンプル中の1つ又は複数のタンパク質のレベル(処置後)を測定するステップ、
(c)測定された1つ又は複数のタンパク質のそれぞれについての処置前のレベルに対する処置後のレベルの比率を計算するステップ
を含み、
コントロールと比較した、測定された1つ又は複数のタンパク質の比率の増大が、対象が組み合わせ療法に奏効する可能性が高いことを予測し、
アンタゴニストがアテゾリズマブでもCS−1001でもない、方法。 - がんが、子宮内膜がん、非小細胞肺がん(NSCLC)、腎細胞癌(RCC)、尿路上皮がん、頭頚部がん、黒色腫、肝細胞癌、乳がん、卵巣がん、胃がん、大腸がん、膀胱がん、膠芽腫、胆道がん、グリオーマ、メルケル細胞癌、ホジキンリンパ腫、非ホジキンリンパ腫、子宮頸がん、進行性の又は難治性の固形腫瘍、小細胞肺がん、非扁平非小細胞肺がん、線維形成性黒色腫、小児の進行性の固形腫瘍又はリンパ腫、メソテリン陽性の胸膜中皮腫、食道がん、肛門がん、唾液腺がん、前立腺がん、カルチノイド腫瘍、原始神経外胚葉性腫瘍(pNET)、及び甲状腺がんからなる群から選択される、請求項1に記載の方法。
- 対象から処置前に得られた生体サンプル及び処置後に得られた生体サンプルが、血液サンプル、血清サンプル、又は血漿サンプルである、請求項1又は2に記載の方法。
- 生体サンプルが血清サンプルである、請求項3に記載の方法。
- 1つ又は複数のタンパク質が、IFN−γ、CXCL9、CXCL10、及びCXCL11からなる群から選択される、請求項1〜4のいずれか一項に記載の方法。
- 1つ又は複数のタンパク質がCXCL9及び/又はCXCL10である、請求項1〜4のいずれか一項に記載の方法。
- 1つ又は複数のタンパク質がFGF−19及び/又はFGF−23である、請求項1〜4のいずれか一項に記載の方法。
- レンバチニブの薬学的に許容できる塩が、レンバチニブメシル酸塩である、請求項1〜7のいずれか一項に記載の方法。
- PD−1アンタゴニストがPD−1のアンタゴニストである、請求項1〜8のいずれか一項に記載の方法。
- PD−1のアンタゴニストがペムブロリズマブである、請求項9に記載の方法。
- レンバチニブ又はその薬学的に許容できる塩が1日に1回投与され、ペムブロリズマブが3週間ごと又は6週間ごとに投与される、請求項10に記載の方法。
- 処置後の生体サンプルが、組み合わせ療法の開始の少なくとも1週間後に対象から得られる、請求項1〜11のいずれか一項に記載の方法。
- 処置後の生体サンプルが、組み合わせ療法の開始の1週間〜24ヶ月後に対象から得られる、請求項12に記載の方法。
- 処置後の生体サンプルが、組み合わせ療法の開始の1週間〜4週間後に対象から得られる、請求項13に記載の方法。
- がんが子宮内膜がんである、請求項1〜14のいずれか一項に記載の方法。
- 子宮内膜がんが、進行性の子宮内膜がんである、請求項15に記載の方法。
- 組み合わせ療法に奏効すると予測される又は奏効する可能性が高いと予測された対象に対して組み合わせ療法を継続するステップをさらに含む、請求項1〜16のいずれか一項に記載の方法。
- がんを有するヒト対象を処置する方法であって、
(a)IFN−γ、IL−10、CXCL9、CXCL10、CXCL11、CXCL12、FGF−19、及びFGF−23からなる群から選択される1つ又は複数のタンパク質の比率が増大していると判定されたヒト対象に、レンバチニブ又はその薬学的に許容できる塩及びPD−1アンタゴニストを含む組み合わせ療法を投与するステップ
を含み、
1つ又は複数のタンパク質の比率が、組み合わせ療法を投与する前にヒト対象から得られた生体サンプル中の1つ又は複数のタンパク質のレベル(処置前)及び組み合わせ療法を投与した後に得られた生体サンプル中の1つ又は複数のタンパク質のレベルを測定し、比率を決定することによって得られ、

- 1つ又は複数のタンパク質が、IFN−γ、CXCL9、CXCL10、及びCXCL11からなる群から選択される、請求項18〜21のいずれか一項に記載の方法。
- 1つ又は複数のタンパク質がCXCL9及び/又はCXCL10である、請求項18〜21のいずれか一項に記載の方法。
- 1つ又は複数のタンパク質がFGF−19及び/又はFGF−23である、請求項18〜21のいずれか一項に記載の方法。
- レンバチニブの薬学的に許容できる塩が、レンバチニブメシル酸塩である、請求項18〜24のいずれか一項に記載の方法。
- PD−1アンタゴニストがPD−1のアンタゴニストである、請求項18〜25のいずれか一項に記載の方法。
- PD−1のアンタゴニストがペムブロリズマブである、請求項26に記載の方法。
- レンバチニブ又はその薬学的に許容できる塩が対象に1日に1回投与され、ペムブロリズマブが対象に3週間ごと又は6週間ごとに投与される、請求項27に記載の方法。
- 処置後の生体サンプルが、組み合わせ療法の開始の少なくとも1週間後に対象から得られる、請求項18〜28のいずれか一項に記載の方法。
