Abstract

変性円板疾患及び関連する下背部痛は、痛みの主要な原因の1つであり、先進国の人口の約70%〜85%に影響している。変性円板疾患のための現在の治療では、痛みの最小化、脊椎の安定化、及び運動性の改善又は維持に重点が置かれている。通常、治療には、ステロイド注入及び鎮痛剤、理学療法、体重減量、及び手術など、痛み及び炎症の管理技術のいくつかの組合せを含む。これらの治療のどれも、変性疾患を治癒又は予防することはない。

A.SHHシグナル伝達経路活性化剤
いくつかの実施形態では、SHHシグナル伝達経路活性化剤は、そのように機能する、任意の小分子であることが可能である。いくつかの実施形態では、SHHシグナル伝達経路活性化剤は、SHHである、SHH活性を模倣する、Patched活性に拮抗する、又はスムーズンド活性を刺激する。いくつかの実施形態では、SHHシグナル伝達経路活性化剤は、スムーズンド(SMO)及び亜鉛フィンガー転写因子の神経膠腫関連腫瘍遺伝子ホモログ(GLI)ファミリーなどのSHH経路の下流エフェクターである。ある実施形態では、SHHシグナル伝達経路活性化剤は、ブラキウリ(Bra)のアップレギュレーターである。

キット、例えば、対象において、SDDを治療するため、又は1又は2以上のSDDに関連する症状を治療するためのキットも、本明細書で提供される。キットは、例えば、SHHシグナル伝達経路活性化剤又はSHHシグナル伝達経路活性化剤を含む医薬組成物、及び使用説明書を含有してよい。キットは、SHHシグナル伝達経路活性化剤を含む医薬組成物を有するバイアルを含んでよい。キットは、あらかじめ充填されたシリンジを含んでよい。使用説明書は、処方情報、投薬情報、保管情報などが含有されてよい。
[実施例]
マウス仙骨の形成
ほとんどのマウス系統は、7つの頸椎、13の胸椎、5〜6つの腰椎、4つの仙椎を有する(A. and Michie、1958、Wellik、2007)。マウスにおける仙骨形成のタイムラインを決定するために、新生児(生後4日(P4,postnatal day four))、急速成長期(4週)、縦成長期間の末期(12週及び14週)及び、中年齢(1年齢)の範囲のFVBマウス由来の仙椎円板(S1/S2、S2/S3、S3/S4)をX線によって比較し(図1B)、ヘマトキシリン及びエオシン(HE,hematoxylin and eosin)染色された中央冠状切片において、組織形態計測的解析を実施した(図1A、C〜F)。すべての仙椎円板は、P4で、網状のNP細胞及び組織されたAF層を有して、組織学的に正常であった(図1A)。4週間後、すべての仙椎円板は、網状のNPを保持し続けたが、一方、AFは2つの最も頭部側の仙椎円板(S1/S2及びS2/S3)で組織破壊されていた。12〜14週齢までに、NP細胞は、2つの頭部の円板で崩壊し、かつ該円板の高さ及びNP空間が、S3/S4と比較して劇的に減少した(図1A、D及びEにおいてS1/S2をS3/S4と比較)。これらの2つの円板のAFは、より組織破壊されたようにみえ、検出可能な層もなく、かつ血管体のようにみえる構造を含有し、これは加齢に関連する円板変性時にみられることが知られている(Kauppila, L. I. (1995). Ingrowth of blood vessels in disc degeneration. Angiographic and histological studies of cadaveric spines. J Bone Joint Surg Am 77, 26-31)、(Nerlich et al.、2007)。これを確認し、内皮細胞マーカーCD31の免疫染色によって、それぞれの円板を定量した(PECAM−1、図1H及びI)。免疫蛍光法により、4週齢という早い時期にAFに血管体が存在することが確認された。
NP細胞によるSHHシグナル伝達は、円板の生後の成長及び分化に必要とされ、かつ年齢とともに減少する(Dahia, C. L., Mahoney, E. and Wylie, C. (2012). PLoS One 9, e98444)。S1〜S3の仙椎円板の崩壊が、SHHシグナル伝達の喪失(又はSHHシグナル伝達への応答の喪失)に関連しているかどうかを決定するため、S1/S2、S2/S3及びS3/S4の円板でのSHHの免疫染色を、成長及び分化時に実施した。図3Aには、S1/S2において4週以降のSHHの発現が劇的に減少するが、正常な構造が長く維持されるS3/S4では、その発現ははるかに遅く減少することが示される。この結果は、SHHの転写標的CK19及びBraについても示される。CK19及びBraの発現は、それらの発現が1年齢まで検出可能であるS3/S4の円板と比較して、4週以降のS1/S2の円板においてSHHの発現に対応して減少した(図3B及びC)。S2/S3の円板の免疫染色データは、結果がS1/S2の円板と類似したために、示していない。3つの仙椎円板のすべてにおけるSHH、CK19及びBraの免疫蛍光強度を定量化した結果を図4A〜Cに示す。
この試験では、SHHシグナル伝達の再活性化によって、仙椎円板の崩壊を回復することができるかどうかを試験した。これは、S1/S2の円板においてSHHの発現がすでに減少し、かつNP細胞がそれらの網状の表現型を失い始め、円形となって共に凝集しはじめる、12週齢で実施された。この研究のために、本発明者らは、CK19CreERT2由来系統を使用した。最初に、4カ月齢のCK19CreERJ+,−R26mTmGに2つの用量のタモキシフェンをを投与して、最も頭部の仙椎円板を通る中央冠状切片を分析することによって、CK19を発現し、かつ組換えを受けることができる細胞の量を調査した。図7Aは、S2/S3の円板(白い矢印の頭で示す)において、この週齢までにCK19を発現する細胞がほとんどないことを示す。

Genesis 45, 593-605)と交配させた、SHHGFPCre/+;R26mTmGでは、SHH由来の脊索細胞のみがCreを発現し、膜局在tdTOMATO(mT)をフロックスアウト(flox out)し、膜局在EGFP(mG)を「オン」にして緑色でマークされるが、一方、残りの細胞は、mTを発現し続け、「赤色」のままである。骨格的に成熟した18カ月齢(18M)のSHHGFPCre/+;R26mTmGマウスにおける系統追跡(lineage tracing)は、NP全体が、排他的にSHH由来細胞(SHH+)で構成されていることを示す(図9a〜b)。この研究で使用される系統を含め、マウスのほとんどの系統には6つの腰椎がある。18カ月齢のL5/L6の円板は、網状のNP細胞及び組織化されたAF層を有して、輪郭が明らかな構造を維持した(図9a)。対照的に、腰仙(L6/S1)の円板は、より少なくより丸いNP細胞及びNP空間に移動する組織破壊されたAF層をともなう加齢した表現型を示す(図9b)。24カ月齢までに、NP空間において、SHH+の緑色の細胞はみられず、椎間円板は低細胞化し、かつ円板の高さの減少とともに崩壊する(図9c)。アグリカン(ACAN)の免疫染色は、18カ月齢〜24カ月齢の円板の崩壊に関連するECMの喪失を示す(図10a及びb)。
新生児マウスNP細胞と比較して、1年齢のマウスの円板において、SHH及びその下流ECMマーカーの発現は、大幅に減少する(Winkler, T., Mahoney, E. J., Sinner, D., Wylie, C. C. and Dahia, C. L. (2014). Wnt signaling activates Shh signaling in early postnatal intervertebral discs, and re-activates Shh signaling in old discs in the mouse. Genesis 45, 593-605)を交配させ、誘導可能なSHHレポーター(SHHCreERT2/WT;R26mT/mG)系統を生成し、かつ最後のタモキシフェン投与の2日後に腰椎脊椎を収集することによって、12カ月齢及び24カ月齢のSHH発現NP細胞の集団を分析した。12カ月齢では、主にNP周辺に存在している、AF及びEPにより近い位置にあるNP細胞のサブセットのみがSHHを発現していた(図11a)。これらの少数のSHH+NP細胞は、網状のNP及びよく組織化されたAFを備えた円板構造を維持するのに十分であると考えられる。24カ月齢までには、崩壊した腰椎円板は、SHH+細胞が維持されず、低細胞化した(図11b)。これらの結果から、加齢にともなうSHH発現の喪失が、円板細胞数、ECMタンパク質、及び円板の高さの喪失をもたらすことが示唆される。

Cholesterol modification of sonic hedgehog is required for long-range signaling activity and effective modulation of signaling by Ptc1. Cell 105, 599-612)]を、CK19CreERT2/+(Means, A. L., Xu, Y., Zhao, A., Ray, K. C. and Gu, G. (2008). A CK19(CreERT) knockin mouse line allows for conditional DNA recombination in epithelial cells in multiple endodermal organs. Genesis 46, 318-323)と交配させ、中年齢マウスのNP細胞におけるSHHを標的化した。約11カ月齢の時点で、マウスに、タモキシフェンを2回、強制経口投与し、5カ月間加齢させた。HnE染色は、実験終了時での対照における同じレベルと比較して、SHH cKOの腰椎円板における劇的な組織学的変化を示した(図12a)。SHH cKOの円板における組織学的変化が、SHH及びその標的の喪失によるためであることを確認するために、対照及びSHH cKO群の腰椎円板におけるSHHの免疫蛍光分析を実施した(図12b)。SHH発現が、対照の円板では検出可能であったが、SHH cKOの円板では検出されなかった。また、免疫蛍光分析は、対照及びSHH cKOの腰椎円板においてECMマーカーであるACANの喪失を示した(図12c)。これらの研究は、椎間円板の変性及び加齢が、SHHの喪失によるものであり、かつSHHが円板の健康維持に不可欠であるという仮説が正当であることを立証する。

Dev Dyn 231, 57-71)アレルを保持する三重変異体を生成することによって、SHHを一過性に過剰発現させた。11カ月齢にて、タモキシフェン投与後のCK19発現NP細胞においてrtTAが活性化された。さらに12〜13カ月齢から、マウスは、飲用水においてドキシサイクリンを与えられて、これ以降rSHHパルスと呼ばれる、(tetO)7CMV−rSHHアレルからのrSHH発現を刺激した。対照マウスは、同じ遺伝子の組合せを有しておらず、rSHH発現もなかった。次いで、マウスを18カ月に加齢させた。図13aは、18カ月齢の対照マウス及びrSHHパルスマウスの腰椎円板を示す。対照マウスの円板は、NP空間の減少、円板内のごくわずかな細胞、及びAFの層及び構造の喪失を含む、加齢に関連する変性の兆候を示す。対照的に、rSHHパルスされた円板は、網状の及び拡散されたNP細胞、並びにAF細胞の組織化された層を有して、非常に健康にみえた。SHHの免疫染色(図13aにおいて赤色で示す)は、12〜13カ月齢からrSHHの発現を刺激することで、NPがこれらの円板においてさらに多くのSHHを産生し続けるように再プログラムされたことを示す。ECMマーカーであるACANの免疫染色(図13b)は、SHHの条件付き及び一過性の過剰発現が、円板のECMの維持をもたらしたことを示す。これらの結果は、中年齢マウスの円板をSHHに一過性に曝露すると、円板構造、細胞数、及びECM産生の維持を介して、円板が再活性化され、円板の加齢を遅らせることができることを示唆する。
下背部痛の円板療法及び治療のためのSHH又はSHHシグナル伝達の活性化の可能性を試験するために、リガンドのスムーズンド受容体の下流を活性化する、SAGと呼ばれるヘッジホッグシグナル伝達経路の小分子アゴニストをマウスに投与した。成体マウスに、水、又は10mg/kg体重の濃度で水に溶解したSAGのどちらかを腹腔内投与した。マウスを7日間、毎日処置した。次の日に、RNA単離、並びにSHH及びその下流標的のリアルタイムRT−qPCR解析のために、腰椎円板を収集した。RT−qPCR解析(図14)は、SAGをマウスに注入することにより、対照と比較してSHHのmRNA発現が増加したことを示す。SHHのmRNA発現のアップレギュレーションは、その下流標的のPtch1及びGli1の発現の増加にも関連した。そして、SHHシグナル伝達の増加が、結果として、Sox9及びCol2のようなECMマーカーの発現を増加させた。これらの結果は、円板は、体内において最大の無血構造であるが、ヘッジホッグアゴニストの全身送達により、椎間円板におけるSHHシグナル伝達経路を首尾よく活性化できる証拠を提供する。
ヒト椎間円板の再生のために、SHHシグナル伝達及びその下流標的を標的する可能性を決定するために、第1に、外科的廃棄物として廃棄されるNP細胞におけるSHHのmRNA発現を試験した。このような試料には、若年及び高齢患者の両方から収集されたNP細胞が含まれた。図15は、若年患者由来のヒトNP細胞はSHHを発現するが、SHHの発現は年齢とともに減少することを示す。この分析は、マウス及びヒトの円板が、両方において、加齢にともなってSHHの発現を喪失することにより、分子的に類似することを示す。そのため、このような所見は、SHHシグナル伝達を標的することによる加齢マウス円板の再活性化が、患者に同様の効果をもたらす可能性があることを示唆する。
タモキシフェン誘導性CK19CreERT2/+系統を使用した、11カ月齢のマウスのNP細胞におけるSHH(SHHflx/flx)の条件付き標的化が、SHHの喪失の5カ月以内の加齢円板と同様の表現型を示した(図16A)。免疫染色は、SHH標的化の後に、Bra発現が喪失したことを示す(図16B)。
