本明細書に記載される方法を用いてAAV8ベクターを作製した。ベクターは、5’AAV2 ITR、TBGプロモーター、イントロン、hOTCco、WPREエレメント、ウシ成長ホルモンポリA、および3’AAV2 ITRをその中にパッケージングしている。このベクターの発現および動態学を、WPREエレメントを有するかまたは有さない自己相補的AAV8ベクターと比較した。結果は、WPREエレメントを有する一本鎖構築物が、WPREエレメントを欠いたベクターよりも優れており;同等の用量では、両方の一本鎖ベクター(WPREの有るものおよび無いもの)が、測定した時点でWPREを欠いた自己相補的ベクターよりもロバスト性が低いことを示す。

A.本明細書に記載されるhOTCco配列を含有する複製欠損レンチウイルスベクターは、プラスミドpLenti−GIII−CMV−GFP−2A−PuroのラットOTC遺伝子配列インサート[Applied Biological Materials(ABM)Inc.;Canadaから市販される]を、所望のhOTCco配列[配列番号3、4、5、9および10]で置換することによって作製することができる。ウイルスは製造業者の説明書に従って作製される。ABMシステムは、形質導入および標的細胞のゲノムDNAへの組み込みの後にレンチウイルスベクターの自己不活性化を保証するために3’ALTRのU3領域におけるエンハンサー欠失を含み;パッケージングシグナルを全てが欠いている異なるプラスミドに由来する、パッケージング、複製および形質導入(Gag/PoI/Rev)に必要な最少のレンチウイルス遺伝子を含有し;さらに、Gag、PoI、またはRev遺伝子はどれもパッケージングされたウイルスゲノムに組み込まれず、従って、成熟ウイルスには複製能力がない。

この研究では、実施例1に記載したように作製されたscAAV8.TBG.hOTCcoLW4を、新生児期(早期)発症OTCDのマウスモデルにおいて動物を救出するために使用した。エクソン2−3の欠失によりOTC KOマウスを作製し、このマウスの特性を、OTCのヌル変異を有するヒト患者に対する類似性に関して特徴付けた。要約すると、我々の研究室でエクソン2−3の欠失により作製したOTCノックアウト(KO)モデルは、ヒトにおける重篤なOTCDの新生児期発症型を厳密に模倣する。新生オスOTC KO子は、肝臓内のOTC発現が存在しないために血漿アンモニアレベルが上昇しており、生後24時間以内に必然的に死亡する。正常に繁殖するヘテロ接合性のメスは、正常な血漿アンモニアレベル、低下した肝臓OTC酵素活性、上昇した尿オロト酸レベル、および場合により野生型(WT)同腹子と比べてより低い体重を有する。出生直後に1〜3×10e10GC/子の用量で、実施例1に記載されるように作製されたscAAV8−hOTCcoベクターを単回注射すると、OTC KO子を救出し、6週間まで延命させることができる。長期間の補正を達成するために、4週齢のOTC−KOマウス群に、実施例1に記載されたように作製したscAAVrh10−hOTCcoベクターの2回目のベクター投与を受けさせた。
。救出された子はその正常な同腹子よりも低い体重を有し、被毛疎および皮膚異常の一過性の表現型を有した。最も重要なことに、その血漿アンモニアレベルが正常範囲であった。しかしながら、効力は、新生児期の高速の肝臓増殖中のベクターゲノムの喪失のために6週間を超えて維持することができない。4週齢OTC−KOマウスにおけるscAAVrh10−hOTCcoベクターの2回目のベクター投与は、さらに成人期まで延命させることができる。最高齢マウスは18月齢に達した。長期間救出マウスは、正常に近い血漿アンモニアレベルを示すが、これらのマウスの一部では尿オロト酸レベルが著しく上昇した。異なる年齢(6、12、および18月齢)のヘテロ接合性マウスからの肝臓サンプルにおけるシリウスレッド染色は、11歳のOTCD患者からの肝臓サンプルと類似して、高齢(18月齢)OTC−KOヘテロ接合性メスマウスにおいて肝線維症を示した。
2月齢のOTC−KOヘテロ接合性マウスに、1×1010、3×1010、および1×1011ベクターゲノムコピー/マウスにおいて、コドン最適化ヒトOTC遺伝子(配列番号5)をコードする自己相補的AAV8ベクターの単回の尾静脈注射を受けさせた。ベクター処置の1週間後に、3つのベクター用量群の全てのマウスは正常な尿オロト酸レベルを有し、これは研究の間中(16カ月間)保持された。病理分析のために肝臓サンプルを18月齢の処置マウスから採取し、年齢を適合させた未処置のヘテロ接合性マウスおよびWT同腹子と比較した。全ての処置マウスは、肝臓全体に線維症を有した未処置のヘテロ接合性動物とは対照的に、WTと同様の正常な肝臓組織像を示した。結論として、AAV8sc−hOTCcoベクターの単回注射は、OTC−KOヘテロ接合における肝線維症を予防し、OTCD患者における肝線維症の補正のために大きな可能性を有することができる。

- 前記発現カセットがさらに、イントロン、コザック配列、ポリA、および転写後調節エレメントのうちの1つまたは複数を含む、請求項1〜8のいずれか一項に記載の組換えウイルスベクター。
- 前記組換えウイルスベクターが、少なくとも1つのITR配列および合成hOTCを含む核酸配列がその中にパッケージングされたAAVカプシドを含む組換えAAVベクターである、請求項1に記載の組換えウイルスベクター。
- 前記AAVカプシドがAAV8、AAV9および/またはAAVrh10から選択される、請求項10に記載の組換えウイルスベクター。
- AAVカプシドを有し、かつその中に、少なくとも1つのAAV逆位末端反復(ITR)配列と、ヒトの少なくとも成熟オルニチントランスカルバミラーゼ(hOTCase)をコードする改変核酸配列と、肝細胞におけるhOTCaseの発現を誘導する発現制御配列とを含む発現カセットがパッケージングされた組換えアデノ随伴ウイルス(rAAV)であって、前記発現制御配列が肝臓特異的プロモーターを含み、前記hOTC核酸配列
が、少なくとも配列番号1の成熟hOTCに関して野生型hOTC配列と80%未満の同一性であり、少なくとも配列番号5の成熟hOTC、またはそれと少なくとも約96〜約99.9%同一である核酸配列、または少なくとも配列番号9の成熟hOTC、またはそれと少なくとも約96〜約99.9%同一である核酸配列を含む、組換えアデノ随伴ウイルス。 - 前記AAVカプシドがAAV8、AAV9、またはAAVrh10から選択される、請求項12に記載のrAAV。
- 前記発現カセットがさらに、5’AAV逆位末端反復(ITR)配列および3’ITR配列を含む、請求項12または13に記載のrAAV。
- 前記少なくとも1つのAAV ITRが、D配列および末端分解部位が欠失された5’ITRを含む、請求項12または13に記載のrAAV。
- 前記5’および3’ITRがAAV2に由来する、請求項12または13に記載のrAAV。
- 前記合成hOTCが配列番号3のコード配列を有する、請求項12〜16のいずれか一項に記載のrAAV。
- 前記合成hOTCが配列番号4のコード配列を有する、請求項12〜16のいずれか一項に記載のrAAV。
- 異種トランジット配列を有する成熟ヒトオルニチントランスカルバミラーゼを少なくとも含むキメラオルニチントランスカルバミラーゼをコードするhOTC遺伝子を含むウイルスベクターであって、前記成熟ヒトオルニチントランスカルバミラーゼ由来のコード配列が、配列番号3、4、5、8または9の核酸配列のものから選択される、ウイルスベクター。
- 担体と、請求項1〜11のいずれか一項に記載のベクター、請求項12〜18のいずれか一項に記載のrAAV、および/または請求項19に記載のウイルスベクターの有効量とを含む医薬組成物。
- ヒト患者のオルニチントランスカルバミラーゼ欠損症に関連する疾患または病状を処置する方法で使用するための、請求項1〜11のいずれか一項に記載のウイルスベクター、請求項12〜18のいずれか一項に記載のrAAV、および/または請求項19に記載のベクター。
