Abstract
Description
APPAのレベル/機能活性に基づいて又は有糸***遅延の表現型を示す細胞を同定することにより浸潤性乳癌を発症し易い患者の同定を可能にする。したがって、***組織サンプル内の増殖性病変は、非浸潤性のままである可能性が高い又は浸潤性になり易いのいずれかとして分類することができる。
のDNA、主にPAPPAプロモーター領域のメチル化によるものであることを突き止めた。CpGジヌクレオチド内のシトシンへのメチル基の共有結合による付加により主に生じるDNAのメチル化は、遺伝子のプロモーター領域中で主に起こり、これはそこに大部分のCpGアイランドが見出されるからである。ハイパーメチル化は転写のサイレンシングを引き起こす。
2000; 28(8)及びWidschwendler et al, Cancer Res., 2004; 64:3807-3813に記載されている。代替的方法には、Combined Bisulphate Restriction Analysis法、Methylation-sensitive Single Nucleotide Primer Extension法、及びCpGアイランドマイクロアレイの使用が含まれる。DNAメチル化を調べるための市販のキットが利用可能である。したがって、本発明は、患者の浸潤性乳癌に進行するリスク又は乳癌を判定するための、PAPPA遺伝子又はその制御プロモーター配列のメチル化状態を決定する従来の方法を用いる。

iii.例えばABI Step one plusソフトウェアを用いて、データを分析し、Ct値を計算するステップ;
iv.サンプルのPAPPA:COL2A1比を陽性対照サンプル(例えば、SssI処理HeLaゲノムDNA)のPAPPA:COL2A1比で割り、100を乗じることにより特異的座位における完全にメチル化されたPAPPA分子のパーセンテージを計算するステップ。
につながるように変異している可能性がある。点変異、欠失、ヘテロ接合性の消失、転座等は全てPAPPA遺伝子の転写活性を失わせ得る。遺伝子レベルでの改変がPAPPAの発現を低減(又は消失)させ得るが、遺伝子レベルでの改変(変異)により機能的活性が低減又は消失したPAPPAの発現が生じることもあり得る。したがって、本発明は、PAPPA活性レベルが診断を助けるために用いられると予想する。活性消失の原因となる変異ホットスポットが同定されてもよく、患者サンプル中のそのようなホットスポットの同定も診断に寄与し得る。
ヘテロ二本鎖分析、タンパク質切断試験、RNASE A切断法、化学的/酵素ミスマッチ切断、DNAチップ上でのアレル特異的オリゴヌクレオチドハイブリダイゼーション、ブロッキング試薬を用いた(野生型アレルの増幅を抑制するため)アレル特異的PCRと続いてのリアルタイムPCR、PCR産物の直接シークエンシング、パイロシークエンシング、及び次世代シークエンシングシステムが含まれる。
発現レベルを検出するためのノーザンブロッティング技術又は定量的PCRの使用は当該技術分野で周知である。定量的PCRに基づく遺伝子発現解析用のキット、例えばキアゲン社製のQuantitectシステムが市販されている。ハイブリダイゼーションに基づく核酸アレイシステムを用いた複数のサンプル中の発現レベルの同時解析が当該技術分野で周知であり、これも本発明の範囲内である。当業者に理解されるように、変異特異的PCRを用いることもできる。

行する増殖性病変のリスク及び/又は再発性非浸潤性疾患のリスクがあることを示している。例えば、組織サンプル内の細胞の5個が有糸***中であるとして同定された場合、有糸***遅延表現型が同定されるため並びに/又は浸潤癌に進行する増殖性病変のリスク及び/若しくは再発性非浸潤性疾患のリスクが判定されるためには、これらの細胞の少なくとも2個が前期/前中期になければならない。より好ましくは、有糸***遅延表現型を示す患者に由来する***組織サンプル中の前期/前中期にある有糸***細胞の割合は33%、35%、40%、45%、50%、55%、60%、65%超である。典型的には、有糸***中の非浸潤性健康***組織細胞の「対照」値は前期/前中期にある約10〜25%、例えば23%の細胞である。

組織標本
ホルマリン固定及びパラフィン包埋された組織を、UCLの病理学部(Department of Pathology)(UCL病院、英国、ロンドン)のアーカイブから取得し、これには以下が含まれた:浸潤性乳癌(n=182、このうち156例を有糸***期分布の分析に含めた);乳管上皮内癌(DCIS;n=81、69例が評価可能);***縮小術標本に由来する正常***組織(n=33、評価可能性、不適);妊娠患者に由来する正常***組織(n=5、全て評価可能);結腸腺癌(n=41、全て評価可能);膀胱移行上皮癌(n=27、全て評価可能);陰茎扁平細胞癌(n=33、全て評価可能);胃腺癌(n=21、全て評価可能);悪性黒色腫(n=21、全て評価可能);小細胞肺癌(n=30、全て評価可能);及び非ホジキンリンパ腫(n=29、全て評価可能)。利用可能な組織学的材料及び臨床病理学的情報に基づいて症例を選択した。組織標本は、診断時に資格を有する病理学者に検査され、世界保健機関(WHO)の基準に従って組織学的サブタイプ及び核異型度を評価されたものである。標本上に少なくとも5個の有糸***細胞が見られた場合、症例は有糸***期分布の分析に評価可能とした。Joint UCL/UCLH Committees on the Ethics of Human Researchから倫理的承認を得た。
細胞単分子層を調製するために、0.5×106個の細胞を、Cytospin 4 cytocentrifuge(サーモサイエンティフィック社製)を用いて800×gで5分間スライドガラス上にサイトスピンし、風乾し、4%中性緩衝ホルマリン中で一晩固定した。
28日間の免疫化プロトコール(ユーロジェンテック社(Eurogentec))に従ってPAPP−Aウサギポリクローナル抗体(PAPP−A PAb)を合成ペプチド(aa1384〜1399)に対して産生させた。使用したその他の抗体には、ミリポア社のセリン10でリン酸化されたヒストンH3(H3S10ph;06−570)、ダコ社のPAPP−A(A0230)、シグマ社のβアクチン(AC−15)、BDファーミンゲン社のCD29(HUTS−21)、ケミコン社のβ1−インテグリン(MAB1959)、及びインビトロジェン社のAlexa Fluor 594が含まれる。
切片の脱パラフィン、抗原賦活化、及び免疫染色はBond III Autostainer及びBond Polymer Refine Detectionキット(ライカ社製)をメーカーの指示書に従って用いて行った。熱を介した抗原賦活化及びタンパク質消化をそれぞれH3S10ph及びPAPPA抗原に用いた。以下の希釈で一次抗体を40分間アプライした:PAPP−A、1/200;H3S10ph、1/4000。脱パラフィンステップのない同じプロトコールでサイトスピン調製物を免疫染色した。一次抗体なしのインキュベーションを陰性対照とし、扁桃腺及び胎盤の切片をそれぞれH3
S10ph及びPAPPA抗体の陽性対照として用いた。
各組織サンプルに由来する2つの連続切片及び各細胞系につき2つのサイトスピン調製物を、有糸***期分布を分析するためにH3S10phについて免疫染色した。拡大率400倍の5〜20個のフィールドから得られた最低5個の有糸***細胞をCV12 CCDカメラ及びイメージキャプチャーソフトウェア(image capturing software:SIS)を用いて取り込んだ。標本上の有糸***細胞が5個未満であった場合、症例を分析から除外した。従来の形態学的基準に従って有糸***細胞を前期/前中期、中期、後期、又は終期に分類した。
組織切片を脱パラフィンし、マイヤーヘマトキシリンで5秒間染色し、風乾した。拡大率100倍のフィールドを針顕微解剖(needle micro−dissect)し、1mg/mlのプロテイナーゼK(シグマ アルドリッチ社製)55μl中、55℃で48時間インキュベートした後、ゲノムDNAを抽出した。
浸潤性乳癌(n=182)、DCIS(n=81)、及び正常***(n=34)の細胞系及び症例に由来するゲノムDNAをMethyLight分析(Widschwendter, M. et
al. Cancer research (2004) 64, 3807-3813)に用いた。浸潤性乳癌の9例、DCISの6例、及び正常***の4例はDNA抽出に充分な材料が標本上に見られなかったため分析から除外した。NanoDrop分光測定法によりDNA濃度を決定し、参照遺伝子を用いたqPCRによりDNAの品質を検証した。全ての***サンプルの平均遺伝子増幅は34.39サイクル(SD=2.07)と算出された。全てのサンプルについて、EZ DNA Methylation−Goldキット(ザイモリサーチ社(Zymo Research)製)をメーカーの指示書に従って用いて400ngのゲノムDNAをバイサルファイトで改変した。非改変SssI処理ゲノムDNA(ニュー・イングランド・バイオラボ社製)を陽性対照として用いた。バイサルファイト改変DNAは使用時まで−80℃で保存した。MethyLightを用いた定量的PCR分析を全てのサンプルに行った。MethyLightのプライマー及びプローブのヌクレオチド配列は、目的の遺伝子のプロモーター又は5’末端領域中に設計した。特異的座位での各MethyLight反応は平均5〜10個のCpGジヌクレオチドをカバーした。分析した全座位のプライマー及びプローブ(メタビオン社(Metabion)製)の詳細なリストを表1及び2に示す。バイサルファイト改変DNAに特異的に設計された2セット(目的の遺伝子用及びインプットDNAを標準化するための参照遺伝子(COL2A1)用のメチル化セット)のプライマー及びプローブを各座位に用いた。SssI処置ヒト白血球細胞DNA(高度にメチル化されている)を用いてメチル化DNAに対する反応の特異性を別個に確認した。サンプルの目的の遺伝子:COL2A1比をSssI処理ヒト白血球細胞DNAの目的の遺伝子:COL2A1比で割り、100を乗じることにより、特異的座位で完全にメチル化されている分子のパーセンテージ(PMR;メチル化基準に対するパーセンテージ)を計算した。
BT549、T47D、BT474、MDAMB157、MDAMB453、MCF10A、ヒト***上皮細胞(HMEpC)はRodriguez-Acebes et al, Am. J. Pathol, 2010; 177:2034-2045に記載されているように培養した。HeLa Kyoto細胞は、10%FCS(インビトロジェン社製)を補充したDMEM(インビトロジェン社製)中、37℃、5%CO2で培養した。
二重チミジンブロック(シグマ社)によりHeLa Kyoto細胞をS期で同調させた。簡潔に述べると、終濃度3mMのチミジンを培養培地に18時間添加した後、新鮮な培養培地に9時間放出し、3mMチミジンを用いて2回目のブロックを17時間行った。終濃度5ng/mlのPlk1阻害剤BI2536(セレックケム社製)で24時間処理することによりHeLa Kyoto細胞をM期で同調させた。細胞の同調をフローサイトメトリーで確認した。
細胞増殖評価及びフローサイトメトリーによる細胞周期分析はRodriguez-Acebes et al, Am. J. Pathol, 2010; 177:2034-2045に記載されている通りに行った。
PAPPA mRNAを標的とする特異的RNA二本鎖(アンビオン社のs10042;センス5’−GAGCCUACUUGGAUGUUAAtt−3’(配列番号37)及
びアンチセンス5’−UUAACAUCCAAGUAGGCUCtg−3’(配列番号38)、あるいは二本鎖のプール(ON TARGETplus SMARTpool、Dharmacon社製)を用いたRNAiによりPAPPAのサイレンシングを行った。非標的化siRNA(Stealth RNAi Negative Control Med GC、インビトロジェン社製)を陰性対照として用いた。全てのトランスフェクションはLipofectamine 2000(インビトロジェン社製)を用いて行った。終濃度100nMのPAPPA siRNAを用いて効率的なノックダウンを達成した。トランスフェクション後の記載の時点で細胞を回収した。qRT−PCR及び/又はウェスタンブロットによりノックダウン効率を評価した。
Tudzarova et al, EMBO J., 2010; 29:3381-3394に記載されているように、細胞から全RNAを単離し、100ngの全RNAを用いてqRT−PCRを行った。プライマー配列は以下の通りである:PAPPAフォワード5’−ACAGGCTACGTGCTCCAGAT−3’(配列番号39)及びリバース5’−CTCACAGGCCACCTGCTTAT−3’配列番号40);RPLPO(インバリアント対照として使用されるリボソームタンパク質)フォワード5’−CCTCATATCCGGGGGAATGTG−3’配列番号41)及びリバース5’−GCAGCAGCTGGCACCTTATTG−3’(配列番号42)。
免疫蛍光では、HeLa Kyoto細胞をカバースリップ(12mm #1 VWRインターナショナル社製)上で成長させ、前述したように同調させた。同調させた細胞をPBSですすぎ、1%PFA中で固定し、0.1%トリトンX−100/0.02%SDSを用いて透過処理し、2%BSA中でブロッキングした。ブロッキングステップ後、ホスホヒストンH3(H3S10ph)抗体を1/500希釈で1時間アプライし、PBSで3回洗浄した後、Alexa Fluor 594抗体を1/300希釈で1時間添加した。カバースリップをPBSで3回洗浄し、減衰しないDAPIの入ったVectashield(ベクターラボラトリーズ社製)を用いてマウントした。
細胞増殖評価及びフローサイトメトリーによる細胞周期分析は上記のRodriguez-Acebesに記載されているように行った。
PAPPA及びZMPSTE24(対照)はメトジンシンスーパーファミリーの亜鉛依存性メタロプロテイナーゼである。全長ヒトPAPPA cDNA(NM_002581.3)及びZMPSTE24 cDNA(NM_005857.2)をpCMV6−XL5ベクター(オリジーン社製)にクローニングした。T75フラスコ中で培養した約2×106のT47D細胞を40μgのPAPPA又はZMPSTE24 cDNAでトランスフェクトした。トランスフェクションの48時間後及び72時間後に細胞を回収した。qRT−PCR及びウェスタンブロットによりPAPPA及びZMPSTE24発現レベルを決定した。
2つの異なるPAPP−A siRNA(アンビオン社のs10042及び104028)の標的となる領域内のヒトPAPPA cDNA(NM_002581.3)の2つの別個の部位に8個のサイレント変異を導入した。変異cDNAをpCMV6−XL5ベクター(オリジーン社製)にクローニングし、BT549細胞を20μgのPAPPAレスキュープラスミドでトランスフェクトした。PAPPA+mut過剰発現の24時間後
に、100nMのPAPPA特異的siRNA(アンビオン社のs10042又は104028)を用いて内因性mRNAをノックダウンした。内因性mRNAノックダウンの48時間後に細胞を回収した。
細胞外マトリックス(ECマトリックス)を介した浸潤をBoyden chamberアッセイ(QCM Invasion Assay、ミリポア社製)をメーカーの指示書に従って用いて測定した。簡潔に述べると、無血清培地中で24時間、PAPPA ON−TARGETplus SMARTpool又は対照オリゴでBT549細胞をトランスフェクトした。T47D細胞をPAPPA又はZMPSTE24発現コンストラクトで24時間トランスフェクトした後、無血清培地中で24時間飢餓状態にした。5%BSAを含むRPMI培地中にBT549及びT47D細胞を回収し、カウントし、2.5×105細胞を各浸潤チャンバー中に播種した。48時間(BT549)又は72時間(T47D)インキュベートした後、浸潤チャンバーインサートをPBSで洗浄し、4%パラホルムアルデヒド中で5分間固定し、0.1%クリスタルバイオレットで染色し、フィルター上のランダムな領域に由来する細胞をカウントした。アッセイは3連で行った。
(Rizki, A. et al., J. Cancer research (2007) 67, 11106-11110に記載されているように)生きた細胞を懸濁液中で免疫染色し、2%PFA中で固定し、Rodriguez-Acebes
et al, Am. J. Pathol, 2010; 177:2034-2045に記載されているようにFACSを行った。蛍光ピークを中央値の値で評価し、一次抗体(抗CD29 HUTS−21、BDファーミンゲン社製)と一緒にインキュベートしなかったサンプルを用いて補正した。β1−インテグリンをマスキングするために、浸潤アッセイの間、20μg/mlのブロッキング抗体(抗β1−インテグリンMAB1959、ケミコン社製;Vincourt, J. B. et al.
(2010)Cancer research 70, 4739-4748)を培養培地に加えた。
前悪性及び悪性組織の各標本について前期/前中期にある有糸***細胞の割合を計算した。種々の最低数の有糸***細胞について、前期/前中期にある細胞の割合を用いて(プールされた)他の悪性腫瘍に由来する分化中の乳癌の受信者動作特性(ROC)曲線を作製した。最低必要数をたった5個の有糸***細胞とすることでROC曲線が損なわれないことが明らかになった(図3)。可能な限り多くの標本を用いるために、有糸***遅延の分析の評価可能性閾値は、標本1個当たりに観察される有糸***細胞少なくとも5個に設定した。有糸***細胞の少なくとも3分の1が前期/前中期にあった場合、標本を「遅延」とした。この条件は、乳癌と他の悪性腫瘍との区別に関する感度と特異性のバランスをとることにより導かれた:評価可能な乳癌標本の94.9%で有糸***細胞の少なくとも3分の1が前期/前中期にあり、評価可能な他の悪性腫瘍の94.1%で前期/前中期にある有糸***細胞が3分の1未満であった(図4)。ピアソンのカイ二乗検定及びイェーツの連続補正を用いて、有糸***遅延のある評価可能な標本の割合を標本の出所(乳癌、DCIS、他の悪性腫瘍)間で比較した。マン・ホイットニー検定を用いて前期/前中期にある有糸***細胞の割合の中央値を標本の出所間で比較した。記載されている全ての有意性の確率は両側検定である。
乳癌では初期有糸***像が多い
皮膚、肺、結腸、胃、膀胱、陰茎、及びリンパの癌を含む7つの一般的なヒト腫瘍タイプに由来する評価可能な組織標本(n=患者202例)において、有糸***細胞の大部分は中期にあった(図1a〜b及び図5a〜b)。顕著に対照的に、本発明者らは、他の悪性腫瘍を合わせた群のたった6%(202例中12例)に対し、乳癌の95%(評価可能患者156例のうち148例)で強い前期/前中期濃縮(前期/前中期にある少なくとも3分の1の有糸***細胞と定義される)を見出した(P<0.0001、ピアソン検定及びイェーツ補正)。有糸***細胞の割合の平均は乳癌では58%(中央値56%)であり、他の悪性腫瘍では23%(中央値23%)であった(P<0.0001、マン・ホィットニー検定)(図1b〜c及び図5a)。このことは、初期の有糸***の遅延又は停止が乳癌の特徴であることを示している。これは、正常に増殖している***組織は有糸***期分布が乱れておらず、有糸***細胞の23%(中央値24%)が前期/前中期にあったので、疾患組織に特異的であった(図1b及び図5b)。重要なことに、本発明者らは、非浸潤性の乳管上皮内癌(DCIS)病変の80%(評価可能患者69例のうち55例)で既に明確な有糸***遅延表現型を検出できることを見出した(他の悪性腫瘍の群との比較でP<0.0001)、DCIS病変では、前期/前中期にある有糸***細胞の割合の平均が48%(中央値45%)であった(他の悪性腫瘍の群との比較でP<0.0001)(図1c及び図6)。したがって、細胞培養モデルにおける遺伝子サイレンシングによって最初に観察された(Neumann, B. et al. Nature (2010) 464, 721-727)特定の有糸***表現型についての腫瘍スクリーニングにより、試験したほぼ全ての乳癌において予想外に高頻度な初期有糸***像(前期/前中期)が同定され、以前には認識されていなかったこの種の腫瘍における有糸***進行の遅延が明らかとなった。
MitoCheckスクリーニング(Neumann, B. et al.)において初期有糸***遅延は非常に特異的且つ比較的珍しい有糸***表現型であった。本発明者らが乳癌で観察したのと同様な初期有糸***表現型となる候補遺伝子を培養ヒト細胞におけるダウンレギュレーションにより同定するために、乳癌組織において本発明者らが観察した表現型に形態学的に最も似ている前期/前中期クラスに特にフォーカスしてMitoCheckデータベース(www.mitocheck.orgでアクセス可能)をサーチした(図7)。ゲノム全体のデータセット中、KIF11、PLK1、TUBB2C、TPX2、PAPPA、SGOL1、及びPSMD8が前期/前中期クラスの有意な増加を示し(図8)、これは乳癌で検出されたような前期/前中期における遅延又は停止を示している。これらの個々の遺伝子のノックダウンに対する時間分解的な表現型の応答を定量的にスコア化したところ、7つの遺伝子全てについて、一次表現型としての前中期停止、次いで二次表現型(例えば、細胞死又は多葉状の核形状)が示された(図8)。KIF11、PLK1、TUBB2C、及びTPX2を標的としたRNAi実験では、トランスフェクションの12〜25時間後には既に前中期にある細胞のパーセンテージが高く、一方、PAPPA、SGOL1、及びPSMD8のノックダウンは前中期表現型の浸透率がそれより低かったが、全体的に同様な表現型プロファイルを示した(図8)。有糸***表現型の結果としての細胞死がPAPPA及びSGOL1を除く全ての遺伝子で有意であり(図8)、このことから、これらは有糸***異常が細胞死によりクリアされないと予想される腫瘍抑制遺伝子の最も有力な候補である。
プロモーターのメチル化は、癌発生中の腫瘍抑制因子の機能喪失の一般的なメカニズムであるので、本発明者らは、7つのMitoCheck候補遺伝子のいずれかのエピジェネティックなサイレンシングを、本発明者らが乳癌で発見した有糸***遅延表現型に関連付けられるという仮説を立てた。実際、MethyLightアッセイ(Widschwendter,
M. et al. Cancer research (2004) 64, 3807-3813)により、7つの候補遺伝子のうち、PAPPAのみが浸潤性乳癌及び非浸潤性DCIS病変において遺伝子の5’制御領域が強くハイパーメチル化されていることが示された(図9a)。乳癌の46%(メチル化を評価した患者173例中80例)及びDCIS病変の45%(評価した患者75例中34例)がPAPPAのハイパーメチル化を示した(PMR>1;メチル化基準遺伝子に対するパーセンテージ)。対照的に、PAPPAは正常***組織サンプルの大部分(評価した患者30例中27例)においてメチル化されていなかった(図9b;乳癌9例、DCIS6例、及び正常***4例はDNAの保存性が悪かったためMethyLight分析に利用可能でなかった)。このことから、PAPPAが乳癌に見られる強い前期/前中期遅延を説明する最も有力な候補となった。PAPPAプロモーターのメチル化が実際に遺伝子サイレンシングを引き起こしたかどうかを調べるために、本発明者らは、市販の抗PAPPA抗体(ダコ社製)を用いて組織切片の免疫標識を行い、アフィニティー精製されたウサギ抗PAPPA PAb(図10)を用いてウェスタンブロッティングを行った。免疫発現分析により、メチル化PAPPAプロモーターを有し且つ有糸***遅延表現型を示す非浸潤性及び浸潤性の乳癌の実に96%(患者84例中81例)がPAPPAタンパク質を発現していないことが示された(図9c)。対照的に、非遅延/PAPPA非メチル化乳癌の大部分(73%、患者11例中7例)及び正常増殖妊娠***組織(n=患者5例)は、主に細胞膜で強いPAPPA免疫染色を示し、これは分泌型タンパク質と一致する(図9c)。
本発明者らは、患者組織で観察されたPAPPA発現消失と有糸***遅延との間の関連を再現した乳癌細胞系を入手し、RNAiによるPAPPAノックダウンが、プロモーターのメチル化により遺伝子がサイレンシングされていないBT549中で前期/前中期の遅延を誘発するかどうかを最初に調べた(図9e〜f)。対照siRNAと比べ、PAPPA mRNAを標的とした4つのRNA二本鎖のプールによるBT549細胞のトランスフェクションは、転写産物レベルを約70%、PAPP−Aタンパク質レベルを約60%低下させた(図11a)。ホスホヒストンH3(H3S10ph)について免疫標識されたBT549サイトスピン調製物の分析により、実際、PAPPAのノックダウンがこの細胞系において、強く前期/前中期遅延表現型を誘発することが明らかとなり(図11c)、これはMitoCheckスクリーニングにおいてHeLa細胞で観察された表現型と非常に似ていた(Neumann, B. et al. Nature (2010) 464, 721-727)。前期/前中期にある有糸***BT549細胞の割合増加と一致して、PAPPAノックダウンは、細胞集団倍加時間を顕著に長くし(図11d)、同時にG2/MのDNA含有量の細胞の割合が約2倍に増え(図11e)、有糸***指数が3倍上昇した。本発明者らは、転写産物の異なる領域を標的とする単一siRNA二本鎖(siRNA−28及びsiRNA−42)でBT549細胞をトランスフェクトすることにより、PAPP−A枯渇により生じ
る前期/前中期遅延表現型を検証した(図12)。RNAi表現型がPAPPA枯渇による特異的なものであることを確認するために、本発明者らは、siRNA−28及びsiRNA−42に抵抗性のPAPPA cDNAバリアントを過剰発現させた。このコンストラクトの発現はPAPPAタンパク質発現を回復させ、2つの単一siRNA二本鎖のいずれかにより生じた有糸***遅延表現型をレスキューした(図12)。T47D細胞等のハイパーメチル化PAPPAプロモーターを有する乳癌細胞における有糸***遅延表現型をPAPPA消失が引き起こす場合(図9d〜e)、本発明者らは、この実験系中、PAPPA過剰発現が表現型をレスキューするはずであると考えた。実際、T47D細胞をPAPPA cDNA(PAPPA+)でトランスフェクトするとPAPPAのmRNA及びタンパク質のレベルが回復し(図11b)、有糸***遅延表現型が完全に逆転し、PAPPAの発現が正常なBT549細胞と非常に似た有糸***期分布となった(図11c)。総合すると、これらの結果は、初期有糸***の進行にPAPPAが必要であること及びエピジェネティックなサイレンシングを介したPAPPAダウンレギュレーション(T47D細胞)又はRNAiによる実験的PAPPAダウンレギュレーション(BT549細胞)が強い前期/前中期遅延表現型を引き起こすことを示している。
次に、本発明者らは、初期有糸***進行の混乱が新生物***細胞にどのような生物学的利点をもたらすのかと考えた。この疑問に取り組むために、通常の臨床調査中に各乳癌標本について決定された有糸***遅延表現型と臨床病理学的特徴との関連を探すことから始めた(n=評価可能患者156例)。この分析により、有糸***遅延と腫瘍分化(グレード)、形態学的サブタイプ(浸潤性管、小葉、混合型、粘液性、及び微小乳頭)、分子サブタイプ(luminal、Her−2、又はトリプルネガティブ/basal−like)、エストロゲン/プロゲステロン受容体状態、結節転移、又は異数性との間には関連がないことが明らかになった。しかし、特に、有糸***遅延表現型が、調べたほぼ全ての浸潤性乳癌標本に存在するが(95%、評価可能患者156例中148例)、非浸潤性DCIS病変では一部にしか存在しない(80%、69例中55例)ことから、この有糸***異常は浸潤性の獲得に関連している可能性が考えられる。この具体的な仮説を試験するために、本発明者らは、PAPPAノックダウンによりBT549細胞中で有糸***遅延表現型を誘発し(あるいは、外因性PAPPA発現によりT47D細胞中で正常な有糸***進行を回復させ)、マトリゲルコートBoyden chamberアッセイにより操作細胞の浸潤性を測定した。並行して、本発明者らは、フローサイトメトリーを用いて、よく特徴付けられている乳癌の浸潤マーカーであるβ1−インテグリンの細胞表面レベルを決定した。PAPPAのサイレンシングは、浸潤細胞数の顕著な(2倍)増加と関連した(図13a〜b)。PAPP−A枯渇BT549細胞の浸潤性増大はβ1−インテグリン細胞表面レベルの増加にも反映されていた(図13c)。特に、PAPPAノックダウンに関連する浸潤性の増大は、細胞をBoyden chamberに移す前にβ1−インテグリンブロッキング抗体でBT549細胞を処理することで完全に逆転された(図13d)。逆に、外因性PAPPA発現による正常有糸***期分布の再確立はT47D細胞の浸潤性を強く低減した(図13a〜b)。これらの結果は、PAPPA機能の消失が、初期有糸***の進行を遅らせ、乳癌細胞がより浸潤性になる能力を増大させることを示している。
外因的に添加されたIGF−1は、前期/前中期遅延表現型を示す乳癌細胞において有糸***の正常な進行を回復させる。
イラビリティを増大させることにより、PAPPA活性は、IGF依存性シグナル伝達を増大させ、細胞成長及び増殖を促進する。したがって、PAPPAはIGF経路を介したシグナル伝達を調節することにより乳癌細胞中における有糸***進行に影響を与える可能性があるという仮説を立てることができる。このことから、生体内で腫瘍細胞と非常に似ている(すなわち、PAPPAプロモーターメチル化及び有糸***遅延表現型を示す;図14)T47D乳癌細胞を組換えIGF−1で処置することにより、この細胞系において有糸***の正常な進行が回復すると考えられる。これらの実験のために、T47D細胞を48時間血清飢餓にした。トリパンブルー生体染色を用いて細胞の生存率を確認した。終濃度100μMの組換えヒトIGF−1(Imgenex社製)で細胞を処理し、回収した。細胞は24時間後に回収し、PBSに再度懸濁し、スライドガラス上にサイトスピンし、前述したようにH3S10ph免疫染色を行った。実際、IGF−1の添加は前期/前中期にある有糸***T47D細胞の割合を約半分に低減し(図15)、これはPAPPAを発現するBT549乳癌細胞で見られるレベルに近い。この発見から、当業者は、抗有糸***剤を用いた追跡処置が癌細胞殺作用を増強すると推論することができる。したがって、有糸***ブロックを取り除く第1の薬物及び前中期段階後の有糸***を標的とする第2の薬物を用いた逐次処置により、腫瘍細胞が第2の薬物に対して化学療法剤感受性になる。
細胞培養
T47D細胞は、10%FCS及び10μg/mlインスリンを補充したRPMI培地(ATCC)を用いて培養した。細胞は37℃、5%CO2、湿度95%以上で培養した。TrypLE(商標) Express(ライフテクノロジーズ社製)とインキュベートした後、細胞を回収した。Countess(登録商標) Automated Cell Counter(ライフテクノロジーズ社製)を用いてトリパンブルー排除により細胞密度及び生存率を決定した。PDT=(t2−t1)/3.32×(log n2−log n1)(式中、tはサンプリング時間、nはサンプリング時の細胞密度である)により集団倍加時間を計算した。図14Aに示す顕微鏡写真は、ライカ社製倒立顕微鏡に装着したパナソニック社製デジタルカメラを用いて撮影した。
60〜70%のコンフルエンシーに達したT47D細胞をトリプシン処理により回収した。培地上清を、トリプシン処理細胞と共にプールし、194×gで3分間遠心した(このステップは、全ての有糸***細胞の保持を確実にするため及び有糸***シェイクオフによるロスを回避するために含めた)。細胞ペレットを洗浄し、PBSに再懸濁し、濃度を2×106細胞/mlとした。次いで、再懸濁した細胞をCoulterフローサイトメトリーチューブに移し、ボルテックスを用いて撹拌しながら1.5mlの氷冷100%エタノールを滴下することにより固定した。次いで、細胞を氷上に30分間置いて固定した。エタノール固定後、194×gで5分間遠心することにより細胞をペレット化した。上清を注意深く除去し、細胞を2mlのPBS(ボルテックスで撹拌しながら滴下)で洗浄した。細胞を最終的に300μlのDNA Prep PI溶液(ベックマン・コールター社製)に再懸濁し、暗黒下、室温で10分間インキュベートした後、Navios Flow Cytometerを用いて細胞周期を分析した。Multicycle−AVソフトウェアを用いてデータを分析した。
Ambion PureLink RNA Miniキット(ライフテクノロジーズ社製)をメーカーの指示書に従って用いて全細胞RNAを単離した。qPCR反応は、SuperscriptIII Platinum SYBR Green One−Step qRT−PCRキット(ライフテクノロジーズ社製)をメーカーの指示書に従って用
いて行った。PCR反応はStepOne Plus Real Time PCRシステム(ライフテクノロジーズ社製)を用いて行った。400ngの鋳型RNA及び200nMの最終プライマー濃度を個々の各PCR反応に用いた。PCR産物を更に、図14Dに示すようにアガロースゲル電気泳動にかけた。
Bradfordタンパク質アッセイキット(ピアス社製)をメーカーのプロトコールに従って用いてタンパク質濃度を決定した。Novex(登録商標) 4−20%Tris−グリシンSDS PAGE(ライフテクノロジーズ社製)により、20〜30μgの細胞質ゾルタンパク質及びMagicMark(商標)(ライフテクノロジーズ社製)を分離した。iBlot(登録商標)ドライエレクトロブロッティングシステム(ライフテクノロジーズ社製)を用いてタンパク質をポリアクリルアミドゲルからPVDFメンブレン上に転写した。簡潔に述べると、10%ミルクを補充したPBS中でメンブレンを1時間ブロッキングした。ポリクローナル抗PAPP−A抗体(アカデミックな協力者からの寄贈)を用いて更にメンブレンをプローブした。このステップは穏やかに撹拌しながら4℃で一晩行った。一次抗体とのインキュベート後、メンブレンをPBSで10分間、5回洗浄した。10%ミルクの入ったPBS中のHRPコンジュゲート二次ヤギ抗ウサギ抗体(ダコ社製)をメンブレンに加え、室温で1時間インキュベートした。ECLSelect(商標)キット(GEヘルスケア社製)の等体積の試薬A及びBをメンブレンに加え、室温で3分間インキュベートした。図14Eに示す画像はGeneGnome化学発光検出システム(シンジーン社(Syngene)製)を用いて取得した。メンブレンを抗βアクチン(シグマ アルドリッチ社製)抗体で再度プローブして、各レーン中の全タンパク質のローディングが等しくなるようにした。
QIAamp DNAキット(キアゲン社製)をメーカーのプロトコールに従って用いてT47D細胞からゲノムDNAを抽出した。単離したDNAを、NanoVue分光光度計を用いて定量化した。各反応についてEZ DNA Methylation−Gold kit(ザイモリサーチ社製)をメーカーの指示書に従って用いて400〜500ngのゲノムDNAをバイサルファイト改変した。バイサルファイト改変DNAは必要時まで−80℃で保存した。非改変SssI処理ゲノムDNA(ニュー・イングランド・バイオラボ社製)を陽性対照として用いた。バイサルファイト改変DNA用に2セットのプライマー及びプローブを設計した:インプットDNAについて標準化するためのPAPP−A及びコラーゲン2A1用のメチル化セット。TaqMan(登録商標) Unive
rsal PCR Master Mix、No AmpErase(登録商標) UNG(ライフテクノロジーズ社製)を20ngのDNA、0.3μMのプローブ、並びに0.9μMのフォワード及びリバース両方のプライマーと用いてqPCR反応を行った。
T47D細胞を実験前に48時間血清飢餓状態にした。Countess(商標)自動細胞計数装置(ライフテクノロジーズ社製)を用い、生体染色トリパンブルーを用いて細胞の生存率を決定した。細胞を終濃度100μMの組換えヒトIGF−1(IMR−233、Imgenex社製)で処理した。24時間後にTryple express(商標)(ライフテクノロジーズ社製)を用いて細胞を回収し、PBSに再懸濁した。次いで、細胞をスライドガラス上にサイトスピンし、前述したようにH3S10ph免疫染色した。
結果を図14及び15に示す。外因性IGF−1はT47D細胞において有糸***遅延表現型を逆転させることにより、細胞の有糸***を可能にする。したがって、有糸***中に作用する化学療法剤を細胞に標的化させて有効な治療法を提供することができる。
Claims (25)
- 患者の増殖性病変が浸潤性乳癌に進行するリスク及び/又は再発性非浸潤性疾患のリスクを判定する方法であって、前記患者から得られた***組織サンプル中のPAPPAの存在及び/又はレベルを検出することを含み、PAPPAが存在しない又は対照より低いレベルで存在する場合、浸潤癌への進行リスク及び/又は再発性疾患のリスクがある、方法。
- 患者の増殖性病変が浸潤性乳癌に進行するリスク及び/又は再発性非浸潤性疾患のリスクを判定する方法であって、前記患者から得られたサンプル中におけるPAPPA遺伝子又はその制御若しくはプロモーター配列中の機能喪失に関連する遺伝子変化の存在を検出することを含み、遺伝子変化が存在する場合、浸潤癌への進行リスク及び/又は再発性疾患のリスクがある、方法。
- 前記PAPPA遺伝子又はその制御若しくはプロモーター配列中の機能喪失に関連する遺伝子変化がメチル化である、請求項2に記載の方法。

- 前記患者が、前記患者から得られたサンプル中におけるPAPPA消失又はPAPPA遺伝子若しくはその制御若しくはプロモーター配列中の機能喪失に関連する遺伝子変化の存在を検出することにより、治療レジメンに係る処置に好適な候補であるとして最初に同定され、PAPPA消失又は遺伝子変化がある場合、前記患者は治療レジメンに係る処置に好適な候補であるとして同定される、請求項16〜20のいずれか一項に記載の治療レジメン。
- PAPPA消失又はPAPP遺伝子若しくはその制御若しくはプロモーター配列中の機能喪失に関連する遺伝子変化がメチル化によるものである、請求項21に記載の治療レジメン。
- 乳癌の処置又は防止に用いるための、細胞***中の分子イベントを標的とする化学療法剤であって、細胞を有糸***遅延から解除する分子で先に処置された患者に投与される、化学療法剤。
- タキサン及びビンカアルカロイドを含む群から選択される、請求項23に記載の使用のための化学療法剤。
- 前記分子が、脱メチル化剤、IGF受容体アゴニスト、好ましくはIGF−1、又はPAPPAタンパク質である、請求項23又は24に記載の使用のための化学療法剤。
