Abstract
Description
本出願は、その内容が、参照によりその全体が本明細書に組み入れられる、2011年5月26日に出願された米国仮出願第61/490,505号に関連する。
ガンの臨床的管理は起源の各部位に特異的であって、ほとんどは、疾患の段階(すなわち、どの程度まで腫瘍が局所的に浸入したか、または転移によって他の臓器に広がったか)に依存する。手術および/または局所の放射線療法は、一般には、化学療法、モノクローナル抗体もしくはサイトカイン療法、または転移性疾患に対する処置である全身照射に処された原発腫瘍に対する最適な処置である。最近、最初の樹状細胞療法であるプロベンジ(Provenge)が、4ヵ月分の進展という利点を示して、前立腺ガンについて認可された。診断は依然として、生検の組織学的分析に基づく。分子マーカーは、もしそれらが薬物(例えば、ハーセプチン)の選択において助けになるならば、基準となることもある。しかしながら、免疫応答のプロファイルを作ることが通常の臨床の場で行われることはない。

A.定義
「抗体」という用語は、その最も広い意味で用いられ、具体的には、ヒト、非ヒト(たとえば、マウス)、およびヒト化モノクローナル抗体(完全長のモノクローナル抗体を含む)、ポリクローナル抗体、多重特異的抗体(たとえば、二重特異的抗体)およびそれらが所望の生物活性を示す限り抗体断片をその範囲に含む。

本発明の1つの態様において、サブドミナント抗原または抗原上のサブドミナントエピトープに対して患者由来のT細胞をエクスビボで増殖させた後、これらのT細胞を患者に注入または投与することによって、免疫応答のバランスを再調整することができる。サブドミナント抗原またはエピトープに対するT細胞を、エクスビボで(患者の免疫調節環境から離れて)組織培養において増殖させる。以前に優勢であった細胞を圧倒するほど十分に細胞を増殖させた後、細胞を患者に再度注入して、細胞のバランスを偏らせ、優勢ヒエラルキーを治療的に切り替える。さらに好ましい態様において、治療としてデノボで導入されるこの多数のT細胞は、抗原、感染性物質、腫瘍、または臓器に対応するT細胞の5%より多い。内因性のT細胞数を減少させる前処理物質(すなわち、化学療法剤)によって患者を前処置すると、注入した細胞に有利となるように、比率をさらに偏らせることができる。

別の態様において、本発明者らは、特定のタイプの腫瘍に対して必要なサブドミナント抗原を同定することができれば、そのタイプの腫瘍を、患者を試験する必要なく、多数のサブドミナント抗原に対するT細胞によって処置することができると提唱する。同様に、同じ腫瘍上の多数の抗原を標的とすることにより、耐性、たとえば併用化学療法を減少させるであろう。たとえば、リンパ腫の40%に存在するサブドミナント抗原であるEBV LMP2を標的とし、かつ生存している(リンパ腫の50%に存在する)1種類のT細胞株を開発することができれば、これらの2つの抗原に対して特異的なT細胞株によってリンパ腫のおよそ80%を標的とすることができるであろう。リンパ腫全体に対する産物の臨床試験を実施例5で実証する。3年目まで、リンパ腫全体に対する産物によって処置した患者の、進行なしでの生存は、治療前に抗原を試験する産物と同等である。これは、併用化学療法と同様に、同じ腫瘍上の多数の抗原に対するCTLの応答が、腫瘍が免疫を逃れる可能性を減少させ得るからである。全てのリンパ腫(1種類の抗原に関して陽性であるそのサブセットを単に処置する必要はない)を1種類のT細胞産物によって処置できることは、新しい産物の考え方である。本発明の他の態様において、本発明者らはまた、実質的に任意のガンに関する類似のガン全体に対する産物を主張する。
肝炎および肝細胞ガン
慢性肝炎患者では、HBs抗体は産生されないが、HBc抗体が生成される(Ganem D et al 2004)。急性肝炎患者では、両方の抗原に対する抗体が生成される。新生児の90%超および年齢1〜5歳の小児の30%が慢性型を発症するが、成人は90%超の確率でウイルスが急激に消失する。肝細胞ガンの95%がB型肝炎ウイルスによる慢性感染症に関連しており、HBsAgがしばしば、HCCの細胞表面上に存在する。これらの知見を考慮して、本発明者らは、HBsが肝炎および肝細胞ガン患者においてサブドミナントであったか否か、およびT細胞の増殖が、サブドミナント抗原に対する免疫応答のバランスを再調整して治療的でありうるHBs抗原に対するCTLを生成することができるか否かを試験することを選択する。

疫学的研究により、多発性硬化症を発症する患者のリスクはEBV抗体力価と相関することが証明されている。EBVに感染する前および初回感染後数年間、数百から数千人の人を追跡することによって、Ascherioらは、MSを発症した患者305人を調べることが可能であった。MSのリスクはEBV感染後急激に増加した(Ascherio A et al. 2010)。EBNA-1 400-641に応答するメモリーCTLは、同様にEBVキャリアである健康な人と比較すると、他のEBV抗原に対する応答と比較してMS患者において上昇した(Lunemann JD et al. 2006)。EBNA-1特異的Th1細胞は、自己抗原の交叉認識によって、またはバイスタンダー反応機構によって自己免疫を維持することができるように思われる。さらに、トランスジェニックマウスの研究により、LMP2aを発現するB細胞は、正常な寛容のチェックポイントを迂回して、自己免疫疾患の発生を増強することが示唆されている(Swanson-Mungerson M 2007)。そのため、本発明者らは、EBVおよびEBV関連ガンの処置としてEBVサブドミナントエピトープに対して反応性のT細胞を調べたところ、EBVサブドミナントエピトープに対して反応性のT細胞はまた、自己免疫におけるバランスを元に戻すためにも有用でありうる。EBV潜在抗原(EBNA-1、LNP1およびLMP2)に応答するT細胞を増殖させて導入することによって、本発明者らは、免疫応答のバランスを再調整することができ、EBVとの関連を有する自己免疫疾患において寛容を再誘導することができることを提唱する。同じことが他の自己免疫疾患に関連する他のウイルスについても当てはまるであろう。たとえば、コクサッキーウイルスB3などのピコルナウイルスは、心筋炎/拡張型心筋症、1型糖尿病、脳炎、筋炎、精巣炎、肝炎を引き起こす。
免疫調節の原理の1つは、T細胞の他のサブセットに対するTregのバランスである。本発明の別の態様は、エクスビボでのT細胞増殖をTreg細胞に向かわせることである。この場合、サブドミナントエピトープを標的とする代わりに、免疫応答のバランスを再調整するための別の方法として、ドミナントエピトープに対するTregを生成する。1つの態様において、そのように生成されたTreg T細胞を、単独の治療用産物として用いる。別の態様において、Tregサブセットを、サブドミナントエピトープまたはサブドミナント抗原に対して増殖させた他のサブセット由来のT細胞と併用して用いる。Tregは、FACSソーティングを用いて非抗原特異的に増殖させた後、抗CD23および抗CD28コーティングビーズによって増殖させるが(Putnam et al. 2009)、今日まで養子免疫療法のために抗原特異的にTregを増殖させた人はいない。本明細書において、異なる抗原、特に自己免疫疾患、移植片対宿主病、または移植の拒絶において観察される免疫応答において、ドミナントである抗原に対して特異的なTregを確立するために有用な方法が記述される。

なお別の態様において、ドミナント抗原に対するTregを、サブドミナント抗原に対してそれ自身反応性であるTH1、TH2またはCTLサブセットのT細胞と組み合わせることができる。そのような組み合わせは、応答のバランスをサブドミナントエピトープに向けてより完全に切り替えるであろう。本発明者らは、関節炎において2つのタイプのT細胞のあいだに相乗効果があることを証明した(実施例6)。
代わりの態様において、他の健康なドナーの樹状細胞(または放射線照射PBMC)を用い、上記のTreg培養条件を用いて健康なドナーのPBMCからTregを増殖させる。このようにしてMHCの各々に対する同種異系反応性Treg株を確立してバンクを作製する。本発明者らが上記で考察したように、MHCの80%を、20〜50個のMHCハプロタイプに対して生成されたTreg株によってカバーすることができる。これらの株の各々を-80℃で1用量のアリコートとして凍結することができる。臓器移植またはBMTを行う場合、ミスマッチMHCに対して反応性のTreg 5×107個/m2もまた患者に移植する。このようにして、同種異系拒絶または移植片対宿主病が緩和される。
気体透過性メンブレン装置は、培養技術の好ましい態様である。メンブレンは気体透過性であることから、培養の規模は、メンブレンの表面積および細胞を増殖させるために必要な培地の体積によって決定される。これらの気体透過性装置の例には、Hyperstack(Corning)またはGrex(Wilson Wolf)が挙げられる。このタイプのバイオリアクターの有用な特色の1つは、細胞培養プロセスの段階的な規模拡大が可能である点である。本発明者らのアプローチの標準化の一部として、本発明者らは、産生セットにおいて用いるための標準的なCO2インキュベータ中にスライドするタイプのバイオリアクターを設計している。異なる態様において、本発明者らは、自動産生施設において標準的な積み重ね型温室に適合するバイオリアクターを設計している。自動化に関して意図されるバイオリアクターは2つの大きさに作製される:1つは、個々の患者に関する自己細胞を増殖させるためのバイオリアクター、および第二のより大きいバージョンは、同種異系T細胞株の商業的産生のためのバイオリアクター。改善された方法において、自動化のために細胞培養装置を四角形形状へと改変すると、標準的なCO2インキュベータの溝にスライドする。気体透過性メンブレンがフラスコの底部にあり、棚の上に載せられると遮断されることから、これは有意な利点である。フラスコを棚にすることによって、メンブレンへの気流はより良好となる。重要な属性はバイオリアクターの重量を支えるフラスコの側面の突縁である。1つの態様において、スライドを棚の支持体に形成するステンレススチールのフレームにバイオリアクターを据え付ける。別の態様において、それらはプラスチックの一部として成型される。態様の1つにおいて、それらは棚全体の設置面積を有する。別の態様において、それらは棚の1/2、1/4または1/3または1/5を構成する。好ましい態様において、インキュベータは、New Brunswick, Forma, ThermoElectron, Nuaire, ESCOによって作製される。好ましい態様において、インキュベータは空気もしくは水で包み込む設計であるかまたは他の設計でありうる。なお別の態様において、それらは金属フレームである棚の中に、または温室中の平板に適合する。別の好ましい態様において、平板はロボットが移動して処理する。なお別の態様において、バイオリアクターは、採取前の浮遊液へと細胞を戻すロッカーを含むがこれらに限定されない様々な販売用処理装置に適合する。
1.実施例1:EBV潜伏感染症、リンパ腫および鼻咽頭癌腫
血液中の抗体によって測定されるように、世界の人口の90%は、EBV(単球増加症の原因ウイルス)に曝露されてきた。EBVはB細胞に潜伏し、そのタンパク質の大部分はなくなるが、非常に低レベルの潜伏抗原LMP1、LMP2および、時々EBNA-1を発現する。これらの蛋白質の免疫原性は弱いが、ウイルスが潜伏状態であっても該ウイルスを維持するのに必要とされる。それらは主にB細胞に由来するゆえに、リンパ腫試験の40%はEBV潜伏抗原に対して陽性である。かくして、これらの抗原は、養子細胞療法におけるCTL応答を発生させる標的として働くことができる。加えて、他の腫瘍(例えば、胃ガンの10%)がそうであるように、鼻咽頭癌腫もまたEBV潜伏抗原を発現する。CTLがEBVリンパ腫を治療するのに用いられてきた一方で、現在の産生方法は時間を消費するものであり(3〜6ヵ月)、反復刺激としてEBVで形質転換されたB細胞を用いるので面倒である。さらに、LMP2に対してこのようにして作成されたCTLは、産生後LMP2に対して検出可能な応答を有していたものがバッチの半分しかないT細胞のバッチを生じさせた。本発明者らは、この理由として、時間の50%でLMP2に対する細胞を増殖させるCTLを生じさせたEBVで形質転換したLCL細胞におけるEBVタンパク質由来のドミナントエピトープの存在によるものであると考えた。大きな腫瘍を持つ患者において、伝統的なプロセスによって産生されたCTLで処置された患者の52%は完全な応答を示した。先行技術ではLMP2に対する応答の存在または欠如にかかわらず全てのCTLが同等であると考えられていたが、本発明者らは、これは変動する臨床的応答に対する一つの理由であろうと考えた。さらに、本発明の有効なモデルが実際当てはまると予測された。従って、LMP2に対するCTL発生の効率を増加させることが臨床的に重要であろう。
刺激剤としての3種の異なる抗原提示細胞:EBVで形質転換されたリンパ芽球系細胞系(LCL)の提示および増殖;サイトカインの増加を伴う樹状細胞(DC)の提示;サイトカインの増加を伴うIFNγマクロファージ(MAC)の提示を用い、T細胞系を10人の患者から調製した。抗原提示の間に、これら3種の各々を、EBNA-1(aa90〜325を欠失したEBNA-1)、LMP1(aa1〜43およびaa260〜315を欠失したLMP1)およびLMP2(1つのプラスミドがaa1〜399を発現し、第2のプラスミドがaa400〜497を発現する2つのプラスミド中のLMP2A)のサブドミナントエピトープを発現する3つのプラスミドの混合物で刺激した。本発明のプロトコルを用いてそのように生じさせたT細胞系を、次いで、51Cr放出アッセイで検討した。
Elispotアッセイを行って、特定の抗原に対して産生されたT細胞の数を決定した。Elispot γIFN 96ウェルの二フッ化ポリビニリデンで裏打ちしたプレート(Millipore, Bedford, MA)を、15μg/mlの抗-IFNγモノクローナル抗体1-DIK(MABTECH,ストックホルム,スウェーデン)で被覆した。ウェルあたり5×106個のPBMCを、各々が、タンパク質の各々からのものであるペプチドミックス2μMと共に加え、37℃、5% CO2で一晩インキュベートした。細胞を捨て、1μg/mlのビオチニル化抗-IFNγモノクローナル抗体7-B6-1(MABTECH)と室温にて2〜4時間インキュベートし、続いて、ストレプトアビジン結合アルカリ性ホスファターゼ(MABTECH)とさらに2時間インキュベートした。試料の数に応じて、alk-phos基質キット(Bio-Rad Richmond,CA)からのリン酸5-ブロモ-4-クロロ-3-インドリルおよびニトロブルーテトラゾリウムとの30分間の反応後、解剖顕微鏡を用いるか、またはAIDELISPOTリーダー(Autoimmun Diagnostika,ストラスブルグ,ドイツ)にて、スポットをカウントした。各スポットは、105個の PBMCあたりのスポット形成細胞(SFC)として報告された細胞であった。これらのアッセイにおいて、陽性対照は細胞+10μg/ml PHAであり、かつ陰性対照はペプチドを含まない細胞単独であった。
LMP2を最適化のために選択した。というのは、それはEBNA-3に対するサブドミナント抗原であって、認識されるEBVサブドミナントエピトープの大部分は、コンセンサスソフトウェアおよびPBMCテストを用いて同定されたこのタンパク質上にあるように見えたからである。認識されたサブドミナントエピトープの数を増加させる努力において、LMP2を2つ以上のプラスミドに分割した。CMVプロモーターおよび人工ATGおよびポリAの制御下にあるpシャトルまたはpUC19プラスミドにおいて、以下のプラスミドを構築した:

以下の実験において、抗原プロセシングを増加させて、サブドミナントエピトープの産生と応答性であるT細胞に有利であるように設計された別の変形を施して、上記実施例1で概説したのと同様にT細胞を増殖した。
慢性肝炎のマウスモデルが最近開発されており、これは、B型肝炎における一次および二次免疫応答の実験を可能とする(Publicover J et al. 2011)。このHBVtgRAGマウスは、15世代にわたってウイルス複製およびビリオンの構成的な放出が可能である、C57BL/6がバックグラウンドのHBV-複製トランスジェニックマウス(HBVRpl)と、RAG-1欠乏マウスとの間の交雑種であり、TおよびB細胞を生じさせることができないる。108個の脾臓細胞をC57BL/6マウスから移動させると、免疫系が再度構成され、肝炎感染症に対する一次免疫応答のモデルとなる。この脾臓細胞を若い(3〜4週齢)マウスに投与すれば、(B型肝炎に感染した若い子供と同様に)この動物は慢性肝炎を発症する。ヒトと同様に、それらはHBcを排除するが、HBsは高いレベルで血清中に残る(図1)。慢性肝炎のこのモデルを以下の実験で用いた。

HBs Agに対するモノクローナル抗体
10-H05ネズミIgG1 HBsAg(Fitzgerald, Concord, MA)
10-H05AネズミIgG2a HBsAg(Fitzgerald, Concord, MA)。
HLA-A*0201であったHBVおよびHCCを持つ5人の患者について、彼らの液性および細胞性免疫応答に関する免疫プロファイルを作成した。前処理臨床実験室的テストでは、B型肝炎コア抗原(HBc Ag)に対する抗体の高い力価が示されたが、B型肝炎表面抗原(HBs Ag)に対しては示されなかった(図10)。

5×105のCTLを再懸濁させ、ペプチド(最終濃度10-5〜10-9M)を含む100μlの1×PBS 1%FCS中で1時間インキュベートし、次いで、Golgi Plug(BD Biosciences,San Diego,CA)を加え、細胞を5%CO2において37℃で5時間インキュベートした。ペレット化して200μlのPBS 1%FCS中で洗浄し、4℃にて、表面抗原(CD4フルオレセインイソチオシアネートおよびCD8アロフィコシアニン(Pharmingen, Becton Dickinson)について30分間染色した。固定および透過化処理溶液中への再懸濁に続いて、抗ヒトIFNγフィコエリスリン(Pharmingen, BD)の1/20希釈物で、氷上で細胞を30分間染色した。1回洗浄し、PBS 1%FCSに再懸濁して、FACS Canto(Becton Dickinson)で分析した。
ガンの臨床的診断および治療は、その起源の組織、分化および局所的および全身転移の程度を決定するための腫瘍の生検およびイメージングを含む。ガン遺伝子または腫瘍サプレッサー遺伝子中の遺伝子的欠陥に対する診断、および化学療法剤または生物学的製剤を決定するためのアッセイは時々行われるが、一般には、患者は、その特異的ガンおよび段階に応じて、外科的処置、化学療法および放射線の組合せで処置される。同様に、種々の患者における免疫系が異なる技術を用いて研究されてきたが、この情報は患者の臨床的管理では用いられてこなかった。免疫系のバランスを再調整するための細胞療法の出現に伴い、本発明者らは、ガン患者の管理における免疫プロファイリングの使用を確立させなければならなかった。本発明者らは、その患者においてサブドミナントであって腫瘍に対する免疫応答のバランスを再調整するために患者に再度注入することができるT細胞をインビトロで成長させるのに用いることができる抗原を選択するのに用いる、標準化された免疫プロファイリング方法を開発した。治療の後に患者に関して再度プロファイルを作成して、この治療法が免疫系のバランスをうまく再調整したかを決定する。このような治療方法は新規であって、臨床的応答および生存の延長をもたらすことができる。同一のアプローチを感染性疾患および自己免疫、および臓器移植で用いることができる。
T細胞は、優れたガス交換用のガス透過性膜上で層状に増殖し、必要な密度まで増殖するのに十分な体積の培地を有する。
臨床的には、患者は、段階IVの転移性黒色腫を持つと診断された52歳の男性であった。彼は、IL-2と組み合わせた、ダカルバジン(DTIC)およびテモダール(テモルゾロマイド)化学療法薬物の組合せ投与レジメンに対して応答しなかった。
再発した急速に進行する非ホジキンリンパ腫を持つ150人の成人患者を3つのアームにランダム化した:A:リツキサン+CHOP;B:腫瘍中のEBV LMP1&LMP2抗原についての試験、続いてEBV LMP1&LMP2 T細胞のバランス再調整、およびC:リンパ腫全体:抗原に対するアッセイは無し;EBV LMP1、LMP2、生存、MAGE A3に対して応答するように増殖したT細胞での治療。リンパ腫生検試験の40%がEBV LMP2に対して陽性、生存に関する試験は50%が陽性であり、およびMAGE A3に関する試験は15%が陽性である。標準的治療であるR-CHOP投与レジメンを用い、1 m2あたり5×107〜2×108のT細胞を投与した。
コラーゲン誘導関節炎モデル(CIA)は、DBA/1マウスにおける異種コラーゲンII(CII)での免疫化によって誘導することができる、リウマチ性関節炎(RA)のモデルである。
Tregの増殖を刺激するのに用いられる263〜270免疫ドミナントペプチド(IL-2+ラパマイシン)。
T細胞の増殖を刺激するのに用いられる286〜300サブドミナントペプチド(IL-15)。
図30Aは正常なラットを示し、図30Bはヒトプロテオグリカンで免疫化したラットを示し、および図30CはT細胞で処理したラットを示す。
Claims (19)
- 患者の免疫優勢ヒエラルキーを変える方法で使用するための、患者においてサブドミナント抗原またはエピトープを認識することができるT細胞。
- T細胞が、
(a)患者から得られた試料中の少なくとも一つのサブドミナント抗原またはエピトープを同定すること、および
(b)該サブドミナント抗原またはエピトープを認識することができるT細胞を培養すること
によって得られる、請求項1記載の使用のためのT細胞。 - 免疫優勢ヒエラルキーを変えることが、患者における感染症、ガン、炎症、臓器移植片拒絶、または移植片対宿主病を処置/予防/軽減する、請求項1または2記載の使用のためのT細胞。
- 患者の免疫優勢ヒエラルキーを変えるための医薬の製造における、患者においてサブドミナント抗原またはエピトープを認識することができるT細胞の使用。
- T細胞が、
(a)患者から得られた試料中の少なくとも一つのサブドミナント抗原またはエピトープを同定すること、および
(b)該サブドミナント抗原またはエピトープを認識することができるT細胞を培養すること
によって得られる、請求項4記載の使用。 - 免疫優勢ヒエラルキーを変えることが、患者における感染症、ガン、炎症、臓器移植片拒絶、または移植片宿主病を処置/予防/軽減する、請求項4または5記載の使用。
- 患者から得られた試料中の少なくとも一つのサブドミナント抗原またはエピトープを同定すること、および
該サブドミナント抗原またはエピトープを認識することができるT細胞集団を培養すること
によって得ることができる、患者の免疫優勢ヒエラルキーを変える際に使用するためのT細胞集団。 - 治療で使用するための、請求項7記載のT細胞集団。
- (a)患者試料中のドミナント抗原またはエピトープ、およびサブドミナント抗原またはエピトープを同定する段階;
(b)該サブドミナント抗原またはエピトープを認識することができるT細胞を培養する段階;および
(c)患者の免疫優勢ヒエラルキーを変えるために、有効数の該T細胞で該患者を処置する段階
を含む方法。 - (a)患者試料中の少なくとも一つのサブドミナント抗原またはエピトープを同定する段階;
(b)該サブドミナント抗原またはエピトープを認識することができるT細胞を培養する段階;および
