Description
本出願は、35USC§119(e)に基づき、2008年2月7日に出願された米国特許仮出願第61/027,032号の優先権を主張するものであり、これによりその全内容が参照により組み込まれる。
「異常増殖」という用語は、本明細書で使用される場合、任意のタイプの細胞の任意の不必要な過増殖と定義され、前記細胞は正常細胞サイクル進行の制約下になく、前記増殖は腫瘍又は任意の癌の発症に帰着する場合がある。
アト−ナル(atonal)タンパク質相同体1(Atoh1又はアトーナル)は、ショウジョウバエ(Drosophila)神経系の発生における細胞運命決定に重要な役割を果たすと考えられている塩基性ヘリックス−ループ−ヘリックス(bHLH)ドメイン含有タンパク質をコードする前神経遺伝子である(Jarman et al., Cell, 73:1307-1321, 1993)。Atoh1は、コクヌストモドキ(Tribolium castenium)(赤コクヌストモドキ)、トラフグ(Fugu rubripes)(フグ)、ニワトリ(Cath1)、マウス(Math1)、及びヒト(Hath1)で同定された相同体と、進化的に保存されている(Ben-Arie et al., Hum. Mol. 本発明は、細胞においてAtoh1発現を増加可能な化合物を提供する。幾つかの実施形態では、Atoh1発現の増加は著しい増加である。幾つかの実施形態では、Atoh1発現の増加は、例えば、基線を超えた約1〜10%、11〜20%、21〜30%、31〜40%、41〜50%、51〜60%、61〜70%、71〜80%、81〜90%、91〜100%、101〜200%、201〜300%、301〜400%、401〜500%、501〜1000%、1001〜10000%、10001〜100000%もしくはそれ以上の範囲であってもよい。Atoh1の増加は、倍増として表現することもでき、例えば100%の増加は1倍の増加であり、1000%の増加は10倍の増加などである。その代わりに又はそれに加えて、Atoh1発現の増加は、細胞、例えば非聴覚有毛細胞(つまり聴覚有毛細胞以外の細胞、例えば先駆細胞又は幹細胞)の、聴覚有毛細胞への又はそれへと向かう分化を促進するのに十分である。

本発明は、Atoh1発現の増加から利益を得る可能性のある疾患及び/又は障害を治療するための、本明細書に記載されている化合物の使用に関する方法を提供する。一般的に、本治療法は、Atoh1の発現レベルを増加させ、それにより標的細胞の部分的な又は完全な分化を促進するために、本明細書に記載されている化合物の1つ又は複数を使用することを伴う。そのような治療から利益を得ることができる疾患は、Atoh1レベルの増加が、疾患、例えば、Atoh1発現の増加に起因する完全な又は部分的な分化細胞が、(1)喪失又は損傷細胞又は組織、例えば機能的細胞、例えば聴覚有毛細胞を置換する役目を果たす、及び/又は(2)細胞、例えば癌細胞の有害集団の増殖を防止する疾患の1つ又は複数の症状を治療するものである。
有毛細胞を生成可能な細胞は内耳に存在するが、内耳における自然な有毛細胞再生は低いことが広く受け入れられている(Li et al., Trends Mol. Med., 10, 309-315 (2004);Li et al., Nat. Med., 9, 1293-1299 (2003);Rask-Andersen et al., Hear. Res., 203, 180-191 (2005))。その結果、喪失又は損傷有毛細胞は、自然な生理学的過程(例えば、細胞分化)により適切に置換することができず、有毛細胞の喪失が生じる。多くの個体では、そのような有毛細胞の喪失により、例えば感音性聴力損失、聴覚障害、及び平衡障害が生じ得る。内耳の有毛細胞の数を増加させるという治療戦略は、有毛細胞喪失、例えばこれらの症状の1つ又は複数を有する患者に有益であろう。
細胞増殖は、通常、複数のチェックポイント及び保護機構により管理される厳密に制御された過程である。これらのチェックポイント又は保護機構の1つ又は複数が、例えば、遺伝子突然変異により迂回される又は破綻すると、異常細胞増殖が生じる。異常細胞増殖の結果は、癌性増殖又は腫瘍の形成である。最も悪性度の高い癌性増殖は、典型的には侵襲性及び転移性である。それほど悪性度の高くない良性増殖は、侵襲性又は転移性ではないが、転移性になる可能性を保持していることが多い。
Atoh1発現は、小脳及び背側脊髄においても報告されており、例えば発生に重要な役割を有している(Bermingham et al.、上記、及びHelms et al.、上記)。Atoh1は、内耳に見出されるもの以外の神経細胞及び組織の細胞分化の促進に役割を担っているのは明らかである。したがって、本明細書に記載されている化合物及び医薬組成物は、Atoh1発現の増加から利益を得る可能性のある、そのような組織の疾患及び/又は障害の治療にも使用することができる。
直接療法
投与経路は、治療されている疾患に依存して変動する。有毛細胞喪失及び/又は感音性聴力損失は、全身投与及び/又は局所投与を使用した直接療法を使用して治療することができる。幾つかの実施形態では、当該投与経路は、患者の医療提供者又は臨床医により、例えば患者を評価した後で決定することができる。幾つかの実施形態では、個々の患者の療法を個別に作成することができ、例えば1つ又は複数の化合物、投与経路、及び投与の頻度を個別化することができる。或いは、治療は、例えば1つ又は複数の事前に選択された化合物及び事前に選択された投与経路及び投与の頻度を使用する標準的治療方針を使用して実施することができる。
一般的に、本明細書に記載されている細胞療法は、内耳の成熟細胞タイプ(例えば有毛細胞)への又はそれに向かう細胞の完全な又は部分的な分化を促進するためにin vitroで使用することができる。その後、そのような方法から生じる細胞を、そのような治療を必要とする患者に移植するか又は埋め込むことができる。好適な細胞タイプを識別及び選択するための方法、選択された細胞の完全な又は部分的な分化を促進するための方法、完全に又は部分的に分化した細胞タイプを識別するための方法、及び完全に又は部分的に分化した細胞を埋め込むための方法を含む、これらの方法を実施するのに必要な細胞培養法は、下記で説明されている。
本発明で使用するのに好適な細胞には、本明細書に記載されている化合物の1つ又は複数と例えばin vitroで接触した際に、内耳の成熟細胞、例えば有毛細胞(例えば、内及び/又は外有毛細胞)に完全に又は部分的に分化可能な細胞が含まれるが、これらに限定されない。有毛細胞に分化可能な例示的な細胞には、幹細胞(例えば、内耳幹細胞、成体幹細胞、骨髄由来幹細胞、胚性幹細胞、間葉系幹細胞、皮膚幹細胞、iPS細胞、及び脂肪由来幹細胞)、前駆細胞(例えば、内耳前駆細胞)、支持細胞(例えば、ダイテルス細胞、柱細胞、内部支持細胞、視蓋細胞、及びヘンゼン細胞)、及び/又は生殖細胞が含まれるが、これらに限定されない。内耳感覚細胞を置換するための幹細胞の使用は、Liら(米国特許公開第2005/0287127号明細書)、Liら(米国特許第11/953,797号明細書)に記載されている。内耳感覚細胞を置換するための骨髄由来幹細胞の使用は、Edgeら、(国際公開PCT/US2007/084654号パンフレット)に記載されている。iPS細胞は、例えば、Takahashi et al., Cell, Volume 131, Issue 5, Pages 861-872 (2007);Takahashi and Yamanaka, Cell 126, 663-76 (2006);Okita et al., Nature 448, 260-262 (2007);Yu, J. et al., Science 318(5858):1917-1920 (2007);Nakagawa et al., Nat. Biotechnol. 26:101-106 (2008);及びZaehres and Schoeler, Cell 131(5):834-835 (2007)に記載されている。
一般的に、分化は、内耳の成熟感覚細胞、例えば有毛細胞への又はそれへと向かう細胞の完全な又は部分的な分化を促進するのに十分な時間、好適な標的細胞及び/又は細胞集団を、本明細書に記載されている化合物の1つ又は複数と接触させることにより促進することができる。
本明細書で開示されている化合物の1つ又は複数と接触した細胞を分析して、完全なもしくは部分的な分化が生じたかどうかを決定することができる。上述したように、そのような決定は、組織特異的遺伝子の存在又は非存在を分析することにより実施することができる(細胞の選択を参照)。その代わりに又はそれに加えて、有毛細胞を生理学的検査により識別し、細胞が、成熟有毛細胞又はらせん神経節細胞に特徴的なコンダクタンスチャネルを生成するかどうかを決定することができる。そのような細胞は、上述のマーカーを使用して、らせん神経節細胞と区別することができる。
例えば、上述の方法により生成された部分的及び/又は完全に分化した細胞は、細胞懸濁液などの形態で、蝸牛の管腔などへの注射により耳の中に移植又は埋め込むことができる。注射は、例えば、耳の円形窓から又は蝸牛を取り囲む骨嚢からであってもよい。これらの細胞は、円形窓から内耳道の聴神経幹又は鼓室階に注射することができる。
幾つかの実施形態では、本発明は、上述の直接投与法及び細胞治療法を使用し、本明細書に記述されている化合物の1つ又は複数を用いて患者を治療するための方法を提供する。
投与経路は、治療されている疾患に依存して変動する。異常細胞増殖及び/又は癌は、直接治療を使用して、例えばCDERデータ標準マニュアル、バージョン番号004(fda.give/cder/dsm/DRG/drg00301.htmから入手可能)に記載されているような、食品医薬品局が認可した方法の1つ又は複数に従い、例えば、全身投与及び/又は局所投与を使用して治療することができる。
本明細書に記載されている化合物の1つ又は複数を含有する(つまり活性成分として)医薬組成物は、意図した投与方法に従って製剤化される。
本明細書に記載されている化合物及び医薬組成物の毒性及び治療効能は、LD50(集団の50%に致死的な用量)及びED50(集団の50%に治療上有効な用量)を決定するために培養中の細胞又は実験動物のいずれか一方を使用して、標準的医薬手順により決定することができる。毒性と治療効果と間の用量比は、治療指数であり、比率LD50/ED50として表現することができる。高い治療の指数を示すポリペプチド又は他の化合物が好ましい。
本明細書に記載されている化合物及び医薬組成物は、分化するように誘導された細胞(例えば、例えば有毛細胞若しくは有毛様細胞に分化した幹細胞、前駆細胞、及び/又は支持細胞)及び/又は有毛細胞に分化可能な細胞として、キットで提供することができる。当該キットは、本明細書に記載されている化合物及び医薬組成物並びにそのような細胞の組み合わせも含むことができる。当該キットは、(a)本化合物を含む組成物などにある1つ又は複数の化合物、(b)分化するように誘導された細胞(例えば、例えば有毛細胞若しくは有毛様細胞に分化した幹細胞、前駆細胞、及び/又は支持細胞)及び/又は有毛細胞に分化可能な細胞、(c)情報素材、及び(a)〜(c)の任意の組み合わせを含む。幾つかの実施形態では、(a)及び/又は(b)は、(a)及び/又は(b)を直接的に患者の耳(例えば、中耳又は内耳)に直接投与するのに好適な注射器(例えば、事前充填された使い捨ての単回投与注射器)で提供することができる。幾つかの実施形態では、(a)及び/又は(b)は、(a)及び/又は(b)を直接的に患者の耳(例えば、中耳又は内耳)に直接投与するのに好適な、上述のようなカテーテル及びポンプシステムで提供することができる。該情報素材は、本明細書に記載されている方法及び/又は本明細書に記載されている方法用の作用剤の使用に関する説明用素材、指示用素材、マーケッティング用素材、又は他の素材であってもよい。例えば、該情報素材は、聴覚有毛細胞喪失聴覚及び/又は異常細胞増殖を有するか又は発症するリスクがある対象体を治療するための化合物の使用に関する。当該キットは、分化作用剤を細胞(培養中の又はin vivoの)に投与するための、及び/又は細胞を患者に投与するための器具一式も含むことができる。
ハイスループットスクリーニングの最適化
Math1エンハンサー及び最小プロモーターにより制御されるルシフェラーゼ遺伝子を安定的に発現するヒト胚腎臓(HEK)細胞系統を、144,000個の低分子のハイスループットスクリーニング(HTS)に使用して、Math1発現(つまり、転写及び/又は翻訳)を増加させる化合物を識別した。そのような化合物は、幹細胞、前駆細胞、及び支持細胞の、有毛細胞への又はそれへと向かう変換を増加させるために使用することができる。そのような化合物を識別するスクリーニングは、Liら(米国特許公開第2005/0287127号明細書)、Liら(米国特許出願第11/953,797号明細書)により記載されている。
Atoh−1発現の活性化因子を識別するハイスループットスクリーニング
細胞を384ウエルプレートにまき、増殖因子の非存在下、5%CO2、37℃で、終夜結合を可能にさせた。Math1活性化は、ルシフェラーゼ検出剤を添加した後のプレートリーダーでの発光増加により測定した。発光を、24、48、及び72時間で評価した。これらの条件を使用して、ハーバード大学神経変性疾患創薬研究所(LDDN、Harvard University Laboratory's for Drug Discovery in Neurodegeneration)の低分子ライブラリーに含有されている144,000個の化合物をスクリーニングした。
RT−PCRによる陽性化合物の評価
実施例2で識別された110個の化合物の群から無作為に選択した10個の化合物を、以下のようにさらに評価した。HEK細胞を、1ウエル当たり100000細胞の密度で、96ウエルプレートに播種した。播種1日後、0.1、1、及び5μMの各陽性化合物を、10%ウシ胎仔血清(FBS)DMEM溶液のウエルごとに添加した。その後、該化合物を添加した48時間後に細胞を溶解させ、Math1発現を、上述のようにルシフェラーゼリポーターアッセイを使用して、及びリアルタイムPCRを使用して分析した。
有毛細胞分化による陽性化合物の評価
内耳幹細胞を、実施例2に記載されている110個の陽性化合物の群から無作為に選択した化合物CP.−0000540と接触させた。
陽性化合物の第2段階評価
陽性化合物を全て、それらがMath1 mRNAを上方制御する能力について評価し、実施例2でなされた観察を確認した。
陽性化合物の第3段階評価
陽性化合物を、マウス内耳由来幹細胞からの有毛細胞の収率を増加させるそれらの能力について評価した。陽性化合物を、感覚神経性疾患マウスモデルの内耳でin vivoでも評価する。
内耳前駆細胞分化の評価
限定数の陽性化合物を、骨髄由来内耳前駆細胞の有毛細胞への分化を促進するそれらの能力について評価する。実験は、最初は、骨髄由来内耳前駆細胞(例えば、間葉系幹細胞(MSC))、及びルシフェラーゼ発現がミオシンVII1エンハンサー領域及びプロモーターにより駆動されるルシフェラーゼリポーターコンストラクトを使用して実施する。これは、有毛細胞の強力なプロモーターである。ミオシンVIIaエンハンサー領域及びプロモーターも、GFPに作用可能に連結されていた。陽性結果は、ミオシンVIIa用に改変された、実施例3〜6に記載されている方法を使用するRT−PCR及び免疫細胞化学法を使用して確認する。
化合物の薬学的特徴付け
最大半量阻害濃度(IC50)及び半数致死量又は致死量50%(LD50)を、実施例2で識別された化合物の各々について、標準的検査技術を使用して決定した。IC50は、生物学的又は生化学的機能の阻害における、化合物の有効性の尺度である。LD50は、試験集団のメンバーの半分を死滅させるのに必要な化合物の量である。
マウス蝸牛から単離された内耳前駆細胞を使用した化合物の特徴付け
実施例2に記載されている方法により識別された化合物を、マウス蝸牛から単離されたマウス内耳前駆細胞の有毛細胞への分化を促進するそれらの能力に関して試験した。
マウスコルチ器官外植片を使用した化合物の特徴付け
実施例2で識別された化合物を、マウスコルチ器官外植片で新しい有毛細胞の形成を促進するそれらの能力に関して試験する。
化合物の最適化
ナノモル範囲の効能及び細胞毒性の低減を提供するために、化合物を最適化する。
in vivo薬物動態、毒性、及び製剤研究
化合物を、マウス脳への脳室内(icv)注射を使用して投与し、用量研究を実施して、この経路経由で投与することができる化合物の最大用量を決定する。
動物難聴モデルを使用したin vivo研究
10週齢マウスを、約116dB SPLのオクターブバンド(8〜16kHz)音に2時間曝露する。CBA/CaJマウスでは、この音量は、蝸牛の下半分全体の外有毛細胞を破壊し、内毛細胞及び支持細胞は、蝸牛中央のより限定された領域で破壊される(Wang et al., J. Ass. Res. Otolaryngol., 3:248-268, 2002)。さらに、ニューロンは、内毛細胞が破壊される領域で7日以内に縮退し始める。そのようなマウスを、回復期間後にABR及びDPOAEについて検査する。蝸牛を解剖し、凍結切片又は全載標本の切断後にそれらを免疫染色した。細胞***は、凍結切片を染色するBrdU及び抗体を注射することにより動物中で評価する。細胞死はTUNEL法により評価する。有毛細胞、支持細胞、及びらせん神経節ニューロンの数を計数した。
