Abstract
Description
The present invention relates to a clip device and a manufacturing method thereof. For example, the present invention relates to a biological tissue clipping apparatus that inserts into a body cavity endoscopically and clips the biological tissue, and a method for manufacturing the same.
2. Description of the Related Art Conventionally, in medical applications, for example, a clip device used for hemostasis has a bent shape that opens toward a distal end portion, and a clip that grips an object to be gripped by a change in the opening width of the distal end portion. And a presser tube that is inserted from the end side and changes its opening width. Such a clip is often manufactured by cutting and bending a thin metal plate. This metal plate is used as a plurality of arms, and when the clip is inserted into the presser tube from the base end side, it is mechanically closed from a state where it is greatly expanded in a V shape. Thus, the living tissue can be grasped.
As such a clip device, for example, in Patent Document 1, the clip arm is manufactured by bending a thin plate made of an alloy such as stainless steel, and the clip is fixed by tearing the control wire when the grip state is determined. -The assembly is described.
However, the conventional clip device and the manufacturing method thereof have the following problems.
In the technique described in Patent Document 1, when a clip is pulled into a capsule that is a presser tube and the clip arm is closed at the time of insertion into an endoscope, the metal plate constituting the clip arm is plastically deformed. , There is a problem that the opening width becomes smaller than before pulling in when protruding. This is a particularly remarkable phenomenon in the configuration in which the opening width is greatly widened in the initial state so that the living tissue can be easily grasped.
On the other hand, it is also possible to adopt a flexible material or a thinner and softer structure so that the clip arm does not undergo plastic deformation. However, in this case, since the rigidity of the clip arm is low, the deflection of the clip arm becomes large, and the amount of force for grasping the living tissue at the tip of the arm is insufficient, so that the living tissue cannot be firmly grasped. There is.
The present invention has been made in view of the above problems, and a clip device capable of preventing a decrease in opening width and a decrease in gripping force of a clip even when the clip is repeatedly opened and closed, and a clip device thereof An object is to provide a manufacturing method.
In order to solve the above-described problems, a clip device of the present invention has a bent shape that opens toward a distal end portion, and grips an object to be gripped by a change in the opening width of the distal end portion. And a pair of gripping members made of a metal material having an elastic coefficient larger than that of the elastic member.
Further, in the clip device according to the present invention, the elastic member and the gripping member are formed by connecting plate-like portions having concave and convex fitting portions that are fitted in close contact with each other in the opening / closing direction of the clip. In the clip device of the present invention, it is preferable that the elastic member is integrally formed with the bent portion.
Further, in the clip device according to the present invention, the gripping member is disposed when the clip is pulled into the presser tube with the object to be grasped sandwiched between the tip parts and the tip parts being separated from each other. It is preferable that the presser tube is locked by coming into contact with the opening.
In the clip device of the present invention, it is preferable that the gripping member is connected to the inside of the elastic member in the opening / closing direction.
The manufacturing method of the clip device of the present invention has a bent shape that opens toward the distal end, and grips the object to be gripped by a change in the opening width of the distal end, and the clip is inserted from the proximal end side. A holding tube that changes the opening width, and the clip includes an elastic member made of metal glass that forms a bent shape portion on a proximal end side, and one end side forms the distal end portion and the other end side is the elastic member. And a pair of gripping members made of a metal material having a larger elastic coefficient than that of the elastic member. A gripping member forming step for forming a metal gripping member having a large thickness, a gripping member disposing step for disposing the gripping member in a mold, and a metal glass material melted in the mold is filled and cooled. To, by solidifying the amorphous state, thereby forming an elastic member made of metallic glass, and a method of and an elastic member connecting step of connecting to said gripping member.
Moreover, in the manufacturing method of the clip device of the present invention, in the holding member forming step, an uneven fitting portion is formed in the plate-like portion at the end of the holding member, and in the elastic member connecting step, the unevenness of the holding member is formed. It is preferable to form a concave / convex fitting portion that fits in close contact with the concave / convex fitting portion of the gripping member on the elastic member by filling the fitting portion with the metallic glass material.
In the clip device manufacturing method of the present invention, in the gripping member arranging step, a mold having a molding space for transferring the shape of the bent shape portion on the proximal end side of the clip is used as the mold, and one end side is A pair of gripping members constituting the tip portion of the clip are arranged so that the other ends thereof are located in the molding space, and in the elastic member connecting step, the metallic glass material is filled into the molding space. According to the clip device and the manufacturing method thereof of the present invention, the bent shape portion on the proximal end side of the clip is formed of a metallic glass elastic member, and a pair of gripping members having a larger elastic coefficient than the elastic member are connected to the distal end portion. Since the clip is provided, even if the clip is repeatedly opened and closed, there is an effect that it is possible to prevent a decrease in opening width when the clip is opened and a decrease in gripping force of the clip.
Embodiments of the present invention will be described below with reference to the accompanying drawings. In all the drawings, even if the embodiments are different, the same or corresponding members are denoted by the same reference numerals, and common description is omitted.
[First Embodiment]
First, the clip device according to the first embodiment of the present invention will be described.
FIGS. 1A and 1B are schematic cross-sectional views showing an opened state and a gripped state of the clip device according to the first embodiment of the present invention. Fig.2 (a) is a typical perspective view which shows the structure of the clip of the 1st Embodiment of this invention. FIG.2 (b) is A view in FIG.2 (a). FIG. 3A is a B view in FIG. FIG.3 (b) is CC sectional drawing in Fig.3 (a).
As shown in FIGS. 1A and 1B, the clip device 1 of the present embodiment is a device that can perform hemostasis of the biological tissue Bt, for example, by holding the biological tissue Bt. It can be used by being inserted into the body cavity of a living body through the lumen of the endoscope or the like.
The schematic configuration of the clip device 1 includes a
As shown in FIG. 1 (a), the
As shown in FIGS. 2A and 2B, the

Specific dimensions of the
The size of the plate-
As a material of the
The metal glass is an amorphous state in which a molten metal raw material composed of a plurality of metal elements that can constitute the metal glass is rapidly cooled to, for example, about 100 K / second until the temperature is not lower than the critical cooling rate and not higher than the glass transition temperature. It can be manufactured by solidifying as it is.
Such amorphous alloys do not have the grain boundaries found in ordinary crystalline metals, and are subject to grain boundary corrosion (a phenomenon in which corrosion proceeds along the grain boundaries) due to the grain boundaries. Since it does not occur, it has excellent corrosion resistance. For this reason, it can be suitably used in vivo.
Furthermore, since the occurrence of solidification shrinkage unlike that of crystalline metal is small, it has high-accuracy transferability with respect to the mold and low expansion against heat. Further, it is known that its physical properties are easy to elastically deform, has a wide elastic deformation range, and has a higher strength.
Examples of the alloy material that forms such a metallic glass include a zirconium (Zr) -based alloy, a titanium (Ti) -based alloy, and a magnesium (Mg) -based alloy.
Since these metallic glasses are also non-magnetic materials, for example, MRI (each magnetic resonance imaging) inspection can be performed, so that they are particularly suitable for a clip device for medical use. In such applications, the relative permeability of the metallic glass is desirably 1.06 or less, and more desirably 1.02 or less.
However, when non-magnetism is not required, such as for applications that do not require MRI inspection, it is also possible to employ iron (Fe) based alloy metal glass.
In this embodiment, a Zr-based alloy having a composition of Zr 55 Cu 30 Ni 10 A 15 is employed as an example. The longitudinal elastic modulus of this metallic glass is about 90 GPa.
The amount of strain that can be elastically deformed is 2% or more. On the other hand, the elastically deformable strain amounts of SUS304 and Elgiloy (registered trademark) that can be used for the gripping

In the present embodiment, as an example, the band width of the gripping

For this reason, in this embodiment, each through-
Further, in the present embodiment, since the through-

Moreover, in order to be able to firmly grasp the biological tissue Bt, the material of the grasping

Suitable materials for the gripping
These are materials having a high longitudinal elastic modulus and high mechanical strength, good corrosion resistance, and chemical stability, so that they can be suitably used in vivo.
In the present embodiment, Elgiloy (registered trademark) is adopted as an example.

FIG. 5 is a process explanatory diagram for explaining a gripping member forming process in the manufacturing method of the clip device according to the first embodiment of the present invention. FIG. 6 is a process explanatory diagram for explaining an elastic member connecting process in the manufacturing method of the clip device according to the first embodiment of the present invention.
In order to manufacture the clip device 1 of the present embodiment, as shown in FIG. 4, a gripping member forming step S1, a gripping member arranging step S2, an elastic member connecting step S3, and an assembly step S4 are performed in this order.

Thus, the gripping member arranging step S2 is completed.
Next, an elastic member connecting step S3 is performed. In this step, as shown in FIG. 6, the metallic glass material M melted in the

Next, the operation of the clip device 1 having such a configuration will be described.
FIGS. 7A and 7B are operation explanatory views of the clip device according to the first embodiment of the present invention.

When the
Thus, in the clip device 1, even if the
When the living tissue Bt, which is the object to be grasped, is arranged between the grasping
Since the longitudinal elastic modulus of the gripping

Therefore, since neither the holding

According to the clip device 1 of the present embodiment, a clip in which a bent portion on the proximal end side is formed by a metal

[First and second modifications]
Next, first and second modifications of the present embodiment will be described.
Fig.8 (a) is typical sectional drawing which shows the shape of the uneven | corrugated fitting part used for the clip apparatus of the 1st, 2nd modification of the 1st Embodiment of this invention. FIG. 8B is a schematic plan view in the case of the first modification. FIG. 8C is a schematic plan view in the case of the second modification.
The first modified example is a modified example of the uneven fitting portion in the clip device 1 of the first embodiment. In the
Hereinafter, a description will be given centering on differences from the first embodiment.

In this case, it is preferable to fill the recessed

By having such a concavo-convex fitting portion, even when a shearing force is applied between the plate surfaces 3d and 4d during the gripping and releasing operation of the clip device 1, the concavo-convex fitting portion prevents displacement in the shearing direction. Therefore, the close contact state of the plate surfaces 3d and 4d can be maintained.

Hereinafter, a description will be given centering on differences from the first embodiment.

As shown in FIG. 8A, the cross-sectional configuration of such a concave-convex fitting portion is the same as that of the first modified example, and therefore has a tapered shape that prevents the same as in the first modified example. Even if not, for example, stronger bonding strength can be obtained as compared with the case where the planes between the plate surfaces 3d and 4d are bonded to each other.
[Third to Sixth Modifications]
The 3rd-6th modification is a modification of the cross-sectional shape of the uneven | corrugated fitting part in the clip apparatus 1 of the said 1st Embodiment similarly to the said 1st, 2nd modification. In either case, the
FIGS. 9A, 9B, 9C, and 9D are schematic views showing the shapes of the concave-convex fitting portions used in the clip devices of the third to sixth modifications of the first embodiment of the present invention, respectively. FIG.

[Second Embodiment]
Next, a clip device according to a second embodiment of the present invention will be described.
FIG. 10A is a schematic front view showing the configuration of the clip according to the second embodiment of the present invention. FIG.10 (b) is the elements on larger scale of the D section in Fig.10 (a).

Hereinafter, a description will be given centering on differences from the first embodiment.
As shown in FIG. 10A, the

As the adhesive 86, a resin adhesive such as an epoxy-based adhesive can be used, but it is also possible to perform welding with a low melting point solder or a brazing material in order to further increase the strength.
In any case, it is necessary to be a joining material having safety compatible with living bodies.

FIG. 13 is a process explanatory diagram for explaining an elastic member connecting process in the manufacturing method of the clip device according to the second embodiment of the present invention.
In order to manufacture the clip device of the present embodiment, as shown in FIG. 11, a gripping member forming step S11, a gripping member arranging step S12, an elastic member connecting step S13, an elastic member connecting step S14, and an assembly step S15 are performed in this order. Do.
The gripping member forming step S11 is a step of forming the gripping

Thus, the gripping member arrangement step S12 is completed.
Next, the elastic member connecting step S13 is performed. In this step, as shown in FIG. 12, the metal spring material M is filled in the

Next, the elastic member connecting step S14 is performed. In this step, as shown in FIG. 13, the plate-

Above, elastic member connection process S14 is complete | finished.

In the above description of each embodiment and each modification, the concave / convex fitting portion between the gripping member and the elastic member has been described as an example in which a convex portion is provided on the elastic member side and a concave portion is provided on the gripping member side. This is because the convex portion having a retaining shape can be more easily formed by molding an elastic member using metal glass than a gripping member not using metal glass.
However, as long as the gripping member can be processed, a concave portion may be provided on the elastic member side, and a convex portion may be provided on the gripping member side, or a concave and convex portion may be provided in each case.
Further, in the description of each of the above embodiments and each modification, an example in which the protrusions forming the protrusions have a columnar shape, a truncated cone shape, a conical shape, or an oblique cone shape, except for the second modification example. As described above, the cross section perpendicular to the protruding direction of the protrusion is not limited to such a circular cross section, and may be a convex polygon, a concave polygon such as a star, an ellipse, an irregular shape, or the like. . The same applies to the cross section of the recess.
In the above description of the third and fourth modified examples, the example in which the concave / convex fitting portion is formed by separating and deeply forming the concave hole portion has been described, but a large number of shallow holes may be provided. For example, the
Moreover, in the description of each of the embodiments and the modifications described above, the structure in which the bonding strength is reinforced is formed only by the concave / convex fitting portion.
