Description
本願は、2010年9月30日に出願した米国特許仮出願第61/388575号の利益を主張し、その内容全体を参照によって本書に援用する。
特定の実施形態では、本開示は、楕円形の弁輪または楕円形の装着フレームを、患者の自己弁内に移植するシステムおよび方法に向けられる。楕円形の大動脈弁装着フレームの例示的実施形態を以下でより詳細に記載する。
特定の実施形態では、本開示は、楕円形の弁補綴物を患者の体内に移植するシステムおよび方法に向けられる。特定の態様では、楕円形の弁構造物は、生体組織から構成することができる。
特定の実施形態では、本開示は、楕円形弁補綴物の移植が、折畳み可能かつ拡張可能な楕円形の弁構造をステント内に配置した状態で圧縮可能かつ拡張可能な楕円形のステントを含む、システムおよび方法に向けられる。該実施形態の実施に際して、胸部または鼠径部の小さい入口開口を介して楕円形の弁補綴物を前進させるために、挿入カテーテルが使用される。楕円形の弁構造物は生体組織で構成することができる。
発明者らは、健康なヒトの大動脈弁の高解像度マルチスライスコンピュータ断層撮影(CT)血管造影図を入手して解析した。1mmの軸線方向スライスを用いて、Mathematicaで10個の正常な大動脈根のCT血管造影図から弁構造物のx、y、およびz座標を生成した。弁葉および洞の座標の三次元最小二乗回帰を使用して、半球モデルと楕円モデルとを比較した。全ての根構造物の形状および寸法を評価した。
1.5の軸比(長軸/短軸または0.66の短軸/長軸の軸比)の楕円形の断面および10°外側に広がったポストを持つ現在記載した弁輪内装着フレームを子ウシで試験し、以下の詳述する通り有望な結果を得た。
Claims (19)
- 哺乳動物の大動脈弁内に移植するための医療装置であって、大動脈弁中の血流の方向に対し垂直な面で楕円断面形状を含む装置。
- 基部と、基部に取り付けられた交連支持部材とをさらに備え、交連支持部材が、楕円断面の外側縁に対し垂直な線から8°〜12°の角度で基部から外側に広がる、請求項1に記載の装置。
- 交連支持部材が、楕円断面の外側縁に対し垂直な線から約10°の外側角度で基部から離れて広がる、請求項2に記載の装置。
- 楕円の短軸に対する楕円の長軸の比は約1.2〜1.8である、請求項1に記載の装置。
- 楕円の短軸に対する楕円の長軸の比は約1.5である、請求項4に記載の装置。
- 楕円断面の長軸の長さは約10ミリメートルから約35ミリメートルの間である、請求項1に記載の装置。
- 楕円断面の短軸の長さは約8ミリメートルから約25ミリメートルの間である、請求項1に記載の装置。
- 装置がプラスチック、高分子、金属、熱可塑性樹脂、樹脂、またはそれらの組み合わせを含む、請求項1に記載の装置。
