Abstract
Description
本出願は、2006年11月22日に出願された米国特許仮出願第60/860,733号、および2007年11月2日に出願された代理人整理番号3933.1007-000のRandall L. Carpenterによる発明の名称「Use of M1 Muscarinic Antagonists to Treat Disorders of Brain Development」の米国特許仮出願の恩恵を主張する。前記出願の全教示は、参照により本明細書中に援用される。
精神遅滞、ダウン症候群、脆弱X症候群および自閉症は、学習、記憶、言語、社会的技能および行動などの日々の機能に影響を及ぼす発育上の遺伝的な障害である。精神遅滞、ダウン症候群、脆弱X症候群を有するヒトに対して現在利用可能であり、日々の機能を補助する治療計画としては、例えば抗うつ薬および抗精神病薬などの一連の医薬による行動修正および治療が挙げられる。しかし、これらの治療計画は、高頻度で効果的ではないか、または長期間の使用、特に抗精神病薬の使用により望ましくない副作用を生じ得る。従って、精神遅滞、ダウン症候群、脆弱X症候群および自閉症を治療するための新規の改良された効果的な方法を開発する必要がある。
本発明は、精神遅滞、ダウン症候群、脆弱X症候群および自閉症を有するヒトの治療方法に関する。

自閉症の一連の障害を有する23歳の女性(身長61インチ、体重170ポンド)を入院させ、腸全体を休ませる所定の期間でTPNを始めるためのポートカテーテル置換について準備した。腸の運動を改善するために、バクロフェンを、5mg 1日3回(1日3回)として処方した。約24時間以内に、腹部の痛みは和らいだように思われ、GI運動が機能的になり、経口食事が可能になった。次の数日間および数週間にわたって、10年間変わっていなかった認知および行動領域に改善が見られた。

・175mg トパマックス:痛みおよび4 gran mal 発作(2002年以来なし)の病歴;

・500mg マグネシウムフィリップ1日3回 便秘を治療するため;
・ダルコラックス 便秘の必要に応じて;
・100mg トラマドール 必要に応じて(6〜8時間毎);および
・オキシコドン 痛みをなくすため
・バクロフェン 20mg(10mg 1日あたり2回);
・トパマックス 125mg; および
・ザンタク 150mg
である。
自閉症の一連の障害を有する12歳の男性(身長=54インチ、体重=64ポンド)を、バクロフェンで治療した。バクロフェン(約5mg 1日2回)を、胃食道逆流症状を減らそうとするバクロフェンで最初に投与した。次の数日間および数週間、言語に対する関心および応答の増大ならびに口頭で意思伝達する試み等の改善が、いくつかの認知および行動領域に見られた。「心地よく、冷静および幸せそうに見える」などの気分および情動における劇的な改善も見られた。学校教師との認知/教育活動に従事する注意、関心および動機付けの増大も見られた。毎日の行動を記録する学校職員は、薬物治療の変更に関して知らされず、バクロフェンがこの被験体に処方されたことを知らなかった。毎日のスコアがバクロフェン療法の開始後の5週間に対して平均化され、前記のバクロフェン療法の開始直後の5週間の平均スコアと比較した。
・平均17+2(平均+SE)から6+1エピソード/日に減少した、他人を引っ掻く、殴るおよび蹴ることのエピソード

自閉症の一連の障害を有する9歳の男性(身長=46インチ, 体重=47ポンド)を、バクロフェンで治療した。バクロフェン(5mg 1日2回)を、胃食道逆流症状を減少しようと開始した。次の数日間および数週間、言語に対する関心および応答の増大ならびに意思伝達する自発的な試み等の改善が、認知および行動領域に見られた。また、患者は、睡眠パターン、気分および情動における改善が見られた。認知/教育活動に従事する注意、関心および動機付けの増大が学校職員によって見られた。毎日の行動を記録する学校職員は、薬物治療の変更に関して知らされず、バクロフェンがこの被験体に処方されたことを知らなかった。毎日のスコアは、バクロフェン療法を開始した後の5週間に対して平均化され、前記のバクロフェン療法の直ぐの開始を5週間の平均スコアと比較した。改善は、驚愕/震え行動の頻度で見られ、この被験体における短い(およそ2秒間)発作行動を反映すると考えられる。バクロフェン治療の開始後に、驚愕震えのエピソードは、平均66+24(平均+SE)から16+5エピソード/日に減少した。同様に、意思伝達する自発的な試みの事象は、平均11+1から21+2エピソード/日に増大した。

ラセミ体バクロフェンを、自閉症の一連の障害を有する21歳の被験体(12人の男性および9人の女性)を治療するために投与した(表1参照)。被験体は、抗癲癇薬(N=14)、抗精神病薬(N=8)および鎮痙薬(N=6)などの様々な薬剤で調節されなかった重大な行動問題を有した。被験体に投与されるバクロフェンの用量を、約8.5ヶ月の治療の最大期間について、約2.5mg 1日/2回(1日2回)〜最大約15mg 1日/3回(1日3回)の週ベースで滴定した。
5人の脆弱X症候群の男性被験体を治療するためにラセミ体バクロフェンを投与した(表2)。これらの被験体は、典型的な向精神薬では完全に制御されない深刻な行動の問題を有した。5人の被験体のうち4人において同時抗精神病薬治療にバクロフェンを加えた。用量を約2.5mg BIDから最大で約20mg TIDまで、最長約4ヶ月の期間で滴定した。医師がバクロフェン治療による改善の全体的な印象を、「ずっと悪くなった」、「悪くなった」、「やや悪くなった」、「変化なし」、「ややよくなった」、「よくなった」または「ずっとよくなった」の範囲の7つのカテゴリー階級で評価した。医師の評価が「ずっとよくなった」または「よくなった」のいずれかであった場合、被験体を「改善された」とみなし、評価が「やや悪くなった」、「変化なし」または「ややよくなった」の場合、「改善せず」とみなし、「悪くなった」または「ずっと悪くなった」と評価された場合、「悪化した」とみなした。5人の患者のうち2人は、いらいら、攻撃性および動揺の減少などの行動の改善を示した。他の改善の範囲として、クラス参加の増加および多動の減少が含まれた。2人の被験体は、明白な改善は示さず、投薬計画は、なお調整されている。1人の被験体は、過剰な眠気、いらいらの増加、叫ぶおよび罵ることで示されるように悪化し、バクロフェンを中止した。
ラセミ体バクロフェン
市販のバクロフェン(Sigma Chemical Co.,St. Louis,Mo)は、R-およびS-異性体の50:50ラセミ混合物である。ラセミ体バクロフェンの結合親和性をより良好に理解するため、Novascreen Biosciences Corporation (Hanover,MD)からSide Effect Database (SED)のライセンス許可を得た。Side Effect Databaseは、10μM ラセミ体バクロフェンに対して二重にスクリーニングされた76個の分子標的を含む。これらの標的は、バクロフェンの副作用、誤作用および治療標的の重要な媒介物質であり得る。Side Effect Databaseによれば、バクロフェンは、GABA-Bに選択的であり、10μMで、その他の75個の分子標的への結合は約50%未満であることがわかった。
バクロフェンのR-異性体(R-バクロフェン)は、S-異性体よりも強力なGABA-Bアゴニストである。バクロフェン(Sigma Chemical Co.,St. Louis,MOから購入)のR-(約99% R-バクロフェン)およびS-(約99% S-バクロフェン)異性体に対する結合親和性を評価した。R-およびS-バクロフェンの両方を10μMで、74個の分子標的(29個の神経伝達物質関連標的、1個のステロイド、3個のイオンチャネル、1個のセカンドメッセンジャー、4個の増殖因子/ホルモン、13個の脳/腸管ペプチド、12個の一般的な酵素、9個のキナーゼ、ならびに2個の細胞系および機能性標的)に対して二重にアッセイした。試験により、R-およびS-バクロフェンがGABA-Bの選択的アゴニストであること、また、R-異性体はS-異性体よりも強力なGABA-Bアゴニストであることが確認された(表3)。表3に示されるように、GABA(B)結合アッセイは、ラット皮質膜を用いて行なった。IC50およびKi値は、R-バクロフェンがS-バクロフェンよりも約10〜約15倍強力であり、R-/S-バクロフェンは効力が中間であることを示す。
実験目的
実験の目的は、GABA-BレセプターアゴニストR-バクロフェンおよび/またはS-バクロフェンの投与後に、ガラス玉覆い隠し行動が減少されるかどうかを調べることであった。これらの実験において、Fmr1 KOマウスは、R-バクロフェンまたはS-バクロフェンのいずれかのi.p.注射を受け、ガラス玉覆い隠し行動が試験された。
実験の結果は、R-バクロフェンおよびS-バクロフェンがFmr1 KOマウスにおいてガラス玉覆い隠し行動を改変することを示す。試験前にR-バクロフェンのi.p.注射を1時間受けたFmr1 KOマウスにおいてガラス玉覆い隠し行動に有意な用量関連改変があった。具体的には、10mg/kgの用量を受けたFmr1 KOマウスは、0、0.3、1、3または6mg/kgを受けたマウスよりも有意に少ないガラス玉を覆い隠した。また、試験前にS-バクロフェンのi.p.注射を1時間受けたFmr1 KOマウスにおいて、ガラス玉覆い隠し行動に有意な用量関連改変があった。具体的には、1.0mg/kgを受けたマウスは、0.0mg/kg治療マウスよりも有意に多くのガラス玉を覆い隠した。また、50mg/kgを受けたマウスは、0、0.3、1、6または30mg/kgのS-バクロフェンを受けたマウスよりも少ないガラス玉を覆い隠した。
この実験からの所見は、R-バクロフェンで治療したFmr1 KOマウスにおいてガラス玉覆い隠し行動に用量関連減少があることを示し、R-バクロフェンが、このアッセイで評価される不安感様/強迫性/反復性行動形態を減少させることを示す。しかしながら、S-バクロフェンも、1mg/kgの用量でガラス玉覆い隠し行動を用量関連様式で改変した。実際、Fmr1 KOマウスは、より多くのガラス玉を覆い隠し、この用量が所望の効果と反対の効果を有し得ることを示す。約50mg/kgの用量を受けたFmr1 KOマウスは、より少ないガラス玉を覆い隠し、この高用量が、このアッセイで評価されるこの不安感様/強迫性/反復性行動形態を減少させることを示す。これらのデータは、R-バクロフェンがS-バクロフェンよりも、ガラス玉覆い隠し試験でシミュレーションされるこの不安感様/強迫性/反復性行動形態の減少に適当であり得ることを示す。
C57BL/6J(9世代)遺伝的背景のFmr1 KOマウスをThe Jackson Laboratory (Bar Harbor,ME)から得た。C57BL/6Jを戻し交配してN11世代のFmr1マウスを確立した。本試験のためのすべてのマウスは、Fmr1ヘテロ接合性雌マウスをFmr1野生型雄マウスと交配することにより作製した。雄Fmr1 KO(Fmr1-/y)マウスのみをこの試験において試験した。12時間の明:暗サイクル(6時AMに点灯、6時PMに消灯)の室内にマウスをゲージあたり2〜5匹で収容し、食餌と水は自由に与えた。2〜4ヶ月齢(20〜30グラム)の雄マウスをすべての実験に使用した。一般に、行動試験は、9時AMから3時PMの間に行なった。試験開始時、マウスは3〜4ヶ月齢であった。マウスの遺伝子型を知らない実験者が実験を行なった。
標準的なマウスケージにトウモロコシの穂軸床敷を10cm敷き詰めた。20個の小さい(1.5〜2cm)黒ガラス玉を床敷上に等間隔で配置した(約1〜2cm離して)。マウスをケージ内に配置し、ガラス玉を探して覆い隠させた。約20分後、マウスを出し、覆い隠されたガラス玉の数(ガラス玉は、50%より多くが床敷下にある場合を「覆い隠された」という)を記録した。
ラセミ体バクロフェン
脆弱Xノックアウトマウスへのラセミ体バクロフェン(6mg/kg、12mg/kg)の投与により、ガラス玉覆い隠し行動が用量依存的様式で減少した(図1)。
図2は、試験1時間前にR-バクロフェンのi.p.注射を受けたFmr1 KOマウスにおいて、ガラス玉覆い隠し行動に有意な(p<0.005)用量関連改変があったこと示す。具体的には、10mg/kgの用量を受けたFmr1 KOマウスは、0、0.3、1、3または6mg/kgを受けたマウスよりも有意に少ない(p<0.05)ガラス玉を覆い隠した。治療順序の効果は統計学的に有意ではなかった(p>0.05)。

実験目的
実験の目的は、GABA-BレセプターアゴニストR-バクロフェンおよび/またはS-バクロフェンの投与後に、Fmr1 KOマウスにおいて聴原性発作に対する感受性が低減されるかどうかを調べることであった。これらの実験において、Fmr1 KOマウスは、R-バクロフェンまたはS-バクロフェンのいずれかのi.p.注射を受け、聴原性発作が試験された。
実験の結果は、R(+)-バクロフェンはFmr1 KOマウスにおいて聴原性発作を低減させるが、S(-)バクロフェンは低減させないことを示す。ビヒクル処理およびS(-)-バクロフェン処理(3mg/kg)Fmr1 KOマウスの約70〜80%は、聴原性発作を示す。対照的に、3mg/kgのR(+)バクロフェンで処理したFmr1 KOマウスは、33%しか発作を示さない。
この所見は、R(+)-バクロフェンはFmr1 KOマウスにおいて聴原性発作を低減させたがS(-)-バクロフェンは低減させなかったこと、およびR-バクロフェンはS-バクロフェンよりも、Fmr1 KOマウスにおいてこの環境誘導型の発作の低減に有効であることを示す。
聴原性発作実験に使用した方法は以前に記載されたもの(Yan,Q.J.,et al,Genes Brain Behav. 3:337-359 (2004);Yan,Q.J.,et al,Neuropharm. 49:1053-1066 (2005))。簡単に、Fmr1ノックアウトマウスを、アッセイの約60分前にビヒクル、R-バクロフェンまたはS-バクロフェンのいずれかでi.p.処理した。マウスを大音量に曝露し、次いで、発作の発生について観察した。主な終点は、ほとんどの場合で呼吸停止および死をもたらす持続性強直性発作である痙攣重積状態の頻度とした。
R-バクロフェン(3mg/kg)は、聴原性発作の誘導を有意に予防したが、同じ用量のS-バクロフェンは、ビヒクル以上には有効でなかった(図4)。
これらの所見は、R-バクロフェンはS-バクロフェンよりも、Fmr1 KOマウスにおいてこの環境誘導型の発作の低減に有効なようであることを示す。
実験目的
実験の目的は、GABA-BレセプターアゴニストR-バクロフェンおよび/またはS-バクロフェンの投与後に、Fmr1 KOマウスにおいてオープンフィールド活動性が改変されるかどうかを調べることであった。これらの実験において、Fmr1 KOマウスは、R-バクロフェンまたはS-バクロフェンのいずれかのi.p.注射を受け、オープンフィールド活動性について試験された。
実験の結果は、R(+)-バクロフェンおよびS(-)バクロフェンがFmr1 KOマウスにおいてオープンフィールド活動性を低下させたことを示す。ビヒクル処理野生型(WT)同腹子対照と比べて、ビヒクル処理Fmr1 KOマウスは、全活動性によって測定したとき、オープンフィールドにおいて有意により活動性であった。試験1時間前にR(+)またはS(-)-バクロフェンのi.p.注射を受けたFmr1 KOマウスにおいて総距離で測定される探索活動性に有意な用量関連低下があった。具体的には、6mg/kgの用量のR(+)-バクロフェンを受けたFmr1 KOマウスは、ビヒクルを受けたFmr1 KOマウスと比べて活動性が低かった(総距離が減少)。同様に、50mg/kgの用量のR(+)-バクロフェンを受けたFmr1 KOマウスも、ビヒクルを受けたFmr1 KOマウスと比べて活動性が低かった(総距離が減少)。
これらのデータは、R(+)またはS(-)-バクロフェンで処理したFmr1 KOマウスにおいて、探索活動性に用量関連減少があったことを示す。脆弱X症候群を有する個体と同様、雄Fmr1欠損マウスはより活動性であったが、オープンフィールドアリーナを動いた総距離で測定されるこの活動性の増大は、バクロフェンで標準化され得る。データは、Fmr1 KOマウスにおいて探索活動性を低下させるのに必要とされるS-バクロフェンの用量は、R-バクロフェン処理マウスで必要とされるよりも多いことを示す。したがって、R-バクロフェンはS-バクロフェンよりも、オープンフィールド試験でシミュレーションされる活動性の増大の型の低下に有効であり得る。
訓練の1時間前に、試験品および対照品を0.1ml/10g体重の容量でマウスに腹腔内(i.p.)注射によって投与した。
R(+)-バクロフェン
図5は、ビヒクル処理WT対照と比べてビヒクル処理Fmr1 KOマウスでは、運動活動性に有意な(p<0.001)増大があったことを示す。また、試験1時間前にR-バクロフェンのi.p.注射を受けたFmr1 KOマウスにおいて移動総距離に用量関連改変があった。6mg/kgの用量を受けたFmr1 KOマウスは、ビヒクル処理Fmr1 KOマウスよりも有意に活動性が低かった(p=0.018)。
図5は、ビヒクル処理野生型(WT)対照と比べて、ビヒクル処理Fmr1 KOマウスにおいて運動活動性に有意な(p<0.05)増大があったことを示す。また、試験の1時間前にS(-)-バクロフェンのi.p.注射を受けたFmr1 KOマウスにおいて、移動総距離に用量関連改変があった。具体的には、50mg/kgの用量を受けたFmr1 KOマウスは、ビヒクル処理Fmr1 KOマウスよりも有意に活動性が低かった(p=0.021)。
これらのデータは、R(+)-バクロフェンで処理したFmr1 KOマウスにおいて運動活動性に用量関連減少があることを示し、R(+)-バクロフェンは、このアッセイで評価されたように、Fmr1 KO多動を減少させることを示す。また、S-バクロフェンは、Fmr1 KO活動性を用量関連様式で減少させた。低用量のR(+)-バクロフェンは、S(-)-バクロフェンと比べてFmr1 KO多動の減少に有効であった。R(+)バクロフェンはS(-)-バクロフェンよりも、Fmr1 KOマウスにおいてオープンフィールド試験によってシミュレーションされたように、多動の型の低減に適当であり得る。
R-バクロフェンは、S-バクロフェンよりも、GABA-Bアゴニストとして10倍より大きく強力であり(表3)、また、ガラス玉覆い隠し行動の抑制に関してより強力である(図2および3)。同様に、R-バクロフェンはS-バクロフェンよりも、聴原性発作の抑制(図4)およびオープンフィールド試験(図5)に関して強力である。
ジシクロミンは、Fmr1ノックアウトマウスにおいてガラス玉覆い隠し行動を低減する(図6)。ガラス玉覆い隠し行動は、不安感関連の強迫性および保続性行動を反映していると考えられており、したがって、脆弱X症候群および他の脳発達障害を有する被験体において一般に観察される症状を模倣する。同様に、ジシクロミンは、Fmr1ノックアウトマウスにおいて聴原性発作を抑制した(図7)。Fmr1ノックアウトマウスにおける聴原性発作への感受性増大は、FXSを有する被験体における発作障害の発生率の増加のモデルと考えられる。
方法
Fmr1 KOマウスをC57BL/6J遺伝的背景で交配した。雄Fmr1 KO(Fmr1-/y)マウスのみをこの試験において試験した。標準的なマウスケージにトウモロコシの穂軸床敷を10cm敷き詰めた。小さい(1.5〜2cm)黒ガラス玉を床敷上に等間隔で配置した。マウスをケージ内に配置し、ガラス玉を探して覆い隠させた。約20分後、マウスを出し、覆い隠されたガラス玉の数(ガラス玉は、ガラス玉の50%より多くが床敷下にある場合を「覆い隠された」とみなした)を記録した。マウスは、試験前にジシクロミンで約60分間処理した。
この背景系統のFmr1ノックアウトマウスは、野生型(wt)同腹子よりも多くのガラス玉を覆い隠した。ジシクロミンの投与により、ガラス玉覆い隠し行動が用量依存的様式で低減された(図6)。
これらのデータは、ジシクロミンで処理したFmr1 KOマウスにおいて、ガラス玉覆い隠し行動に用量関連減少があることを示す。ジシクロミンは、このアッセイで評価される不安感様/強迫性/反復性行動形態を減少させた。
方法
使用した方法は上記のとおりである。簡単に、Fmr1ノックアウトマウスをアッセイの約60分前にビヒクルまたはジシクロミンのいずれかでi.p.処理した。マウスを大音量に曝露し、次いで、発作の発生について観察した。主な終点は、ほとんどの場合で呼吸停止および死をもたらす持続性強直性発作である痙攣重積状態の頻度とした。
ジシクロミンは、聴原性発作の誘導を有意に妨げた(図7)。
これらのデータは、ジシクロミンは、Fmr1 KOマウスにおいて発作を有効に低減させることを示す。

Claims (63)
- ダウン症候群、脆弱X症候群および自閉症からなる群より選択される少なくとも1つの状態を有するヒトに、少なくとも1つのγ-アミノ酪酸アゴニストを含む組成物を投与する工程を含む、ヒトの治療方法。
- γ-アミノ酪酸(GABA)アゴニストがGABA(B)アゴニストである、請求項1記載の方法。
- GABA(B)アゴニストがバクロフェンを含む、請求項2記載の方法。
- バクロフェンが、ヒトに投与される組成物中のR-バクロフェンおよびS-バクロフェンの総量に対して少なくとも約51モル%のR-バクロフェンである、請求項3記載の方法。
- バクロフェンが少なくとも約75モル%のR-バクロフェンである、請求項4記載の方法。
- バクロフェンが少なくとも約90モル%のR-バクロフェンである、請求項4記載の方法。
