Claims (28)
- 少なくとも1つの頭蓋内カテーテルと経皮アクセス機器とを備える、治療薬を中枢神経系に送達する装置において、前記経皮アクセス機器は、少なくとも1つの体外表面と、少なくとも1つの皮下表面とを有する本体を備え、前記本体は、移植された頭蓋内カテーテルに連結するための少なくとも1つのポートを画定し、前記ポートは、前記機器の前記体外表面からアクセス可能であるが、前記ポートの前記内腔と前記体外表面との間に封止体が設けられることを特徴とする装置。
- 前記経皮アクセス機器は3つ以上のポートを備えることを特徴とする請求項1に記載の装置。
- 前記経皮アクセス機器は前記本体から延びるフランジを備えることを特徴とする前記請求項のいずれかに記載の装置。
- 前記フランジは湾曲しているが、前記経皮アクセス機器が移植されるとき、皮膚の表面に対して実質的に平行である周辺縁部領域を有することを特徴とする請求項3に記載の装置。
- 前記フランジは管が通過できる開口を備えることを特徴とする請求項3から4のいずれか一項に記載の装置。
- 前記フランジは有孔材料を含むことを特徴とする請求項3から5のいずれか一項に記載の装置。
- 前記経皮アクセス機器の一部が組織内殖を促進する材料から作製され、かつ/または前記材料で被覆されることを特徴とする前記請求項のいずれかに記載の装置。
- 前記経皮アクセス機器の前記封止体は栓の形態にあることを特徴とする前記請求項のいずれかに記載の装置。
- 前記経皮アクセス機器の前記封止体は抗バクテリアまたは抗菌材料を含むことを特徴とする前記請求項のいずれかに記載の装置。
- 前記経皮アクセス機器の前記体外表面は、前記ポートに対するアクセスを可能にするための開口を備えることを特徴とする前記請求項のいずれかに記載の装置。
- 前記経皮アクセス機器の前記体外表面に装着可能な少なくとも1つのキャップを備えることを特徴とする前記請求項のいずれかに記載の装置。
- 移植後の治癒過程中に前記経皮アクセス機器を安定化させるための術後キャップを備えることを特徴とする請求項11に記載の装置。
- 前記経皮アクセス機器に固定されうるキャップを備えることを特徴とする請求項11から12のいずれか一項に記載の装置。
- 前記少なくとも1つのキャップは抗菌性の裏当てを有することを特徴とする請求項11から13のいずれか一項に記載の装置。
- 前記1つまたは複数の頭蓋内カテーテルは、1本の管を経由して前記経皮アクセス機器の前記1つまたは複数のポートに装着されることを特徴とする前記請求項のいずれかに記載の装置。
- 流体薬剤を送達するための少なくとも1本の導管を有する送達ユニットを備え、前記導管には、前記導管およびポートが流体連通するように、前記経皮アクセス機器の前記ポートの中へ挿入するための雄型連結部分が設けられることを特徴とする前記請求項のいずれかに記載の装置。
- 前記送達ユニットは3本以上の導管を備え、前記経皮アクセス機器は3つ以上のポートを備えることを特徴とする請求項16に記載の装置。
- 前記送達ユニットおよび経皮アクセス機器は、前記送達ユニットおよび経皮アクセス機器が唯一の様態で嵌り合うように配置され、それによって同じ導管が同じポートに連結されることを保証することを特徴とする請求項16または請求項17に記載の装置。
- 前記経皮アクセス機器には案内部材が設けられ、前記送達ユニットには対応する穴が設けられることを特徴とする請求項18に記載の装置。
- 前記送達ユニットを前記経皮アクセス機器に固定するための固定部材を備えることを特徴とする請求項16から19のいずれかに記載の装置。
- 3つ以上の頭蓋内カテーテルとともに使用する経皮アクセス機器において、少なくとも1つの体外表面と、少なくとも1つの皮下表面とを有する本体を備え、前記本体は、前記3つ以上の頭蓋内カテーテルに連結するための3つ以上のポートを画定し、前記3つ以上のポートは、前記機器の前記体外表面からアクセス可能であるが、各ポートの内腔と前記体外表面との間に封止体が設けられることを特徴とする経皮アクセス機器。
- 移植されたカテーテルとともに使用する経皮アクセス機器において、少なくとも1つの体外表面と、少なくとも1つの皮下表面とを有する本体を備え、前記本体は、移植された機器に連結するための少なくとも1つのポートを画定し、前記ポートは、前記機器の前記体外表面からアクセス可能であるが、前記ポートの内腔と前記体外表面との間に封止体が設けられ、フランジが前記経皮アクセス機器の前記本体から外向きに延びて、前記フランジは湾曲しているが、前記経皮アクセス機器が移植されるときに皮膚の表面に対して実質的に平行に位置する周辺縁部領域を有することを特徴とする経皮アクセス機器。
- 移植時に、前記フランジの前記周辺縁部領域は皮下組織の上に着座することを特徴とする請求項22に記載の経皮アクセス機器。
- 体外部分および皮下部分を備える経皮アクセス機器を移植する方法において、任意適切な順序で、
(a) 前記経皮アクセス機器の全体が通過することを可能にするサイズの第1の切開部を被験者の皮膚の中に作成するステップと、
(b) 前記体外部分のみの通過を許容するサイズの第2の切開部を前記被験者の皮膚の中に作成するステップと、
(c) 前記第1の切開部を経由して、前記経皮アクセス機器を受け入れるのに十分なサイズである皮下ポケットを前記第2の切開部の下に形成するステップと、
(d) 前記経皮アクセス機器を前記第1の切開部に通過させて前記ポケットの中へ入れ、かつ前記体外部分が前記第2の切開部を通過するように、前記経皮アクセス機器を前記ポケットの内部に配置するステップと、
を含むことを特徴とする方法。 - 前記経皮アクセス機器の前記皮下部分は本体から延びるフランジを備えることを特徴とする請求項24に記載の方法。
- 治療薬を中枢神経系に送達する方法において、任意適切な順序で、
(i) 被験者の神経系に流体を送達するための1つまたは複数のカテーテルを移植するステップと、
(ii) 経皮アクセス機器を前記被験者に移植するステップであって、前記経皮アクセス機器は、前記1つまたは複数のカテーテルと流体連通するポートを備え、前記ポートはまた、前記経皮アクセス機器の体外表面からアクセス可能である、移植するステップと、
(iii) 前記経皮アクセス機器を経由して治療薬を前記1つまたは複数のカテーテルに送達するために少なくとも1つの外部ポンプを使用するステップと、
を含むことを特徴とする方法。 - ステップ(iii)は定期的に実施され、前記外部ポンプは、治療薬の前記送達が必要とされないときに前記経皮アクセス機器から切り離されることを特徴とする請求項26に記載の方法。
- ステップ(i)は複数のカテーテルを移植するステップを含み、ステップ(iii)は治療薬を各カテーテルに送達するステップを含んで、各カテーテルに対する前記送達は、異なる注入率および異なる注入継続時間の少なくとも一方を使用して実施されることを特徴とする請求項26に記載の方法。
