本調査の目標は、椎間板変性の様々な段階を区別する高分解能マジック角回転法NMR分光法の能力を判断することである。17の椎間板が、人の死体から取り出され、かつ化学組成及び変性レベル(トンプソングレード)の増大に対する椎間板の化学的環境変化を決定する、1次元及び2次元の(TOCSY)1HによるHR-MAS分光法、並びに緩和時間T1及びT2計測を用いて解析された。重要な発見は、線維輪から取られた試料に係るスペクトルと、髄核から取られた試料に係るスペクトルとは非常に似ていること、並びに椎間板の線維輪及び髄核試料に係るスペクトルにおいて、トンプソングレードの増大に対して視覚的に明らかな変化が観測されたこと、である。N-アセチルとコリン領域との領域比、及びコリンと炭水化物領域との領域比によって、個々の比の重なりを最小にしながら、トンプソングレードが増大する椎間板間での区別が可能となる。椎間板スペクトルの化学成分に係る緩和時間T1及びT2の変化は、トンプソングレードの増加に対する椎間板の脱水並びにプロテオグリカン及びコラーゲンマトリックスの衰弱程度の変化を反映しているように思われる。この調査結果は、椎間板の変性に関連する化学変化を検出する生体内分光法の利用を支持している。
A.細胞組織の取得
本調査は、我々の調査所の審査委員会による承認を得た。腰椎は、n=17体の死体(範囲:22歳から85歳)から外科的手法によって取り出され、-80℃に冷却された。得られた腰椎は、死体解剖用のこぎり及び外科用メスで分離された。椎間板の周りの骨は、椎間板から除去及び分離された。3mmの組織診が、取り出された椎間板の線維輪及び髄核領域で開けられた。これらの組織診は、互いに近接するように開けられ、円柱対称であった。平均質量は15.2±3.4mgだった。所与の位置から並んだ3の試料はまた、スペクトルの再現性をテストするのにも用いられた。トンプソングレーディングは、それに忠実であるという大多数の見解が得られている表示器で実行される。全体では、8のトンプソングレード1を有する試料、6のトンプソングレード3を有する試料、及び6のトンプソングレード5を有する試料について調査された。
HR-MASデータは、バリアンのINOVA分光計を用いて、1.0±0.5℃で、かつ回転速度2250Hzで得られた。INOVA分光計は11.75Tで動作する。またその分光計には4mmのgHXナノプローブが備えられている。1次元スペクトルでは、20000Hz(40ppm)のスペクトル幅にわたり、40000の複合点(complex point)が得られた。その際の条件は、90°のパルス幅、2秒のHOD分子のプリサチュレーション周期、32の過渡状態、8秒の繰り返し時間(>最長の緩和時間T1の5倍)、2秒の取得時間(>最長の緩和時間T2の5倍)、及び3:36分の全取得時間であった。楕円形状の試料チャンバ及び漏れを防ぐ気密性ねじの上部プラグを有する、特別に設計された18μlのジルコニウム回転子を用いて、試料は解析された。各試料について、0.75wt%のトリメチルシリルプロピオン酸ナトリウム(3-(trimethylsilyl)propionic-2,2,3,3-d4acid)(D2O+TSP、シグマ-アルドリッヒ社製)を含む3.0μlの酸化重水素がピペットによって回転子の底部に注入された。その後、細胞組織試料の質量が測定され、加えられた。
データは、バリアン社製VNMR6.1Cソフトウエア(バリアン社製)をオンラインで用いるか、又はACD/ラボ1D及び2DNMR処理用ソフトウエア、バージョン7.0(アドバンストケミストリー社)をオフラインで用いることによって処理された。1次元のFIDsソフトウエアは、指数関数でアポダイゼーションされた。その指数関数は、取得時間の逆数に等しい線広がりを有し、フーリエ変換され、位相が補正され、かつ0.0ppmでのTSPが参照された。緩和時間は、指数関数の最小2乗法による回帰解析を用いて計算された。表の(最小)2乗フィットが有する標準エラーが10%未満の場合にのみ緩和時間は用いられた。TOCSYデータは、F1について3×Nの線形予測を用いて処理され、1024の複合点にゼロが入り(F1のみ)、かつ両方の観測軸についてガウシアンによる重み付けを用いてアポダイゼーションが行われた。
A.トンプソングレード微分
トンプソングレード1、3,及び5を有する椎間板の線維輪及び髄核に係る代表的な1次元HR-MASスペクトルが、図1-3に図示されている。
1次元のプロトンスペクトルで観測される共鳴を特定するため、2次元のTOCSYスペクトルが得られ、観測されたクロスピークの化学シフトは、これまでに報告されてきた化学シフト値と比較された。図5Aは、トンプソングレード1の椎間板に係るTOCSYスペクトルを図示している。調査された全部で8のトンプソングレード1の椎間板では、限られた数のクロスピークのみが観測できた。そのクロスピークには、アラニン(1.49,1.79ppm)、乳酸(1.35,4.16ppm)及びプロテオグリカンポリマーの炭水化物部分に関連するプロトン、が含まれる。対照的に、調査された6の変性したトンプソングレード5の椎間板に係るTOCSYスペクトルは、より多くの検出可能なクロスピークを示した。そのクロスピークには、イソロイシン(0.92,1.32ppm)、ロイシン(0.98,1.72ppm)、リシン(1.73,3.04ppm)、プロリン(1.73,1.93ppm)、グルタミン(2.14,2.46ppm及び2.14,3.79ppm)、グルタミン酸(2.1,2.36ppm)、ヒドロキシプロリン(2.45,3.45ppm)、及びエタノールアミン(3.15,3.83ppm)が含まれる。TOCSY実験は、1DHR-MASスペクトルでの炭水化物領域(3.5-4.2ppm)での分解可能な共鳴は、多数のアミノ酸、エタノールアミン含有化合物、及びコンドロイシン硫酸塩の崩壊産物に係る砂糖のC-Hプロトンから生じる複合ピークであった。全椎間板の1DHR-MASスペクトルは、3.21ppm及び3.23ppmで2の一重項を示した。3.21ppm及び3.23ppmの一重項は、自由コリン(Cho)及びホスホコリン(PC)の化学シフトにそれぞれ対応する。TOCSY実験はまた、3.55×4.07ppmでのメチレンプロトンのクロスピーク、及び3.62×4.18ppmでのPCのメチレンプロトンのクロスピークをも示した。またスペクトルのコリン領域では、複数の他の小さく、広い共鳴も存在する。これら特定されていない。
図6A及び図6Bは、トンプソングレード1、3及び5の椎間板で分解可能な共鳴の平均緩和時間T1及びT2をそれぞれ示す。全トンプソングレードの髄核及び線維輪で分解可能な共鳴の緩和時間T1及びT2のみが測定された。この結果、0.9,1.32,1.49,2.04,3.21及び3.67ppmでの共鳴の緩和時間T1及びT2が測定された。髄核及び線維輪領域で測定された緩和時間T1とT2との間には大きな違いはなかった。従ってこれらの領域の緩和時間は、椎間板のグレード比較に係る統計学的有意性を増大させるために追加された。各トンプソングレードからのこれらの共鳴に係る平均のT1及びT2測定は、大きな変化を示した。そのため、椎間板変性の増大に対する代謝体でのT1及びT2について明らかな傾向は得られなかった。T1の値についての観測可能な傾向は、最初にトンプソングレード3の椎間板で減少し、続いてトンプソングレード5の椎間板で増大した。T2については、すべての観測された共鳴について一貫した傾向は存在しないが、N-アセチル(2.04ppm)、コリン(3.21ppm)及び炭水化物C-H共鳴(3.67ppm)については、トンプソングレードの増加と共に増加する傾向を示した。
プロトンHR-MASスペクトルは、椎間板の線維輪及び髄核領域から取られたスペクトルと非常に似ていた。2のスペクトルは、主としてコンドロイシン硫酸塩からの大きなN-アセチル共鳴及び炭水化物共鳴、並びにコリン含有化合物、脂質/乳酸及び複数のアミノ酸からの共鳴を示した。髄核の性質がよりゲル状であるため、トンプソングレード1の椎間板についての線維輪と比べて、髄核内では一貫して高いスペクトル分解能が観測された。
まとめると、プロトンHR-MASは、椎間板の線維輪及び髄核領域から取られたスペクトルと非常に似たスペクトルを供する。トンプソングレードの増大に対する重要でかつ視覚上明らかな変化が、椎間板からの線維輪及び髄核試料に係るプロトンHR-MASスペクトル内で観測可能である。定量的には、Nアセチル領域での共鳴に係る代謝体のピーク領域とコリン領域での共鳴に係る代謝体のピーク領域との比は、個々の比の重なりを最小にしながらトンプソングレードが増大する椎間板を差別化するのに有用である。椎間板に係るスペクトルの化学成分の緩和時間T1及びT2の変化は、椎間板変性についてこれまでに報告されてきた水の緩和時間の変化とは似ていない。変性の増大に対する椎間板スペクトルの化学成分に係る緩和時間変化は、変性の増大に対する、椎間板の脱水及びプロテオグリカンとコラーゲンマトリックスの破壊程度の両方を反映する。NMR分光の生体内撮像装置は、椎間板変性に関連する化学変化の検出に有用である。
痛みを伴う変性椎間板を評価する従来の画像化法では、形態に係る基準に焦点が当てられてきた。しかし、形態上の知見と患者の症状との間の関連が弱いということはよく知られていることである。この生体調査の目的は、外科手術が行われた患者から取られた椎間板の組織片内の生化学的マーカーを正確に評価する道具として、定量的高分解能マジック角回転法(HR-MAS)NMR分光法を利用することである。痛みを伴う椎間板変性の生化学的兆候を特定するため、背痛のために椎間板切除が行われた患者から得られた椎間板のスペクトルと、脊椎側弯症のために外科手術が行われた患者からなる参照用母集団のスペクトルとが比較された。
スペクトルデータは、4mmのgHXナノプローブが備えられたバリアン(Varian)社製INOVA分光計を用いて、11.7T(500MHz)、1℃、及び2250Hzの回転速度で得られた。特別に設計された35ulの回転子を用いて、椎間板の痛みを伴う患者(n=6)、並びに前部及び/又は後部の脊椎固定術が行われた患者(n=4)、での外科手術で除去された椎間板細胞組織について調査された。基準として、D2O+0.75%TSPを有する細胞組織試料(シグマ-アルドリッヒ(Sigma-Aldrich)社)についての定量的プロトンスペクトルが得られた。各細胞組織試料について、スピンエコー回転子で同期されたCarr-Purcell-Meiboom-Gill(CPMG)パルスシーケンス(nt=128,at=2.0s,TR=5s,エコー時間=80ms)が得られた。乳酸共鳴(1.31ppm、二重項)、プロテオグリカン(PG)に関連するn-アセチル共鳴(2.04ppm、一重項)、及びコラーゲン破壊領域(col)(3.30-4.00ppm)が解析され、椎間板の組織片が比較された。これらの領域は、図7A及び図7Bで図示されたより詳細なスペクトル部分で注釈が与えられている。
図7A及び図7Bは、(A)椎間板に起因する痛みを伴う患者、及び(B)脊椎側弯症の患者、の典型的な80msの1HのCPMGスペクトルを図示している。プロテオグリカンのn-アセチル共鳴(PG)、乳酸、及びコラーゲン破壊領域(col)が示されている。
本実験から得られた結果は、生化学マーカーが椎間板の痛みと相関する過程を評価するのに有用であることを示唆している。これまでの調査は、プロテオグリカン合成及び全体の健康へのpHの影響について報告している。乳酸濃度が増大することで、椎間板材料の実効的pHは、水溶液中の自由H+の増大に起因して減少する。このことにより、プロテオグリカンの合成が妨げられているものと思われる。

- 細胞組織の変性又は痛みに関連する前記細胞組織の第1領域の特性を診断するのに有用な情報を供するシステムであって、
コンピュータで読み取り可能な形式で書かれたコンピュータで読み取り可能なソフトウエアプログラムであって、前記細胞組織の前記第1領域から取られた核磁気共鳴(NMR)スペクトルからのデータを処理するように備えられ、かつさらに前記処理されたデータからの前記有用な情報を供するように備えられているソフトウエアプログラム、

コンピュータで読み取り可能な形式で書かれたコンピュータで読み取り可能なソフトウエアプログラムであって、前記NMRスペクトルに関連するデータを処理し、かつ前記処理されたデータに基づく前記有用な情報を供するように備えられているソフトウエアプログラム;及び
前記プログラムを動作させるように備えられた処理装置;
を有し、
前記有用な情報は、前記細胞組織変性に関連する少なくとも1の化学的因子の共鳴に少なくとも部分的に基づく、

コンピュータで読み取り可能な形式で書かれたコンピュータで読み取り可能なソフトウエアプログラムであって、前記NMRスペクトルに関連するデータを処理し、かつ前記処理されたデータに基づく前記有用な情報を供するように備えられているソフトウエアプログラム;及び
前記プログラムを動作させるように備えられた処理装置;

前記有用な情報は、前記痛みに関連する少なくとも1の化学的因子の共鳴に少なくとも部分的に基づく、
医療診断システム。 - 患者の骨格関節に関連する細胞組織の特性を特定又は評価する方法であって:
前記特性の特定又は評価を可能にするように備えられた前記細胞組織のNMR分光に関連する情報を処理する手順;
前記特性の程度が特定又は評価可能となるように、前記細胞組織に関連する、NMR分光システムからの分光情報を供する手順;若しくは
前記特性の程度が特定又は評価可能となるように、前記細胞組織に対する、実質的に非侵襲の画像化診断装置からの、前記細胞組織に関する情報を供する手順;又は
上記手順の2以上を組み合わせた手順;
を有する方法。 - 椎間板の少なくとも一部についての特性の程度を評価する方法であって、
当該方法はNMR分光システムによって取り込まれた前記一部に関連するスペクトルからデータを取得する手順を有し、かつ
前記スペクトルは前記の特性の程度を少なくとも部分的に示唆する情報を供する、
