Abstract

股関節を再構築する関節形成術用の大腿骨部品（１）は、大腿骨キャップ（２）、及び軸（６）を含み、上記大腿骨キャップ（２）は骨盤骨（１５）の中にセットされるカップ（３）と噛み合うように適応され、上記大腿骨キャップは凸状の表面（４）及び凹状の表面（５）を持ち、上記軸（６）は上記大腿骨キャップの凹状の表面に取り付けられ、少なくとも上記軸の一部が可溶性の材料で構成されることで特徴付けられる。

Description

発明の詳細な説明
〔技術分野〕
本発明は股関節の再構築に関するものであり、特に股関節を再構築する関節形成術用の大腿骨部品に関するものである。しかしながら、本発明は股関節の再構築に限定されるものではなく、人間及び他の哺乳動物における別の球窩関節に応用して利用することができる。
〔発明の背景〕
股関節を再構築する概念は、１９５０年代の後半以来知られている。股関節の再構築は、骨の大部分を完全に削除するのではなく、衰弱した、又は病変した股関節の範囲内で骨表面だけを置換することが好ましいという事実に頼っている。このアプローチは、大腿骨の頭部及び頚部を保存するという利益を有する。これは、完全な股関節の自然なオフセット及び前傾を残し、ほぼ同等の脚の長さを維持する。球窩関節におけるより大きなサイズの球は、脱臼の問題を減少させる。骨の上に負荷されるストレスは比較的自然である。さらに、磨耗率が低い金属を使用することができる。
股関節を再構築する関節形成術では、骨盤窩の病変部分が除去される。置換カップは骨盤骨の中にセットされる。大腿骨の関節表面及び大腿骨頭部は作り変えられ、大腿骨キャップは大腿骨の上にセットされて上記カップと噛み合うように適応される。
外科手術中にキャップと大腿骨との位置合わせを援助するために、キャップは軸を備えている。当該軸は、キャップの位置合わせを可能にし、大腿骨キャップの金属及び／又はセメントを備えた骨メッシュまでの関節の安定も付与する。
この軸は、有益である一方、大腿骨内部の微小破壊及び不自然なストレスと同調して、大腿骨の衰弱を引き起こすことができる。このことは、患者にとって顕著な痛みの原因となり得、長期的には大腿骨を衰弱させるかもしれない。
忍耐、結果的に比較的長期にわたる回復期間、及びその後の病院滞在により受ける心的外傷を最小限にするために、上記軸を除去可能にすることが試みられた。そのような装置の１つはＧＢ２３７２７０７（ＭｃＭｉｎｎ）に記述されている。その開示では、軸は、大腿骨キャップに接続された第１の軸部分、及び第２の除去可能な軸部分を有している。この装置では、第２の軸部分を除去することの利点がある一方、残りの部分は大腿骨頭部に残る。以前の先行技術に対するいくつかの同様の不利が、残っている第１の軸部分に存在する。
本発明の目的は、先行技術における欠陥の少なくとも１つを克服もしくは改善する、股関節を再構築する関節形成術用の大腿骨部品を提供すること、又は代替物を提供することである。
本発明の更なる目的は、先行技術における欠陥の少なくとも１つを克服もしくは少なくとも改善する、ヒトを含む哺乳動物のための球窩関節を再構築する関節形成術用の骨部品を提供すること、又は代替物を提供することである。
〔発明の概要〕
第１の態様によれば、本発明は、大腿骨キャップ及び軸を含む、股関節を再構築する関節形成術用の大腿骨部品であり、上記大腿骨キャップは骨盤骨の中にセットされるカップと噛み合うように適応され、上記大腿骨キャップは凸状の表面及び凹状の表面を持ち、上記軸は上記大腿骨キャップの凹状の表面に取り付けられ、少なくとも上記軸の一部が可溶性の材料で構成されることで特徴付けられる。
好ましくは、第１の実施形態では、上記大腿骨キャップの凹状の表面は軸穴を含む。
好ましくは、上記軸は全部が可溶性の材料で構成される。
好ましくは、特定の実施形態では、上記軸はマグネシウム合金で構成される。
好ましくは、他の実施形態では、上記軸はアルミニウム合金又は亜鉛合金で構成される。
好ましくは、さらに他の実施形態では、上記軸はポリグリコール酸又はポリ乳酸のような酸で構成される。
好ましくは、さらに他の実施形態では、上記軸はトリメチレン炭酸塩共重合体のような分解性高分子で構成される。
好ましくは、さらに他の実施形態では、上記軸はヒドロキシアパタイトで構成される。
好ましくは、上記可溶性の材料は骨形成を援助する。
好ましくは、１つの実施形態では、上記軸穴及び軸の上部末端はネジ切りされている。
好ましくは、第２の実施形態では、上記軸穴及び軸の上部末端はバヨネット接続（ｂａｙｏｎｅｔ ｆｉｔｔｉｎｇ）を含んでいる。
好ましくは、第３の実施形態では、上記軸穴及び軸の上部末端は円錐台接続（ｔｒｕｎｃａｔｅｄ ｃｏｎｅ ｆｉｔｔｉｎｇ）を含んでいる。
好ましくは、第４の実施形態では、上記軸穴及び軸の上部末端はトラニオン接続（ｔｒｕｎｎｉｏｎ ｆｉｔｔｉｎｇ）を含んでいる。
第２の態様によれば、本発明は、大腿骨キャップ及び軸を含む、股関節を再構築する関節形成術用の大腿骨部品であり、上記大腿骨キャップは骨盤骨の中にセットされるカップと噛み合うように適応され、上記大腿骨キャップは凸状の表面及び凹状の表面を持ち、上記軸は上記大腿骨キャップの凹状の表面に取り付けられ、少なくとも上記軸の一部は溶解され得ることで特徴付けられる。
第３の態様によれば、本発明は、少なくともその一部が可溶性の材料で構成されていることで特徴付けられる軸を含む、ヒトを含む哺乳動物のための球窩関節を再構築する関節形成術用の骨部品から成る。
好ましくは、第１の実施形態では、球窩関節は股関節である。
好ましくは、第２の実施形態では、球窩関節は肩関節である。
好ましくは、１つの実施形態では、軸はマグネシウム合金で構成される。
好ましくは、他の実施形態では、軸はアルミニウム合金又は亜鉛合金で構成される。
好ましくは、他の実施形態では、軸はポリグリコール酸で構成される。
第４の態様によれば、本発明は、少なくともその一部が溶解され得ることで特徴付けられる軸を含む、ヒトを含む哺乳動物のための球窩関節を再構築する関節形成術用の骨部品が記述される。

〔発明の最良の形態〕
図１〜３を参照しながら、ヒトの股関節を再構築する関節形成術用の大腿骨部品１を説明する。大腿骨部品１は凸状の表面４及び凹状の表面５を持つ大腿骨キャップ２を含む。大腿骨キャップ２はカップ３と噛み合うように適応される。大腿骨キャップ２及びカップ３は鋼のような金属で構成される。
大腿骨キャップ２は大腿骨１６の上にセットされるために適応され、さらに患者の骨盤骨１５の中にセットされるカップ３と噛み合うようにも適応される。使用に当たり、この大腿骨キャップ２とカップ３との噛み合わせが球窩関節を構成することが理解できる。
大腿骨部品１は、大腿骨キャップ２に取り付けられるために適応される軸６をさらに含む。軸６は大腿骨キャップ２から除去可能であるように適応される。
大腿骨キャップ２はネジ切りされた雌穴８を含む。軸６はその上部末端にネジ切りされた末端９を有し、それは前記雌穴８と噛み合うように適応されている。これにより大腿骨キャップ２から容易除去される軸６を可能とすることが理解できる。
軸６は、可溶性金属、分解性高分子、又は酸などの「可溶性の（ｓｏｌｕｂｌｅ）」材料で構成される。可溶性金属は、マグネシウム合金、アルミニウム亜鉛合金、又は他の任意の可溶性合金が挙げられる。分解性高分子はトリメチレン炭酸塩共重合体が挙げられる。酸はポリグリコール酸又はポリ乳酸が挙げられる。使用可能なもう１つの可溶性材料は、ヒドロキシアパタイトが挙げられる。さらに、これらを組み合わせて、又は他の吸収可能な材料が使用できることも理解されるに違いない。
用語「可溶性の（ｓｏｌｕｂｌｅ）」は、材料が溶解又は液化され得ることを意味する。適切な材料は、体内への挿入時には、容易な挿入を可能とするため十分に硬くなければならないが、時間が経てば溶解される。
マグネシウム合金を使用する利点は、骨形成、及び骨成長の合成刺激剤を援助し得ることである。これは、大腿骨１６に生じる骨再生が比較的瞬時に起こることを保証するのに役立つ。それゆえ、回復期間及び患者の病院滞在が短くなる可能性がある。
この実施形態では、軸６の可溶性の材料に対する患者の身体の自然処理作用の影響の結果としての溶解処理により、軸６が除去されることが好ましい。
配置上、可溶性の軸６は時間の経過とともに溶解し、大腿骨１６は軸６の溶解により残った空間を満たすように再生することが理解できる。大腿骨１６から軸６を除去することの利点は、治癒するにつれて、骨が保護されたというよりはむしろ自然のストレスを受けるということである。これは、骨が治癒するとともに、骨折や骨の破損の機会が少なくなるという利点を持つ。
使用に当たり、ガイド・ワイヤー（図示せず）は外側面から大腿骨１６を通って挿入される。カニューレが挿入されたドリル（図示せず）は、大腿骨１６の外側面から大腿骨頭部に通じる通路（図示せず）を開けるために利用される。大腿骨部品１は大腿骨頭部に適合され、軸６は穴を空けられた通路からカニューレが挿入されたドリルを押し出す。その後、軸６は通路から除去され、通路は骨再生を刺激するための骨移植片で充填される。
以上は、本発明の好ましい実施形態のただ１つだけを記述したに過ぎず、当業者にとって明白な修正は、本発明の範囲から逸脱することなく、それに対して行うことができる。
軸６と大腿骨キャップ２との間の噛み合わせは、バヨネット接続（ｂａｙｏｎｅｔ ｆｉｔｔｉｎｇ）、円錐台接続（ｔｒｕｎｃａｔｅｄ ｃｏｎｅ ｆｉｔｔｉｎｇ）、及びトラニオン接続（ｔｒｕｎｎｉｏｎ ｆｉｔｔｉｎｇ）を含む、任意のタイプの接続を備え得ることが意図される。
大腿骨キャップ２及びカップ３は鋼で構成されるが、コバルト・クロムのような任意の他の適切な材料が使用されてもよい。
大腿骨部品は股関節を再構築する関節形成術における使用のために記述されているが、本発明はヒトや他の哺乳動物における任意の球窩関節の置換に使用することができる。
上述の実施形態においては、軸６は完全に可溶性であるが、示していない他の実施形態においては、軸６の任意の部分が可溶性の材料で構成されていればよいことが理解されるに違いない。例えば、軸６の上部のネジ切りされた末端は鋼のような材料で構成されていてもよく、一方、残りの部分が可溶性の材料で構成されていてもよい。
軸６の可溶性材料に対する身体の自然処理作用に加えて、軸６の溶解処理のスピードアップを助けるために、患者に施された医薬や治療のような適切な触媒が適用されてもよい。
用語「含む（ｃｏｍｐｒｉｓｉｎｇ）」は、本明細書において「持つ（ｈａｖｉｎｇ）」又は「含む（ｉｎｃｌｕｄｉｎｇ）」の包括的な意味で使用され、「のみからなる（ｃｏｎｓｉｓｔｉｎｇ ｏｎｌｙ ｏｆ）」という排他的な意味で用いられるのではない。

Claims (22)

  1. 大腿骨キャップ、及び軸を含み、
    上記大腿骨キャップは、骨盤骨の中にセットされるカップと噛み合うように適応され、凸状の表面及び凹状の表面を持ち、
    上記軸は、上記大腿骨キャップの凹状の表面に取り付けられ、少なくともその一部が可溶性の材料で構成されることで特徴付けられる、
    股関節を再構築する関節形成術用の大腿骨部品。
  2. 上記大腿骨キャップの凹状の表面は軸穴を含んでいる、請求項１に記載の股関節を再構築する関節形成術用の大腿骨部品。
  3. 上記軸は全部が可溶性の材料で構成される、請求項１に記載の股関節を再構築する関節形成術用の大腿骨部品。
  4. 上記軸はマグネシウム合金で構成される、請求項１〜３のいずれか１項に記載の股関節を再構築する関節形成術用の大腿骨部品。
  5. 上記軸はアルミニウム合金又は亜鉛合金で構成される、請求項１〜３のいずれか１項に記載の股関節を再構築する関節形成術用の大腿骨部品。
  6. 上記軸はポリグリコール酸又はポリ乳酸のような酸で構成される、請求項１〜３のいずれか１項に記載の股関節を再構築する関節形成術用の大腿骨部品。
