Description

The present invention relates to a double-lumen catheter, and more particularly to a slide-type double-lumen catheter in which an inner cylinder is slidable in an outer cylinder while forming a blood removal gap between the inner and outer tubes.

The extracorporeal circulation treatment requires a route for extracting blood and returning blood, so-called blood access. The most common means for ensuring blood access is placement of a catheter in the central vein, and a double lumen catheter having a blood removal cavity and a blood delivery cavity is known.

By the way, blood has a coagulation function when there is no appropriate blood flow or when it is in contact with a foreign substance. Since a catheter is a foreign substance for a living body, if it is placed in blood, a thrombus will be generated.

Therefore, in order to prevent blood clots due to blood clots in the lumen of the venous catheter, so-called heparin lock is performed after the extracorporeal circulation, in which an anticoagulant is injected into the catheter.

However, many of the conventional double lumen catheters are of a partition type in which a partition wall extending in the length direction is formed inside a single tube, and two flow paths of a blood removal cavity and a blood supply cavity are formed by the partition wall. Yes (see FIG. 11 of Patent Document 1). In the double lumen catheter having such a structure, the heparin saline solution is released to the blood vessel side in a short time, and blood may flow into the catheter to cause a thrombus.

Therefore, an enlarged diameter portion is provided at the distal end of the inner cylinder, and when the inner cylinder is retracted into the outer cylinder when not in use, the enlarged diameter section abuts against the inner wall of the outer cylinder and the gap between the outer cylinder and the inner cylinder. There has been proposed a sliding double lumen catheter configured to block the blood flow path to prevent the release of heparin saline and prevent the generation of thrombus (for example, Patent Document 1). The slide-type double lumen catheter has a position fixing tool at the rear part, and this position fixing tool has a diameter-expanded portion projecting from the outer cylinder and opening the blood removal cavity tip between the inner cylinder and the outer cylinder tip. The position of the inner cylinder is fixed with respect to the outer cylinder at a position where the portion is formed and a position where the enlarged diameter portion is accommodated in the outer cylinder and closes the blood removal cavity tip opening.
JP-A-9-253214

By the way, when it is necessary to replace the catheter, in the conventional bulk-type Dalbrumen catheter that is not slidable, for example, as shown in FIG. A blood vessel is secured through a guide wire (not shown) in the indwelling portion of the catheter 10 cut at the location indicated by, and the rear portion 11 is cut. After removing the indwelling portion of the catheter 10, a new catheter is used as the guide wire. Replacement is done through.

On the other hand, in the slide type double lumen catheter, if the rear part of the catheter is cut and the guide wire is passed through as described above, the inner cylinder is free with respect to the outer cylinder. There is a risk that. For this reason, the slide-type double-lumen catheter is marked with a warning that prohibits cutting, and the handling of the product is thoroughly explained. However, when the rear portion of the catheter is not cut, a guide wire is passed from the rear portion of the catheter. In this case, the rear portion needs to be sterilized, which is troublesome.

Therefore, an object of the present invention is to provide a slide type double lumen catheter having a structure in which an inner cylinder does not come out of an outer cylinder even when a rear portion of the catheter is cut.

In order to achieve the above object, a sliding double lumen catheter according to the present invention is an inner tube that is slidably inserted into an outer tube and that defines a flow passage gap between the inner tube and the inner wall of the outer tube. A cylinder, a diameter-enlarged portion that is provided at a tip portion of the inner cylinder and closes the gap for the flow path by drawing the inner cylinder into the outer cylinder, and a locking portion provided on an outer peripheral surface of the inner cylinder A locked portion that is provided on an inner peripheral surface of the outer cylinder and that is locked by the locking portion, and the locking portion is a passage for securing a flow path of the flow path gap. And is configured to be locked to the locked portion of the outer cylinder when the inner cylinder is pushed out of the outer cylinder for a predetermined length.

The locked portion is preferably formed by constricting the tip of the outer cylinder to reduce the inner diameter of the tip of the outer cylinder.

It is preferable that the locking portion protrudes radially on the outer peripheral surface of the inner cylinder.

It is preferable that the locking portion extends along the length direction of the inner cylinder.

In order to achieve the above object, a sliding double lumen catheter according to the present invention is slidably inserted into an outer cylinder and an inner wall of the outer cylinder to define a flow passage gap. And an enlarged diameter portion that is provided at a tip portion of the inner cylinder and closes the flow path gap by drawing the inner cylinder into the outer cylinder. The outer cylinder and the enlarged-diameter portion are connected by a thread-like member so as to limit the extruded length to a predetermined length.

According to the slide type double lumen catheter according to the present invention, the locked portion is locked to the locking portion, thereby preventing the inner tube from coming out of the outer tube.

An embodiment of a sliding double lumen catheter according to the present invention will be described with reference to FIGS.

1 to 4 are longitudinal sectional views showing a main part of a sliding double lumen catheter. In these drawings, the rear portion including the position fixing has the same configuration as the conventional one, and is not shown.

The sliding double lumen catheter 1 includes an outer cylinder 2 and an inner cylinder 3 that is slidably inserted into the outer cylinder 2. Between the inner peripheral wall of the outer cylinder 2 and the outer peripheral wall of the inner cylinder 3, a flow passage gap X for blood flow is defined. The outer cylinder 2 and the inner cylinder 3 are formed by flexible tubes.

The inner cylinder 3 has a diameter-enlarged portion 4 fixed to the tip portion thereof. The enlarged diameter portion 4 is configured to close the flow path gap X between the outer cylinder 2 and the inner cylinder 3 by drawing the inner cylinder 3 into the outer cylinder 2.

A locking portion 5 is provided on the outer peripheral surface of the inner cylinder 3, and a locked portion 6 that is locked by the locking portion 5 is provided on the inner peripheral surface of the outer cylinder 2.

In the example shown in FIGS. 1 to 4, the locked portion 6 is formed by constricting the tip end portion of the outer cylinder 2 and reducing the inner diameter thereof. The inner diameter of the locked portion 6 is set to a dimension that does not hinder the flow of blood removal.

On the other hand, the locking portion 5 includes a fin-like portion that protrudes radially on the outer peripheral surface of the inner cylinder 3 and extends along the length direction of the inner cylinder 3. In the locking portion 5 having such a shape, a passage for securing a flow passage for the flow passage gap X is formed between adjacent fin-like portions. The locking portion 5 is preferably as thin and small as possible in order to reduce blood flow resistance.

The locking portion 5 is provided at a position where the locking portion 5 is locked to the locked portion 6 of the outer cylinder 2 when the inner cylinder 3 is projected from the outer cylinder 2 by a predetermined length. Specifically, the inner cylinder 3 can be pushed out to a position where a blood removal cavity distal end opening is formed between the inner cylinder and the outer cylinder distal end, and the distal end side of the cut position is sufficient. It should be noted that the inner cylinder push-out position for forming the blood removal cavity distal end opening (the flow path gap X distal end opening) between the inner cylinder and the outer cylinder is determined by a position fixing tool (not shown) in the rear portion of the double lumen catheter.

When the extracorporeal circulation treatment is performed, the slide type double lumen catheter having the above configuration operates a position fixing tool (not shown) in the rear portion to push the inner cylinder 3 out of the outer cylinder 2 for a predetermined length. A blood removal cavity tip opening is formed between the distal end of the outer cylinder, blood is removed through the channel gap, and blood is fed through the lumen of the inner cylinder 3 (FIG. 1). At this time, the flow of the blood flow is not hindered because the passage of the blood flow is formed in the lock portion 5 by causing the locking portion 5 to project radially on the outer peripheral surface of the inner cylinder 2. When the treatment is interrupted or stopped, the inner cylinder 3 is drawn into the outer cylinder by the operation of the position fixing tool, and the distal end of the outer cylinder 2 is blocked by the enlarged diameter portion 4 to block the blood removal (FIG. 3). And if necessary, even if the rear portion is cut and separated at a required location (see phantom line C in FIG. 11), the locking portion 5 is locked to the locked portion 6, so that a guide wire is inserted. However, the inner cylinder 3 does not leave the outer cylinder 2 and enter the blood vessel (FIG. 4).

Compared to the conventional non-sliding double lumen catheter that divides blood from one side of the lumen by dividing the lumen into half, the sliding double lumen catheter can remove blood from the entire periphery of the inner tube. The obstruction of the blood removal space due to suction on the inner wall is improved. By adopting a configuration in which the locking portion 5 extends along the length direction of the inner cylinder, a certain resistance is obtained against the bending of the inner cylinder 3 and the outer cylinder 2, and as a result, blood removal due to suction to the inner wall of the blood vessel. The effect of preventing occlusion of the cavity can be further enhanced.

Moreover, the sliding double lumen catheter having the above-described configuration can be manufactured without greatly changing the manufacturing process.

In the above example, the inner cylinder has a form in which the blood feeding cavity also serves as a guide wire passage hole. For example, as shown in FIG. 5, the blood feeding cavity 3a and the guide wire hole 3b are separated in the inner cylinder 3. It is also possible to adopt a form extending in parallel with each other.

In the above example, the blood feeding cavity outlet of the inner cylinder is provided at the tip of the enlarged diameter portion. However, the blood feeding cavity outlet is not limited to the tip of the enlarged diameter portion, and as shown in FIG. The outlet of the blood feeding cavity 3a can be opened on the peripheral surface of the inner cylinder 3 in the vicinity of the diameter portion 4.

Further, as shown in FIG. 6, for example, the locked portion 6 is formed with a longitudinal groove 2 a in which the locking portion 5 is slidably fitted on the inner wall of the outer cylinder 2, and a terminal portion at the tip of the longitudinal groove 2 a. For example, it can be provided at a position away from the distal end of the outer cylinder 2 (a position closer to the distal end than a position where it can be cut).

Further, the locking portion 5 may be replaced with, for example, a hook-shaped metal mesh (not shown) attached to the outer peripheral surface of the inner cylinder 2 instead of the fin-shaped one.

Further, as shown in FIG. 8 and FIG. 9, a cutout portion 2b for expanding the opening area is provided at the opening end of the outer cylinder 2, and the enlarged diameter portion 4 can be formed so as to conform to the shape of the opening end. . Thereby, even if the front end of the outer cylinder 2 is tapered, the opening area of the front end of the outer cylinder 2 is increased, and the amount of inflow of blood removal can be increased.

In still another embodiment, as shown in FIG. 10, the outer cylinder 2 and the enlarged diameter portion 4 are connected by a thread-like member 7 such as nylon teg. As a result, the length of the inner cylinder 3 that is pushed out from the outer cylinder 2 can be limited to a predetermined length. Therefore, the rear portion is cut and cut off at a required location (see the phantom line C in FIG. 11) as necessary. Even if the guide wire is inserted, the inner cylinder 3 does not leave the outer cylinder 2 and enter the blood vessel.

The preferred embodiments of the present invention have been described above. However, the present invention is not limited to the above-described embodiments, and various modifications can be made without departing from the scope defined by the claims.
