Description

The present invention relates to a biological tissue collection device that collects biological tissue in the body.

Japanese Laid-Open Patent Publication No. 2001-70307 (Patent Document 1) discloses a biological tissue collecting device that is deeply inserted into a body organ and collects a deep tissue for a biological examination or the like. This biological tissue collection device has a mantle pipe inserted into a sheath member, and a puncture member is inserted and arranged in the mantle pipe, and the distal edge of the mantle pipe is used as an outer blade, and the puncture member is connected to the outer blade. The inner blades facing each other and the accommodating recess for taking in the biological tissue are provided.

When using this biological tissue collection device, puncture the tip portion of the internal organ, puncture the puncture member from the mantle pipe, and pull the puncture member back with the puncture member. Is cut out and taken into the housing recess.
JP 2001-70307 A

The conventional biological tissue collecting device cuts the biological tissue that has entered the receiving recess by protruding the puncture member from the outer pipe and pulling back the puncture member with the outer blade and the inner blade while the biological tissue is punctured. Collect. That is, the living tissue that has entered the housing recess is cut by the outer blade and the inner blade. For this reason, if the living tissue does not sufficiently enter the housing recess, the living tissue easily escapes from the housing recess when cutting with the outer blade and the inner blade. In addition, when an organ is punctured, the living tissue does not easily enter the housing recess, and the amount of collected living tissue tends to be insufficient. It may happen that it cannot be collected in one operation.

The present invention has been made paying attention to the above-mentioned problems, and an object of the present invention is to provide a biological tissue collection device that can reliably collect biological tissue.

The present invention includes an insertion portion to be inserted into a body, a gripping member that is disposed at a distal end of the insertion portion, can be radially expanded, and can grip a living tissue to be cut, and the gripping member is radially expanded. A first operating means operable in a contracted state for gripping a living tissue to be cut, and a puncture portion provided at a distal end side portion and a grasping portion at a proximal end portion A biological tissue collecting apparatus comprising: a cutting member that is movable forward and backward in which a blade for cutting a biological tissue grasped by the member is formed; and a second operation unit that advances and retracts the cutting member. .
Then, by extending the gripping member radially, the living tissue around the puncture unit can be gripped, and after the gripping member grips the biological tissue, the cutting member is moved to the proximal side to The biological tissue is cut and collected with a blade.

According to the present invention, a living body tissue in a body can be collected easily and reliably.

<First Embodiment>
A biological tissue collection device according to a first embodiment of the present invention will be described with reference to FIGS.

As shown in FIG. 1, the biological tissue collection device 1 according to the present embodiment includes an elongated sheath 2 as a flexible insertion portion, an annular tip 3 arranged and fixed at the distal end of the sheath 2, A cutting member 4 that is disposed on the distal side of the tip tip 3 so as to face the tip tip 3 and that can move forward and backward in the longitudinal axis direction of the sheath 2, and a retractable member that protrudes and retracts into the cutting member 4. In addition, the gripping portion 5 that advances and retreats independently of the cutting member 4 and naturally expands in a free state, and is operated at the proximal end (proximal end) of the sheath 2 to operate the cutting member 4 and the gripping portion 5. And an operation unit 6 including an operation means for performing the operation.

As shown in FIG. 2, the cutting member 4 is formed by a cylindrical member whose tip is closed, and forms a storage portion 7 for storing a biological tissue collected by the internal space. Further, the outer diameter of the cutting member 4 is formed to be substantially the same as the outer diameter of the sheath 2.

The outer peripheral portion of the distal end of the cutting member 4 is formed in a tapered conical shape, and the puncture portion 8 whose tip is a pointed portion 8a is formed by this conical portion. Further, a cutting edge-shaped blade 9 for cutting a biological tissue to be collected is formed on the entire periphery of the proximal end (proximal end) of the cutting member 4. Further, the distal end of the operation wire 10 extending through the internal space of the cutting member 4 is connected to the closed distal end portion of the cutting member 4. This operating wire 10 constitutes operating means for operating the cutting member 4 to advance and retract in the axial direction of the sheath 2.

As shown in FIG. 2, a control operation member 11 having the grip portion 5 connected to the tip is inserted into the sheath 2. The control operation member 11 is formed of a hollow flexible member formed in a sheath shape by, for example, a metal close-wound coil. A cylindrical fixing member 12 for holding the grip portion 5 is fixedly attached to the tip of the control operation member 11. The control member 11 constitutes an operating means for operating the grip portion 5 to advance and retract in the axial direction of the sheath 2. The operation wire 10 is inserted into the control operation member 11. The control operation member 11 and the operation wire 10 can be operated independently of each other.

As shown in FIG. 2, the grip portion 5 has a plurality of arms (members) 13. Each arm 13 is made of, for example, an elastic metal wire. By fixing the base portion of each arm 13 to the fixing member 12, the tip side portion of each arm 13 is radially outwardly spread outward. Has expandability. In the free state shown in FIG. 2, each arm 13 extends outward obliquely rearward (proximal direction) and extends outward beyond the outer peripheral positions of the sheath 2 and the cutting member 4.

A claw 13a facing inward is provided at the tip of each arm 13, and each claw 13a is caught by a living tissue and does not slip. Therefore, the biological tissue can be reliably grasped by the arm 13.

Next, with reference to FIG. 3, the operation part 6 which operates the said cutting member 4 and the said holding part 5 is demonstrated. The operation unit 6 includes a grip 15 connected to the sheath 2, a slider 16 connected to the control operation member 11, and a knob 17 connected to the operation wire 10.

The slider 16 includes an operation knob 18 for a user to operate, and a fixing member 19 that connects the operation knob 18 to the control operation member 11.

The grip 15 has a lumen 20 along the axial direction, and the fixing member 19 is movably inserted into the lumen 20. In addition, the grip 15 is provided with a plurality of grooves 21, 22, and 23 for fixing the operation knob 18 at predetermined positions so as to be shifted in the front-rear direction. Then, by selecting one of the plurality of grooves 21, 22, and 23 and locking the operation knob 18, the position relative to the sheath 2 can be selected and the grip portion 5 can be fixed.

As shown in FIG. 3, a fixing member 25 is provided in the middle of the operation wire 10 protruding from the fixing member 19. The fixing member 25 is provided with a large-diameter convex portion 26. Further, the lumen 20 of the grip 15 in the operation unit 6 is formed with a recess 27 for fitting and locking the protrusion 26 of the fixing member 25. The operation wire 10 can be fixed at a predetermined position in the advancing / retreating direction by engaging the convex portion 26 of the fixing member 25 with the concave portion 27.

Hereinafter, the operation when using the biological tissue collection device 1 will be described. First, as shown in FIG. 4, the gripping part 5 is housed in the cutting member 4, and then the cutting member 4 is pulled to the proximal end side as shown in FIG. 5 until the cutting member 4 hits the distal tip 3. Retreat. At this time, as shown in FIG. 3, the slider 16 and the knob 17 of the operation unit 6 are in a retracted position state. Moreover, the outer periphery of the sheath 2 and the cutting member 4 will be in a substantially continuous state.

Next, it introduce | transduces in a body cavity through the channel of the endoscope which is not shown in the state shown in FIG. In the case of laparoscopic surgery, the endoscope channel can be introduced through other surgical instruments without directly using the endoscope channel.

And the puncture part 8 is protruded from the channel front-end | tip of an endoscope, and the puncture part 8 is punctured to the target site. In this puncture state, as shown in FIG. 6, the slider 16 is pushed toward the distal end, and the slider 16 is fitted and locked in the most advanced groove 21 to fix the cutting member 4. Then, the fixing member 25 of the operation wire 10 is fitted into the recess 27 of the grip 15, and the operation wire 10 is automatically fixed to the grip 15. At this time, as shown in FIG. 4, the distal end portion of the biological tissue collection device 1 is in a state of being pushed forward from the distal end tip 3 of the sheath 2 with the cutting member 4 retracting the grip portion 5.

Next, as shown in FIG. 7, without operating the knob 17, the operation knob 18 is operated to pull the slider 16 back to the proximal end side, the operation knob 18 is engaged with the groove 22, and the control operation member 11. To fix. Then, as shown in FIG. 2, the arm 13 of the gripper 5 is expanded in a radially expanding state. For this reason, each arm 13 penetrates into the living tissue and pierces and holds the living tissue.

Next, with the slider 16 fixed to the grip 15, the knob 17 is pulled toward the proximal end, and only the puncture portion 8 is slightly pulled toward the proximal side. Then, the living tissue grasped by the arm 13 is cut by the blade 9 of the cutting member 4, and can be cut out and collected in a state where the living tissue is stored in the storage unit 7. At this time, each arm 13 contracts by hitting the peripheral edge of the proximal end (proximal end) of the cutting member 4, but keeps grasping the living tissue, so that the living tissue is not missed. In particular, when there is a nail 13a having a reversed shape, the living tissue to be collected is not missed. In this manner, the living tissue is grasped by the arm 13 of the grasping unit 5 and cut off, so that the living tissue can be reliably collected.

Thereafter, the slider 16 is released from the grip 15, and the slider 16 is retracted, whereby the living tissue is stored in the storage portion 7 and pulled out of the body as shown in FIG. For this reason, the collected biological tissue can be reliably recovered.

<Second Embodiment>
Based on FIG. 9, a biological tissue collection apparatus for an endoscope according to a second embodiment of the present invention will be described. In the present embodiment, the number of arms 13 of the grip portion 5 in the first embodiment described above is increased from two to three to four. The effect | action by this is the same as that of the case of 1st Embodiment mentioned above.

In the present embodiment, by increasing the number of arms 13, the force for grasping the living tissue is increased, the grasping stability is improved, and the living tissue can be reliably collected.

<Third Embodiment>
Based on FIG. 10, a biological tissue collecting apparatus for an endoscope according to a third embodiment of the present invention will be described. In this embodiment, the main body portion of the fixing member 12 of the grip portion 5 and the plurality of arms 13 in the first embodiment described above are integrally formed. The operations and effects of this embodiment are the same as those of the first embodiment described above. In addition, by forming the arm 13 integrally with the fixing member 12 of the grip portion 5, the manufacture is easy and the assemblability can be improved, and the cost is further reduced.
