Abstract
Description
The present invention relates to a medical guide wire for guiding a catheter used in treatment or examination of a necessary part of a human body.
A guide wire for a catheter is used as a guide for introducing a catheter when performing treatment or inspection of a site where surgical operation is difficult or treatment with minimal invasiveness. A general guide wire operation method is that a doctor or the like gives a slight bending heel to the tip in advance (reshaping) and then accommodates it in the catheter and protrudes several mm to several cm from the tip of the catheter. A doctor applies a slight torsional force from the proximal end side of the wire and selectively passes through the branching portion to insert it into a target site such as a blood vessel. Therefore, it is necessary for the guide wire to reliably transmit the torque (torque transmission) in response to the twisting operation of the proximal end portion, and to pass through the meandering lumen portion or narrowed portion of the blood vessel or the like. (Passability), it is necessary to selectively enter the branch point in a desired direction (selectivity, pushability).
At present, as a medical guide wire, a stainless steel or piano wire metal wire is used as an inner core and a coil is wound around the end of the wire and a super elastic shape memory alloy is used. They are roughly classified into those used as cores and coated with synthetic resin.
However, if a material such as a stainless steel guide wire that has a strong back and can be pushed in and is easily plastically deformed is used for the inner core, the blood vessel wall may be damaged in the meandering lumen or stenosis, or the meandering blood vessel may be damaged. There is a concern that it will expand. Another problem is that the guide wire that is plastically deformed at the time of removal damages the blood vessel wall. In addition, stainless steel guidewires have long been regarded as problematic because of their extremely poor transmission of torsional forces in meandering blood vessels and poor selectivity and operability at bifurcations. On the other hand, a superelastic alloy guide wire has a problem that it cannot be reshaped but has a low bending resistance in a bent state although it has a high torsional force transmission in a meandering blood vessel. Accordingly, there has been a demand for a reshapable guidewire that solves the above-described problems, and that can be easily and safely performed by an operator, yet has high elasticity and moderate waist strength.
SUMMARY OF THE INVENTION An object of the present invention is to provide a guide wire having flexibility that does not damage a blood vessel wall while having an appropriate waist strength. It is another object of the present invention to provide a guide wire with excellent torque transmission property in which a distal end portion surely responds to a twisting force of a proximal portion even in a curved blood vessel.
In light of the above-mentioned problems, the present inventors have conducted extensive research and as a result, the present invention is achieved by the following.

Furthermore, by performing a treatment for reducing the sliding resistance on the surface of the covering portion as necessary, it is possible to exhibit excellent performance in reachability to a target site and passage through a constricted portion.
Hereinafter, preferred embodiments of the present invention will be described with reference to the drawings.

The medical guide wire 1 has a length of 150 cm, but various lengths of 10 cm to 400 cm can be applied depending on the type. The outer diameter is 0.89 mm (0.035 inch), but can be set to 0.2 to 2.0 mm depending on the type. The outer diameter is substantially the same from the distal end to the proximal end, but the distal end may be made thinner.

The titanium alloy of the

In addition, as a titanium alloy containing a β phase, the metastable βTi alloy described above may be used as long as the guide wire has suitable properties such as tensile strength, longitudinal elastic modulus, rigidity, 0.2% proof stress or bending elastic modulus.

And it is preferable that it is a core material which has two or more of each characteristic.

For example, 110 kgf / mm 2 or more 200 kgf / mm 2 or less in tensile strength or 5,000 kgf / mm 2 or more 12,000kgf / mm 2 or less longitudinal elastic modulus and the predetermined roundness of titanium alloy, the tensile strength or modulus of longitudinal elasticity comprises the numerical range It may be a titanium alloy having a stiffness and a rigidity having the above numerical range, or a titanium alloy having a rigidity having a predetermined roundness of the cross section and the above numerical range.

When the core material of the guide wire is formed of a titanium alloy containing a β phase as in the present invention, it is difficult to kink, and both the tenacity that is difficult to plastically deform and high torque transmission properties can be obtained. The guide wire can be inserted into the guide, and the catheter can be inserted into the outer surface of the guide wire reliably and smoothly. In other words, the tip portion can be a guide wire with high operability that can respond to the torsional force of the hand portion almost certainly while maintaining high strength and high elasticity. In addition, the tip portion can be shaped by reshaping as necessary, and since the core material has an appropriate waist strength, the pushability at the narrowed portion is good and the selectivity at the branching portion is also high. On the other hand, by selecting a high-elasticity and high-strength titanium alloy containing a high-contrast metal such as Sn or Mo, it is possible to select a conventional super-elastic alloy such as Ni-Ti wire or stainless steel such as SUS. In addition, it is possible to express a high contrast property, and there is an advantage that an operator can confirm an entire image of the guide wire, thereby enabling an accurate operation.
As described above, the medical guide wire has been described. However, the titanium alloy that has both the tenacity and the high torque transmission property that are difficult to kink and plastically deform can be used for various medical devices. For example, it can also be used as a core material for a PTCA catheter equipped with a balloon, that is, a so-called on-the-wire catheter core material. This catheter is a kind of PTCA catheter that uses a portion ahead of the balloon as a guide wire. Since there is no guide wire lumen like the over-the-wire type catheter described later, the outer diameter of the balloon portion is small. Can be small. This catheter is inserted in a state where the core material protrudes into the tube by a predetermined length, and the balloon is fixed to the tube tip and the middle of the protruding portion of the core material. Further, the pushability can be improved by providing a core material on the lumen or wall surface of the over-the-wire catheter. Furthermore, the present invention can also be applied to a core material of a device that removes a calcified stenotic lesion by scraping with a rotating body.
Examples of core materials applicable to the present invention will be described below, but the embodiments are not limited to the following.
(Example 1) Using a hot rolled material having an outer diameter of 9.0 mm made of 15 wt% vanadium, 3 wt% tin, 3 wt% chromium, 3 wt% aluminum, and the remaining titanium, the surface of the wire was first peeled off. A wire with an outer diameter of 8.3 mm was produced. Next, roll drawing was performed to obtain an outer diameter of 2.0 mm, and solution treatment was performed by line annealing at 800 to 810 ° C. for 10 minutes. Subsequently, dry drawing was performed by die drawing to obtain a wire rod having an outer diameter of 0.48 mm. The obtained wire was finally finished by aging treatment at 500 ° C. for 4 hours.
Table 1 shows the mechanical properties of the Ti-15V-3Sn-3Cr-3Al alloy wire produced as described above.
In addition, the test method and calculation method of each numerical value are as follows.
Tensile strength, longitudinal elastic modulus, and 0.2% proof stress were calculated in accordance with a metal material tensile test method (JIS Z2241) at room temperature at a tensile speed of 5 mm / min and a distance between chucks of 50 mm.
The rigidity is measured by using a fine torsion tester manufactured by Takachiho Seiki Co., Ltd. at room temperature with a twisting speed of 10 times / minute and a gauge distance of 25 mm. The value of the obtained torque is expressed as rigidity G = 32LT. 1 / πd 4 θ (L: gauge distance, T 1 /: torque value at θ, d: wire diameter, θ: angle, calculated at 90 ° = 1.57) and calculated.
The roundness of the cross section of the wire rod was measured according to JIS B0419 by arbitrarily measuring five points in the length direction of the portion excluding the thinned portion.
The bending elastic modulus was measured by performing a three-point bending test. Specifically, a sample was placed on a three-point bending jig with a fulcrum of 30 mm (sample length: 40 mm), and the center was pushed in at 2 mm accurately at room temperature at a speed of 5 mm / min, and the flexural modulus was obtained from the load at that time. The test method and calculation of numerical values were in accordance with JIS K7203. The value of the moment of inertia I is obtained by substituting the [pi] d 4/64 for a circular cross-section (d: wire diameter). The following examples were obtained in the same manner.
(Example 2) A hot rolled material having an outer diameter of 9.0 mm made of 15 wt% molybdenum, 5 wt% zirconium, 3 wt% aluminum, and the remaining titanium was used. First, the surface of the wire was peeled to obtain an outer diameter of 8.3 mm. A wire was prepared.

(Example 3) Using a hot rolled material having an outer diameter of 9.0 mm composed of 22 wt% vanadium, 4 wt% aluminum, and the remaining titanium, the surface of the wire was first peeled to produce a wire having an outer diameter of 8.3 mm. . Next, roll drawing was performed to obtain an outer diameter of 2.0 mm, and solution treatment was performed by line annealing at 800 to 810 ° C. for 10 minutes.
