Abstract
Description
The present invention relates to a medical guide wire for guiding a medical instrument such as a catheter used for treatment or examination to a predetermined position in a body cavity such as a blood vessel, and more specifically, even if the diameter is reduced. The present invention relates to a medical guide wire that has a good torque transmission property, has a high bending elasticity (strong waist), does not easily cause permanent distortion even when bent, and is easy to insert.
In some cases, it may be necessary to insert the catheter into place in the blood vessel for treatment or examination. The catheter is generally excellent in flexibility, and it is difficult to push the catheter into a predetermined position inside the blood vessel by itself. Therefore, it is common practice to insert a guide wire into the blood vessel in advance and guide the catheter to a predetermined position in the blood vessel along the guide wire.
The main requirement for guidewires is that they have sufficient pushing characteristics to push them through the patient's vasculature or other lumens of the body without kinking them. However, they also need to be sufficiently flexible so as not to damage the blood vessels through them or other lumens of the body.
Efforts have been made to achieve both the push-in characteristics and flexibility of the guide wire and make them more suitable for use (see Patent Document 1 and Patent Document 2). However, these two properties are, in many parts, the exact opposite of each other, making one larger makes the other smaller.
Conventional guidewires for PTCA and other vascular procedures are typically placed around an elongated wire core having one or more tapered sections near its distal end and a distal portion of the wire core A flexible coil spring. By setting it as such a structure, it aims at coexistence of sufficient indentation characteristic and the flexibility in a distal end part.
Conventionally, the wire core wire and the coil spring are made of stainless steel, and the elastic elongation is about 0.9%. Therefore, the tip portion is not sufficiently flexible, and is inserted into a blood vessel having a strong bend of R = 20 mm or less. In many cases, permanent deformation occurs and subsequent operation in the blood vessel becomes difficult.
It has also been proposed to use a superelastic alloy such as nickel / titanium, which exhibits superelastic properties, for the guide wire. When a load is applied, a property that undergoes a relatively large deformation with a constant stress, but recovers from the deformation when the load is removed is usually called “superelasticity”.
However, if the entire guide wire is made of a superelastic alloy, the elastic modulus of the superelastic alloy is relatively low, so it becomes easy to bend to the proximal end (base end) of the guidewire, and the guidewire is pushed in. There is a problem with the characteristics.
In addition, a guide wire in which the base end portion of the guide wire is made of stainless steel and the tip portion is made of a superelastic alloy has been proposed, but it is difficult to join the stainless metal and the superelastic alloy, and the joining is insufficient. The strength of the joint becomes a problem. Further, the rigidity changes abruptly before and after the joint, which often makes wire manipulation difficult.
The present invention has been made in view of such a situation, and the object thereof is to provide good torque transmission even when the diameter is reduced, and has high bending elasticity (strong waist) and permanent deformation even when bent. It is an object of the present invention to provide a medical guide wire that does not easily occur and is easy to insert.
In order to achieve the above object, a medical guide wire according to the present invention comprises:
A medical guide wire having a wire core wire having a distal end side small diameter portion and a proximal end side large diameter portion having a relatively larger outer diameter than the distal end side small diameter portion,
The wire core wire is composed of a carbon steel material containing iron as a main component and containing 0.3 to 1.5% by weight of carbon,
A heat treatment for improving the spring property is performed on at least a part of the wire core wire.
The heat treatment for improving the spring property is not particularly limited, and is any one of austempering, ausforming, aus roll, and patenting, and preferably austempering.
The condition for the heat treatment for improving the spring property is not particularly limited. For example, the longitudinal elastic modulus of the part before processing is smaller than 200 GPa, whereas the longitudinal elastic modulus of the processed part is 200 GPa or more. It is such a condition. Alternatively, the conditions are such that the longitudinal elastic modulus of the portion before the treatment is 100 and the longitudinal elastic modulus after the treatment is 200 to 300.
The maximum outer diameter of the wire core is not particularly limited, but is preferably 0.350 mm or less, and more preferably 0.260 mm or less.
In the present invention, preferably, the distal end side small diameter portion is not subjected to the spring property improving heat treatment, and the proximal end side large diameter portion is subjected to the spring property improving heat treatment.
The inventors of the present invention, even if the wire core wire is made of a carbon steel material and is reduced in diameter by applying a specific heat treatment, torque transmission (the rotational torque on the proximal end side is the distal end side). In order to complete the present invention, it has been found that a medical guide wire can be provided that is easy to insert, and has a high bending elasticity (high stiffness), is hardly deformed even when bent, and is easy to insert. It came.
Since the guide wire of the present invention has high spring properties due to heat treatment, even if the diameter is reduced, the torque transmission is good and the bending elasticity is high. Therefore, when inserted into a body lumen such as a blood vessel, the tip of the wire is directed to a desired branching direction of the blood vessel while rotating the wire around the axis, and the wire is advanced without buckling. be able to. In addition, since the guide wire of the present invention is unlikely to be permanently deformed even when it is bent, there is no possibility of causing permanent deformation even when the guide wire passes through a very tortuous passage such as a coronary artery system.
Furthermore, in this invention, since it is not the structure which connects dissimilar metals, the intensity | strength of a connection part does not become a problem. Moreover, since the heat treatment of the wire core wire can be performed relatively easily, the productivity is high and the guide wire can be manufactured at low cost.

First embodiment As shown in Fig. 1, a
The
The carbon steel material constituting the
In the present embodiment, the
In the austempering treatment, the steel material is heated at a temperature of 800 to 900 ° C. in a non-oxidizing atmosphere to stabilize the austenite, and then the isothermal transformation is caused in a salt bath of about 300 to 400 ° C. to form a bainite structure. How to get. By performing this treatment, the core wire can be prevented from being distorted or cracked, and can be provided with toughness (including tensile strength and elongation) and wear resistance. Also, this treatment has less heat treatment distortion.
The ausforming process is a method in which an external force is applied to a steel material in the austenite state. In other words, steel that has both high strength and toughness that cannot be obtained by ordinary quenching and tempering by adding plastic working before austenite undergoes martensitic transformation and then martensite. can get. Specifically, the steel material of stable austenite heated at a temperature of 800 to 900 ° C. is rapidly cooled to a temperature of 450 to 550 ° C., and plastic working (for example, drawing) is performed in a metastable state of austenite, Get a bainite texture.
The aus roll process is the same concept as the aus forming process, but the bainite transformation is promoted more than the aus forming process. Specifically, the steel material of stable austenite heated at a temperature of 800 to 900 ° C. is rapidly cooled to a temperature of 450 to 550 ° C., and plastic working (for example, drawing) is performed in a metastable state of austenite, By maintaining the temperature, the bainite transformation is promoted.
The patenting process is a general heat treatment method used for manufacturing a piano wire, and is a processing operation close to an austempering process. Specifically, a constant temperature transformation (a bainite structure is obtained) is caused at 400 to 500 ° C., followed by cold drawing, and then the constant temperature transformation and cold drawing are repeated.
By such heat treatment, for example, the longitudinal elastic modulus of the part before processing is smaller than 200 GPa, whereas the longitudinal elastic modulus of the processed part becomes 200 GPa or more. Or the longitudinal elastic modulus of the part before processing can be set to 100, and the longitudinal elastic modulus after processing can be set to 200-300. In particular, by adopting the austempering treatment, a preferable wire core wire can be realized as a medical guide wire because permanent deformation hardly occurs and insertion property is improved.

As a result, as shown in FIG. 2, for example, when the

The present invention is not limited to the above-described embodiment, and can be variously modified within the scope of the present invention.
Hereinafter, although this invention is demonstrated based on a more detailed Example, this invention is not limited to these Examples.

Evaluation As shown in Fig. 6, Table 1, and Table 2, the guide wires of Examples 1 and 2 have better torque transmission characteristics than Reference Examples 1-1 to 2-3. It was confirmed that the indentation characteristics were excellent and the permanent set was small. In particular, as shown in Example 2, it was confirmed that even when the outer diameter of the wire core wire was reduced, the torque transmission characteristics were good, the pushing characteristics were excellent, and the permanent distortion was small.
Due to this characteristic, the guide wire of the present invention can be advanced without buckling in the middle even when it is inserted into a thin and strong blood vessel such as a cerebral blood vessel. It was confirmed that it was possible to communicate well to the distal end.
