BACKGROUND OF THE INVENTION [0002] Glaucoma is an important public health problem because it is a major cause of blindness. Blindness due to glaucoma includes blindness in both central and peripheral vision, which greatly affects the functions of individuals living independently.
TECHNICAL FIELD Glaucoma is an optic neuropathy (optic nerve disorder) that usually occurs in an environment of elevated intraocular pressure. As the pressure inside the eye increases, it is related to changes in the appearance (“depression”) and function (“blind spot” in the visual field) of the optic nerve. If the pressure remains high for a sufficiently long time, complete blindness occurs. High pressure proceeds in the eye due to internal fluid imbalance.
The eye is a hollow structure that contains a clean fluid called “aqueous humor”. This aqueous humor is made at a rate of about 2.5 microliters per minute by the ciliary body in the posterior chamber of the eye. This fluid, made at a roughly constant rate, passes around the lens and enters the anterior chamber of the eye through the pupil opening in the iris. Upon entering the anterior chamber, this fluid flows out of the eye through two different paths. In the “uveal scleral” pathway, this fluid penetrates between the muscle fibers of the ciliary body. This pathway accounts for approximately 10 percent of human aqueous outflow. The main pathway of human aqueous humor outflow is the “tubule” pathway, which includes the trabecular meshwork and Schlemm's canal.
The trabecular meshwork and Schlemm's canal are located at the junction of the iris and sclera. This junction or corner is called the “corner angle”. The trabecular meshwork is a wedge-shaped structure that extends around the periphery of the eye. The trabecular meshwork is composed of collagen beams arranged in a three-dimensional sieve structure. These beams are aligned with a single layer of cells called trabecular cells. The space between the collagen beams is filled with extracellular material made by trabecular cells. These cells also create enzymes that break down extracellular material. Schlemm's canal is adjacent to the trabecular meshwork. The outer wall of the trabecular mesh overlaps the inner wall of Schlemm's canal. Schlemm's canal is a tubular structure that extends around the periphery of the cornea. In adults, Schlemm's canal is thought to be divided into a set of autonomous dead ends by a septum.
Aqueous humor flows through the space between the trabeculae, passes through the inner wall of the Schlemm's canal, enters the Schlemm's canal, passes through a set of approximately 25 collecting channels that flow out of the Schlemm's canal, and the sclera Enter the superior venous system. Under normal circumstances, the production of aqueous humor is equal to the outflow of aqueous humor, and the intraocular pressure is maintained almost constant in the range of 15 to 21 mmHg. In glaucoma, resistance in the tubule outflow system is abnormally high.
In primary open-angle glaucoma, the most common form of glaucoma, the abnormal resistance is thought to occur between the outer surface of the trabecular meshwork and the inner wall of Schlemm's canal. Abnormal metabolism of trabecular cells causes excessive accumulation of extracellular material in this region or abnormally “hard” material. Primary open-angle glaucoma accounts for approximately 85 percent of all glaucoma. Other forms of glaucoma (such as closed angle glaucoma and secondary glaucoma) also include a decrease in efflux through the tubule pathway, but increased resistance may be due to mechanical occlusion, inflammatory debris, cell occlusion, etc. Due to other causes like.
As resistance increases, aqueous humor cannot be drained fast enough and is stored. When aqueous humor is stored, intraocular pressure (IOP) within the eye increases. Increasing intraocular pressure compresses the optic nerve axons and impairs vascular recruitment to the optic nerve. The optic nerve transmits images from the eye to the brain. Some of the optic nerve is more susceptible to IOP than other eye components. Although several studies have been conducted to protect the optic nerve from elevated pressure, the only therapeutic approach currently available for glaucoma is to reduce intraocular pressure.
Clinical treatment of glaucoma is being addressed step by step. Medication is often the first treatment option. When administered either locally or orally, these medications either reduce aqueous humor production or increase spillage. Currently available drugs are accompanied by a number of serious side effects including congestive heart failure, dyspnea, hypertension, depression, kidney stones, aplastic anemia, sexual dysfunction and death. Medication compliance is an important issue because more than half of glaucoma patients do not follow the correct dosing schedule.
Laser trabeculoplasty is often performed when intraocular pressure is not reduced properly by medication. In laser trabeculoplasty, thermal energy from a laser is applied to several non-adjacent points in the trabecular meshwork. It is considered that such laser energy stimulates the metabolism of trabecular cells in some way and changes extracellular substances in the trabecular meshwork. In approximately 80 percent of patients, aqueous humor outflow is promoted and IOP is reduced. However, the effect does not last very long and 50 percent of patients show elevated intraocular pressure within 5 years. This laser surgery cannot usually be repeated. In addition, laser trabeculoplasty is not an effective treatment for primary open-angle glaucoma in patients younger than 50 years, nor is it an effective treatment for closed-angle glaucoma and many secondary glaucoma. Filtration surgery is performed when the intraocular pressure is not reduced sufficiently by laser trabeculoplasty. In filtration surgery, a hole is made in the sclera / corner area. This hole allows aqueous humor to flow out of the eye via another route.
The most commonly performed filtration procedure is trabeculectomy (trabeculectomy). In trabeculectomy, a posterior incision is made in the conjunctiva, which is a transparent tissue covering the sclera. This conjunctiva is turned forward, and the sclera is exposed at the corneal limbus. A partially thick scleral flap is created and cut into half the thickness and placed into the cornea. The anterior chamber is placed below the scleral flap and the deep scleral part, and the trabecular meshwork is excised. The scleral flap is sutured loosely in place. The conjunctival resection is tightly closed. After the operation, the aqueous humor passes through a hole below the scleral flap and collects in a high space below the conjunctiva. This fluid is then absorbed through the blood vessels in the conjunctiva or enters the tear film through the conjunctiva.
Travelectomy has many problems. Fibroblasts present in the superior sclera can proliferate and migrate, leaving scars on the scleral flap. Failure due to scarring may occur especially in children and young adults. Eighty percent of eyes with successful first trabeculectomy will cause scarring failure during the 3-5 years after surgery. In order to minimize fibroblasts, surgeons are currently administering antifibrotic agents such as mitomycin (MMC) and 5-fluorouracil (5-FU) to the scleral flap at the time of surgery. The use of these agents has increased the success rate of trabeculectomy, but has increased the prevalence of hypotony. Hypotension is problematic in that it develops when aqueous humor flows out of the eye too quickly. If the intraocular pressure is too low (usually less than 6.0 mmHg), the eye structure is deflated and visual acuity decreases.
Travelectomy creates a passage for draining aqueous humor to the surface of the eye. At the same time, trabeculectomy also creates a passage for bacteria that normally live on the surface of the eyeball or eyelid to enter the eye. If this happens, an intraocular infection called endophthalmitis may occur. Endophthalmitis often causes permanent and severe blindness. Endophthalmitis can occur at any time after trabeculectomy. The fear increases with the presence of thin vesicles that occur after administration of MMC or 5-FU. Another factor that contributes to infection is the vesicular sequence. Eyes with poor trabeculectomy are about five times as likely to have eye infections as eyes with fine vesicles. Thus, the initial trabeculectomy is performed with superior techniques under the eyelid in either the nasal quadrant or the temple quadrant.
In addition to scarring, hypotony and infection, there are several other trabeculectomy complications. Small vesicles may break and cause severe ocular hypotension. Small vesicles can be stimulated and the normal tear film can rupture, causing blurred vision. Patients with vesicles are generally unable to wear contact lenses. All complications of trabeculectomy stem from the fact that aqueous humor is diverted from the inside of the eye to the outside surface of the eye.
When intraocular pressure does not decrease successfully due to trabeculectomy, the next surgical stage is often the aqueous humor short circuit device. Prior art aqueous humor diversion devices are silicone tubes attached to one end of a plastic (polypropylene or other synthetic resin) plate. In the aqueous humor short-circuit device, an incision is formed in the conjunctiva to expose the sclera. This plastic plate is sewn behind the surface of the eye, usually over the equator. A sufficiently deep hole is formed in the eye by a corneal ring, usually by a needle. Through this hole, the tube is inserted into the eye. The outer portion of the tube is covered with either the donor sclera or pericardium. The conjunctiva is replaced and the incision is tightly closed.
In the prior art aqueous humor bypass device, aqueous humor is drained through the silicone tube to the surface of the eye. This aqueous humor is then absorbed by the thicker orbital tissue. The outer end of the tube is protected from fibroblasts and scar formation by the plastic plate. A number of complications are associated with the aqueous humor diversion device. The thick wall of scar tissue that occurs around the plastic plate provides some resistance to outflow and, in many eyes, limits the reduction in intraocular pressure. In some eyes, ocular hypertension occurs because the flow through the tube is not restricted. Many physicians tie an absorbable suture around the tube and wait for the suture to dissolve after surgery once sufficient scar tissue has been successfully formed around the plastic plate. Some devices include pressure sensitive valves inside the tube, but these valves may not function properly. Surgery includes surgery in the posterior orbit, and many patients suffer from eye muscle imbalance and double vision after surgery. In prior art aqueous humor short-circuit devices, bacteria can potentially enter the eye and cause endophthalmitis.
Prior art includes U.S. Pat. No. 4,936,825 (providing a tubular shunt from the anterior chamber to the corneal surface for the treatment of glaucoma), U.S. Pat. No. 5,127,901 (from the anterior chamber to the conjunctiva). U.S. Pat. No. 5,180,362 (teaches helical steel implants arranged to provide outflow from the anterior chamber to the subconjunctival space), and Several aqueous humor short circuit devices are included, such as US Pat. No. 5,433,701 (which widely teaches a short circuit from the anterior chamber to the corneal or subconjunctival space).
In addition to these conventional aqueous humor short-circuit devices, other prior art devices for glaucoma surgery use skewers or other porous materials to divert excess aqueous humor from the anterior chamber to the outer ocular surface. A wick-shaped component was used. Examples include U.S. Pat. No. 4,634,418 and U.S. Pat. No. 4,787,885 (teaching surgical treatment of glaucoma using an implant composed of a triangular skewer (light core)), and U.S. Pat. No. 4,946,436, which teaches the use of a porous device to short the anterior chamber to the subscleral space. These patents do not teach placement in Schlemm's canal.
Some prior art criteria for glaucoma management were for Schlemm's canal, but these did not include long-term indwelling catheter placement. In U.S. Pat. No. 5,360,399, viscous material is injected through a pre-formed curved tube made of plastic or steel in order to expand the trabecular meshwork by hydraulic pressure and hydrodivide. It is taught to temporarily place the tube in Schlemm's canal. This tube is removed from Schlemm's canal after injection. Since the tube is directed outward from the eye for injection, the angle of intersection of the outflow element and the pre-formed curved element in Schlemm's canal is approximately 90 degrees with respect to its curvature plane. At the same time, it is 180 degrees away from the anterior chamber. Thus, no part of the device of US Pat. No. 5,360,399 leads to the anterior chamber. Furthermore, the tube is provided with an infusion cuff element that is larger in diameter relative to that part inside Schlemm's canal and functions as an adapter for cleaning. This device is therefore not suitable for shorting the aqueous humor between the anterior chamber and Schlemm's canal.
Most of the problems that have arisen with current glaucoma treatment devices and treatment methods have arisen because aqueous humor escapes from the inside of the eye to the surface of the eye. There is a need for a number of physiological systems to facilitate the outflow of aqueous humor from the anterior chamber into Schlemm's canal. In the overwhelming majority of patients with glaucoma, resistance problems exist between Schlemm's canal and the anterior chamber. Schlemm's canal itself, the collecting channels and the episcleral venous system are all intact. When aqueous humor is encouraged to flow directly into Schlemm's canal, the internal horn region is occupied by a single line of non-proliferating trabecular cells, which minimizes the scarring that normally occurs with external filtration methods. Let's go. When the aqueous humor is encouraged to flow directly into Schlemm's canal, Schlemm's canal is part of the normal efflux system and is biologically made to handle the normal volume of aqueous humor. Pressure sickness will be minimal. If the aqueous humor is facilitated to flow directly into Schlemm's canal, complications such as endophthalmitis and leakage will be eliminated.
SUMMARY OF THE INVENTION The present invention is directed to a novel shunt and associated surgical procedure for the treatment of glaucoma that is placed to divert aqueous humor from the anterior chamber of the eye into Schlemm's canal. Thus, according to the present invention, as was done with most prior art short circuit devices, the normal for draining aqueous humor out of the anterior chamber rather than shorting to the sclera or another anatomical site. Natural physiological pathways are promoted. It is a further object of the present invention to provide a permanent indwelling catheter to provide more outflow of aqueous humor from the anterior chamber to Schlemm's canal for glaucoma management.

Fluid communication can be facilitated by an aqueous humor guide channel in either the proximal portion or the distal portion, as described below. Fluid communication can also be facilitated by, for example, a solid proximal or distal portion wicking function in the device.
According to this invention, it is an inventive shunt, and at least a part of the shunt tube is accommodated in the circumferential direction in order to divert the aqueous humor from the anterior chamber of the human eye into the Schlemm tube Also provided is a shunt embodiment comprising a body of biocompatible material adapted to such a size and shape, which facilitates the transfer of aqueous humor from the anterior chamber into Schlemm's canal. Embodiments of the device of the present invention can be manufactured without the proximal portion of the embodiment extending into the anterior chamber. According to the aqueous humor guide channel, the movement of aqueous humor from the anterior chamber into Schlemm's canal can be promoted. Fluid communication can also be facilitated, for example, by a wicking function of the solid body portion.
The present invention contemplates many different configurations for the aqueous humor guide channel. However, it is a condition that each configuration facilitates the movement of aqueous humor from the anterior chamber to Schlemm's canal, such as by providing a lumen, fold, wick or capillary action. For example, the aqueous humor guide channel may be a completely sealed lumen, a partially sealed lumen, or a saddle-shaped channel that is at least partially open. The present invention provides a solid monofilament or reticulated polymer, such as Proline ™ (polypropylene), to provide wicking or dilating functions to facilitate movement of aqueous humor from the anterior chamber to Schlemm's canal Is intended to be inserted into Schlemm's canal. Such a wicking extension or an extension for extension can also form grooves or folds along any part of its overall length so that the cross-sectional shape is polygonal or star-shaped. The device of the present invention can be composed of a solid, matrix, mesh, windowed or porous material, or a combination thereof.
A traditional glaucoma lecture explained that Schlemm's canal in adults is divided into separate tubes by a septum, making it impossible to pass the suture completely. Previous studies on adult eyes in Eyebank showed that Schlemm's canal was actually open. The suture can pass all around the Schlemm's canal. It has not been determined so far that Schlemm's canal is not divided into multiple dead ends by a septum, but is open throughout its circumference in normal adult individuals. The present invention takes advantage of such knowledge to create and maintain the natural physiological outflow of aqueous humor from the anterior chamber to Schlemm's canal and to the collecting channel while approaching Schlemm's canal.
According to this invention, the usage method of the said short circuit apparatus is also provided. One embodiment of the invention relates to a surgical method for diverting aqueous humor from an anterior chamber into Schlemm's canal with a device implanted to extend from inside the anterior chamber of the eye to Schlemm's canal It is. The portion of the device that extends into Schlemm's canal can be made from a flexible material, such as silicone, that can be housed within a portion of Schlemm's canal radius, curvature, and diameter. The outer diameter of the proximal portion is about 0.1 mm to 0.5 mm, or about 0.3 mm. Prior work has shown that the preferred diameter of this proximal portion is from about 0.23 mm to about 0.28 mm, more preferably from about 0.23 mm to about 0.26 mm. All or part of this device may be solid, porous, tubular, bowl-shaped, windowed or pre-curved.

Since the inventive device is an implant, the device can be assembled from materials that are compatible with the tissues and fluids it contacts. The device may be a biodegradable material or a non-biodegradable material. The device is preferably not absorbed, corroded or otherwise structurally damaged while held in situ within the living body. Furthermore, it is equally important that the eye tissue and aqueous humor are maintained without being adversely affected by the presence of the implanted device. Several materials are available to meet the manufacturing and medical specifications of this shunt. In some exemplary embodiments of the invention, the
In the embodiment shown in FIGS. 1-4, the
Due to the properties of the
Furthermore, the

The
In an exemplary embodiment of the invention, the shorting device is a proximal portion at a “T-shaped” junction, as shown in FIGS. 1A-1E, 2, 3A-3D, 4 and 5C. The distal portion of the implant that intersects the device is bi-directional, or, as shown in FIG. 5A, a “V-shaped” junction between the proximal and distal shorted portions One of the one-way types. The unidirectional short-circuit device may include a distal portion that is inserted in both directions inside the Schlemm's canal. In the case of a unidirectional short-circuit device, only the distal short-circuit portion is placed inside Schlemm's canal. In these exemplary embodiments, “non-linear fluid communication path” means at least some portions of the shunt where the fluid passage is not straight. Examples of non-linear shunts are the bi-directional “T” -shaped shunt and the uni-directional “V” -shaped shunt, or a shunt with two channel openings that are not arranged in a straight line when implanted. is there.
The surgical anatomy associated with this invention is shown in FIG. 6A. FIG. 6A shows the
In yet another embodiment of the present invention, not shown, the shorting
Therefore, according to the present invention, an aqueous humor short circuit device for diverting aqueous humor in the eye from the anterior chamber into the Schlemm's canal, which is large enough to be accommodated in a circumferential direction in a part of the Schlemm's canal A distal portion having at least one end side profiled and shaped and at least one end side sized and shaped to be received within the anterior chamber of the eye A proximal portion having a fixture extending therefrom to maintain the position of the end side of the proximal portion within the anterior chamber of the eye, An aqueous humor short circuit device is provided in which the device allows fluid communication from the proximal portion in the anterior chamber to the distal portion in Schlemm's canal. In another embodiment, such a fixture may extend from the distal portion of the device to help stabilize the implant in Schlemm's canal.
Fixture extensions (“fixtures”) from multiple proximal portions or distal or proximal portions in various embodiments described below and in various embodiments apparent to those of skill in the art in view of this disclosure. Generically) provides various improvements from this short circuit device. This fixture facilitates implantation and proper placement of the device as the proximal portion is advanced into the anterior chamber and then pulled back into place until it contacts the edge of the anterior chamber. . As described further below, a surgical procedure for implantation can form a shelf designed to capture this fixture. This allows the surgeon to determine how much of the proximal portion extends into the anterior chamber. This fixture configuration also allows a surgical alternative to first implanting the proximal portion into the anterior chamber and then placing the distal portion into Schlemm's canal. The fixture also helps to secure the shorting device in the desired position in the anterior chamber and Schlemm's canal with minimal movement during normal use.
The fixture can be assembled by simply thickening the constituent material of the shunt, such as silicone, or it can be made from another material attached thereto. Further, the fixture can be assembled by removing excess material. The fixture can extend from the proximal portion in virtually any functional shape, such as circular or piercing. FIG. 7A shows the anatomical relevance of the surgical arrangement in an exemplary embodiment of the invention showing the
Thus, the fixture can extend circumferentially around the proximal portion or extend therefrom only in one or more directions. FIG. 8A shows one embodiment of this device having a bi-directional
The device includes a fixture positioned adjacent to the outer surface of the anterior chamber to facilitate surgical placement and fixation of the device, with or without a corresponding fixture adjacent to the inner surface of the anterior chamber. May be provided. Thus, possible configurations for stabilizing the implant are placed inside the anterior chamber and inside the Schlemm's canal to secure the device to the trabecular mesh between the anterior chamber and Schlemm's canal. It is a device with a fixture.
It should be understood that the anchor can extend in any shape and size in any direction that facilitates implantation or fixation of the device. For example, FIG. 9 shows that the surface of the bidirectional
FIG. 10 shows that the proximal end comprises a larger single
The fixture, like the rest of the device, is rough, grooved, or porous to promote the growth of cells such as fibroblasts and stabilize the implant from migration. Can be composed of quality material. The proximal and distal ends of the device are designed to prevent the attraction of new tissue such as fibroblasts that may grow at the surgical site and to block the flow of aqueous humor therethrough Can be manufactured. Thus, the proximal portion of the device can be manufactured to extend 0.1-3 mm, preferably about 0.5 mm, beyond the entrance into the anterior chamber. As discussed above, the angled tip of the proximal inlet will provide a range of lengths along the proximal portion that extends into the anterior chamber.
The distal portion should extend similarly beyond the surgical site followed by the fibroblast growth site. Thus, the distal portion may have a length of approximately 4 mm to 6 mm, again taking into account the diversity about the angled end. The single-type or double-type lumen short-circuit device according to the present invention can be manufactured by a conventional molding method or extrusion method. In the case of extrusion manufacturing, the single lumen can then be partially interconnected to form a dual lumen device, or each of the co-extruded dual lumen devices. The lumen can define a distal portion that can be partially separated and extend in different directions. Such a device is preferably configured so that it does not twist when it is wrapped around a 0.25 mm object.
In some cases, the device may include one or more visual indications on the surface of the device to assist in proper placement in the anterior chamber or Schlemm's canal. Such an indication at the distal end could be used to confirm that the distal end was properly inserted into Schlemm's canal, and such an indication at the proximal end would be , Will prevent over-insertion or under-insertion into the anterior chamber.
In some cases, the device may be selectively coated or infiltrated with a therapeutic agent. For example, where ingrowth is desired for stabilization, certain growth factors may be present, and antifibrosis such as 5-fluorouracil or mitomycin at the end entrance where occlusion should be avoided. A drug may be present. More generally, the device may be provided with a coating of antibiotics or carboxylic acid anhydrase inhibitors. Agents that promote collagen degradation within the trabecular meshwork can also be used.
According to the present invention, a method of implanting and using a short circuit device is provided. The surgical procedure required to insert this device requires an effort through the conjunctival flap. A partially thick scleral flap is then formed, cut into half thicknesses and placed into a normal cornea. When the rear surface of the Schlemm's tube is confirmed, it is inserted into the Schlemm's tube from the rear. Schlemm's canal and / or the anterior chamber can be expanded and lubricated by injection of viscoelastic and / or cell division agents. Some suitable viscoelastic compositions, some devices and methods for injecting them into the eye, are described in US Pat. No. 5,360,399, the contents of which are incorporated herein by reference. Incorporated into the book. Care should be taken when using some viscoelastic compositions as part of this invention to prevent over-expansion and rupture of Schlemm's canal. The proximal portion of the shunt is then inserted through the Schlemm's canal and the inner wall of the trabecular mesh into the anterior chamber within the range of angles created by the iris and cornea. In some cases, an incision is required from Schlemm's canal through the trabecular meshwork to the anterior chamber, which facilitates movement of the proximal portion therethrough. One arm of the distal portion of the shorting device is grasped and screwed into Schlemm's canal. In a similar manner, the other arm (when present) of the distal portion of the shorting device is inserted into Schlemm's canal in the opposite direction from the beginning. Scleral flaps and conjunctival wounds are closed in a conventional manner.
The following procedure may be performed to insert a bidirectional shunt into Schlemm's canal, and in particular to insert a fixation shunt as disclosed on the surface of a surgically prepared tissue shelf. .

The periocular area is washed with a surgically acceptable disinfectant such as a poboxine solution. Then, an eyelid examination mirror is arranged.
A dome-based conjunctival incision is made at the limbus. Ensure hemostasis with either a bipolar cautery or diathermy device.
A 3-4 mm x 3-4 mm scleral flap is made that extends to the depth of approximately 100 into the choroid.
Cut the scleral flap at the front and peel the Schlemm's canal outer wall.
This tearing is subsequently performed to form a cornea shelf above the trabecular meshwork. At the discretion of the surgeon, a reserve suture is placed through the scleral flap to maintain the scleral flap in place.
At the surgeon's discretion, the opening to the Schlemm's canal on both sides of the scleral flap is widened using a viscous tube construction cannula and a viscoelastic agent (eg, hyaluronic acid sulfate or hyaluronic acid / chondroitin sulfate).
A perforation is made at the corneal margin distal to the surgical site.
At the surgeon's discretion, viscoelastic drugs and miotic drugs (carbacol or acetylcholine) are injected into the anterior chamber to deepen the area.
Remove the GMP shunt from the individual. Insert the distal part of the shunt into Schlemm's canal on both sides.
Using a keratotomy blade or a 21 gauge needle, enter the anterior chamber along the cornea shelf.
Insert the proximal portion of the tube into the anterior chamber.
The scleral flap is closed with a 10-0 nylon knot suture. First, one suture is placed at the base, and then one suture is placed along the two sides. They then hide these suture knots.
The anterior chamber is deepened by injecting balanced salt solution through the perforations.
The scleral flap is tested with a cellulose sponge. If there is a leak, place a 10-0 nylon suture to achieve a waterproof seal.
The conjunctiva is closed with an appropriately sized absorbable suture.

Results of preclinical studies in animal eyes Research device A study on 16 pigs was performed using a short-circuit device, which was 1.0 mm in length at the proximal end forming a Y-shape. Two silicone (65A durometer) tubes having a length of 7 mm, an inner diameter of 0.125 mm, and an outer diameter of 0.250 mm joined to each other by a silicone adhesive are provided. The device was implanted in one eye of each animal and the other non-implanted eye served as a control.
Surgical procedures For each animal, a surgical preparation (eyelash trimming and betadine disinfection) was performed in the area around the study eye. Each animal was then anesthetized with isoflurane. A blepharoscope was used to open the eyelids under aseptic conditions. The surgical microscope was suspended in place. A peripheral corneal control thread was placed to rotate the eye to expose the upper nasal limbus. A foramenal conjunctival incision was made in the sclera and hemostasis was ensured with a bipolar cautery. A 4 × 4 mm triangular scleral flap with a partial thickness was made on the limbus, and it was cut forward and placed into the cornea. At the base of this first flap, a deeper second flap was formed and was cut forward to peel off the pig equivalent of Schlemm's canal. This deeper flap face was then angled forward to form a cornea shelf. Viscoelastic agents (hyaluronic acid sulfate and chondroitin sulfate) were instilled into Schlemm's tubular space on either side using a viscous tube cannula. The distal side of the bidirectional glaucoma shunt was inserted into the space on either side of the site to be peeled off. A cornea shelf was placed in the anterior chamber and a viscoelastic drug was instilled into the anterior chamber. The proximal (radial) portion of the shunt was inserted into the anterior chamber through the keratoconus shelf. The scleral flaps were tightly closed with 10-0 nylon sutures and then their knots were hidden. The conjunctiva was closed with absorbable suture. The control yarn was removed. Subconjunctival galamycin and decadron were instilled internally. Tobramycin-decadrone ointment was applied to the eye. These animals were awakened and returned to the dormitory.
In each case the final goal of the surgery was achieved. Schlemm's tubular space was accurately positioned and exposed at 16 locations of 16 eyes, and the device was successfully implanted without complications. Neither the eye structure nor the device was damaged during the implantation. The surgical site was closed properly without difficulty. During this procedure, there was no observable contact between the device and the corneal endothelium, anterior chamber depression, anterior bleeding requiring cleaning, iris lacerations, and no contact between the device and the iris. . All animals well tolerated surgery and anesthesia.
Clinical observation All animals well tolerated the transplantation procedure. None of the animals showed postoperative pain or discomfort, as evidenced by rubbing, loss of appetite or sleep. There were no vision-threatening complications caused by the implantation of this device. In particular, there was no chronic inflammatory reaction, wrinkles of surrounding tissues, choroidal detachment or bleeding, retinal detachment, or infection with this device.
The pigs incorporated into this study were normal animals without glaucoma. At the baseline, the average intraocular pressure in the right eye and the average intraocular pressure in the left eye were equivalent. Three months after surgery, the intraocular pressure in the study eye associated with this device was 14% lower than the intraocular pressure in the contralateral (controlling) eye (n = 16 animals).
It was unexpected that IOP would decrease in the study eye because the eye did not suffer from glaucoma and therefore did not become an area of abnormal resistance to detour formation. . Nevertheless, this device will exhibit even lower pressures in these normal eyes. None of the eyes had hypotonia or ocular hypertension. Therefore, the intermediate phase of this study showed the potential for a pressure drop of this device in the body of an animal model not suffering from glaucoma.
In-vivo fluid flow viewing At 3 months, two devices were explanted from two eyes for pressure flow testing. In these eyes, a foramenal conjunctival incision was made across the scleral flap. This scleral flap was gently cut away from the surrounding tissue and was cut forward to expose Schlemm's tubular space. The device was identified inside the space, the distal portion of the device was removed from the space, and the proximal portion was left inside the anterior chamber. At this point, aqueous humor was observed to flow from the anterior chamber through the shunt and out of the distal tube, and in vivo flow through the device was presented.
Although several embodiments described so far are exemplary, the present invention contemplates a wide variety of shapes and configurations for the shunt to provide fluid communication between the anterior chamber and Schlemm's canal. To do. Therefore, some embodiments described thus far are not intended to be limited to the scope of the claims and their equivalents.
