Chem. 27:3296−3301 (1997))。

(Irwin et al., Interpretation of positive results of a methacholine inhalation challenge and 1 week of inhaled bronchodilator use in diagnosing and treating cough−variant asthma (Archives of Internal Medicine. 157(17):1981−1987, (1997))。
【0013】

化合物Iの経口的処置(30mg/kg,b.i.d.)

Field of the Invention The present invention relates to the use of CSBP / p38 inhibitors in the treatment of cough-related diseases promoted by CSBP / p38-mediated inflammation.
[0002]
BACKGROUND OF THE INVENTION The inflammatory response of the lung or airways of the lung is derived from macrophages and epithelia, such as TNF-α and IL-1β, which promote the expression of vascular adhesion molecules (ICAM-1, E-selectin) and neutrophil chemotaxis Is believed to produce destructive oxidants and proteases in concert with cytokines, or chemokines such as IL-8 (Warner et al., Am J. Respir Crit Care Med. 160: S1-S79 (1999)). ).
[0003]
Inflammatory cytokines (TNF-α, IFN-γ, IL-4, IL-5) and chemokines (IL-8, RANTES, eotaxin) can regulate or support chronic airway inflammation (Barnes et al., Pharmacol Rev. 50). : 515-596 (1998)). The production and action of many potential mediators of airway inflammation has been shown to depend on the MAP kinase or p38 kinase (p38 MAPK) cascade induced by stress (Foltz et al., J. Biol. Chem.). 27: 3296-3301 (1997)). Various inflammatory mediators activate p38 MAPK, which in turn activates downstream markers of the MAPK system, including other kinases or transcription factors, thus creating potential for an amplified inflammatory process in the lung.
[0004]
Interfering with the biochemical processes created by this cascade indicates an effective and novel use for intervention with inhibitors of CSPB / p38. The present invention is directed to new findings regarding treatment, including prevention, of cough hyperactivity in mammals having increased eosinophils in the airways or having inflammation in the airways and coughing.
[0005]
SUMMARY OF THE INVENTION The present invention is directed to a method for treating cough hyperactivity associated with airway inflammation and / or preventing cough hyperactivity associated with airway inflammation in a mammal in need of such treatment. Use of a CSBP / p38 kinase inhibitor for the treatment of
The present invention also provides a CSBP / p38 kinase for the treatment comprising prevention of an inflammation-promoted cough-related disease in a mammal in need of such treatment, comprising the prevention of inflammation-promoted cough-related disease. Related to the use of inhibitors.
The present invention also relates to the use of CSBP / p38 kinase inhibitors in eosinophilic bronchitis and cough variant asthma.
[0006]
DETAILED DESCRIPTION OF THE INVENTION IL-1, TNF, and other cytokines affect a wide range of cells and tissues, and these cytokines and other leukocyte-derived cytokines are important and critical inflammatory mediators of a wide range of disease states and conditions. It is. Thus, inhibition of these cytokines is beneficial in controlling, reducing and ameliorating many of these disease states.
[0007]
In particular, the present invention is directed to treatments that include the prevention of eosinophilic inflammation and cough in the respiratory tract. The present invention is also directed, where appropriate, to treatments involving the prevention of eosinophilic bronchitis (different from asthma), and treatments involving the prevention of cough variant asthma. These diseases are secondary to other respiratory diseases such as viral infections that exacerbate asthma (induced by infection), chronic bronchitis, chronic obstructive pulmonary disease, otitis media, and sinusitis It is directed to the treatment of inflammation induced in the airways. The respiratory viral infection to be treated in association with smoke-related airway inflammation may be associated with a secondary bacterial infection such as otitis media, sinusitis, or pneumonia.
[0008]
The disease associated with cough promoted by hypercough or inflammation may be a direct result of eosinophil activity or may be related thereto. It may be the result of, or related to, the production of certain cytokines that may mediate these phenomena.
[0009]
As used herein, the term treatment may include prophylaxis for use in treatment groups that may be susceptible to such airway inflammation and / or cough. It may reduce the symptoms of a patient, ameliorate the symptoms, reduce the severity, reduce the morbidity or induce other changes in the symptoms, and improve the therapeutic effect.
[0010]
The mechanism of action for inhibition of cytokines by cytokine inhibitory anti-inflammatory agents (CSAIDs) is well known in the art. The present invention will show that CSAID inhibitors are useful in treating eosinophilic inflammation and cough in the airways.
[0011]
Clinically, eosinophilic bronchitis manifests as chronic cough and eosinophilic sputum, but without the abnormal airway function seen in asthma. In contrast to cough in patients without eosinophilic sputum, cough responds to anti-inflammatory therapeutics, such as inhaled corticosteroids (Niimi et al., Eosinophilic inflammation in cough variant asthma, European Respirator). Journal 11 (5): 1064-9, (1998)).
[0012]
Patients with cough variant asthma may have the following criteria: (1) have not been diagnosed with asthma; (2) complain of cough for a period of at least 3 weeks; (3) wheezing; Does not complain of short breathing or chest tightness; (4) physical test is normal; (5) spirometry results are normal or near normal; (6) bronchial excess during bronchial stimulation challenge test There is evidence of responsiveness; and (7) a favorable response to asthmatic drug application (Irwin et al., Interpretation of positive results of a methacholene inhalation related to the related matters. ng cough-variant asthma (Archives of Internal Medicine. 157 (17): 1981-1987, (1997)).
[0013]
Unlike conventional anti-cough agents such as codeine or dextrometrophan, p38 kinase inhibitors have no direct anti-cough activity but reduce airway eosinophilia and normalize hypercough It seems to let. Therefore, the use of p38 inhibitors reduces excess cough or hypercough and returns to normal levels, which can then be appropriately treated with conventional drugs and / or therapeutic agents. The use of p38 inhibitors allows maintenance of patients with increased cough response, especially unproductive cough, due to other existing diseases or treatments. Such increased cough response may be modulated or reduced by the use of this innovative anti-inflammatory therapeutic.

Biological Examples The following are examples that show how to determine the utility of p38 inhibitors in the treatment of hypercough disease or inflammation-promoted cough.
When evaluated for the first time, the anti-cough activity in question is evaluated after a pre-treatment period of 10 to 30 minutes by intraperitoneal injection or 1 hour in the case of oral administration. The animals (guinea pigs) are then inhaled with citric acid and challenged with a cough. A model of citric acid-induced cough is shown in FIG.
The tool compound selected as a p38 MAP kinase inhibitor in this inflammatory response is trans-1- (4-hydroxycyclohexyl) -4- (4-fluorophenyl) -5-[(2-methoxy) pyrimidin-4-yl] imidazole And here also referred to as compound I.
The compound's effect is then evaluated on the cough hyperresponse that occurred 72 hours after exposure to the antigen or LTD4. Animals are treated with the agent before and / or after challenge with antigen or LTD4, but not on the day of citrate challenge. The model of hypercough induced by antigen or LTD4 is shown in FIG.
The effects of the known antitussives dextromethorphan and codeine on citric acid-induced cough in guinea pigs are shown in FIG.
[0040]
In conscious guinea pigs, inhalation of citric acid (CA; 0.4% for 1 minute) elicited 11 to 15 coughs during exposure and during the 12 minute monitoring period. Sensitized animals exposed to inhaled ovalbumin became cough-excessive for several days (50-80% more frequently than CA-induced cough), and bronchoalveolar lavage indicated that There was a positive correlation with polycythemia.
Compound I has no direct anti-cough effect, but suppresses airway eosinophilia and normalizes the hypercough condition. The effect of Compound I on direct anti-cough activity is shown in FIG.
Similarly, inhalation of LTD4 (10 μg / ml per minute) increased the number of coughs and airway eosinophils 72 hours after exposure. Oral treatment with Compound I (30 mg / kg, bid) blocked the eosinophilia induced by LTD4 and normalized the cough response as shown in FIG.
These findings provide evidence for the role of eosinophils in maintaining increased cough responsiveness and support the usefulness and efficacy of using p38 MAP kinase inhibitors for this type of treatment. is there.
