一般に、本方法は、溶解した親油性成分を一緒に混合し、溶媒を蒸発又は凍結乾燥させて均一な脂質フィルムを形成することを含む。溶媒の蒸発は、成分の安定性を保つ任意の適切な手段によることができる。薬剤の大部分がその化学構造、又はその化学構造と平衡状態にある化学構造を保持する限り、SN−38は安定であると言うことができる。SN−38と平衡状態にある化学構造は、特に、高いpHではより大きな溶解性を与えるが、pHを下げるとSN−38に変わる構造を含む。

体重約70kgのヒトでは、例えば、体重1kg当たり約0.1〜2mg又は約0.5〜1mgのSN−38を投与することができる。好ましくは、体重1kg当たり約0.5〜2.0mgのSN−38を投与する。しかしながら、上述した投与量から逸脱することも必要であり、特に、治療する患者の性質及び体重、疾患の性質及び重大性、製剤の性質、そして医薬を投与するならば、投与する時間又は間隔の相関関係としてそうすることが必要であり得る。従って、活性化合物の上記量未満で処理すれば十分である場合もあるし、活性化合物の上記量を超過しなければならない場合もある。SN−38の特別に要求される最適投与量及び投与のタイプは、利用できる方法により当業者によって決定され得る。適切な量は、経験的研究によって決定されるように、過度の毒性を有さない治療有効量である。

カルジオリピン含有組成物の重要な利点は、該組成物がSN−38に付す癌細胞での多剤耐性の調節方法を提供するということである。特に、本組成物は、SN−38を用いる化学療法に付した癌細胞の、それへの耐性を発達させる傾向を減少させ、タキソール又はドキソルビシンなどの他の治療剤への耐性を発達させる癌細胞の傾向を減少させる。従って、SN−38と併用して、他の薬剤を本治療で有利に使用することができる。

本発明を説明したが、ここである種の実施例に言及する。実施例は、説明の目的のためだけに提供され、限定することを意図するものではない。

(実施例1)
SN−38(3マイクロモル)を、3マイクロモルのカルジオリピンを含むクロロホルムに溶解することができる。この混合液に、ヘキサンに溶解した14マイクロモルのホスファチジルコリン及びクロロホルムに溶解した10マイクロモルのコレステロールを加えることができる。混合物を穏やかに撹拌し、溶媒を、真空下、30℃未満で蒸発させて脂質と薬剤との薄い乾燥フィルムを形成することができる。次いで、2.5mlの生理食塩溶液を加え、渦状攪拌によって成分を激しく混合してリポソームを形成することができる。次いで、フラスコで渦状攪拌して多重膜リポソームを提供することができ、所望により、音波処理器中で音波処理して小さい単一膜リポソームを提供することができる。SN−38のカプセル化効率は、被検リポソームのアリコートを適切な水性溶媒中に一晩透析するか、又は被検リポソームのアリコートを50,000×gで1時間遠心することによって決定することができる。その後、リポソーム画分をメタノールに溶解し、逆相HPLCなどの高圧液体クロマトグラフィー(HPLC)を用いる標準法によって分析する。一般に、リポソーム中のSN−38のカプセル化効率は、最初の投入用量の80〜95%であろう。

(実施例2)
種々の量の薬剤及び脂質を用いて同様の実験条件を利用することができる。例えば、6μM SN−38、6μMカルジオリピン、28μMホスファチジルコリン及び20μMコレステロールの濃度を、適切な溶媒にそれらを溶解し、溶媒を蒸発させ、そして乾燥した脂質/薬剤を5mlの7%トレハロース−生理食塩溶液などの適切な水性溶媒中に分散させることによって使用することができる。リポソームの水和は、混合物を渦状に攪拌及び/又は音波処理することによって容易に行うことができる。次いで、所望の場合、リポソームを透析することができ、上述のように、リポソーム中のSN−38のカプセル化パーセントを測定することができる。典型的には、SN−38のカプセル化は、HPLCでアッセイすると、約75%より多く、より一般には約85〜95%以上であろう。

(実施例3)
体積2.5ml中に3μMの薬剤、15μMジパルミトイルホスファチジルコリン、1μMカルジオリピン及び9μMコレステロールを使用することによって、SN−38をリポソーム中にカプセル化することができる。薬剤と脂質との混合物を真空下で蒸発させ、等体積の生理食塩溶液に再懸濁することができる。残りの方法は上述したものと同様であることができる。SN−38のカプセル化効率は一般に、このシステムで75%より高いであろう。

(実施例4)
本実施例において、実施例1に記載の方法によって製造した、2μM SN−38、2μMのホスファチジルセリン、11μMホスファチジルコリン、2μMカルジオリピン及び7μMコレステロールを含むリポソームは、SN−38のカプセル化効率が75%より多いと予想される。

(実施例5)
本実施例において、溶液中に1mg/mlより多いSN−38を含むリポソームが示される。
約35−40℃に温めた約1kgのt−ブチルアルコールへ、約0.2gのコハク酸D−α−トコフェロール酸(D−α−tocopherol acid succinate)を加えることによって、脂質フィルムを製造する。トコフェロールが溶解するまで、溶液を約5分間混合する。約6.0gのテトラミリストイルカルジオリピンを溶液に加え、その溶液を約5分間混合する。約10gのコレステロールを溶液に加え、その溶液をもう約5分間混合し、次いで約30gの卵ホスファチジルコリンを加え、もう5分間混合する。得られた約11gの脂質溶液を凍結乾燥し、脂質フィルムを生成する。
リポソームSN−38を製造するために、8〜10のpHを有するアルカリ性水溶液中に薬剤を溶解することによって、SN−38の1.2mg/ml溶液を製造する。このSN−38溶液の約15mlを、脂質フィルムを含むバイアルに加える。バイアルを温和にかき混ぜ、室温で30分間水和させ、2分間激しく渦状に攪拌し、浴型音波処理器中で最大強度で10分間音波処理する。リポソーム溶液のpHを酸性pHに下げる。この方法を用いて、SN−38の90重量%超が、リポソームの形態で脂質と複合体を形成する。

Claims (66)
- 哺乳動物宿主における真核細胞増殖によって引き起こされる疾患の治療方法であって、SN−38と脂質とを含む複合体(ここで、SN−38の50重量%超が、該脂質の一部との複合体中にある)の治療有効量及び医薬上許容される賦形剤を含む医薬組成物を製造すること、及び該組成物を哺乳動物宿主に投与することを含む方法。
- SN−38複合体がリポソームである、請求項1に記載の方法。
- 脂質が、ホスファチジルコリン、ホスファチジルエタノールアミン、ホスファチジルセリン、ホスファチジルグリセロール、ホスファチジン酸、ホスファチジルイノシトール、スフィンゴミエリン、ステロール、トコフェロール、脂肪酸、カルジオリピン、及びそれらの混合物からなる脂質群から選択される脂質の少なくとも1つである、請求項1に記載の方法。
- 脂質がカルジオリピンを含む、請求項1又は2に記載の方法。
- カルジオリピンが、天然カルジオリピン及び合成カルジオリピンからなる群から選択される、請求項1に記載の方法。
- 哺乳動物宿主がヒトである、請求項1に記載の方法。
