【発明の属する技術分野】
本発明は、被検体に関する撮影方向の異なる複数の画像のデータを指定断面の奥行きに応じて加算することにより断面上の部位の像が鮮明で断面以外の部位の像が不鮮明な断層画像データを再構成する機能を有するX線断層撮影装置に関する。

【従来の技術】
X線断層撮影装置は、X線の撮影方向を変えながら、被検体のある領域を繰り返し撮影し、撮影方向の異なる複数の画像のデータを加算することにより、撮影方向に概垂直な方向の面の断層画像データを再構成する装置である。この撮影において、それぞれの撮影方向における画像データをそれぞれデジタル画像で保存し、再構成計算を行うと、再構成可能領域での任意の面での断層画像や三次元表示を行うことができる。このようなデジタル三次元断層撮影装置は、例えば特開平10−295680号公報に示されている。

このような装置では、X線CT装置と異なり、一般にX線の透過する領域の全てではなく、一部の領域の画像が再構成される。これは、可動範囲を小さくして患者周りの空間を確保していることをこのような装置の特徴の一つとしているからである。

図22は、従来例におけるX線管球の移動と再構成可能領域を示したものである。矢印101はX線の撮影方向であり、矢印101の始点の位置にX線管球の焦点(X線発生点)がある。X線管球の焦点から発生したX線は、X線錐102の形状に放射状に拡がる。符号103はX線錐2の中心線を示している。この中心線103の向きを撮影方向と定義する。X線錐102は被検体104を通過し、X線検出器105の受光面上に投影される。X線管球を矢印106の方向に移動しながらX線撮影を繰り返す。このとき、X線管球の位置及び方向は、全ての撮影位置でX線中心103が被検体104内の一点(不動点)を常に通過するように制御される。a,b,cは代表的な撮影位置を示している。実際にはX線管球を一端位置aから中心位置bを経て他端位置cまで移動しながら、その間に数十以上の位置で撮影を繰り返すことにより数十以上の枚数のX線画像のデータを収集する。収集した数十以上の画像のデータを加算することにより、組織形態が特定の断面で鮮明になり、他の位置ではボケた断層画像データが再構成される。再構成可能な領域119は、一端位置aから中心位置bを経て他端位置cに至るまでに繰り返した撮影の全ての撮影位置におけるX線錐102の和の領域として与えられる。ある断面111において、再構成可能領域119内の中央付近の部分では像が鮮明であるが、再構成可能領域119から外れた端部分では不鮮明に表示される。

この場合、関心部位108が、再構成可能範囲119に入っているかどうかについては、a,cの両位置で見えていれば、領域に入っていることは分かる。a,b,cの全画像上で関心部位の像113が画像中央にあれば、関心部位108は不動点に一致していることが分かる。関心部位の像113がa,b,cの位置で画像上の位置が変化していれば、関心部位108は不動点から外れていることが分かる。

関心部位108が不動点から外れていることが分かっても、関心部位108が再構成可能領域119内に収まっているのか、あるいは再構成可能領域119から外れてしまっているのか分かり難い。つまり、再構成可能領域119に対する関心部位108の2次元的な位置関係は理解できても、奥行き方向の位置関係は直接的には分からない。再構成可能領域119は上述したように位置aから位置cまでの全ての撮影位置におけるX線錐2が重畳する領域として与えられるので、再構成可能領域119の幅は奥行き方向の位置に応じて変化する。例えば、図22の位置関係を、画像109,118,110上で関心部位像113を見ても、再構成可能領域119との奥行き方向に関してその位置関係が分からないので、関心部位108が再構成可能領域119内に収まっているのか否か分からない。図22では関心部位108は再構成可能領域119の手前端にあり、このままでは、関心部位108は再構成可能領域119に収まっているものの、関心部位108の周囲組織は殆ど再構成可能領域119から外れて不鮮明で観察は難しい。このような事態は、撮影後画像再構成して初めて分かる。

さらに、再構成可能領域119そのままでは先端は狭くなって周囲が見にくいこと、計算量を減らして短時間で画像を再構成したいことから、装置が表示する再構成可能領域を、上記領域よりも小さい領域107に設定することがある。この場合は、上記のように、全ての位置で見えていたとしても、実際に画像表示する再構成可能領域7にあるとは限らなくなり、撮影したい部位が再構成範囲にあるのかが、ますます分かりにくくなってしまう。このとき不動点を示すガイド112に代えて、再構成範囲の中心を示すガイドを表示することも有用である。

また、撮影装置全体を小さく構成するために、大型の平面検出器を使用する場合は、検出器に入射する方向が逐次変化するが、この変化に対応してX線の散乱線を低減するグリッドは、適切なものが提供されていない。グリッドの形状を変化させる方法が特開2000−217813号公報に示されているが、これを応用すると、製造コストが高くなるし、変形の応答性が断層撮影の撮影時間に対して十分とは考えられない。

以上のように、従来のX線三次元断層撮影装置では、実際に画像表示する再構成可能領域と観察したい関心部位の位置関係が把握しにくいため、位置合わせが難しく、再度撮影を再度行って、時間の無駄や被曝の増加を生じることが考えられる。また、X線の散乱線を低コストで有効に除去することが難しいため、装置のコストが高くなったり、画質が低下する問題がある。

本発明の目的は、関心部位が再構成可能領域に入っているか否かを直接的に視認できるX線断層撮影装置を提供することにある。

本発明は、X線管球とX線検出器との間に配置された被検体に関する複数の撮影方向にそれぞれ対応する複数の画像データを収集し、指定された再構成範囲に基づいて該再構成範囲を含む領域の画像データを再構成するX線断層撮影装置において、撮影方向の異なる少なくとも2枚のX線画像上で同一の関心部位に関する2次元位置を指定する手段と、前記指定された2次元位置に基づいて前記関心部位と前記再構成範囲との位置関係を計算する手段と、前記画像データの再構成可能な範囲を記録する手段と、前記再構成範囲を変更する手段と、前記再構成可能な範囲又は前記再構成範囲を表す2次元画像を生成する手段と、前記再構成可能な範囲又は前記再構成範囲と前記関心部位との位置関係を表すマークと前記2次元画像とを共に含む画像を構築し、表示する手段とを具備する。

【発明の実施の形態】
以下、図面を参照して本発明によるX線断層撮影装置を好ましい実施形態により説明する。図1は本実施形態に係るX線断層撮影装置のブロック図である。図2は、図1のX線管球部とX線検出器部の配置を示している。X線管球部51は、X線管球20と、X線管球20を移動可能に支持及びその移動を駆動するために構成されたX線管球支持機構52と、X線管球20及びX線管球支持機構52を収容するケーシング23とから構成される。X線管球20には高電圧発生部61から高電圧が印加される。それにより被検体4に、X線管球20に取り付けられたX線絞り21で成形されたX線が照射される。X線管球部51は、床面上に設置された支持機構53に移動可能に支持される。X線検出器部54は、2次元状に配列された複数のX線検出素子を有するX線検出器5と、X線検出器5を収容するケーシング24とから構成される。X線検出器部54は、寝台56の昇降及び体軸移動可能な天板26上に載置された被検体4を挟んでX線管球部51に対向するように床面上に設置された支持機構55に支持される。X線管球部支持機構53とX線検出器部支持機構55とは別体でもよいし、図2に示すように、一体構造25で構成されていても良い。図3には、X線管球部51とX線検出器部54はそれぞれ、支持機構30,31に固定され、独立に移動できる構成を例示している。X線管球部51の上部にはアーム29を介してCCDカメラ(光学カメラ)28が配置される。CCDカメラ28の位置及び向きは、天板26上に仰向きで横たわっている被検体4の光学像を正面から撮影することが可能に調整されている。

撮影制御部60は、再構成可能範囲と関心部位との位置合わせ段階と、位置合わせ完了後に実際に被検体4を断層撮影する段階とにおいて、それぞれの段階に応じた撮影手順で高電圧発生部61、X線管球部支持機構53、X線管球移動機構部52、X線検出器5、X線検出器部支持機構55、寝台56の各動作を制御するために設けられている。

X線検出器5には、位置合わせ段階及び断層撮影段階において、撮影制御部60の制御によりX線検出器5から繰り返し出力されるX線画像データを撮影に同期して順次記憶するためのデータ記憶部62が接続される。再構成部63は、システム制御部65の制御のもとで、断層撮影段階において撮影され、データ記憶部62に記憶されている複数の撮影方向にそれぞれ対応する複数のX線画像のデータを、操作者が入力部66を介して任意に指定された再構成範囲を含む断面の位置に基づいて加算することにより、当該再構成範囲を含む断面に対応する断層画像データを再構成するために設けられている。断面には、再構成範囲とその周辺が含まれる。再構成範囲は、再構成可能範囲内の領域であり、その断面内の組織形態が鮮明に描画される。一方、同じ断面内であっても、再構成範囲の周囲、つまり、再構成可能範囲から外れた領域では、組織形態は鮮明には描画されない。

なお、空間座標系を、説明の便宜上、図2、図4に示すように既定する。ここでは空間座標系は、X線管球部51の配置により決まる。X線管球20は移動機構52により移動される。移動機構52はX線管球20からのX線錐の中心線が一点(不動点)で交差するように構成されている。この不動点を原点として、X線管球部51の中心からX線検出器部54に向かう奥行き方向をZ軸、前後方向にX軸、上下方向にY軸を既定する。

表示プロセッサ68は、計算された関心部位の位置に基づいて、再構成可能範囲をXZ又はYZの2次元で表すフレーム上に、関心部位の位置を表すマークを重畳する。フレーム上には、関心部位の位置を表すマークとともに、操作者が指定した再構成位置を表すラインマークを重畳するようにしても良い。この再構成可能範囲を表すフレームと、関心部位の位置を表すマークとの位置関係から、前後方向X(又は上下方向Y)に関する再構成可能範囲と関心部位との位置関係とともに、奥行き方向Zに関する再構成可能範囲と関心部位との位置関係が理解され得る。

位置合わせ計算部72は、再構成可能範囲と関心部位の位置とに基づいて、関心部位の位置に対する再構成可能範囲の中心位置又は任意の指定位置の空間座標上でのズレ方向とその距離とを計算するために設けられている。計算されたズレ方向と距離に基づいて、システム制御部65は支持機構53、55を制御してX線管球部51及びX線検出器部54の位置を変更することにより、またはX線管球移動機構52を制御してX線管球20の移動軌跡を変更することにより、再構成可能範囲の中心位置又は任意の指定位置が被検体内の関心部位に一致させることができる。

次に本実施形態の動作を説明する。まず、位置合わせ段階の初期において、被検体4の関心部位を再構成可能範囲に対して大まかに位置決めをする。大まかな位置決めには様々な方法があり、そのいずれを採用しても良い。例えば、X線管球部51とX線検出器部54とに対して、寝台天板26の位置関係がほぼ一定に設定できるように構成されていれば、寝台天板26に被検体4が乗る位置のガイド、例えばマーキングや、突起物などを備え、この位置合わせマーカに従って被検体4を寝台天板26に乗せ、また、頭部は、所定の頭部固定器によって固定する。それにより被検体4の関心部位を再構成可能範囲に対して大まかに位置決めをすることができる。また、通常のX線撮影装置やX線CT装置などで使用されているような、十字型の投光器や円形の光線を発する光源をX線管球部51に備え、被検体4に投光し、その光に合わせて被検体4の位置を調整し、また投光の大きさから位置関係を判断し、X線管球部51とX線検出器部54の両方またはいずれかの位置を調整することによって、空間座標系上での被検体4の位置を特定させることができる。また、X線検出器部51またはX線管球部54と被検体4との距離を計測する例えば超音波距離センサを設け、X線検出器部51またはX線管球部54から被検体4までの距離を測定して表示する。この表示をガイドにX線管球部51とX線検出器部54の両方またはいずれかの位置を調整することによって、被検***置を空間座標系で特定できる。または、支持アーム29を所定の位置としたCCDカメラ28の像を見て、再構成可能領域表示が関心部位の位置を含むと思われる位置になるように、X線管球部51とX線検出器部54の両方またはいずれかの位置を調整する。

上記位置合わせマーカを基準とする場合は、例えば図5に示すように、位置合わせマーカはその側面36、上面37が透明な板で構成され、各面から見た方向での、再構成可能領域の中心に十字線のようなガイド39が記されている。マーカ38は、側面36に貼り付けられ、このマーカ38がX線の写る位置によってX線管球20とX線検出器5の位置関係を校正するものであり、X線画像上の或る程度所定の位置に写るように位置合わせがなされることによって、校正が可能になっている。また、再構成可能領域の位置もこのマーカ位置から所定の位置に設定することもできる。先ずガイド39が被検体4の皮膚面での関心部位に合うように位置合わせマーカ全体を患者に対して位置合わせを行う。次にX線を透視し、X線画像上でマーカが所定の位置に写るように、X線管球部51とX線検出器部54の両方またはいずれかの位置を調整する。この所定の位置は、マーカ38と中心ガイド39の位置関係から計算されるものである。

なお上述では被検体4を側面から見ることを説明したが、被検体4の正面からのX線画像を見たい場合は、管球20の振り角を変えて移動する構成としても良い。これは図6のようにX線管球部51の角度を変える構成や、図7のように管球20の振り角度だけを変えてX線管球部51を直線移動する構成などを採用することにより実現可能である。この場合、正面からのX線画像上に撮影したい部位をマーキングし、X線管球20の角度や位置を戻した後、マーキングした位置が、再構成可能領域で、透視方向から見て中心となるように移動する手段を設けると操作性が良い。

なお、被検体4の位置をX線管球部51、X線検出器部54に対して合わせる方法としては、正確に位置を合わせる方法と、ある程度正確に合わせた後、補正する方法がある。概略の位置合わせができており、位置のズレ量が分かっていれば、画像再構成時に補正することによって、画像を得ることができる。また、概略の位置合わせができていれば、位置合わせマーカを同時に撮影し、マーカの位置から補正することもできる。例えば、片側から光線を発し、投光位置をガイドとすることによって設置位置を調整する。あるいは、三次元位置測定装置を設け、位置関係を表示してもよい。あるいは、機械的に位置が決まるように連結部材で結合したり、連結部材の長さや角度を測定できるセンサを組み込んで、相対位置関係を測定するようにしても良い。

X線管球部51に対してX線検出器部54の位置を合わせるには、例えば図8に示すように、3本のレーザビームを使用する方法がある。32はレーザビーム発光部でありX線管球部51に設置され、互いに平行でない3本のレーザビーム33を発する。レーザビーム33はX線検出器部54に投光される。投光される面には、図9に示すように3個のターゲット34が設けられている。レーザビームの投光が3個ともこのターゲットに一致するように、X線管球部51に対してX線検出器部54を移動する。レーザビームの投光が3個ともこのターゲットに一致した時点で、X線管球部51に対して所定の位置及び向きにX線検出器部54が設定され得る。

被検体4の関心部位を再構成可能範囲に対して大まかな位置合わせが終了した後に、正確な位置合わせのためにX線透視が行われる。位置合わせ段階での撮影は断層撮影段階の撮影よりも低いX線強度で行われる。低いX線強度を使って一定の周期で撮影が繰り返される。

被検体4は、寝台天板26に仰向きで横たわる。被検体4の頭部は図示しない頭部固定器によって天板26上で固定される。図10において、矢印1はX線の撮影方向であり、矢印1の始点の位置にX線管球20の焦点(X線発生点)がある。X線管球20で発生したX線は、絞り21で成形され、全体としてX線錐3を成す。2はX線錐3の中心線である。X線錐3は被検体4を通過し、X線検出器5の受光面上に投影され、そのX線画像がX線検出器5で撮影される。撮影されたX線画像データはデータ記憶部62に記憶される。撮影制御部60の制御のもとで、X線管球20は移動機構部52により断層撮影段階で予定されている軌道に沿って、直線的又は円弧状に移動する。移動の間、所定周期で繰り返し撮影される。それにより撮影方向の異なる複数のX線画像のデータがデータ記憶部62に記憶される。

図11に示すように、記憶された撮影方向の異なる複数のX線画像の中から例えば移動軌跡の両端a,cに対応する2つの撮影方向に関する2つのX線画像9,10がシステム制御部65により選択され、同一画面内に並べて表示部67に表示される。この2つの撮影方向に関する2つのX線画像9,10には、透視期間中に手動でフリーズ指示した画像を適用しても良い。

図12は、X線管球20が移動する円軌道を例示している。再構成可能領域の位置、大きさ、形状は、X線管球20の振り角ψと、円軌道の半径rと、X線管球20の軌道と被検体4との距離とによって変わる。ここでは、被検体4の周囲の空間を変えないため、振り角ψと、軌道半径rを変える場合について説明する。図13に示すように、X線管球20の振り角ψをψ’に変えることにより、X線中心線が交差する定点pが、p’に移動し、再構成可能領域Rが、中心軸沿って奥行き方向Zに移動して、R’となる。また、図14に示すように、X線管球20の振り角ψを保ったまま、X線管球20が移動する円軌道の半径rをr’に変更することにより、再構成可能領域RからR’に変化する。このように振り角ψと、軌道半径rを変えることにより、再構成可能領域を奥行き方向に沿って移動することができる。
