Abstract
Description
【0001】
【0004】
PRF(Proton Resonance Frequency)の温度特性により、磁気共鳴信号の位相変化Δφと温度変化ΔTの関係は次式で与えられる。
【0005】
【0006】

PRESTO(Principles of Echo−Shifting with a Train of Observations)による撮影を行う場合について説明する。
なお、PRESTOに限らず、例えばSPGR(Spoiled Gradient Echo)やEPI(Echo Planar Imaging)等他の適宜の技法で磁気共鳴撮影を行ってよいのはいうまでもない。
【0043】
治療制御部506としては、例えばWS(Work Station)等が用いられる。
【0056】
各カメラとしては例えばCCD(charge coupled device)カメラ等が用いられる。

TECHNICAL FIELD OF THE INVENTION
The present invention relates to a treatment device, and more particularly, to a device for performing treatment while magnetic resonance imaging of an affected part.
[0002]
[Prior art]
In order to cause harmful tissues in the body, such as cancer, to be necrotized by heat, thermotherapy (thermotherapy) is performed. As one type of thermal treatment, there is a method using energy of a laser beam or a micro wave. These are heated by irradiating a laser beam or a microwave from the tip of a needle-shaped treatment device arranged in the body.
[0003]
In order to properly perform the heat treatment, a temperature distribution image of the affected part is taken by a magnetic resonance imaging (MRI) device. That is, a tomographic image of a treatment target including an affected part under treatment is photographed by a magnetic resonance imaging apparatus, and a phase change from an image before treatment is obtained for each pixel (pixel) of the obtained image, and a distribution image of the phase change is obtained. Generate
[0004]
Due to the temperature characteristics of PRF (Proton Resonance Frequency), the relationship between the phase change Δφ of the magnetic resonance signal and the temperature change ΔT is given by the following equation.
[0005]
(Equation 1)
[0006]
here,
δ: temperature coefficient (about -0.01 ppm / ° C)
γ: magnetic rotation ratio
Bo: magnetic field strength
TE: echo time
According to the above equation, the distribution image of the phase change shows the distribution of the temperature change.
[0007]
Therefore, a treatment boundary line is set on the tomographic image of the affected part obtained by magnetic resonance imaging, an isotherm of an effective treatment temperature is generated from the temperature distribution image obtained by magnetic resonance imaging, and monitoring for the presence or absence of a conflict between the isotherm and the treatment boundary line. Perform therapy (for example, see Patent Document 1).

[0009]
[Problems to be solved by the invention]
For example, the treatment of the tissue of the abdomen such as the liver is performed in a predetermined breath-hold state by managing the respiration of a patient who has undergone general anesthesia with a respiratory management device. At that time, the breath hold is periodically released in order to secure the patient's breathing. However, due to the softness of the tissue, the position of the treatment unit slightly changes in the imaging space each time the breath is held.
[0010]
Since such a change in position appears as a change in the position of the treatment device, automatic follow-up of an imaging section is performed based on the position information. However, a reference image for temperature calculation taken before treatment is referred to as: There is no way to perform such automatic tracking. For this reason, the temperature distribution image becomes inaccurate due to the relative displacement between the two images.
[0011]
Therefore, an object of the present invention is to realize a treatment apparatus capable of obtaining an accurate temperature distribution image of a treatment portion.
[0012]
[Means for Solving the Problems]
The present invention for solving the above-mentioned problems is a treatment means having a needle-shaped treatment device which is used by a practitioner and emits energy for heat treatment from a distal end in a state where a treatment target is punctured, and in a three-dimensional space. Detecting means for detecting the position of the treatment device, and a tomographic image of the object to be treated using magnetic resonance for a cross section including the tip of the treatment device based on the position of the treatment device in parallel with treatment by the treatment device. A photographing means for photographing and creating and displaying a temperature distribution image from this tomographic image and a previously photographed reference image, and photographing a plurality of reference images having different cross-sectional positions on the object to be treated before the treatment. And a control means for performing an optimal reference image according to the position of the treatment device during the treatment.
[0013]
In the present invention, the control unit causes the imaging unit to perform imaging of a plurality of reference images having different cross-sectional positions with respect to the treatment target before treatment and, during the treatment, to generate an optimal reference image according to the position of the treatment device. Since this is adopted, an accurate temperature distribution image of the treatment portion can be obtained regardless of a change in the position of the treatment portion in the imaging space.
[0014]
The plurality of reference images are tomographic images of a cross-section to be taken in parallel with the treatment and a plurality of parallel cross-sections adjacent to both sides thereof, thereby facilitating the adoption of an optimum reference image. It is preferred in that respect. It is preferable that the plurality of reference images are images photographed in a plurality of breath holding states, respectively, in that a reference image suitable for the breath holding state is obtained.
[0015]
It is preferable that a cross section taken in parallel with the treatment is a cross section parallel to the axis of the treatment device, from the viewpoint of obtaining a temperature distribution image in a cross section parallel to the axis. The cross sections taken in parallel with the treatment are two cross sections parallel to the axis of the treatment device and perpendicular to each other, in that the temperature distribution images at the two cross sections parallel to the axis and perpendicular to each other are obtained. preferable.
[0016]
It is preferable that the cross section taken in parallel with the treatment is a cross section perpendicular to the axis of the treatment device, from the viewpoint of obtaining a temperature distribution image in a cross section perpendicular to the axis. The cross-section imaged in parallel with the treatment is a cross-section parallel to the axis of the treatment device and a cross-section perpendicular to the axis of the treatment device. It is preferable in that it is obtained.
[0017]
The sections taken in parallel with the treatment are two sections parallel to the axis of the treatment device and perpendicular to each other, and two sections perpendicular to the axis of the treatment device. This is preferable in that a temperature distribution image is obtained in each of a cross section and a cross section perpendicular to the axis.
[0018]

When coordinate axes perpendicular to each other in the static magnetic field space are x, y, and z, any of the axes can be a slice axis. In that case, one of the remaining two axes is a phase axis, and the other is a frequency axis. Further, the slice axis, the phase axis, and the frequency axis can have an arbitrary inclination with respect to the x, y, and z axes while maintaining the perpendicularity among them. FIG. 2 is a diagram showing a pulse sequence when collecting magnetic resonance signals (gradient echo) for eight views. By repeating such a pulse sequence, for example, a gradient echo signal of 256 views is collected. It is needless to say that the magnetic resonance imaging may be performed using not only PRESTO but also other appropriate techniques such as SPGR (Spoiled Gradient Echo) and EPI (Echo Planar Imaging).
[0043]
As shown in FIG. 2A, the RF excitation is repeatedly performed by the RF pulse at a period TR (repetition time). The period TR is about 20 ms. The RF excitation is performed by the
[0044]
Next, as shown in (d), a readout gradient Gr is applied, and dephasing of the spinout for readout is performed. The application of the readout gradient Gr is also performed by the
[0045]
During the phase encoding period, spin rephase is performed by the slice gradient Gs as shown in (c). A dephase gradient is added to the latter half of the rephase gradient to dephase the phase of the rephased spin.
[0046]
This dephase gradient is for preventing RF-excited spins from generating a gradient echo during the TR period, and is also called a crusher. Therefore, no gradient echo is generated even when the readout gradient Gr is applied as shown in FIG.
[0047]
The readout gradient Gr is applied four times with the polarity alternately inverted. Prior to the application of the readout gradient Gr each time, the phase encoding gradient Gp is applied, and transfer encoding is performed by a predetermined amount.
[0048]
At the end of the TR period, the slice gradient Gs is added to perform a rephase to cancel the crusher, and a readout gradient Gr and a phase encode gradient Gp for phase rewind are added. A similar pulse sequence is repeated in the next TR period.
[0049]
When such a pulse sequence is repeated, four gradient echo signals MR of the spins RF-excited for each period TR occur during the next TR period as shown in (b). This makes it possible to make the effective echo time TE longer than TR. Conversely, by making TR shorter than TE, magnetic resonance imaging with good time resolution can be performed.
[0050]
The gradient echo signal MR is received by the
[0051]
The above operation is repeated over, for example, 64 TR. Each time the operation is repeated, the phase encoding gradient Gp is changed, and a different phase encoding is performed each time. As a result, data collection is performed in the k space along a trajectory as shown in FIG.

Since the treatment is performed in a breath-hold state, the cross section setting is also performed in a breath-hold state. When a plurality of breath holding states are defined, the cross section is set in each breath holding state. That is, when, for example, 92%, 90%, and 88% are defined as the breath holding states, the cross section is set for each breath holding state.
