この特許願は、1995年9月20日に出願された「血管内における温度差を
検出するカテーテル及び方法(Catheters and Methods Detecting Thermal Discr
epancles in Blood Vessels)」という名称のアメリカ合衆国法典第35巻111
(b)に基づく予備出願第60/004,061号の利益を請求している。
発明の背景
この発明は米国心肺血液研究所(National Heart Lung and Blood Institute)
から与えられた研究助成金第91HL07のもとに政府の支援を受けて行われた
ものであり、発明の一定の権利を連邦政府に与えるものである。また、ここに開
示されているこの発明は、米国航空宇宙局(NASA)との契約の研究において行われ
たものであり、1958年の米国航空宇宙条例の305項の条例、公法85−5
68(72 Stat.435;アメリカ合衆国法典42巻2457)に従うものである。
A.発明の分野
この発明は温度差センシングによる、さらに詳しくは温度プローブやカメラ、
カテーテルなどの装置を用いた赤外線センシングによる動脈疾患の医学的診断及
び治療に関する。さらに詳しくは、この発明はカテーテル及び動脈壁における温
度差によって検出可能な動脈疾患へのカテーテルの使用法を提供している。
B.従来の技術の説明 診断における問題点
<プラーク生理学>
アテローム動脈硬化症による冠状動脈疾患は工業国における主要な死亡原因と
なっている。アテローム動脈硬化症プラークは動脈の壁が肥厚した部分である。
一般には、冠状動脈疾患によって死亡した患者は数ダースものアテローム動脈硬
化症プラークを有している。しかし、心筋梗塞、心停止、脳卒中の大部分におい
ては、実際にはこれらの障害のうちの一つが破裂したり、裂開したり、潰瘍を生
じたりするだけである。この破裂、裂開、あるいは潰瘍によって、動脈の内部に
大きな血栓(血液の塊)が形成される。血栓は、動脈の中の血液の流れを完全に
塞いでしまい、心臓や脳に障害を与える。心臓専門医にとって予後及び診断の主
なジレンマは、どのプラークが破裂しそうなのかを如何にして予測するかという
ことである。
大部分の破裂したプラークはコレストロールあるいは壊死組織片の大きなプー
ル及びマクロファージが著しく浸潤した薄い繊維キャップによって特徴付られる
。細胞が基質消化酵素(matrix-digesting enzymes)を放出することが、プラーク
破裂に寄与するものと考えられている。他の血栓は、破裂はしていないが炎症を
起こしているプラーク表面に見られる。
動脈プラークにおける炎症は、動脈壁における一連の生化学的及び機械的変化
の結果である。プラークすなわち動脈壁の肥厚は、コレステロールの蓄積、平滑
筋細胞の増殖、細胞によるコラーゲン細胞間質の分泌、マクロファージの蓄積、
そして最終的には出血(ブリーディング)、血栓(血塊)及び石灰沈着の結果と
して生じる。大多数の意見による理論は、内皮細胞の刺激あるいは生化学的損傷
の結果としてアテローム動脈症プラークが発生するというものである。
血管の内側に沿って並ぶ内皮細胞は、血液凝固の不適切な形成を防止し、その
下にある平滑筋細胞の収縮や増殖を抑制する。大部分の研究者は、内皮細胞が損
傷を受けたとき、あるいは機能障害を被ったときにアテローム動脈硬化症プラー
クが発生すると考えている。内皮細胞の機能障害は、一般にタバコの煙による損
傷、高血清コレステロール(特に酸化された低密度リポタンパク)、(例えば管
分岐点などにおいて見られる)血行力学の変化、ある種のウィルス(単純疱疹ウ
ィルスやサイトメガロウィルス)あるいはバクテリア(例えばクラミジア)、高
血圧、血清中のある種のホルモン因子(アンギオテンシンII、ノルエピネフリン
を含む)、そしてまだ知られていない他の要因によって発生する。内皮細胞に対
するこれらの漸進的な損傷の結果、アテローム動脈硬化症プラークが長年にわた
ってゆっくりと成長する。しかし、今日では、プラークが破裂するとプラークは
急速に成長することが十分に実証されている。
プラークが破裂すると、プラークの表面と血流とが触れる裂開を介して出血が
プラークの中へ流れ込む。血液はプラークのコラーゲンや脂質と接触すると急速
に凝固する(血栓を形成する)。この血塊が成長して血管を完全に又は部分的に
塞ぐ。後者の場合には、一般的に新たな血塊がプラークの壁の中に取り込まれ、
より大きなプラークを形成する。
<破裂の危険性のあるプラーク>
致命的な心筋梗塞のうちの60%〜70%がプラークの破裂に起因していること、及
び単球−マクロファージが上の繊維キャップを劣化させ弱めるメタロプロテナー
ゼ(例えば、コラゲナーゼ(collagenases)、ストロメリシン(stromelysin))を放
出することによって破裂に寄与することがかなりの証拠によって示されている。

残念なことに、プラークの破裂もプラークの薇爛も臨床的な手段で予測するこ

)、及び活性化された循環炎症細胞が不安定な狭心症を有する患者において見ら
れるが、これらの物質が梗塞あるいは死を予測するのかどうかについては、まだ

。しかし、こうした物質の存在は関係する病巣の位置を特定するのに使えないこ
とが知られている。
流れの分岐点と対向する低せん断領域は、アテローム動脈硬化症をより発生し
やすい(クー(Ku)外,アーテリオセロシス(Arterioscerosis)5:292-302(1985
))が、しかし、急性心筋梗塞や突然心臓死の患者はそれまで兆候がないことが
多く、ましてや血管造影像での兆候はない。ファーブ外,サーキュレーション92
:1701-1709(1995)。
いくつかの血管造影データによって、不規則なプラーク形状が、感度は低いが
、血栓のかなり特徴的な指標であることがわかっている。カスキ(Kaski)外,
サーキュレーション92:2058-2065(1995)。これらの狭窄症は、完全な閉塞へと
進行する。一方、軽い狭窄症は同じように進行するが、完全な閉塞までに至るこ

重大ではない狭窄は重大な狭窄よりも数が多く、付随する成長の引金にはならな
いため、実際には急激な閉塞を生じるものが大部分の心筋梗塞の原因になってい

さらに、検死においては、直径が50%以下であるような狭窄を生じていると考
えられる破裂した、あるいは潰瘍を生じているプラークに、かなり閉塞を起こし
た血栓が見つかる。シャー外,サーキュレーション244(1995)。こうした狭窄症
は狭心症を生じたりトレッドミルテストで陽性になる?ことは少ない。(実際、
心筋梗塞で死亡する患者は三血管?疾患あるいは重症の左心室機能障害を有して
いない)ファーブ外,サーキュレーション92:1701-1709(1995)。
50%以下の狭窄を引き起こすプラークの大部分においては、表面の外形は均一
であるが、深部の構造は非常に変化に富んでいて、プラークの寸法にも狭窄症の
程度にも直接の相関はない。パスターカンプ(Pasterkamp)外,サーキュレーシ
ョン91:1444−1449(1995);マン及びデービース,サーキュレーション94:928-
931(1996)。
破裂しそうなプラークを冠状動脈内部超音波を用いて識別する能力を決定する
ためのいくつかの研究がこれまでに行われている。(1)血管造影は冠状動脈の
アテローム動脈硬化症の程度を過小に評価すること、(2)エコーの濃度が濃い
所は通常は密度の高い繊維組織を示していること、(3)エコーの濃度が薄いの
は出血、血栓、あるいはコレステロールの特徴であること、(4)陰影は石灰沈
着を示していること、が知られている。ヨック(Yock)外,カージオ(Cardio)11-1
4(1994);マッパーソン(McPerhson)外,エヌ・イングル・ジェー・メド(N Engl
J Med)316:304-309(1987)。しかし、最近の研究によれば、現在の血管内部超音
波技術では、安定なプラークと不安定なプラークとを識別することができないこ
とがわかっている。ドゥ・フェイテル(De Feyter)外,サーキュレーション92:1
408-1413(1995)。
破裂のプロセスは完全には理解されていない。しかし、最も破裂しやすいプラ
ークは薄いコラーゲンのキャップ(繊維の瘢痕組織)を有し、かつ下側のプラー
クに物理的に弱い箇所を有するようなものであることが知られている。炎症表面
あるいは高密度の活性化マクロファージを有し、上に薄いキャップを被っている
プラークが血栓の危険性があることが知られている。ファン・デル・ワル外,サ

箇所はコレステロールのプールがプラークのより細胞質で繊維の多い部分と接触
する結合部に位置すると考えられている(モデルによる研究や病理学的分析によ
って)。一般に、熱を発生するマクロファージ(炎症細胞)はこうした結合部分
に見られる。これら炎症細胞はコラーゲンや細胞間質の他の成分を劣化させる酵
素
を放出するため、これらの細胞は破裂あるいは裂開のプロセスにとって重要であ
ると考えられている。
<移植血管病理学(Transplant Vasculopathy)>
炎症は移植したドナーの内皮細胞におけるホストTリンパ球の攻撃によって始
まるプロセスである移植臓器の拒絶においても重要な役割を果たす。イェン(Yeu

滑筋細胞のレクリートメント及び増殖は発熱プロセスであり、ストレステスト、
超音波又は血管造影によって血管狭窄を検出するまえに検出が可能である。ジョ
ンソン(Johnson)外,J.Am.Coll.Cardiol.17:449-57(191);セイント・ゴア(
St.Goar)外,サーキュレーション85:979-987(1992)。ドナーの臓器の「非自己
」抗原のホスト攻撃に加えて、移植された多くの組織はこれも発熱性事象である
サイトメガロウイルス感染(cytomegalovirus infection)を生じる。グラタン(Gr
attan)外,JAMA261:3561-3566(1989)。移植生理学におけるこれらの事態は、こ
うした手術から回復しつつある患者において追跡する価値のある事態である。
<再狭窄>
心臓疾患における別の重大な問題は再狭窄である。再狭窄とは(プラークによ
って引き起こされた)元々の狭窄を緩和するための単一あるいは複数の介在技術
が施された動脈が再び狭くなることである。こうした技術にはバルーン血管形成
、アテレタトミー(プラークの切削又は切断)及びレーザ血管形成が含まれる。
冠状動脈のバルーン血管形成は治療における重要な進歩であり、血行力学的に重
大な冠状動脈狭窄(血管の断面直径の70%から99%であるもの)に対して実施され
、成功率は90%である。しかし、患者の約40%においては、血管の中に再狭窄が発
生し、この手術によって得られた治療効果の大部分が失われてしまう。従って、
従来の装置あるいは方法によっては容易に解決することができない心臓専門医に
とっての別の診断及び予後の主要なジレンマはどの患者が再狭窄を生じるのかを
予測することである。
再狭窄は血管形成又はアテレクトミーによるプラークの切除によって動脈壁が
損傷を受けたときに生じる可能性がある。血管壁への損傷によって、その箇所に
おける平滑筋細胞が増殖し、細胞間質を分泌させ、これによって再び動脈が狭く
なる。細胞増殖と分泌は両方とも発熱(熱を発生する)プロセスである。さらに
、血管中におけるマクロファージの集中が再狭窄の危険性に関係する。
どの患者が再狭窄を生じるかを予測するための多くの要因が報告されている。
しかし、これらの研究は互いに相反するものが多く、再狭窄プロセスの予測に大
きく関係する要因は見あたらない。従って、喫煙、高血圧、高コレステロール血
症、不安定狭心症、年齢、性別その他の多くの要因は、せいぜい低い予測能力し
かない。
従来の装置/方法
血管障害を診断し治療するための多くのアプローチや装置がこれまでに提案さ
れている。米国特許第3,866,599号には血液中の酸素を測定するために心臓血管
系の中に挿入される光ファイバカテーテルが開示されている。血液中の酸化レベ
ルを検出するために、光ファイバを使って、まずカテーテルの先端から血液中に
赤外光及び赤色光を照射する。血液によって反射される赤外光及び赤色光は光フ
ァイバを通して酸素濃度計に戻される。反射される赤外光と血液によって吸収さ
れるそれとの比は血液中の酸素飽和状態に比例する。このカテーテルの設計では
、エレメントの末端においてファイバの端部が血管壁と接触しないようにするた
めのリセスも設けられている。また、拡張することによって、ファイバを血管壁
からさらに離間させる外側スリーブが設けられている。しかし、この特許の光フ
ァイバカテーテルは熱を検出することができない。
従来のいくつかの装置においては、温度検出素子が使われている。米国特許第
4,752,141号には、(1)光ファイバと、(2)光ファイバに沿ってファイバの
他端まで放射を導くためのファイバの一端に設けられている可視光あるいは可視
光に近い電磁放射のパルス光源を含んでいる部材と、(3)ファイバの端部又は
端
部近くに配置されていて光を受光し、温度によってこれを変調し、変調された光
を光ファイバの中に導いてセンサまで戻すセンサと、(4)このセンサの放射源
の経路中に少なくとも一つ設けられ、温度変化の大きさに応じて光の特性を変化
させるような光学素子と、(5)ファイバの端部に設けられており、変調光を受
光して、センサの温度を示している励起パルスのあとの発光強度の減衰時間に関
係した機能を測定するための部材とを有するシステムを用いて、温度及び/又は
その他の物理量の光ファイバセンシングが開示されている。これらの温度センサ
は表面に物理的に接触するように設計されており、弾力性を有する物質によって
ファイバの端部に形成されており、そこには燐光材料から成る薄い層が蒸着され
ている。燐光材料は温度に反応し、放射を放出する。この放射はファイバを伝搬
して、センサによって検出される。接触温度を決定するには、温度を測定しよう
とする位置へカテーテルを接触するような形に設置できなければならない。
米国特許第4,986,671号には、エラストマレンズによって形成されている単一
のセンサを有する光ファイバプローブが開示されている。レンズには光を反射し
温度依存性を有する材料がコーティングされており、その上に赤外放射を吸収す
る材料から成る層がコーティングされており、熱特性又は熱伝達特性が求まるよ
うになっている。応用の一つは血管中の血液の圧力、流量及び温度を測定するカ
テーテルである。
熱の検出を行う別の装置を使った他の方法もある。これらの装置の感度及び/
又は毒性についてはわかっていない。米国特許第4,140,393号では、温度プロー
ブとして複屈折性材料を使用している。米国特許第4,136,566号は、温度センサ
として、ガリウムヒ素の温度依存性光吸収特性を利用することを提案している。
米国特許第4,179,927号には、温度依存性光吸収特性を有する気体材料について
記載されている。
他のアプローチでは熱を検出するために励起法を利用している。米国特許第4,
075,493号では、温度センサとしてルミネッセント材料を使用している。一つの
波長範囲の放射を励起して測定装置から光ファイバに沿って伝搬させ、温度依存
性を有するルミネッセント放射がセンサから放出されて伝送用光ファイバに沿っ
て戻され、測定装置によって検出され測定される。光ファイバを用いた測定で最
も
実用性が高いのはルミネッセントセンサである。これは、安定性、温度範囲が広
いこと、温度以外の影響による光の変動が最も小さいこと、センサの寸法が小さ
いことなどの理由による。
他の関係するパラメータの中で特に流体の流速を測定するのに使用できるルミ
ネッセント式光ファイバプローブの例が米国特許第4,621,929号に開示されてい
る。光ファイバに沿ってセンサまで赤外放射を導き、材料層によって吸収させる
。いったん加熱されたら、流体を流してセンサを冷却する。この冷却はルミネッ
セントセンサによって検出される。冷却速度は周囲の液体の熱伝達特性及び流量
に比例する。
米国特許第4,995,398号では、心臓バイパス手術のとき、胸腔を閉じるまえに
手術が成功したことをチェックするためにサーモグラフィを使っている。この特
許には、走査式熱カメラ、画像処理、温度差及び実時間での画像表示の使用が記
載されている。この発明は冷却注入材料(cold injectate)を使っている。冷却注
入材料は、温かい血液と混ざると、離れた吻合部位における狭窄を検出するため
に心臓に焦点が合わせられている熱カメラ上で画像を捕らえることができる。
米国特許第5,046,501号では、アテローム動脈硬化症プラークと、構造的に生
育可能な組織とを、好ましくはレーザから得られ、これらの組織を識別し得る35
0nmから390nmの間の単一の励起波長を有する蛍光ビームを使って識別する方法が
開示されている。この特許の例ではカテーテルは使用されていない。従って、こ
の特許においてはインシチュのイメージングは開示されておらず、教示されてい
ない。さらに、この特許にはプラークの破裂、再狭窄又は移植血管病理を予測す
るための方法は記載されていない。
米国特許第5,057,105号には、ヒータと、キャップと、熱電対と、電源リード
線と、カテーテルを動脈の中へ設置するために末端中央に設けられたルーメンと
を有する高温先端カテーテルアセンブリが開示されている。熱電対はカテーテル
の加熱を連続的にモニタすることによって過熱を防止するために使われている。
形成されたプラークの上に先端を適切に設置すると、先端がプラークを溶かす。
米国特許第5,109,859号には、超音波によってガイドされるレーザ血管形成が
開示されている。これはカテーテルの先端に設けられたレーザと、ガイドのため
に
これもレーザの先端に設けられている超音波装置と、安定性と血液のない環境を
提供するための基端部閉塞バルーンとを有している。明かに、この特許はプラー
ク組織の質量を評価することについても関係している。この特許の超音波装置が
組織構造的な特徴(すなわち、どの細胞及び細胞外基質がプラーク内にあるか)
を識別可能かについては何も教示されていない。従って、この装置はプラーク破
裂を予測するのに使えそうにない。実際、最近の研究によれば、血管内部超音波
ではどのプラークが破裂の危険性があるのかを識別できないことがわかっている

米国特許第5,217,456号には、血管内のガイドワイヤコンパチブルなカテーテ
ルが開示されている。このカテーテルは照射光源と同期式の蛍光検出器とを有し
ている。組織中に蛍光を誘起するような波長の光がカテーテルの側部に設けられ
た開口部から半径方向に放射される。組織から放出された蛍光は同じ開口部を通
してカテーテルの中に入射し、スペクトル分析器へ運ばれる。この情報を使って
、健全な組織をアテローム動脈硬化症プラークから識別することができる。しか
し、この装置は温度差に基づいたプラーク識別を行っていない。
米国特許第5,275,594号には、ターゲット部位からの光放出を分析することに
よってアテローム動脈硬化症プラークと正常な組織とを識別する方法及び装置が
開示されている。このシステムは石灰沈着していないアテローム動脈硬化症プラ
ーク及び石灰沈着したものからの蛍光を励起するためのレーザ光源と分析装置と
を有している。分析装置は、組織によって放出された蛍光のスペクトルがターゲ
ット部位において石灰沈着した又は石灰沈着していないアテローム動脈硬化症プ
ラークを表しているかどうかを、カルシウムを蛍光励起のあとのカルシウム光放
射の時間領域信号に基づいて決定する。アテローム動脈硬化症プラークが識別さ
れたときには、レーザーエネルギを用いてプラークを切除する。
血管内の動脈診断及び修復に対する従来のアプローチは数多くあるが、いくつ
かの能力においてうまくいっていない。特に、従来のカテーテルや方法は、アテ
ローム動脈硬化症プラークの検出と治療を行うための手段を提供していない。な
ぜなら、それらは、プラークの石灰沈着の存在又は欠如は決定することができて
も、破裂し閉塞を起こす危険性のあるプラークと現時点ではそうした危険性のな
いプラークとの違いを識別することができないからである。同様に、従来のアプ
ローチは血管形成又はアテレクトミーのあとに動脈の再狭窄を起こす危険性のあ
る特定の動脈部位を識別するための有効な手段を提供していない。従来のアプロ
ーチはまた移植血管病理を検出するための実際的かつ有効な手段も提供していな
い。従来のアプローチは広範囲にわたる瘢痕組織の領域や細胞浸潤のない脂質プ
ール、又はこれから炎症細胞が増殖する出血や血栓の領域などのように、高温で
はなくて低温の動脈壁領域を有する患者を有効に識別することもできない。
発明の概要
この発明は、熱を発生している炎症細胞や管壁細胞を検出するための赤外セン
シングカテーテルを提供し、医学的患者の傷ついた血管の挙動を予測することに
よって、従来の技術の問題点の少なくともいくつかを克服している。この発明に
よるカテーテルと方法は、他の疾患の中でも特に破裂し閉塞を起こす危険性のあ
るプラークと、現時点では危険性のないプラークとを識別することのできる、ア
テローム動脈硬化症プラークを検出して治療するための有効な手段を提供してい
る。この発明によるカテーテルと方法はまた血管形成又はアテレクトミーのあと
に動脈の再狭窄を起こす危険性のある特定の動脈部位を識別したり、どの患者が
血管病理や組織拒絶のために危険であるかを識別するための有効な手段を提供し
ている。炎症による危険な領域をその高い温度によって識別できたり、普通以下
の温度(血管の残りの部分よりも冷たい領域)が活発に新陳代謝を行っている健
全な細胞がないことを示している(人体の熱は活発な新陳代謝を行っている細胞
から発生するからである)ように、この発明によるカテーテルと方法はまたそれ
までは検出されなかった危険な動脈領域である異常に温度が低い動脈壁を有する
患者を有効に識別することができる。非細胞領域は一般に出血、血栓、コレステ
ロールのプール又はカルシウムの領域であり、これらはすべて高い危険性のある
プラークの指標である。この発明の装置及び方法は、血管壁における温度差を識
別し分析することによって、これらの目的を達成している。
ある点に関しては、この発明は血管における光放射を分析するための装置に関
する。別の点に関しては、この発明は光放射を分析する方法に関する。この方法
はこの発明の装置を使って実行することが最も好ましい。
この発明において特に関心のある光放射は約2〜14μmの波長範囲に光スペク
トルがあるような放射である。赤外の魅力的な特徴は(白色光や紫外線に比べて
)カルシウムを透過することである。ベナロン(Benaron)外,(J Clin Monit)11
:109-117(1995)。
この発明において特に関心のある管はその表面へのアクセスが困難であるよう
な血管である。従って、管の内径が小さすぎて従来の温度プローブ(すなわち、
接触式温度計又は熱電対)では容易にアクセスできないような場合には、この発
明の装置及び方法に有用性が見いだされるであろう。同様に、十分に大きな内径
を有していて温度プローブをアクセスできるが、管の開口部が狭窄しているか、
閉塞しているか、又はブロックされているような場合には、この発明の装置及び
方法に有用性が見いだされるであろう。従って、この発明の装置及び方法の応用
において特に関心があるのは、動脈、静脈、血脈洞、心室、小室などのように、
血清(すなわち血液)を循環させ輸送する管を含む人体の血管である。
この発明は少なくとも一つの光ファイバが使用されているような装置及び方法
に関する。光ファイバは通常は血管の内部に位置するファイバの末端から通常は
血管の外側に位置するファイバの基端まで光放射を伝送することができる。この
発明の光ファイバは2〜14μmの範囲の波長を伝送可能にするに関していくつか
のキーパラメータを有している。これらのキーパラメータの中には、光学的透過
性、可とう性、強度などが含まれる。この発明の光ファイバは非常に細い直径に
押し出しされており、適当な赤外スペクトル領域にわたって伝送を行うようにな
っているファイバである。赤外光ファイバはフッ化ジルコニウム(ZrF4)、
石英、カルコゲナイド(AsSeTe)など、当該分野の技術者にとっては周知の様々な
材料から構成することができる。ZrF4ファイバはこの発明の装置及び方法に
非常に適している。なぜなら、こうしたファイバは、小さい直径で、1メートル
当り90%以上の透過率を有しているからである。
また、この発明の装置及び方法において使用できる光ファイバは調べようとし
ている管壁のその箇所の近傍に設置可能であるようなものである。この基準は光
ファイバの可とう性によって部分的に達成することができる。この基準の他の部
分は光ファイバの直径が非常に細いことによって満たされる。
この発明の装置及び方法は、光ファイバの末端を包み込むバルーンも利用して
いる。ある好ましい実施例においては、このバルーンは対象としている光放射に
対して透明なものである。その場合には、バルーンの外側からの光はバルーンの
外側表面を透過して、バルーンの内側表面や、光ファイバの光放射入射点まで到
達する。この好ましい実施例においては、光源(すなわち管壁、プラーク箇所な
どの箇所)から発する光放射がバルーンを透過するときに、光放射の吸収や反射
又はその他の拡散などはもしあっても非常に小さいことが非常に重要である。こ
うした望ましくない吸収は血液又は他の体液によって引き起こされる。従って、
この発明の目的に対する透明性は特定の光源から発するほぼすべての光放射をバ
ルーンの表面を介して光ファイバまで伝送できる能力を意味している。
この好ましい実施例においては、熱がほぼすべて伝導され、一方ではバルーン
の外側表面に接触するときに光源(すなわち血管壁、プラーク箇所などの箇所)
から発する熱の損失がほとんどないことが重要である。従って、この発明の目的
に対する不透明性(不伝導性)はバルーンの外側表面上の特定の源からの光放射
をほぼすべて吸収することを意味している。そのあと、バルーンの内側表面は光
を発している箇所にすぐ隣接する外側表面上で吸収された放射量に比例した量の
放射を再放出する。この再放出された放射はバルーンの内側に配置されている光
ファイバによって検出される。
この発明の装置及び方法は調べようとしている血管壁に沿った平均的な光放射
とその箇所との間の関心のある光放射における差を検出することのできる検出器
も使用している。ある好ましい実施例においては、この発明の検出器は赤外放射
の差を、50°mKまで、10〜100°mKの範囲で検出可能な感度を有しているような
ものである。
バルーンが、その箇所又は特定の箇所の外側の血管壁部分から直接放出された
放射に対して透明である場合には、検出器はバルーンの外側及び内側の表面を介
して伝送された放射を検出できるようなものである。バルーンが、その箇所又は
特定の箇所の外側の血管部分から直接放出された放射に対して不透明である場合
には、検出器はその箇所に直に接触しているバルーンの外側表面と反対側のバル
ーンの内側表面から再放出される放射を検出できるようなものである。
好ましい実施例においては、この発明の装置及び方法は赤外放射の範囲におけ
る光放射の検出によっている。特に、上述したように、この発明の装置及び方法
においては、2〜14マイクロメートルの範囲を特に対象としている。図2を参照
するとわかるように、様々な一定温度(互いに1°Kだけ異なる300〜310°Kの範
囲の温度をそれぞれ表しているT1、T2、T3)に維持された黒体から放出さ
れる放射に対する曲線を、3〜6マイクロメートルの波長範囲でプロットすること
ができる。図2の挿入図からわかるように、約5.3〜5.6マイクロメートルの範囲
においては、300〜310°Kの範囲にあり互いに1°だけ異なるような一定温度に維
持された黒体は、容易に識別可能な曲線部分を表しており、これらの黒体からは
およそ0.21×1017から0.40×1017フォトンの範囲のフォトンが放出される。従っ
て、同じように識別可能な曲線が得られるような、血管壁及び血管壁上の特定の
箇所からの放射をサンプリングするための波長を選択することが好ましい。
ある好ましい実施例においては、この発明の装置及び方法は少なくとも二つの
ファイバを使用している。血管の軸に沿って複数の箇所で同時に検出を行う場合
など、利点がある場合には、二つ以上のファイバを使用することも明かに可能で
ある。少なくとも二つのファイバが利用される他の好ましい実施例においては、
ファイバのうちの少なくとも一つは参照用ファイバであり、別のファイバが信号
用ファイバである。信号用ファイバはファイバにフォーカスされたすべての光を
末端から基端まで伝送するように設計されたファイバである。逆に、参照用ファ
イバは信号用ファイバの伝送が比較される対照として使用されるファイバである
。従って、信号用ファイバの基端から出射した光放射が参照用ファイバの基端か
ら出射する光放射と比較される場合には、対象とする箇所から放出される放射以
外の放射に起因する信号用ファイバから出射する光放射の相対的な量を容易に求
めることができる。
この発明の装置は、信号用ファイバの末端において、半径方向に生じる光放射
を前記末端及び前記ファイバの中へ導く光学反射面へ光学的に接続されている。
本発明の発明人の何人かが共同発明人になっており、ここで参照されている米国
特許願第08/434,477号には、こうした光学反射面が記載されている。信号用ファ
イバと違って、参照用ファイバは一般に参照用ファイバへ光放射がほとんど入射
しないようにする材料が末端にコーティングされている。
この発明の装置はまた不透明な閉塞用バルーンの内側表面がバルーンの透過ス
ペクトルによって変調された黒体スペクトルを放出するものである。バルーンは
膨張すると、血管内における流体の流れをほぼ制限する。必要とされる流量制限
は、膨張したバルーン末端にすぐ隣接する血管壁に沿った平均背景IR放射が増
大も低下もしないような流れしか生じないようなものである。それに加えて、好
ましい実施例においては、この発明の装置は、バルーンが膨張すると、バルーン
内部のファイバとテスト箇所に最も近接する血管壁との間の血管内の流体の存在
をほぼ排除するようなものである。
使用時には、この発明の装置は血管の軸に沿って設置される。このようにして
、診断用ファイバアレイを診断しようとする箇所の非常に近くに持ってくること
が可能である。ある好ましい実施例においては、その箇所はプラークを含んでい
る箇所である。特に、前述したような装置は装置が近くに持ってこられたプラー
クの中で特に、そのプラークが破裂する危険性のあるものであるかどうかを検知
するときに有用である。大部分の場合には、この発明の装置は血管の内壁におけ
る温度差を診断するために使用される。
この発明の装置は、その最も好ましい実施例においては、カテーテルである。
血管の内部で使用されるカテーテルの通例として、この発明のカテーテルはガイ
ドワイヤといっしょに使用されるように設計されている。例えばこの発明のカテ
ーテルを用いて破裂する危険性のあるプラークを診断したあと、手術医が問題の
あるプラークに隣接する同じ場所へ別の診断装置又はレーザなどの治療装置を持
ってきたいときなどは、ガイドワイヤによって手術医の裁量で取り出したり再び
挿入したりできる。
この発明の装置は、好ましくは、検出器がファイバの基端へ光学的に接続され
、また複数のファイバが存在するときには、それらのファイバの各々の基端へ光
学的に接続されているものである。好ましい実施例においては、検出器は多波長
の放射計である。
こうした放射計は回転式の円形可変フィルタであることが好ましい。このフィ
ルタの透過波長はその回転角度の関数になっている。こうしたフィルタにおいて
は、回転角度を調節することによって、非常に狭い波長帯しか透過しないように
することが可能である。言い換えると、こうしたフィルタはその回転特性によっ
て、非常に選択された波長に対してのみ透過性を有するように作ることができる
。従って、ある好ましい実施例においては、このフィルタはおよそ2〜6マイクロ
メートルの波長を有する放射に対して透明である。非常に好ましい実施例におい
ては、フィルタはおよそ3マイクロメートルの波長を有する放射に対して透過性
を有しているようなものである。
装置に関するこの発明のキーの一つは、約3マイクロメートルをサンプリング
することによって各スペクトルに対する基準を自動的に提供することである。生
物器官において予想される温度範囲である300〜310°Kに対しては、この波長に
おける黒体スペクトルは本質的に同じである。これによって、各信号に対するゼ
ロが与えられ、信号の低波長サイドがロックされる。これを行わないと、垂直方
向が「固定されない」ため、信号を黒体スペクトルに合わせる方法がない。
この発明の装置が比較のためにフィルタを介する複数のファイバからの伝送情
報を利用する場合には、ファイバの末端にずれが設けられていることが好ましい
。従って、信号用ファイバと参照用ファイバの末端が互いにずらされている場合
には、このずれは各ファイバから放出されフィルタ上のほぼ同じ場所を通過する
ようにされた放射をサンプリングできるだけの十分な距離に設けられる。
この発明の装置は、放射計といっしょに使用したときには、伝送された放射を
電気信号へ変換できる少なくとも一つの光電デバイスへ光学的に接続されている
ことが好ましい。この光電デバイスは電気信号をデジタル化する装置(デジタイ
ザ)へ電気的に接続されているものであることが好ましい。
この発明の装置によっていったんデジタル信号を生成したら、およそ300〜310
°Kの間の温度に維持された黒体に対するスペクトル曲線から選択した曲線にこ
のデジタル信号を数学的にフィッティングさせる。制御された黒体の曲線は各黒
体から放出されるフォトンの数として各波長に対してプロットされている。デジ
タル信号を使って血管の内壁における温度差を診断する場合には、人体の典型的
な温度についての知識をもとに、特定の黒体の対照曲線を選択する。
従って、好ましい実施例においては、この発明の装置は血管の赤外放射を分析
するためのカテーテルである。この装置は放射を伝送することが可能であり、血
管の内壁のうちでプラークを含んでいる箇所に近接して血管の軸に沿って設置可
能な少なくとも二つのファイバを有している。ファイバのうちの少なくとも一つ
は参照用ファイバであり、その末端にはファイバの中に光放射がほとんど入射し
ないようにする材料がコーティングされている。他のファイバのうちの少なくと
も一つは信号用ファイバであり、その末端は半径方向に生じる光放射をシャフト
内をそれに沿ってその末端まで導けるような光学反射面へ光学的に接続されてい
る。この装置は各ファイバの末端を包み込むバルーンも有している。このバルー
ンは膨張すると、血管の中の流体の流れをほぼ制限する。また、このバルーンは
ファイバと調べようとする箇所に最も近接する血管壁との間の液体をほぼ排除す
る。バルーンは血管の内側で生じる放射に対して透明又は不透明であり、空間的
に一定の光放射の放出率を備えた内側表面を有している。不透明なバルーンの内
側表面は黒体スペクトルを放出し得るものである。カテーテルはガイドワイヤを
有している。カテーテルは各ファイバの基端へ光学的に接続された検出器も有し
ている。この検出器はその箇所と血管壁に沿った平均的な光放射との間の放射の
差を検出することができる。検出器はまた回転式円形可変フィルタを有する多波
長放射計も有している。フィルタはその透過波長がその回転角度の関数であるよ
うなものである。ファイバの末端はそれぞれのファイバから放出されフィルタの
上のほぼ同じ位置を通過する放射をサンプリングできるだけの十分な距離だけ互
いにずらされている。さらに、放射計は伝送されフィルタに通された放射を電気
信号に変換することのできる少なくとも一つの光電デバイスへ光学的に接続され
ている。電気信号はデジタル化され、このデジタル信号はおよそ300〜310°Kの
間の温度に維持された黒体に対するスペクトル曲線から選択した曲線に対して数
学的にフィッティングさせられる。曲線は各黒体から放出されるフォトンの数と
して各波長に対してプロットされている。
この発明はまた、ある好ましい実施例においては、医学的状態を診断するのに
適した分析方法に関する。従って、この発明は血管壁のある箇所の光放射を分析
する方法に関する。この発明の方法はこの箇所の近くへ放射を伝送することので
きる少なくとも一つの光ファイバを設置する段階を有している。好ましい実施例
においては、ファイバ及びバルーンの設置はカテーテル挿入法を用いて行われる
。分析しようとする箇所の近くへの設置の前又は後に、ファイバの末端を包み込
むバルーンを血管の中で膨張させて、バルーンによって血管内部の流体の流れを
制限する。前に詳述したように、バルーンは熱放射に対して透明又は不透明であ
り、空間的に一定な光放射率を備えた内側表面を有している。この発明の方法は
また放射を検出器までファイバに沿って伝送する段階を必要とする。この検出器
はその箇所と血管壁に沿った平均的な光放射との間の放射の差を検出することが
できる。
さらに詳しく説明すると、この発明は血管に沿ったプラークで破裂する危険性
のあるものを検出する方法に関する。この方法はガイドワイヤを診断しようとす
る血管の中に挿入する段階と、そのあと血管の軸に沿って赤外放射を伝送するこ
とのできる少なくとも二つのファイバを有するカテーテルをガイドワイヤに沿っ
て血管の中へ導入して血管の内壁のうちプラークを含んでいる箇所の近くまで挿
入する段階とを有している。そのあと、この発明の方法のステップを上述したよ
うにして実行する。
この発明はまた血管の内部に多数のプラークを有する患者を外科的に治療する
方法に関する。この方法は複数のプラーク箇所のうちのどの箇所が平均的な血管
壁温度よりも高い温度を有しているかを決定する段階を有している。いったんこ
の決定を行うと、手術医は高い温度を有することがわかったプラーク箇所を除去
又は削減する。この方法は、その決定段階として、血管壁におけるプラーク箇所
の光放射を分析するための前述した方法を有している。危険性のあるプラークを
いったん識別したら、その危険性を軽減するためには多くの治療法が使用できる
。
従って、この発明の目的は、危険性のある特定のプラークを識別することによ
って、破裂する危険性のある冠状動脈のアテローム動脈硬化症プラークを有する
患者を識別することである。この発明の別の目的は、危険性のある特定の動脈箇
所を識別することによって、血管形成又はアテレクトミーのあとに動脈再狭窄の
危険性がある患者を識別することである。この発明の別の目的は移植血管病理の
危険性のある患者を識別することである。この発明の別の目的は頚動脈、脳動脈
、大動脈、腸骨動脈及び大腿動脈においてプラークが破裂する可能性のある箇所
を識別することによって、脳卒中、運動機能障害及びその他の病気に対する危険
性のある患者を識別することである。この発明の別の目的は、広範囲の瘢痕組織
の領域、細胞浸潤のない脂質プール又はこれから炎症細胞が増殖する出血や血栓
の領域などのような高温ではなくて低温の動脈壁領域を有する患者を識別するこ
とである。コレステロールプールの輪郭を求めることはプラークの退化の追跡に
有用である。追跡研究のためのそうした領域の識別によって、将来炎症を起こし
やすい領域が特定される。
この発明のさらに別の目的は、カテーテルによって識別された損傷領域に特殊
な局所治療を施すことである。こうした治療には、炎症(炎症細胞のレクリート
メント、アタッチメント、活性化、増殖)、平滑筋細胞の増殖、又は抗体、転換
成長因子−β(TGF−β)、酸化窒素(NO)、NOシンターゼ、グルココル
チコイド、インターフェロンガンマ、ヘパラント及びヘパリン硫酸プロテオグリ
カン、及びそれらを符号化している種々の相補DNAを含めた内皮細胞の感染を
防止又は制限する治療が含まれるが、それらに限定されるわけではない。
この発明の方法及び装置は多くの利用性がある。それらは冠状動脈又は頚動脈
のアテローム動脈硬化症による疾患及び死亡を低減するであろう。また、それら
は再狭窄の発生を抑え、従って繰り返し血管形成やアテレクトミーを行う必要性
を少なくするであろう。それらはまた臓器移植患者における血管病理の発生を抑
えるであろう。ひいては、これらの成果によって、よりよい健康管理、公衆衛生
の改善、健康管理のコスト低減がもたらされるであろう。この発明のこうした利
用性及びその他の利用性は以下の詳細な説明によってより明かになるであろう。

図3は破裂する危険性のあるプラークの近傍の血管内に設置された図1のカテ
ーテル先端の軸断面図である。
図4(a)は生きた頚動脈プラークの表面温度と細胞密度との関係を描いたグ
ラフである。相対細胞密度は対象とする領域の細胞密度と背景領域におけるそれ
との比である。温度測定は、取り出してから10〜15分後に22人の患者からの24サ
ンプルに対して室温(20℃)で行った。点(温度差0℃)は27の観測を表してい
る。
図4(b)は37℃のチャンバ内で測定したときの生きた人間の頚動脈プラーク
の温度と細胞密度との間の相関を示している。
図5はキャップの厚みの関数としてプラーク表面温度を描いたグラフである。
炎症を起こしていない繊維キャップを有するサンプルに対して面積測定を行って
、ルーメンから下にある細胞クラスタの中心までの距離を測定した。
図6は生きた人間の頚動脈プラーク試料(37℃のチャンバの中で新鮮な状態で
切開された)におけるサーミスタとIRカメラによる測定の間の相関を示してい
る。ここに、r=0.9885、p=0.0001である。
図7はIR放射と上の図6で述べた試料における細胞密度との間の相関を示し
ている。
好ましい実施例の説明
<カテーテルの実施例>
ここで図面を参照する。図1は使用時におけるこの発明の装置の好ましい実施
例を示している。図には、動脈(図示されていない)の内側に設置できるように
なっているカテーテル装置10が示されている。動脈は多数のプラーク箇所(図
示されていない)を有する内壁(図示されていない)を有している。どれかのプ
ラーク箇所が破裂する危険性はこの発明による方法及び装置が適用されるまでは
わからない。
ガイドワイヤ20は外科的に動脈の中へ挿入されており、基端22と末端24
の両方へ延びていることがわかる。ガイドワイヤ20はまたカテーテル装置10
の中を延びていることもわかる。ガイドワイヤ20はカテーテル装置10をプラ
ーク箇所を含んでいる動脈領域へ設置するのに使われる。
カテーテル装置10はその末端(検出器から遠い方の端部)に設けられた膨張
可能なバルーン40と、信号用ファイバ50と、参照用ファイバ60とを有して
いる。膨張可能なバルーン40はその膨張した状態が描かれている。バルーンは
膨張することによって、動脈の内壁及びプラーク箇所へしっかりと当接される。
動脈内における本来の血流の方向に応じて、バルーン40の膨張によって位置3
2又は位置34もしくはバルーンの壁42と動脈の内壁との間の一般に円形の一
連の接触箇所周辺における同様の箇所で、血液の流れがほぼ制限される。これに
よって、干渉を受けることなくカテーテル装置10で測定を行うことが可能にな
る。
バルーン40は弾性材料から形成された壁42を有している。バルーン40の
周辺は膨張によって動脈壁に沿った箇所でバルーンのシーリング又は閉塞が行わ
れるようなものになっている。収縮すると、バルーン40は動脈壁と接触しなく
なり、動脈内における本来の血流が再開される。また、カテーテル装置10を、
例えばカテーテル装置10を使って別のプラーク箇所から放出される放射を測定
する位置などの、動脈内における次の位置まで容易に移動することが可能となる
。バルーン40の膨張/収縮の実行は血管形成又は閉塞切除カテーテル、もしく
は先端にバルーンを付けたカテーテルの製造分野の技術者には周知の多くの方法
によって行うことができる。
バルーン40の目的は、測定しようとする赤外放射の源と末端のカテーテル部
分との間における赤外放射の水による吸収に関連する問題を避けるためである。
膨張して血管壁と接触すると、バルーンの壁42は壁42が最も近接している動
脈部分の温度になる。空隙46はバルーン内壁と信号用ファイバの末端56との
間においてすべての水を排除する。
信号用ファイバ50は透明な先端部分52と不透明な本体部分54とを有して
いる。これらの部分はそれぞれ赤外放射を効率よく透過させることができるよう
に又はできないようになっている。不透明な本体部分54は信号用ファイバ50
がクラッド又はスリーブ56によって覆われている領域である。スリーブ56は
その領域を不透明にし、赤外放射を効率よく透過したり吸収したりできないよう
にしている。透明な先端部分52は単に信号用ファイバ50が露出された領域で
ある。信号用ファイバ50は赤外放射を効率よく伝送することのできる光ファイ
バである。周囲からそうした赤外放射を集めるために、信号用ファイバ50には
集光デバイス58が取り付けられているか、又はその末端に集光デバイス58が
使用されている。集光デバイス58は周囲の赤外放射をファイバの中に集束させ
て、その後伝送させる。
信号用ファイバ50と違って、参照用ファイバ60は透明部分を有していない
。そうではなくて、参照用ファイバ60は不透明な端部62と不透明な部分64
とを有している。これらは両方とも赤外放射を効率よく伝送することができない
。信号用ファイバ50におけると同様に、参照用ファイバ60の不透明な部分6
4は参照用ファイバ60がクラッドあるいはスリーブ66で覆われている領域で
ある。スリーブ66はその領域を不透明にし、赤外放射を効率よく透過したり吸
収したりできないようにしている。不透明な端部62は参照用ファイバ60に研
磨された銀あるいはアルミニウムなどの赤外反射コーティングがコーティングさ
れている領域である。他のすべての点に関しては、参照用ファイバ60は赤外放
射を効率よく伝送することができる光ファイバとして機能するという性能におい
て信号用ファイバ50と同じである。従って、参照用ファイバは信号用ファイバ
50に沿った末端から基端までの任意の温度特性を補償するためのベースライン
として使用することができる。図1に示されているように、参照用ファイバ60
は信号用ファイバ50から基端の方へずらされている。このずれ(末端の方へず
らすことも同様に可能である)は各ファイバによって受光されて伝送される放射
の間に物理的に時間遅れを生じさせる。すぐ下で議論するように、この時間遅れ
は各ファイバの基端から出射する信号ビームと参照ビームがフィルタの同じ空間
部分へ入射するようにするために導入されている。こうすることによって、アラ
イメントの問題やマルチフィルターシステムによって生じるバンドパスの不均一
性をなくすことができる。
動作時には、ファイバとバルーンのアレイ70が熱放射を収集し、この放射を
基端の方へ信号用ファイバ50及び参照用ファイバ60を介して伝送する。両方
のファイバは回転式の放射計80を同じ半径位置82で透過してそれぞれデジタ
イザ92,90へ入射するように配置されている。各光ファイバの伝送からいっ
たんデジタル信号が発生されると、信号用ファイバ50によって伝送されたデジ
タル信号から参照用ファイバ60によって発生された背景信号がコンピュータ9
4によって差し引かれる。その結果得られる調節された信号を黒体のスペクトル
曲線96とコンピュータ94によってフィッティングさせて、特定の箇所の温度
を確定する。
<カテーテル構造>
ここで述べたカテーテル装置のその他の様々な部材に対する材料としては、い
くつかのオプションが存在する。光部品のキーパラメータは光学的透明性、可と
う性及び強度である。高強度のポリエステル及びポリエチレンテレフタレート(
PET)は非常に透明性が高く、非常に薄い壁に容易に押し出し可能である。あ
る実施例において必要とされるように、長い距離にわたって捻れを移動させる(t
ranslate)には高強度の編んだポリエステルが有用である。スペーサ/ベヤリン
グはテフロンから形成することができる。カテーテル全体の可とう性は同じ様な
寸法の心臓血管のレーザを用いた光ファイバ式の血管形成及びアテレクトミー用
カテーテルと大体同じであろう。従って、これらの装置は直径の小さい冠状動脈
まで持っていくことができなければならない。カテーテルの基端(患者の外側)
には、InSbもしくはHgCdTe、TeO2又はTASから成る検出システ
ムを利用した検出器が配置されている。
細長いフレキシブルな光ファイバは一端が保護用シースを介して光コネクタへ
接続されている。光コネクタは熱検出器の中へ摺動させて挿入されるようになっ
ている標準的なものである。光コネクタは一方のサイドに多数の開口部を有して
おり、この開口部を介して流体、空気などの気体がカテーテルの中へ導入され、
またそこから放出される。コネクタはまた圧力を測定するための圧力トランスデ
ューサへ接続するための結合素子も有している。コネクタには結号素子やカテー
テルの圧力ルーメンと連通した開口部が設けられている。結合素子はまた血液サ
ンプルを採取したりカテーテルに生理食塩水を流すために注射器へも接続される
。
カテーテルを形成している材料はナイロン、テフロン(商標名)などの可とう
性プラスチック、ポリ塩化ビニルなどのビニル、ポリウレタン、ポリエチレン、
又は種々のゴム化合物などを含めて一般的に使用される任意のものでよい。一般
に、カテーテルは12.7〜101.6cm(5〜40インチ)の長さであり、約1〜2ミリメー
トルの外径を有している。カテーテル内部のルーメンは様々であるが、一般に直
径は1/2〜1ミリメートルである。
この発明の装置及び材料を用いて検出可能な最小温度差は約0.1℃である。こ
の発明の装置はもっと細かい温度識別が可能であるが、生物学的な変動によって
システムに雑音が入りやすい。大部分の場合には、破裂の危険性があるプラーク
はより危険性が少ないものよりも、少なくとも1.5℃だけ変動している。
<危険なプラーク>
それから一般的に、図3のこの発明の装置及び方法の概観として、赤外センシ
ング用のカテーテル100は潰瘍を生じているアテローム動脈硬化症プラーク1
02を識別している。プラーク102は血小板の集合体103及び血管収縮10
4を伴っている。このプラーク102には炎症細胞105が存在しているため、
その温度はすぐ隣接する血管107よりも高くなっている。この違いがカテーテ
ル100によって検出される。いくつかの内皮細胞108が失われており(老化
、炎症、梗塞、毒素又はバルーンによる傷の結果として)、これによって血小板
109が活性化して、損傷を受けた血管壁110へ付着する。活性化された血小
板109は血管収縮、血小板集合及び平滑筋細胞の成長を引き起こす媒介物を放
出する。これらの媒介物にはADP、セロトニン、トロンボキサンA2、血小板
誘導成長因子、転換成長因子−β及びPF4が含まれる。内皮下層のコラーゲン
111及び脂質112の露出並びに血小板の活性化によって、凝固酵素の酵素活
性が促進される。その結果、血しょう有糸***促進剤の放出及びフィブリノゲン
を引き裂いてフィブリンを形成する酵素であるトロンビンの活性化が起きる。こ
の
プロセスが最も進むと動脈の完全な閉塞になり、その結果、心臓(頚動脈、脊稚
動脈又は大脳動脈の場合には脳)が障害を受ける。
図にはまた単球114も描かれている。単球114は活性化された内皮細胞の
表面上の付着性分子へ付着している。単球は改質コレステロールの取り込み、副
産物として破裂を促進する有糸***促進剤、及び破裂を促進するタンパク質分解
酵素の放出に関係してマクロファージになる。例I:
<方法>
病理学者による肉眼検査のあとに、50の頚動脈内膜除去手術試料を生きた状態
で調べた。試料のうちの約30%に見られた目に見える血栓は一般に緩やかな洗浄
によって除去された。このことは、それらが手術による人工的なものであること
を示唆している。手術に対する目安は、一般に頚動脈狭窄や一過性虚血性発作あ
るいは脳卒中である。
22人の患者からの24の試料を室温(20℃)で調べた。26人の患者からの別の26
の試料を37℃の湿潤インキュベータの中で調べた。
試料を取り出したあと15分以内に、24ゲージニードルティップを有するコール
ーパーマー(Cole-Parmer)モデル8402−20サーミスタ(精度0.1℃、時定数
0.15)を用いて、20の箇所においてタイルルーメン表面の温度を測定した。温度
は再現性があり(±0.1℃)、大部分の測定値は互いに0.2℃以内であることがわ
かった。従って、これを背景温度とした。
大部分のプラークにおいて、高い温度を有するいくつかの箇所がすべて見つか
った。これらの箇所と背景温度に、様々な色の消えないインクでマークを付け(
記録はしたが、病理学者に温度を示すコード付けは行わなかった)、再現性を確
かめるために再測定を行った。次に組織を10%のフォルマリンの中で固定し、長
手方向に切断し、内膜と培地が見えるように包埋し、組織構造を調べるための処
理を行い、ヘマトキシリン及びエオシン又はマソン(Masson)のトリクロームで染
色するか、又は前述したHAM−56及びKP−1抗体(Dako)を用いてマ
クロファージに対する免疫染色を行った。ニッカリ外,サーキュレーション92:

93−1398(1995)。染色した領域の下側の300×400μmの領域におけるキャップの
厚さ及び細胞密度を、マッキントッシュのセントリス(Centris)650及びメ
リーランド州ベススダ(Bethesda,Maryland)の米国衛生研究所(National Ins
titute of Health)からインターネットで利用可能なNIH画像処理ソフト(バ
ージョン1.43)を使って測定した。
ジェット推進研究所(Jet Propulsion Laboratory)のケイ化白金カメラ?を
用いて予備実験も行った。我々はこれをマッハ(Mach)5走査型赤外カメラ(ニ
ューヨーク州ウェストベリー(Westbury,NY)のフレキサーム(Flexitherm))
で校正した。次に、これを0〜100℃の範囲の様々な温度のビーカに入った水で校
正したところ、ほぼ完全な相関y=0.99x+0.31が得られた。ここに、
xは水銀温度計で測定した温度である。このカメラは0.10℃の温度分解能及び0.
15mmの空間分解能を有している。
<結果>
プラークは表面温度の再現性がよく、0.2〜0.3℃(±1.0℃)の変化におさま
る多数の領域を有していた。そして、プラークの37%がプラーク当り1〜5のかな
り温かい(0.4〜2.2℃)領域を有していた。例えば、典型的な場合では、1mm離
れた領域の再現性がよく、0.6℃の温度差を有していた。プラークのルーメン表
面は目に見える異形性を有していたが、温度差は肉眼では明かではなかった。こ
れらの温度差は下側の細胞密度と正の相関があった(r=0.68、p=0.0001)(
図4Aを参照のこと)。この細胞の大部分はマクロファージの形態学的特徴と(
HAM−56及びKP−1との)免疫反応性を有する単核細胞であった。
いくつかの有糸***的特徴が見られた。また、いくつかの泡沫細胞が見られた
が、泡沫細胞が支配的な領域は単核細胞の浸潤のある領域よりも温度が低かった
(そして細胞密度も小さかった)。多くのプラークは少数のリンパ球と肥満細胞
を含んでいた。
温度はキャップの厚さと反比例して変化した(r=-0.38、p=0.0006)(図5
を参照のこと)。最大の相関(r=0.74、p=0.0009)は理論的な予測式ΔT=
相対細胞密度÷キャップ厚さによって与えられる。温度が低い領域は非細胞組織
すなわち新鮮な血栓、出血、瘢痕組織、カルシウム又は炎症浸潤のないコレステ
ロールプールの領域であった。
温度が高い領域はそれらの多くが炎症細胞から成る表面層を有していたが、肉
眼検査では違いがわからなかった。そのうちのいくらかは血小板の集合体を有し
ていた。他の広い領域には炎症細胞はないが、内皮細胞がなかった。これらはお
そらく手術のときに剥ぎ取られたのであろう。なぜなら、検死では、血栓や炎症
がない限り、局所的な剥脱しか通常は見られないからである。ファン・ダム外,

少数の(約20%)プラークは温度的な異形性は検出できなかった。これらの試
料における深部又は表面の炎症領域は染料でマーキングされなかった。これは、
高い温度が測定されなかったことを示している。細胞浸潤物を含んでいる領域の
一部において温度を測定した。それらは隣接する細胞性の少ない領域よりも温度
が高いということはなかった。本発明者この発見が取り出したあと長期間にわた
って室温に維持された試料における代謝活動の減少を反映しているかもしれない
と考えた。
従って、別の一連のプラークを37℃のインキュベータの中で分析した。平均年
齢が68才(50〜86才の範囲)の26人の患者からのこれら26の試料によって、細胞
密度とのより密接な相関(R=0.68、p<0.0001)と、より多くの(試料の93%
)温度的異形性と、一般に1〜3℃の大きな温度範囲とが明かになった。10mmしか
離れていないいくつかの試料が4〜5℃の温度差によって特徴付られた。図4Bを
参照のこと(黒のダイヤモンドで表された点は、相対細胞密度をキャップの厚さ
の二乗で割ったものである。これらの点を直線回帰分析した結果が図に実線で示
されている)。
この発明ではまた0.1℃の温度分解能と0.1mmの空間分解能を有するケイ化白金
の冷却式赤外カメラを使っていくつかの試料を調べた。このカメラは半ビボ試料
の温度的異形性を検出した。図6に示されているように、IRカメラを温度的に
異なったプラークを識別するのに使ったとき、IRカメラは新鮮な状態で切除し
た人間の頚動脈プラーク試料へ直接接触させたサーミスタ測定とよい相関を示し
た(r=0.9885、p<0.0001)。図7はIRカメラで測定した温度のこの相関は
細胞密度測定に対しても見られることを示している。本発明者によると、冷却式
のステアリングアレイカメラはもっとよい温度分解能を有し、空間分解能は10μ
mである。
<結論>
大部分の人間の頚動脈アテレクトミー試料は大部分がマクロファージである下
側の細胞によって発生することが明かな高温の病巣を含んでいる。37℃で調べた
ときには、20℃よりも温度変化は大きかった。これはプラークの温度をより均一
にする20℃における低下した代謝活動と矛盾しない。
体温で調べたサンプルにおいては、1mmの先端を有するサーミスタによって、
同じプラークのうちで10mmしか離されていない異なる部分で4℃もの差を検出す
ることができた。細胞がプローブヘ最も近付いたときに(すなわち、ルーメン自
身のところ又はそのすぐ下側のときに)温度は最高になった。プラークのルーメ
ン表面の大部分は表面の炎症と内皮剥離によって特徴付られるいくつかの領域を
有していた。
表面の炎症のうちの一部の領域しか目に見える血栓と関連付られなかった。大
部分は微小な血栓(例えば、少数のフィブリンのストランドや付着した血小板)
と関連付けられたが、全くそうでない場合もあった。これらの結果は、プラーク
の温度が高いのは剥離して炎症を起こしており、従って血栓の危険性のあるプラ
ークの指標であることを示唆している。
本発明者はまた薄いが健全なキャップのすぐ下側の炎症病巣に関係した少数の
温度の高い領域を見つけた。これらのプラークは破裂の危険性が高いと考えられ
るため、本発明者はインビボでプラークの温度を測定することによってこうした
プラークを識別することが可能であると考えている。例II:
<研究の限界>
本発明者によって識別された潜在的なコンファウンダ(confounder)はアンギオ
ジェネシス(血管新生)である。本発明者は生きたプラークを半ビボで調べた。
インビボでは、バサエバソラム(vasae vasorum=脈管の血管)の存在や状態(tone
)が温度に影響するかもしれない。しかし、プラークのアンギオジェネシスは炎
症と相関があり(ニッカリ外,サーキュレーション92:1393-1398(1995))、ま
た両方ともプラーク破裂の危険因子と考えられているため、温度はインビボでの
予測にやはり使えそうである。
本発明者はまた頚動脈におけるアテローム動脈硬化症プラークに対して真であ
るものが、他の箇所例えば冠状動脈においては真ではないかもしれないというこ
とを考慮しなければならないと考えている。プラークの病理学は二つの箇所にお
いていくらか異なっており(ファン・ダム外,カージオバスキュラ・パソロジ3
:9-17(1994))、危険因子もまた違っている。カネル(Kannel),ジャーナル・カ

<分光、トモグラフィ及び干渉計の可能性>
赤外分光がいくつかの点で有用かもしれない。まず、マクロファージの箇所を
それらが産生する酸化窒素の質量によって確証づけることができるはずである。
なぜな外,酸化窒素は特有の近赤外スペクトルを有しているからである。オーダ
ン(Ohdan)外,移植(Transplantation)57:1674-1677(1994)。コレステロールの
近赤外イメージングは既に行われている。カシス(Cassis)外,アナリティカル・
ケミストリ(Anal Chem)65:1247-1256(1993)。第二に、赤外及び近赤外の波長は
波長が長くなるに従って組織のより深部まで侵入するため、より長い波長によっ
て深部(0.1〜1mm)における代謝の活性度がわかるはずである。
この現象を使って、赤外線コンピュータートモグラフィを開発できるかもしれ
ない。このとき、場合によっては干渉計をいっしよに用いる。入射ビームを可動
ミラーで分割して、参照ビームと組織によっていろいろに散乱され吸収されるビ
ームとに分ける。非同期の反射された波長を再構築して、20μmの分解能で詳細

<危険なプラークの非侵襲検出>
赤外線吸収、対流、組織の放出率の違いによって非侵襲赤外線トモグラフィが
難しいかもしれないため、赤外線検出に代わるものが望ましい。そうしたもの中
には、ガリウム(パスターカンプ(Pasterkamp)外,サーキュレーション91:1444
-1449(1995))、18FDG陽電子スキャニング、放射性同位体で置換された抗マ
クロファージ抗体小片、又は磁気共鳴(フォトンのスピンの緩和が温度依存性を
有するという利点を利用)を用いた炎症の画像化が含まれる。マックフォール(M
ac Fall)外,インターナショナル・ジャーナル・ハイパーサーミア(Int J Hyper
thermia)11:73-86(1995)。
これらの方法は、動いている冠状動脈(特に曲折的で末端の血管)表面の下側
の炎症病巣を検出するための十分な空間分解能を欠いており、プラーク特有の介
在治療を「オンライン」で実施するには使えない。しかし、大動脈、頚動脈、大
腿動脈などの壁が厚くて比較的固定された動脈には、これらの方法における分解

Medlcine)1:69(1995)。ルーメンの炎症を外膜の炎症と区別できるなら、後者
は大動脈の動脈瘤の進行の予測に有用かもしれない。例V:
<治療上の関連(implications)>
ダイエットや薬物で血清のコレステロール濃度を下げることによって死亡率を
低下させることができる。これは、多分、コレステロールの逆輸送によって脂質
コアの寸法が小さくなるためであろう。しかし、今日までで最も説得力のある試
みでも、コレステロール低下治療は冠状動脈死亡率を35%しか引き下げない(そ
して女性にはほとんど利点がない)ことを示している。スカンジナビアン・シン
バスタティン・サバイバル・スタディ・グループ(Scandinavian Simvastatin Su
rvival Study Group),ランセット(Lancet)344:1383-1389(1994)。この発見は
止血
の変量などの他の要因が死亡率に影響していることを示唆している。しかし、同
じ患者においても、プラークの進行あるいは退行は比較的変動している。グール
ド(Gould),サーキュレーション90:1558-1571(1994)。この変動は病巣特有の変
量(例えば狭窄の長さ、表面血栓、低速流又は乱流による低せん断応力、及び血
管収縮など)によって血栓の危険が増大することを示唆している。アルダーマン

)外,(J Am Coll Cardiol)18:904-910(1991)。
「非重大性の」10%〜70%の範囲の狭窄を生じる高温プラークが破裂する危険性
が高い状態にあることが示された場合、それらに血管形成を施すべきであろうか
。拡張の危険性がもっとひどい狭窄症(約1%の死亡率と2%の大動脈−冠状動脈バ
イパス)と同じようであれば、不安定な病巣を長期間にわたる開放性の確率が70
%であり再閉塞の確率が30%であるような病巣に変える利点は何であろうか。最近
の試みがステント(stent)が再閉塞の率を10%〜20%に低下させることを示す前で
も、大規模なエモーリー(Emory)の追跡によって、再狭窄の有る患者と無い患者
では、前者のグループでは繰り返し血管形成あるいはバイパス手術に対する必要
が大きいにもかかわらず、96%の同じ5年生存率が示された。これらのデータは急
な(自発的な)閉塞の短期間の危険性がおよそ5%にしかすぎないなら血管形成は
利点があることを示唆している。例VI:
<医学的治療>
医学的治療は、炎症が表面か、健全なキャップの下側であるかに部分的に依存
する。この区別は、ある日(one day)は、アンギオスコピ(特に発光抗体を使用
して)によって行われるか、又は炎症の可溶性マーカ(Pセレクチン、VCAM
−1、及びその他)のために血液をサンプリングすることによって行われる。磁
気共鳴イメージング、超音波、及び近赤外イメージングも有用であろう。
治療には、単球のレクリートメント、アタッチメント、活性化、あるいはDN
A合成を防止する薬剤(ペプチド、ペプチドミメティクス、オリゴヌクレオチド
、及びその他)を局所的に輸送するものが含まれよう。逆に、アスコルビン酸あ
るいは転換成長因子−β(これは、炎症を生じていない組織における炎症を誘発
したり内皮再生を遅らせたりする可能性があるけれども、大部分のモデルにおい
てはアンギオジェネシス、炎症、及び平滑筋増殖を抑制する作用も行う)によっ
てコラーゲン合成が刺激されるかもしれない。ナサン(Nathan)外,(JCellBol)11
3:981-986(1991)。内皮の再生は塩基性又は酸性の繊維芽細胞成長因子によって
又は特に血管の内皮成長因子によって増強することができる。カセルズ(Casscel

要約すると、生きた人間の頚動脈アテローム動脈硬化症プラークはルーメンあ
るいはルーメンの近くのマクロファージに主に起因する熱的な微小な異形性を有
している。これら温度が高い領域はサーミスタ及び赤外線サーモグラフィによっ
て識別が可能である。温度が高いプラークが実際に血栓(あるいは再狭窄(ゲル
ツ(Gertz)外,サーキュレーション92:1-293(1995);モレノ外,サーキュレーシ
ョン92:1-161(1995))又は外膜の炎症の場合には動脈瘤の破裂の危険性が高い
のであれば、これらの生命を脅かす可能性のある病巣のイメージングと治療のた
めの、カテーテルをベースとした非侵襲の手段を開発することが可能である。こ
れらの技術を使って、磁気共鳴イメージングによって、又は内視鏡検査、検眼鏡
検査、腹腔鏡検査、間接鏡検査又は経頭蓋イメージングによって、炎症の上皮下
クラスタあるいは他の器官の悪性細胞を検出してもよい。
以上、この発明を実施するための好ましい形態を見いだしこれを提案するため
に、この発明を特定の実施例によって説明してきた。当該分野における技術者は
、この明細書を読めば、発明の範囲を逸脱することなく、上述した実施例に対す
る多くの修正及び変更が可能であろう。例えば、この発明を生物医学における例
によって確証してきたが、この発明の装置及び方法は任意の脈管の壁の弱さの分
析に、そうした弱さが温度差を有するか、あるいは有するようにできる限りは、
同じように適用することができる。従って、導管などの人工の脈管を外から加熱
すれば、この発明の装置及び方法を使ってこれを内部分析することができる。こ
うした修正すべてが、添付されている請求項の範囲に含まれている。

Claims (1)
- 【特許請求の範囲】 1. 脈管の光放射を分析するための装置であって、 前記放射を伝送することができるとともに前記脈管の壁の中の箇所に近接して 設置可能な少なくとも一つのファイバと、 前記ファイバの末端を包み込むバルーンと、 前記箇所と前記脈管に沿った平均的な光放射との間の前記放射の差を検出する ことのできる検出器と、 を有し、前記バルーンが前記放射に対して透明であるか又は前記放射に対して不 透明であるとともに黒体の内側表面を有しているような装置。 2. 前記光放射が赤外放射である請求項1記載の装置。 3. 前記脈管が血管である請求項1記載の装置。 4. 少なくとも二つのファイバをさらに有する請求項1記載の装置。 5. 前記ファイバの少なくとも一つが参照用ファイバであり、前記ファイバの 別の一つが信号用ファイバである請求項4記載の装置。 6. 前記信号用ファイバの前記末端が光学反射面へ光学的に接続され、前記光 学反射面が半径方向に生じる光放射を前記末端及び前記ファイバの中へ導くこと ができる請求項5記載の装置。 7. 前記参照用ファイバはその末端において前記光放射が前記参照用ファイバ の中へほとんど入射しないようにする材料がコーティングされている請求項5記 載の装置。 8. 前記不透明なバルーンの前記内側表面が黒体スペクトルを放射する請求項 1記載の装置。 9. 前記バルーンが膨張するによって前記脈管内部の流体の流れをほぼ制限す る請求項1記載の装置。 10. 前記バルーンが膨張することによって前記ファイバと前記箇所に最も近 接する前記脈管の前記壁との間の前記流体をほぼ排除する請求項1記載の装置。 11. 前記設置が前記脈管の軸に沿って行われる請求項1記載の装置。 12. 前記箇所がプラークを含んでいる請求項1記載の装置。 13. 前記プラークのルーメン表面の上又はルーメン表面の下に炎症細胞が存 在するために、前記プラークは破裂する危険性又は血栓の危険性を有する請求項 12記載の装置。 14. 前記壁が前記脈管の内部にある請求項1記載の装置。 15. カテーテルをさらに有する請求項1記載の装置。 16. ガイドワイヤをさらに有する請求項1記載の装置。 17. 前記検出器が前記ファイバの基端へ光学的に接続され、複数のファイバ が設けられている場合には、前記ファイバの各々の基端へ光学的に接続されてい る請求項1記載の装置。 18. 前記検出器が多波長の放射計を有している請求項5記載の装置。 19. 前記放射計が回転式の円形可変フィルタを有し、このフィルタの透過波 長が回転角度の関数である請求項18記載の装置。 20. 前記ファイバがおよそ2〜14マイクロメートルの波長を有する放射に対 して透明である請求項19記載の装置。 21. 前記ファイバがおよそ3〜7マイクロメートルの波長を有する放射に対し て透明である請求項19記載の装置。 22. 前記信号用ファイバ及び前記参照用ファイバの前記末端が各ファイバか ら放出され前記フィルタの上のほぼ同じ場所を通過する放射をサンプリングでき るだけの十分な距離だけ互いにずらされている請求項19記載の装置。 23. 前記放射計が前記放射を電気信号へ変換することのできる少なくとも一 つの光電デバイスヘ光学的に接続されている請求項18記載の装置。 24. 前記光電デバイスが前記電気信号をデジタル化することのできる装置へ 電気的に接続されている請求項23記載の装置。 25. 前記デジタル化された信号がおよそ300〜310°Kの間の温度に維持され た黒体に対するスペクトル曲線から選ばれた曲線に数学的にフィッティングされ 、前記曲線が前記黒体の各々から放出されるフォトンの数として前記波長の各々 に対してプロットされる請求項24記載の装置。 26. 血管の赤外放射を分析するためのカテーテルであって、 少なくとも二つのファイバと、 バルーンと、 ガイドワイヤと、 検出器と、 を有し、前記ファイバが前記放射を伝送することができるとともに前記血管の内 壁の中でプラークを含んでいる箇所に近接して前記血管の軸に沿って設置可能で あり、 前記ファイバの少なくとも一つが参照用ファイバであり、この参照用ファイバ がその末端において前記光放射が前記参照用ファイバの中へほとんど入射しない ようにする材料がコーティングされ、 前記ファイバの残りのうちの少なくとも一つが信号用ファイバであり、この信 号用ファイバの末端が半径方向に生じる光放射を前記信号用ファイバの前記末端 及び前記信号用ファイバの中へ導くことができるような光学反射面へ光学的に接 続され、 前記バルーンが前記ファイバの各々の前記末端を包み込んでおり、前記バルー ンが膨張することによって前記血管内部の流体の流れをほぼ制限し、また前記バ ルーンが前記ファイバと前記箇所に最も近接する前記血管の前記壁との間の前記 流体をほぼ排除し、 前記バルーンが前記放射に対して透明であるか又は前記放射に対して不透明で あるとともに空間的に一定した光放射率を有する内側表面を有し、前記不透明な バルーンの前記内側表面が黒体スペクトルを放射し、 前記検出器が前記ファイバの各々の基端へ光学的に接続され、前記箇所と前記 血管に沿った平均的な光放射との間の前記放射の差を検出することができ、 前記検出器が多波長の放射計を有し、この放射計が回転式の円形可変フィルタ を有し、このフィルタの透過波長が回転角度の関数であり、約3マイクロメート ルの波長を有する放射に対して透明であり、 前記ファイバの前記末端が各ファイバから放出され前記フィルタの上のほぼ同 じ場所を通過する放射をサンプリングできるだけの十分な距離だけ互いにずらさ れ、 前記放射計が前記放射を電気信号へ変換することのできる少なくとも一つの光 電デバイスへ光学的に接続され、前記電気信号がデジタル化可能であり、このデ ジタル化された信号がおよそ300〜310°Kの間の温度に維持された黒体に対する スペクトル曲線から選ばれた曲線に数学的にフィッティングされ、前記曲線が前 記黒体の各々から放出されるフォトンの数として前記波長の各々に対してプロッ トされるカテーテル。 27. 脈管壁中のある箇所の光放射を分析する方法であって、 前記放射を伝送することができる少なくとも一つの光ファイバを前記箇所の近 傍に設置する段階と、 前記ファイバの末端を包み込むバルーンを前記脈管の中で膨張させることによ って前記バルーンによって前記脈管の内部の流体の流れを制限させる段階と、 前記放射を前記箇所と前記脈管壁に沿った平均的な光放射との間の前記放射の 差を検出することのできる検出器まで前記ファイバに沿って伝送する段階と、 を有し、前記バルーンが前記放射に対して透明であるか又は前記放射に対して不 透明であるとともに空間的に一定した光放射率を有する内側表面を有している方 法。 28. 前記光放射が赤外放射である請求項27記載の方法。 29. 前記脈管が血管である請求項27記載の方法。 30. 前記箇所に近接させて少なくとも二つのファイバを設置する段階をさら に有する請求項27記載の方法置。 31. 前記ファイバの少なくとも一つが参照用ファイバであり、前記ファイバ の別の一つが信号用ファイバである請求項30記載の方法。 32. 前記信号用ファイバの前記末端が光学反射面へ光学的に接続され、前記 光学反射而が半径方向に生じる光放射を前記末端及び前記ファイバの中へ導くこ とができる請求項31記載の方法。 33. 前記参照用ファイバはその末端において前記光放射が前記参照用ファイ バの中へほとんど入射しないようにする材料がコーティングされている請求項3 2記載の方法。 34. 前記バルーンの前記内側表面が黒体スペクトルを放射する請求項27記 載の方法。 35.前記ファイバの前記設置が前記脈管の軸に沿って行われる請求項27記載 の方法。 36. 前記箇所がプラークを含んでいる請求項27記載の方法。 37. 前記プラークが破裂あるいは血栓の危険性を有する請求項36記載の方 法。 38. 前記壁が前記脈管の内部にある請求項27記載の方法。 39. 前記ファイバと前記バルーンの前記設置がカテーテル挿入によって行わ れる請求項27記載の方法。 40. 前記カテーテル挿入がガイドワイヤの挿入を含む請求項39記載の方法 。 41. 前記検出器が前記ファイバの基端へ光学的に接続されており、複数のフ ァイバが設けられている場合には前記ファイバの各々の基端へ光学的に接続され ている請求項27記載の方法。 42. 前記検出が前記放射を多波長の放射計へ通す段階を有する請求項31記 載の方法。 43. 前記放射を前記放射計へ通す段階がその透過波長が回転角度の関数であ るような円形の可変フィルタを回転する段階と、前記放射を前記回転式フィルタ へ通す段階とを有する請求項42記載の方法。 44. 前記フィルタがおよそ2〜14マイクロメートルの波長を有する波長に対 して透明であり、これをサンプリングするために使用される請求項43記載の方 法。 45. 前記フィルタがおよそ3〜7マイクロメートルの波長を有する波長に対し て透明であり、これをサンプリングするために使用される請求項43記載の方法 。 46. 前記フィルタがおよそ3マイクロメートルの波長を有する波長に対して 透明であり、これをサンプリングするために使用される請求項43記載の方法。 47. 前記信号用ファイバ及び前記参照用ファイバの前記末端を各ファイバか ら放出され前記フィルタの上のほぼ同じ場所を通過する放射をサンプリングでき るだけの十分な距離だけ互いにずらす段階が設けられている請求項42記載の方 法。 48. 前記検出が前記放射計を少なくとも一つの光電デバイスへ光学的に接続 する段階と、前記放射を電気信号へ変換する段階とによって行われる請求項42 記載の方法。 49. 前記検出が前記電気信号をデジタル化する段階によって行われる請求項 48記載の方法。 50. 前記検出が前記デジタル化された信号をおよそ300〜310°Kの間の温度 に維持された黒体に対するスペクトル曲線から選ばれた曲線に数学的にフィッテ ィングする段階と、前記曲線を前記黒体の各々から放出されるフォトンの数とし て前記波長の各々に対してプロットする段階とによって行われる請求項48記載 の方法。 51. 破裂する危険性のあるプラークを血管に沿って検出する方法であって、 ガイドワイヤを前記血管の中に挿入する段階と、 少なくとも二つのファイバを有する前記ガイドワイヤに沿って前記血管にカテ ーテルを挿入する段階と、 を有し、前記ファイバが前記血管の内壁の中でプラークを含んでいる箇所の近傍 において赤外放射を前記血管の軸に沿って伝送することができ、 前記ファイバの少なくとも一つが参照用ファイバであり、この参照用ファイバ はその末端において前記光放射が前記参照用ファイバの中へほとんど入射しない ようにする材料がコーティングされ、 前記ファイバの残りのうちの少なくとも一つが信号用ファイバであり、この信 号用ファイバの末端が半径方向に生じる光放射を前記信号用ファィバの前記末端 及び前記信号用ファイバの中へ導くことができるような光学反射面へ光学的に接 続され、 前記ファイバの各々の前記末端を包み込むバルーンを膨張させる段階がさらに 設けられ、前記バルーンが膨張することによって前記血管内部の流体の流れをほ ぼ制限し、また前記バルーンが前記ファイバと前記箇所に最も近接する前記血管 の前記壁との間の前記流体をほぼ排除し、 前記バルーンが前記放射に対して透明であるか、又は前記放射に対して不透明 であるとともに空間的に一定した光放射率を有する内側表面を有し、前記不透明 なバルーンの前記内側表面が黒体スペクトルを放射し、 前記赤外放射を検出器へ伝送する段階がさらに設けられ、前記検出器が前記フ ァイバの各々の基端へ光学的に接続され、前記箇所と前記血管に沿った平均的な 光放射との間の前記放射の差を検出することができ、 前記検出器が多波長の放射計を有し、この放射計が回転式の円形可変フィルタ を有し、このフィルタの透過波長が回転角度の関数であり、約3マイクロメート ルの波長を有する放射に対して透明であり、 前記ファイバの前記末端が各ファイバから放出され前記フィルタの上のほぼ同 じ場所を通過する放射をサンプリングできるだけの十分な距離だけ互いにずらさ れており、 前記放射計が前記放射を電気信号へ変換することのできる少なくとも一つの光 電デバイスへ光学的に接続され、前記電気信号がデジタル化可能であり、このデ ジタル化された信号がおよそ300〜310°Kの間の温度に維持された黒体に対する スペクトル曲線から選ばれた曲線に数学的にフィッティングされ、前記曲線が前 記黒体の各々から放出されるフォトンの数として前記波長の各々に対してプロッ トされ、 前記プラークが前記平均的な血管壁温度よりも高い温度を有しているかを決定 する段階がさらに設けられている方法。 52. 脈管の内部に複数のプラーク箇所を有する患者を外科的に治療する方法 であって、 前記複数のプラーク箇所のどれが脈壁の平均温度よりも高い温度を有するのか を決定する段階と、 前記温度が高いことがわかったプラーク箇所を除去あるいは削減する段階と、 を有する方法。 53. 前記決定する段階が前記脈管壁における前記プラーク箇所の光放射を分 析する段階によって行われ、 前記放射を伝送することができる少なくとも一つのファイバを前記箇所の近傍 に設置する段階と、 前記ファイバの末端を包み込むバルーンを前記脈管の中で膨張させることによ って前記バルーンで前記脈管の内部の流体の流れを制限させる段階と、 前記放射を前記箇所と前記脈管壁に沿った平均的な光放射との間の前記放射の 差を検出することのできる検出器まで前記ファイバに沿って伝送する段階と、 をさらに有し、前記バルーンが前記放射に対して透明であるか、又は前記放射に 対して不透明であるとともに空間的に一定した光放射率を有する内側表面を有し ている請求項51記載の方法。 54. 請求項27の方法における請求項1の装置の使用。 55. 臓器、脈管、体腔又は孔における炎症を検出するための請求項1の装置 の使用。 56. 臓器、脈管、体腔又は孔における生きた細胞の不在を検出するための請 求項1の装置の使用。
