DESCRIZIONE DESCRIPTION
del BREVETTO per INVENZIONE INDUSTRIALE avente per titolo: of the PATENT for INDUSTRIAL INVENTION entitled:
“CATETERE PER L’ABLAZIONE DEL DOTTO BILIARE” "CATHETER FOR THE ABLATION OF THE BILIARY DUCT"
The present invention generally relates to the field of catheters for use in surgery and refers in particular to a catheter for the ablation of the bile duct.
Numerous types of catheters are already known which can be used to carry out various treatments on parts or organs within a body by passing through natural ducts or lumens. Such catheters generally have a substantially non-flexible distal part, so that their introduction into the ducts or lumens that have bends or deviations to follow is not really easy, indeed it becomes risky for the walls of the ducts themselves.
The catheters for radiofrequency treatments, then, have a terminal electrode in which various functional elements pass, from one side of which a retractable thermocouple wire usually emerges, but which in any case is rigid, which even less favors the introduction of the catheter. in a duct or lumen through which to reach the area to be treated.
It is therefore an object of the present invention to provide a catheter with a distal part structured to facilitate its introduction into the ducts or lumens of a body, until it reaches the parts to be treated, even in the presence of curves or deviations to be traveled, introduction which it can also be hindered by the hardness of the walls of the ducts themselves.
Another object of the invention is to provide a radiofrequency catheter with a flexible part, also cooled as required and which can also receive a mandrel, a guide wire and / or a thermocouple wire for measuring the temperature in the treated area.
A further object of the invention is to propose and produce a radiofrequency catheter suitable in particular for the ablation treatment of the bile duct.
Said purposes are achieved, according to the invention, with a catheter with several longitudinal tubes or lumens, having a proximal part which is connected, through specific bushes and connectors, to various equipment for supplying and controlling different functions, and a distal bearing part functional elements for the treatment to be carried out, and where said distal part comprises a portion which is flexible and which has an elasticizing spring, an electrode which is also flexible which can be connected to a radiofrequency generator and a guide wire or mandrel.
The distal flexible portion extends beyond a rigid tubular element from one side of which at least one thermocouple needle for detecting the temperature in the area around that part of the catheter exits in a retractable form.
More details of the catheter proposed here will result from the continuation of this description made with reference to the accompanying drawings, indicative and not limiting, in which:
la Fig. 1 mostra, in parziale sezione, un esempio di catetere secondo l’invenzione; Fig. 1 shows, in partial section, an example of a catheter according to the invention;
la Fig. 2 mostra una sezione trasversale ingrandita fatta secondo le frecce A - A sulla Fig. 1 ; Fig. 2 shows an enlarged cross section made according to the arrows A - A on Fig. 1;
la Fig. 3 mostra una sezione longitudinale ingrandita della porzione flessibile distale del catetere; e Fig. 3 shows an enlarged longitudinal section of the distal flexible portion of the catheter; And
Fig. 4 shows, in section, a further enlarged detail of the part of the catheter circled on Fig. 3.
In the example shown, the catheter has a body 10 consisting of several tubes 11, 12, 13, 14 side by side longitudinally and enclosed in an elastic sheath 15.
In a distal part thereof, the catheter body has a tubular element 16 from which extends a portion of flexible catheter 17 made of an appropriate material and having a longitudinal hole 18. At its free end, the flexible portion is provided with a conical tip 19, for example in Teflon, to which a so-called mandrel or a guide wire 20 can be associated. In an area near the tip, the flexible portion 17 preferably incorporates a helical spring 21 suitable and intended to assist the straight position of the same flexible portion after each bending thereof.
In another part between the elasticization spring 21 and the tubular element 16, the flexible catheter portion carries an electrode consisting of a ribbon wire 22 conducting the radiofrequency used for the intended treatment. The wire or tape 22 is wound in a helical shape around the flexible portion to be able to follow the bending thereof without the risk of breaking.
Of the tubes concurrent to form the catheter body, a tube 11 is intended for the passage of a so-called needle 23 which extends until it comes out, in a retractable form, from one side of the tubular element 16 from which the distal flexible portion 17 extends. The needle 23 is tubular, is provided with an external coating 24 and the wires of a thermocouple 25 for measuring the temperature pass through it.
Another tube 12 is intended for the passage of a radio frequency cable 26 which reaches the electrode 22 for its power supply, as needed.
The other tubes 13, 14 are provided, for example, for the circulation of an electrode cooling fluid; they extend up to a cooling compartment 27 next to the electrode.
In the space between the adjacent tubes and enclosed in the sheath, other thermocouple wires 28 can also be housed for detecting the temperature also at the level of the treatment electrode 22.
From its proximal part, the catheter body is constrained, through a bush 29, to a support body 30, and the various tubes are joined by a lump of resin 31, enclosed in said support body, to prevent undue sliding.
Attached to this body, by means of a connector 32, is a handle 33 placed and held between two flanges and two rings 34, 35, respectively.
Said handle carries, in its intermediate part between the two rings 35, a cursor 36 for controlling the longitudinal displacements of the needle 23 in the relative tube 11.
At the rear, the handle carries a connector 37 which can be connected to a radiofrequency generator and respective control instruments. Connector 37 leads to the wires of the thermocouple 25 of the needle 23, the radio frequency cable 26 feeding the electrode and the wires of the thermocouple 28 associated with the electrode 22.
To other parts of the support body are connected, through a first bush 38, the pipes 13, 14 of the cooling circuit each terminating with an attachment 39 and an apparatus for the circulation of the cooling fluid, and, through a second bush 40, the tube through which the guide wire passes which reaches the tip or stylet at the free end of the distal flexible portion of the catheter.
Although many constitutive aspects of the described catheter are certainly known, the distal flexible portion 17 of the catheter as well as its configuration and structure represent a definitely original and advantageous innovation. In fact, as mentioned above, it allows the catheter to be easily introduced up to the parts to be treated, such as the bile duct, otherwise not reachable with known catheters.
