Description

Područje tehnike
Izum je iz područja medicine, točnije područja traumatologije i odnosi se na gornji dio butne endoproteze zgloba kuka s tijelom čiji se poprečni presjek kontinuirano povećava od njegova distalnog kraja prema proksimalnom kraju, a u proksimalnom području tijela se ovalno širi u medijalno/lateralnom smjeru.
Tehnički problem
Tehnički problem kojega rješava izum je da gornji dio butne endoproteze zgloba kuka prije navedene vrste dalje oblikuje tako da je moguće bez oslanjajuće ogrlice uvođenje vertikalnog opterećenja s dovoljno velikim oslanjajućim površinama koje su postignute oblikom.
Stanje tehnike
Ovakav gornji dio proteze za butnu kost je, na primjer, poznat u obliku realizacije koja se može zacementirati iz DE 38 29 361 i koja ima tijelo čiji je poprečni presjek u produženom smjeru prilagođen šupljem prostoru kosti prirodnog femura i ima oslanjajuću ogrlicu u obliku prirubnice na proksimalnom kraju tijela, a koja se naslanja na resekcijski rub prirodnog femura kada je gornji dio proteze za butnu kost ucementiran u femuru.
Budući se kod cementiranja koštani element neujednačeno sakuplja ispod oslanjajuće ogrlice, izravni kontakt između oslanjajuće ogrlice i prirodnog femura se ne može pouzdano ukloniti, osim toga u ovom slučaju postoji opasnost da se tanki segmenti koštanog cementa cijepaju pod opterećenjem i zbog različite prirode materijala se ne mogu pouzdano isključiti i neželjeni relativni pokreti između metalne oslanjajuće ogrlice i prirodnog femura, zbog čega se i koštano tkivo mijenja na nepoželjni način. Osim toga, u pravilu, oslanjajuća ogrlica naliježe samo točkastim dodirom - u danom slučaju preko koštanog cementa - na prirodni femur, pošto konačni položaj gornjeg dijela proteze butne kosti, a time i oslanjajuće ogrlice, zavise od prirode i oblika šupljeg prostora kosti i pri izvođenju resekcijskog reza se ne mogu precizno predvidjeti. Zbog toga se ne mogu otkloniti nedostaci pri uvođenju vertikalnog opterećenja preko oslanjajuće ogrlice na prirodni femur.

Zbog toga je zadaća izuma da gornji dio butne endoproteze zgloba kuka gore navedene vrste dalje oblikuje, tako da je bez oslanjajuće ogrlice moguće uvođenje vertikalnog opterećenja s dovoljno velikim oslanjajućim površinama koje su postignute oblikom.
Ova zadaća će kod gornjeg dijela proteze butne kosti prije spomenute vrste prema izumu biti riješena na taj način, što prema proksimalnom kraju tijela povećanje poprečnog presjeka u ventralno/dorzalnom smjeru slijedi u više stepenika, čija medijalno/lateralna širina od distalnog stepenastog ruba raste prema proksimalnom kraju tijela.
Prednosti izuma su očito u tome, što u proksimalnom segmentu tijela povećanje poprečnog presjeka - radi prilagođenja na šuplji prostor kosti prirodnog femura - slijedi u više stepenika koji u unaprijed određenoj udaljenosti od distalnog kraja tijela naliježu s malom medijalno/lateralnom širinom i čija se medijalna/lateralna širina prema proksimalnom kraju tijela stalno povećava prema poprečnom presjeku tijela. Pošto se pri implanitiranju gornjeg dijela proteze butne kosti, sukladno izumu, šuplji prostor kostiju, pomoću pomagala, u dorzalnom i ventralnom dijelu oblikuje sa stepenastim udublje-njima, stepenasti rubovi proteze s odgovarajućim stepenastim rubovima šupljeg prostora kosti stvaraju uporište koje je omogućeno oblikom i koje osigurava kako primarnu stabilnost tako i trajnu stabilnost i, osim toga, omogućuje primjenu vertikalnog opterećenja na kosti na relativno velikoj površini. Pogodna stepenasta udubljenja u unutrašnjoj stijenci femura postižu se prifliranim pomagalima koji su prilagođeni gornjem dijelu proteze butne kosti i čiji se prostor tada, pri implantiranju, ispunjava homogenom cementnom kapsulom te gornjim dijelom proteze butne kosti. Alternativno, moguće je i implantiranje proteze sukladno izumu bez cementa. Željeno oblikovanje stepenastih udubljenja prema stepenastim izbočinama umjetnog gornjeg dijela butne kosti je moguće na jednostavan način pomoću odgovarajućeg alata, turpije za drvo ili sličnog, pošto se pokretanje alata vrši samo u aksijalnom smjeru i na kraju se proteza odgovarajućim kretanjem postavlja u odgovarajuće pripremljeni šuplji prostor kosti. Otklonjeni su nedefinirani pokreti protetičkog gornjeg dijela butne kosti pri postavljanju.
Osobita je prednost to što je na ventralnoj strani i dorzalnoj strani proksimalnog segmenta tijela postavljen po jedan stepenik, čime se oblikom omogućuje simetrično oslanjanje na obje strane gornjeg dijela proteze butne kosti. Stepenik naliježe malom širinom na unaprijed određenoj udaljenosti od distalnog kraja i proširuje se naviše prema proksimalnom kraju tijela.
Prema jednoj pogodnoj realizaciji izuma, prema ventralnoj strani i dorzalnoj strani tijela raspodijeljeno je uvijek više stepenika, pomaknutih prema proksimalnom kraju stepenasto jedan na drugi. Rub sljedećeg višeg stepenika pri tom pogodno prolazi uvijek na udaljenosti od ruba stepenika ispod njega, tako da se i u lateralnom, odnosno medijalnom smjeru dobiva stepenasta gradacija, pri čemu poprečni presjek tijela prema proksimalnom kraju raste na sve strane i na taj način je približno oblika šupljeg protora kosti. Na ovaj način se postiže pouzdano čvrsto nasjedanje proteze u šupljem prostoru kosti.
Prema jednom pogodnom obliku izuma visina rubova pojedinačnih stepenika je uvijek konstantna, a stepenaste površine su ravne i međusobno paralelne. Ovaj oblik realizacije postiže veliku čvrstoću, pogodna svojstva prihvaćanja opterećenja s pogodnim učvršćenjem.
Alternativno se visina ruba stepenika prema proksimalnom smjeru stalno povećava, čime se postiže dodatna kompresija kosti pri postavljanju proteze.
Pogodno je u proksimalnom području tijela ventralno i dorzalno oblikovana po jedna ravna osnovna površina na kojoj su stepenici pomaknuti u uzdužnom smjeru - kako na dorzalnoj tako i na ventralnoj strani poprečnog presjeka. Ventralna i dorzalna osnovna površina mogu biti međusobno paralelne.
Na proksimalnom kraju tijela je postavljen nasadni konus za učvršćivanje glave proteze pomoću uličnog spoja. Alternativno se može na proksimalnom kraju tijela predvidjeti i vrat proteze s oblikovanom glavom proteze. Osim toga, na proksimalnom dijelu tijela može se lateralno još oblikovati jedan naslon koji povećava primarnu stabilnost neposredno poslije impantiranja dijela proteze, naročito glede rotacijskih opterećenja.
Pogodno, poprečni presjek tijela prema distalnom kraju prelazi u kružni poprečni presjek, a na distalnom dijelu tijela je lateralno oblikovana klinasta površina koja poprečni presjek tijela prema distalnom kraju reducira u segmente kružne površine.
Prema jednom pogodnom obliku izuma, distalni segment tijela proteze je savijen prema ventralnom smjeru pod unaprijed određenim kutom a koji je poželjno u području između 3° i 50°. Na ovaj način je tijelo proteze bolje prilagođeno toku šupljeg prostora kosti, a impantiranje proteze će na taj način biti pojednostavljeno.
Daljnje prednosti oblikovanja izuma su navedene prema pojedinim točkama zahtjeva. U daljnjem toku je pomoću crteža pobliže pojašnjen jedan primjer realizacije.
Slika 1 prikazuje bočni pogled na gornji dio proteze butne kosti u pogledu s ventralne strane;
Slika 2 prikazuje poprečni presjek duž crte A-A sa slike 1 kroz proksimalni dio tijela;
Slika 3 je lateralni pogled na gornji dio proteze butne kosti;
Slika 4 je medijalni pogled na gornji dio proteze butne kosti; i
Slika 5 je pogled u perspektivi na gornji dio proteze butne kosti.
Slike 1 do 5 prikazuju gornji dio butne endoproteze zgloba kuka u različitim pogledima. Gornji dio proteze ima tijelo 2, čiji se poprečni presjek od distalnog kraja 8 tijela stalno povećava do proksimalnog kraja 12. Poprečni presjek tijela 2 je po cijeloj dužini prilagođen prirodnom ljudskom femuru kosti. Poprečni presjek je u distalnom području 4 tijela skoro kružnog oblika, a u proksimalnom području 6 tijela poprima ovalni oblik koji se proteže u medijalno-lateralnom smjeru, dakle u medijalno-lateralnom smjeru se više povećava nego u ventralno-dorzalnom smjeru.
Na proksimalnom kraju 12 tijela 2 nedostaje oslanjajuća ogrlica, ali je predviđen lateralni naslon koji povećava rotacijsku stabilnost, naročito odmah nakon postupka implantiranja.
Na proksimalnom kraju 12 tijela je oblikovan nasadni konus koji je usmjeren pod tzv. CCD-kutom u odnosu na uzdužnu os 3 tijela. Nasadni konus 16 služi za to da se glava proteze (nije prikazana) natakne s odgovarajućim konusnim udubljenjem i time se može tijesno i čvrsto učvrstiti na gornjem dijelu butne proteze. Međutim, alternativno je moguće da se na proksimalnom kraju tijela jednodijelno oblikuje vrat proteze s glavom proteze, u produžetku.
Distalni segment 4 tijela - s unaprijed određenom dužinom je ventralno nagnut ili savijen da bi se na taj način omogućilo bolje prilagođenje oblika na šuplji prostor kosti ljudskog femura, a time ostvarila i mogućnost jednostavnije implantacije. Distalni segment 4 tijela dalje ima prema distalnom kraju 8 tijela lateralno klinasto oblikovanu površinu 10 koja kružni poprečni presjek prema dolje reducira u segmente kružnih površina.
U proksimalnom području tijela 2 je ventralno i dorzalno oblikovana ravna osnovna površina 20, a prema proksimalnom kraju 12 tijela povećava se poprečni presjek tijela u ventralno-dorzalnom smjeru u više stepenika 22, 28, 32 koji s unaprijed određenom visinom naliježu s distalno usmjerenim stepenastirn rubom 24, 30, 34 i tada se prema proksimalnom kraju 12 tijela povećavaju u njihovoj medijalno-lateralnoj širini. Na ventralnoj strani i dorzalnoj strani tijela 2 je postavljen bar po jedan stepenik 22. U prikazanom obliku realizacije se na ventralnoj strani i na dorzalnoj strani tijela nalazi uvijek više stepenika 22, 28, 32 koji su prema proksimalnom kraju pomaknuto raspoređeni jedan na drugom, a njihovi stepenasti rubovi su uvijek na udaljenosti od stepenastog ruba ispod njih i na taj način postižu stepenast porast poprečnog presjeka vratila. Svi stepenici idu do proksimalnog kraja 12 tijela, prvi stepenik 22 se distalno proteže najdalje, on ima najveću lateralno-medijalnu širinu. Drugi stepenik 28 se distalno proteže kraće, medijalno-lateralna širina je reducirana u odnosu na prvi stepenik. Treći stepenik 32 se distalno još kraće širi, njegovo lateralno-medijalno širenje je manje od istog kod drugog stepenika 28 koji prednjači. Svi stepenici u prikazanom obliku relaizacije imaju konstantnu visinu rubova, a stepenaste površine su ravne. Kao što se osobito može vidjeti na slikama 3 i 4, stepenaste površine su time međusobno paralelne. Stupnjevanje na ventralnoj strani odgovara kao zrcalna slika stupnjevanju na dorzalnoj strani tijela 2.

Claims (16)

1. Gornji dio butne endoproteze zgloba kuka, s tijelom čiji se poprečni presjek od distalnog kraja tijela prema proksimalnom kraju tijela kontinuirano povećava, i u proksimalnom području tijela se širi ovalno u medijalno/lateralnom smjeru, naznačen time, što je prema proksimalnom kraju (12) tijela rast poprečnog presjeka u ventralno/dorzalnom smjeru u više stepenika (22, 28, 32), čija se medijalno lateralna širina povećava od distalnog stepenastog ruba (24) prema proksimalnom kraju (12) tijela.
2. Gornji dio butne endoproteze zgloba kuka, prema zahtjevu 1, naznačen time, što se na ventralnoj strani i dorzalnoj strani tijela (2) nalazi bar jedan stepenik (22).
3. Gornji dio butne endoproteze zgloba kuka, prema zahtjevima l ili 2, naznačen time, što je na ventralnoj strani tijela (2) uvijek više stepenika (22, 28, 32) prema proksimalnom kraju (12) tijela (2) pomaknuto raspodijeljeno jedan prema drugom i što je rub (29) sljedećeg višeg stepenika (28) uvijek na udaljenosti od ruba (23) neposredno višeg stepenika (22).
4. Gornji dio butne endoproteze zgloba kuka, prema jednom od prethodnih zahtjeva, naznačen time, što je visina rubova pojedinačnih stepenika (22, 28, 32) konstantna po stepeniku.
5. Gornji dio butne endoproteze zgloba kuka, prema jednom od prethodnih zahtjeva, naznačen time, što je visina rubova svih stepenika (22, 28, 32) jednaka.
6. Gornji dio butne endoproteze zgloba kuka, prema jednom od zahtjeva 1 do 3, naznačen time, što se visina rubova pojedinačnih stepenika u proksimalnom smjeru kontinuirano povećava.
7. Gornji dio butne endoproteze zgloba kuka, prema jednom od prethodnih zahtjeva, naznačen time, što su gornje površine stepenika (22, 28, 32) ravne.
8. Gornji dio butne endoproteze zgloba kuka, prema jednom od prethodnih zahtjeva, naznačen time, što su gornje površine ventralnih stepenika (22, 28, 32) paralelne s gornjim površinama dorzalnih stepenika (22, 28, 32).
9. Gornji dio butne endoproteze zgloba kuka, prema jednom od prethodnih zahtjeva, naznačen time, što je u proksimalnom području tijela (2) ventralno i dorzalno oblikovana po jedna ravna površina (20) na koju je postavljen prvi stepenik (22).
10. Gornji dio butne endoproteze zgloba kuka, prema zahtjevu 9, naznačen time, što su ventralne i dorzalne osnovne površine (20) međusobno paralelne.
11. Gornji dio butne endoproteze zgloba kuka, prema jednom od prethodnih zahtjeva, naznačen time, što je na proksimalnom kraju (12) tijela (2) predviđen nasadni konus (16) za učvršćivanje glave proteze.
12. Gornji dio butne endoproteze zgloba kuka, prema jednom od zahtjeva l do 10, naznačen time, što je na proksimalnom kraju (12) tijela (2) raspoređen vrat proteze s oblikovanom glavom proteze.
13. Gornji dio butne endoproteze zgloba kuka, prema jednom od prethodnih zahtjeva, naznačen time, stoje proksimalnom području tijela (2) lateralno oblikovan oslonac (14).
14. Gornji dio butne endoproteze zgloba kuka, prema jednom od prethodnih zahtjeva, naznačen time, što poprečni presjek tijela (2) prema distalnom kraju prelazi u kružni poprečni presjek.
15. Gornji dio butne endoproteze zgloba kuka, prema jednom od prethodnih zahtjeva, naznačen time, što je u distalnom području (4) tijela (2) lateralno predviđena klinasta površina (10).
16. Gornji dio butne endoproteze zgloba kuka, prema jednom od prethodnih zahtjeva, naznačen time, što je distalni segment (4) tijela savijen u ventralnom smjeru.
