Abstract
1334560 Ligating devices M E J VAN HOORN 25 Nov 1971 [11 Dec 1970 1 June 1971] 54798/71 Heading A5R A surgical instrument for ligating internal structures, e.g. haemorrhoids, in a body cavity comprises two hollow tubes 1, 2 slidable one inside the other; a handle 4 secured to one of tubes 1, 2; a trigger 3 on the handle 4 for producing relative movement of the tubes 1, 2; an auxiliary member, Fig. 3, which is inserted into the normally protruding front end 14 of the inner tube 2 for placing an elastic cord 29 thereon, but is thereafter removed; and an obtivator bar, Fig. 4, which is inserted into the tubes 1, 2 to assist insertion of the device, but is thereafter removed. The handle 4 may, as shown, be attached to the outer tube 1, with the trigger 3 connected to the inner tube 1 directly, or by a rod (40), Fig. 9 (not shown). Alternatively, Fig. 11 (not shown), the trigger (76) causes retraction of a stop (72) against which the inner tube (2) is normally rearwardly biased by a spring (70). The handle 4 may be removably attached to the outer tube 2, Figs. 12, 14 (not shown) by a dovetail tongue-and-groove arrangement. The handle 4 may be attached to the inner tube 2, Fig. 10 (not shown). The body cavity may be illuminated by a light source (20), Fig. 2 (not shown) carried in a side tube 17 attached to the outer tube 1. The inner tube 2 is then either transparent, Fig. 6 (not shown) or has a slot 19 adjacent the side tube 17, and a polished inner surface. Alternatively, Fig. 8 (not shown) the side tube 17 may be mounted on a sleeve (38) which is locked or screwed into the rear end of the inner tube 2. A bundle of optical fibres passes through the side tube (17) and through a flexible tube (39) which is connected thereto and to an intense light source. The instrument may carry a connection (67), Fig. 15 (not shown) to a vacuum pump. One tube may be conical or flared, with an oval cross-section, Figs. 9A and 10A (not shown).
