A gear for a surgical hand motor unit comprises an input shaft 2 driving directly through a central shaft 1 a first output coupling element 3 and driving through three sun-ring planet trains 5, 6, 7 arranged in series around the shaft 1 a second output coupling element 8 at a reduced speed. Various surgical instruments are connected to the end 9 of the gear housing 24 and have coupling elements which engage with the coupling elements 3 or 8 as appropriate. Each planet train has a driving sun gear 4, 30, 47 a driven carrier 26, 32, 36 and a fixed ring gear 27, 33, 37. The carriers 26, 32 of the first and second trains are fast with the suns 30, 47 of the second and third trains respectively and the third carrier 36 is fast with the coupling element 8. The input shaft 2 is driven from an electric or pneumatic motor (18) which screws into the housing 24.
