Abstract
Description
La présente invention concerne une prothèse totale du genou, et une gamme d'éléments modulaires permettant l'obtention de cette prothèse. Une prothèse totale du genou comprend classiquement un implant fémoral et un implant tibial en métal, et un plateau intermédiaire en un matériau favorisant le glissement, notamment en polyéthylène à haute densité. Une telle prothèse comprend également fréquemment un implant rotulien destiné à être fixé sur la rotule osseuse après résection adéquate de cette dernière. L'implant fémoral présente une forme recourbée apte à envelopper l'extrémité inférieure d'un fémur, réséquée en conséquence ; il comprend une paroi antérieure formant la partie supérieure d'une trochlée prothétique, et deux branches recourbées s'étendant depuis le bord inférieur de cette paroi antérieure, ces deux branches délimitant entre elles la partie inférieure de la trochlée prothétique et formant des surfaces condyliennes. L'implant tibial forme quant à lui une paroi supérieure de réception dudit plateau 15 intermédiaire. L'implant rotulien est fréquemment constitué par une pièce en polyéthylène ancrée sur la rotule osseuse dûment réséquée, de resurfaçage de cette rotule osseuse. Il s'avère qu'avec de nombreuses prothèses existantes, il est nécessaire de 20 resurfacer la rotule osseuse au moyen d'un implant rotulien. En effet, à défaut d'un tel resurfaçage, l'articulation prothétique aurait une cinématique anormale et présenterait des pics de contraintes, et le patient serait exposé à des douleurs et il se produirait une usure anormalement rapide de la rotule osseuse. Un autre inconvénient des prothèses existantes est la génération d'un noyau 25 fibreux à la jonction du tendon quadricipital et de la rotule osseuse (phénomène connu sous la dénomination "clunk syndrom"). La présente invention a pour objectif de remédier à ces inconvénients essentiels, en fournissant une prothèse de genou dans laquelle une rotule osseuse ou un implant rotulien peut coopérer dans les meilleurs conditions avec l'implant 30 fémoral prothétique, et dans laquelle le phénomène de génération d'un noyau fibreux à la jonction du tendon quadricipital et de la rotule osseuse est évité. Un autre objectif de l'invention est de fournir une gamme d'éléments modulaires permettant, à partir des mêmes éléments de base, l'obtention d'une telle prothèse, soit non postéro-stabilisée, c'est-à-dire implantée avec conservation du ligament croisé postérieur, soit postéro-stabilisée de première intention, soit postéro-stabilisée de reprise. La prothèse concernée inclut, de manière connue en soi, un implant fémoral comprenant une paroi antérieure qui forme la partie supérieure d'une trochlée prothétique, et deux branches recourbées s'étendant depuis le bord inférieur de cette paroi antérieure, délimitant entre elles la partie inférieure de la trochlée prothétique et formant des surfaces condyliennes. Selon l'invention, - la trochlée présente, vue en coupe transversale, une forme en "aile de 10 mouette", c'est-à-dire présente un sillon médian bordé par deux surfaces convexes arrondies ; - la trochlée présente, entre son extrémité antérieure et son extrémité postérieure, la forme évolutive suivante : - dans sa portion supérieure, c'est-à-dire sa portion coopérant avec la rotule 15 pour des flexions de l'articulation allant de 0° à environ 50° environ, la trochlée a une profondeur inférieure à la hauteur de la nervure médiane longitudinale que comprend la rotule par rapport aux faces latérales concaves bordant cette nervure et a une largeur supérieure à celle de cette même nervure, de telle sorte que la rotule vienne en contact avec l'implant 20 fémoral essentiellement par cette nervure et que lesdites faces latérales concaves soient situées à distance desdites surfaces convexes arrondies ; ladite profondeur va en augmentant progressivement en direction de la portion inférieure de la trochlée, jusqu'à correspondre à la hauteur de ladite nervure au niveau de la jonction de ladite portion supérieure de la trochlée 25 avec ladite portion inférieure de cette trochlée, et ladite largeur va en se réduisant progressivement en direction de ladite portion inférieure de la trochlée, jusqu'à correspondre, au niveau de cette même jonction, à la largeur de ladite nervure ; - dans sa portion inférieure, c'est-à-dire la portion de la trochlée coopérant 30 avec la rotule pour des flexions de l'articulation allant de 50° environ à 120° environ, la trochlée a une profondeur supérieure à la hauteur de ladite nervure par rapport auxdites faces latérales concaves et a une largeur sensiblement égale à celle de cette même nervure, de telle sorte que la rotule vient en contact avec l'implant fémoral essentiellement par lesdites faces latérales concaves, au niveau desdites surfaces convexes arrondies, et que ladite nervure soit à distance du fond de la trochlée. Il se comprend que le terme "rotule" employé ci-dessus recouvre à la fois la rotule osseuse native, lorsque celle-ci-est conservée telle quelle, et un implant rotulien de resurfaçage, lorsqu'un tel implant est utilisé. Ainsi, dans la prothèse selon l'invention, il se réalise un transfert progressif de la surface de la rotule venant en contact avec l'implant fémoral, ce transfert se faisant depuis la nervure centrale, qui est essentiellement active au niveau de ladite portion supérieure de la trochlée, vers lesdites faces latérales concaves, qui sont actives au niveau de ladite portion inférieure de la trochlée. Il résulte de cette structure que la rotule a, dans ladite portion supérieure de la trochlée, une possibilité de translation transversale et de basculement par rapport à la trochlée dans le plan transversal, rendue possible par l'espace existant entre lesdites faces latérales concaves et lesdites surfaces convexes arrondies. Cette possibilité de translation transversale et de basculement s'avère tout à fait favorable à une coopération optimale de la rotule avec l'implant fémoral et donc à un fonctionnement optimal de l'articulation prothétique. Dans ladite portion inférieure de la trochlée, cette possibilité de translation et de basculement est supprimée par la venue desdites faces latérales concaves en appui contre lesdites surfaces convexes arrondies, ce qui est également tout à fait favorable à une coopération optimale de la rotule et de l'implant fémoral. De plus, cette venue desdites faces latérales concaves en appui contre lesdites surfaces convexes arrondies permet de tenir la zone de jonction du tendon quadricipital et de la rotule à distance du bord inférieur de ladite portion antérieure de l'implant fémoral ; il en résulte que la génération d'un noyau fibreux au niveau de cette zone de jonction est fortement réduit, voire éliminé, et que la nervure de la rotule se trouve également à distance de ce bord inférieur et ne risque donc pas de subir un effet de râpe produit par ce bord inférieur. En effet, ce bord inférieur peut être relativement agressif et s'avère avoir une incidence forte sur ladite génération d'un noyau fibreux et sur l'usure de la rotule. De préférence, la prothèse comprend un l'implant rotulien incluant une embase destinée à être fixée à la rotule osseuse, dûment réséquée, et un élément de glissement destiné à être monté sur cette embase, avec possibilité de pivotement par rapport à celle-ci ; cet élément de glissement forme, sur sa face destinée à venir au contact de l'implant fémoral, une nervure longitudinale et deux faces latérales concaves situées de part et d'autre de cette nervure, reproduisant les nervure et faces homologues d'une rotule osseuse, la nervure présentant une forme arrondie vue dans un plan transversal, et ses flancs s'infléchissant de manière à se raccorder avec continuité auxdites faces latérales concaves ; ladite nervure et lesdites faces latérales concaves présentent également, vues dans un plan sagittal, une courbure concave correspondant sensiblement à la courbure de la trochlée dans ce même plan sagittal. De préférence, ladite paroi antérieure de l'implant fémoral forme un fond étendu de la trochlée, c'est-à-dire s'étend en direction de la partie inférieure de la trochlée de telle manière que le bord inférieur de cette paroi antérieure se trouve au niveau de la portion de la trochlée coopérant avec la rotule pour une flexion de l'articulation d'environ 60° à 65°. En d'autres termes, la trochlée présente un fond s'étendant au-delà de la zone précitée des 50° de flexion, à partir de laquelle ladite nervure perd son contact avec ce fond ; il en résulte que, lorsque, au cours d'un mouvement d'extension de l'articulation, le bord inférieur de ladite paroi antérieure vient au niveau de ladite nervure, cette nervure se trouve à une distance de ce bord suffisante pour éliminer tout risque d'effet de râpe ; c'est seulement ensuite, vers 50° de flexion, que ladite nervure viendra à nouveau en contact avec le fond de la trochlée, alors qu'elle aura été majoritairement dépassée par ledit bord inférieur. De préférence, l'implant rotulien comprend des moyens de limitation du pivotement de l'élément de glissement par rapport à ladite embase, empêchant le pivotement de cet élément de glissement au-delà d'une vingtaine de degrés de part et d'autre de la position neutre dans laquelle ladite nervure est parallèle à la trochlée. The depth of the trochlea 12 is gradually increasing towards the lower portion of the trochlea (see FIG. 8), until it corresponds to the height of the rib 22 towards 50 ° of flexion, and the width of this trochlea is gradually reducing towards the lower portion of the trochlea, to correspond to the width of the rib 22 at the same area of the 50 ° flexion. In its lower portion, that is to say, its portion cooperating with the sliding element 23 for bending of the joint from about 50 ° to about 120 ° (see FIGS. 9 to 11), the trochlée 12 has a depth becoming greater than the height of the rib 22 relative to the concave side faces 23 and has a width substantially equal to that of the same rib 22. The sliding element 21 thus comes into contact with the femoral implant 2 essentially by the concave side faces 23, at the condylar faces 11a, and the rib 22, between 50 ° and 60 to 65 °, is at a distance from the bottom of the trochlée 12 (see Figure 9). The aforementioned possibility of transverse translation and tilting, in addition to the possibility of pivoting of the sliding element 21 relative to the base 20, is entirely favorable to optimal cooperation of the patellar implant 3 with the femoral implant 2 and therefore to an optimal operation of the prosthetic joint. In addition, the rib 22 is, between 50 ° and 65 ° of bending, at a distance from the bottom of the trochlée 12 and therefore not likely to undergo a rasp effect that might otherwise produce the lower edge 10a, this edge lower can be relatively aggressive and be a source of wear of this rib.
S'agissant des autres pièces dudit ensemble d'éléments, l'implant tibial 4, également en matériau métallique, comprend un plot inférieur creux 24, d'insertion dans le canal médullaire d'un tibia et une plaque supérieure 25 destinée à recevoir le plateau intermédiaire de glissement 5. Outre le plot 15, il forme deux surfaces latérales articulaires 5a, de coopération avec les surfaces condyliennes 11 a, et comprend une tige inférieure 27 destinée à être reçue de façon ajustée dans le puits que forme intérieurement le plot inférieur 24 de l'implant tibial 4, pour permettre le pivotement guidé du plateau 5 par rapport à l'implant 4. La cage intercondylienne 6 est destinée à être montée de manière amovible sur l'implant fémoral 2, au moyen de deux pattes latérales qu'elle comprend et de deux vis (non représentées, ou, alternativement de deux plots d'ancrage 7. Lesdites pattes latérales sont destinées à être reçues dans des empreintes correspondantes que présente la face distale 2a de l'implant fémoral 2, et les plots d'ancrage 7 comprennent des portions inférieures filetées aptes à être engagées au travers de trous aménagés dans lesdites pattes latérales puis dans des trous taraudés débouchant dans le fond desdites empreintes. La cage intercondylienne 6 est dimensionnée de manière à être apte à loger la partie supérieure du plot de postéro-stabilisation 15. Son extrémité antérieure présente un évidement d'emboîtement 28 apte à permettre l'emboîtement de cette extrémité antérieure sur la proéminence 13. Cet emboîtement permet de facilement et rapidement positionner la cage intercondylienne 6 sur l'implant fémoral 2, laquelle est parfaitement immobilisée par rapport à l'implant fémoral 2 et peut être utilisée comme une embase solide pour le montage d'une tige médullaire 30 d'ancrage au fémur, telle que celle visible sur les figures 12 et 13. Comme cela apparaît sur la figure 1, la paroi supérieure de la cage 6 comprend une série de trois trous sécants alignés dans la direction antéro-postérieure, bordée de deux séries latérales de trois petits trous ; la tige médullaire d'ancrage 30 présente une face inclinée au niveau de son extrémité destinée à être raccordée à la cage 6, et comprend un trou taraudé central débouchant dans cette face et deux petits plots latéraux 30a faisant saillie de cette face, diamétralement opposés ; ce trou taraudé central est destiné à recevoir une vis engagée dans l'un ou l'autre des trois trous sécants que comprend la cage 6 de manière à assurer la fixation de la tige 30 dans l'une des trois positions ainsi rendue permise dans la direction antéropostérieure, et lesdits petits plots latéraux 30a sont destinés à être engagés dans deux des trous desdites séries latérales de petits trous, de manière à permettre un positionnement réversible de la tige 30 et donc une utilisation d'une même tige pour une prothèse droite ou gauche. En référence aux figures 12 et 13, il apparaît que la prothèse 1 de reprise peut être obtenue à partir du même implant fémoral 2 et du même implant tibial 4 que décrits précédemment. Dans ce cas, sont utilisés les autres éléments suivants que comprend la gamme d'éléments selon l'invention : - une deuxième cage intercondylienne 31, de hauteur supérieure à celle de la cage intercondylienne 6, et dont l'extrémité antérieure présente un évidement d'emboîtement 32 apte à permettre l'emboîtement de cette extrémité antérieure sur la proéminence d'emboîtement 13 ; cette deuxième cage intercondylienne 31 comprend la même série de trois trous sécants alignés dans la direction antéropostérieure, et les mêmes séries latérales de trois petits trous que la cage 6 ; - un deuxième plateau intermédiaire 33 comprenant un plot de postérostabilisation 34 ayant une hauteur supérieure à celle du plot 15 ; - une tige médullaire tibiale 35, comprenant un embout conique 35a apte à être 10 inséré étroitement dans la cavité conique distale du plot 24 ; - deux manchons 36 à portions de base évasées ou conique, aptes à être engagés sur les tiges médullaires 30, 35 respectives ; - des blocs de rehausse 37, pouvant être fixés sur une ou plusieurs des faces internes de l'élément fémoral 2 afin de combler un éventuel déficit osseux.With regard to the other parts of said set of elements, the tibial implant 4, also made of metallic material, comprises a hollow lower pad 24 for insertion into the medullary canal of a tibia and an upper plate 25 intended to receive the intermediate slip tray 5.
