Abstract
La présente invention concerne un instrument endoscopique dont la tige comporte une partie pouvant être cintrée. Le cintrage est rendu possible par des découpes transversales (15) qui s'étendent sur pratiquement la circonférence complète de la tige et qui définissent des segments (16) pouvant pivoter l'un par rapport à l'autre. Pour accroître la stabilité de la tige, les segments (16) sont munis, à leurs extrémités libres, de guides (21) qui enjambent une découpe voisine (15) et qui épousent la forme du contour intérieur de la tige. Des éléments d'accouplement (18, 19) s'engageant l'un dans l'autre sont, en outre, intégrés dans les parties latérales de la tige, pour empêcher l'apparition de contraintes excessives dans la tige lors d'un dépassement de sa position droite redressée. La présente invention concerne un instrument endoscopique cintrable, comportant une tige qui présente, dans sa partie cintrable, des découpes transversales subdivisant cette dernière en segments et s'étendant sur un secteur angulaire de plus de 270 , ledit instrument comportant, en outre, un moyen de traction s'étendant en direction longitudinale au- dessus 6 des découpes et transversalement à celles-ci, pour la commande du cintrage
depuis l'extrémité proximale de l'instrument.
Un instrument de ce type général est connu, par exemple, d'après le document de brevet britannique GB 2130885 A. Dans l'instrument endoscopique décrit dans ce document, la tige présente, dans sa partie d'extrémité distale cintrable, une multiplicité de découpes transversales, qui 12 s'étendent sur pratiquement la circonférence totale de la tige et subdivisent ladite partie d'extrémité en segments individuels, reliés entre eux par des barrettes faisant partie de la tige, qui subsistent aux extrémités des découpes. A l'opposé de ces barrettes, est disposé un fil métallique de commande qui s'étend le long de la tige, au-dessus des découpes et transversalement à celles-ci, et qui est fixé, d'une part, à l'extrémité distale 18 de l'instrument et, d'autre part, à un organe de commande prévu sur une pièce de préhension disposée du côté proximal. A l'aide de cet organe de commande, on peut modifier la longueur utile du fil métallique de traction, grâce à quoi la partie distale de la tige peut être plus ou moins cintrée. Les découpes transversales procurent alors les espaces de dégagement nécessaires pour rendre possible un cintrage dans une tige par ailleurs 24 relativement rigide. Ce principe est connu dans le cas des instruments endoscopiques, d'après également la demande de brevet européen
EP 0 521 595 A2 ou le brevet américain US 4 911 148, par exemple.
La caractéristique commune à ces réalisations est que la tige devient instable lorsque sa liberté de cintrage croît, c'est-à-dire lorsque le nombre des découpes transversales augmente et que le secteur angulaire couvert par les découpes transversales augmente également. Il existe alors le risque d'un fléchissement sur le côté, c'est-à-dire en dehors du plan de cintrage prescrit. Il existe, en plus de cela, le risque de voir la tige s'incurver au-delà de sa position droite redressée dans l'autre direction, c'est-à-dire à l'opposé de la direction de cintrage prescrite. Il se produit alors, en général, des déformations permanentes dans les régions des barrettes de la tige,

déformations qui peuvent conduire à brève échéance à une rupture de la tige. Enfin, de telles tiges présentent encore aussi une faible résistance à la torsion. Un inconvénient majeur supplémentaire, que présentent en elles-mêmes toutes les tiges flexibles, est en outre à voir dans le fait que, si 6 le degré du cintrage peut être réglé par immobilisation du moyen de traction, une tige ainsi cintrée n'en reste pas moins flexible en elle-même, c'est-à-dire peut subir des pivotements à l'intérieur de la partie cintrée qui est immobilisée, de telle sorte qu'il en résulte, par exemple, des tronçons plus fortement cintrés et des tronçons moins fortement cintrés. Il serait pourtant souvent souhaitable, dans des utilisations sur des endoscopes, 12 d'avoir une tige flexible ou cintrable qui, une fois le cintrage effectué, se
comporte comme un tube rigide recourbé.
Compte tenu de cet état de la technique, l'invention s'est fixé pour but la réalisation, à l'aide de moyens structurellement simples, d'un instrument endoscopique cintrable qui soit d'une conception telle, que les inconvénients précités soient dans une large mesure éliminés et qu'un 18 instrument intrinsèquement le plus rigide possible soit procuré. Dans un développement de l'invention, il convient que cet instrument puisse être immobilisé dans chaque position de cintrage choisie d'une façon telle, qu'il
se comporte comme un tube rigide recourbé.
Conformément à la présente invention, ce but est atteint, dans un instrument endoscopique cintrable du type spécifié en introduction, grâce 24 au fait que: - la tige présente, au moins dans sa partie cintrable, une section transversale aplatie d'un côté et assume, dans la partie aplatie, une fonction d'articulation, et - le moyen de traction est disposé, sur le côté opposé à la partie aplatie, dans des guides qui épousent la forme du contour intérieur de la tige dans cette région, qui enjambent chacun une découpe transversale respective et qui sont reliés rigidement à un segment, d'un seul côté de
chacune des découpes qu'ils enjambent.
Selon la présente invention, il est donc prévu des guides, qui épousent la forme de la section transversale de la tige dans la région o ils sont placés et qui sont dimensionnés dans le sens de la longueur, d'une

façon telle qu'ils enjambent chacun une découpe transversale respective. Ils ne sont alors reliés rigidement qu'à un seul segment, si bien que l'on obtient de la sorte un guidage de type télescopique, qui accroît dans une très large
mesure la rigidité en torsion de la tige.
L'invention prévoit, par ailleurs, d'intégrer dans la tige, entre 6 des segments voisins, des éléments d'accouplement s'engageant l'un dans l'autre, qui limitent l'angle de pivotement de ces derniers, en particulier dans la direction de dépassement de redressement. Ces éléments d'accouplement s'engageant l'un dans l'autre, qui sont disposés de préférence des deux côtés de la tige entre les guides du moyen de traction et la région aplatie de la tige, doivent être situés le plus près possible des guides, c'est-à-dire le plus 12 loin possible de la région aplatie, pour offrir une sécurité la plus élevée possible contre un fléchissement dépassant le cintrage prescrit autorisé. Les éléments d'accouplement s'engageant l'un dans l'autre sont, de préférence, définis par les découpes transversales elles-mêmes, ce qui assure un gain de place et abaisse en même temps les coûts, quand les découpes transversales
sont par exemple réalisées par découpage au laser ou usinage par étincelage.
18 Conformément à la présente invention, la tige peut être dotée d'un profil en coupe transversale qui ne présente pas la forme classique circulaire, mais est sensiblement ovale et aplati d'un côté. La partie aplatie du profil forme alors, au voisinage des découpes transversales, les points d'articulation. Par le choix de ce profil de la tige, on n'obtient pas cependant que des avantages sur le plan de la stabilité, mais aussi des avantages de 24 disposition spatiale. Comme l'instrument que l'on doit introduire à travers la tige présente en général une section transversale circulaire, on peut utiliser la région convergeant en pointe à l'opposé de la partie aplatie pour y disposer, avec une économie de place, le moyen de traction. Ce moyen de traction, qui doit de toute façon être guidé à l'intérieur de la tige, est selon la présente invention monté, dans cette région, dans des guides qui assurent non seulement le guidage du moyen de traction, mais aussi le guidage
mutuel des segments formés par les découpes transversales.
De préférence, les éléments d'accouplement s'engageant l'un dans l'autre sont constitués par des structures en forme de crochet formées dans la paroi latérale d'un segment, ainsi que par des évidements ayant un contour de forme correspondante, avec un certain jeu, ménagés dans la

paroi latérale du segment voisin. Une telle saillie en forme de crochet peut également être prévue des deux côtés de chacun des éléments d'accouplement s'engageant l'un dans l'autre, pour pouvoir absorber de plus
grands efforts.
Pour permettre d'obtenir un transfert de forces avec un 6 minimum de contraintes dans la barrette qui est formée entre deux segments par la partie aplatie de la tige et qui assume une fonction d'articulation, les découpes sont dotées d'une configuration se terminant sous la forme d'un T, en projection, des deux côtés de la partie aplatie de la tige, afin qu'il se forme non pas un simple point d'inflexion, mais une zone étendue de flexion, et qu'on soit assuré que la flexion de cette zone, dans le cas d'un
12 cintrage de l'instrument, reste dans le domaine des déformations élastiques.
De préférence, la découpe est formée avec un retrait à hauteur du point de croisement du T, pour éviter une concentration des contraintes au niveau de
ce point.
Les guides, qui garantissent une résistance élevée à la torsion de la tige, y compris dans la partie cintrable de celle-ci, sont de préférence 8 réalisés en tôle, sous forme d'éléments façonnés par matriçage et/ou pliage, et sont reliés chacun au segment associé par un soudage par points. Par soudage par points, il faut comprendre, au sens de la présente invention, qu'il s'agit non pas du soudage ponctuel par résistance, mais d'une liaison essentiellement ponctuelle par la matière même, comme celle qui peut être
produite par exemple par bombardement électronique ou soudage au laser.
24 Cela étant, la liaison peut, bien entendu, se faire également sur toute la surface ou uniquement dans la région des bords, suivant les techniques de fabrication actuelles, mais la liaison ponctuelle donne cependant satisfaction, en particulier sur le plan de la rentabilité économique et à l'égard des exigences concernant la solidité. De même, les guides peuvent,
pour un coût avantageux, être découpés dans un matériau profilé.
De préférence, les guides servent non seulement à guider les segments dans la région de leur extrémité libre, mais simultanément aussi à guider le moyen de traction. Il n'est pas absolument nécessaire que le moyen de traction soit guidé directement dans les guides, et il peut aussi être guidé à travers un espace libre subsistant entre les guides et la tige. Par ailleurs,

ces guides peuvent être utilisés pour guider non seulement le moyen de
traction, mais aussi par exemple des fibres optiques.
Les dispositions mentionnées ci-dessus, concernant en particulier le profilage de la tige sous une forme ovale et aplatie d'un côté, les guides disposés du côté de la tige opposé au côté aplati, ainsi que les 6 éléments d'accouplement s'engageant l'un dans l'autre disposés latéralement entre ces régions, procurent chacune, prises individuellement, un accroissement de la stabilité de la tige. Toutefois, ces dispositions seront, de préférence, mises en oeuvre en combinaison, pour non seulement améliorer des caractéristiques individuelles, comnme par exemple la résistance à la torsion ou la résistance à la flexion, mais aussi pour renforcer la stabilité
12 d'ensemble de la tige.
La commande du cintrage se fait par l'intermédiaire du moyen de traction, qui est fixé, d'un côté, à l'extrémité distale de la tige et, du côté proximal, à un organe de commande, lequel est guidé en translation dans une pièce de préhension reliée à la tige. Cet organe de commande présente, de préférence, une denture extérieure qui engrène avec le filetage intérieur 18 d'un écrou moleté monté en rotation dans la pièce de préhension. Le cintrage ou l'extension de l'instrument peut se faire progressivement par une simple rotation de l'écrou dans la pièce de préhension, un autoblocage dépendant de la construction ayant alors lieu, ce qui signifie qu'un angle réglé ne peut pas être modifié par des influences en provenance de
l'extrémité distale de l'instrument.
24 Si l'angle peut être déterminé une fois pour toutes de la manière qui vient d'être décrite, la position angulaire relative des segments individuels reste cependant variable, de sorte que la partie cintrable de la tige présente une certaine flexibilité, qui n'est pas toujours souhaitée. Pour mettre également cette partie cintrée de la tige sous une configuration rigide une fois que le réglage de l'angle de cintrage a été effectué, la présente invention prévoit d'utiliser un tube tendeur, qui bande les segments pouvant pivoter l'un par rapport à l'autre. Pour effectuer ce bandage, on pousse un tube, qui entoure la tige et qui couvre la région s'étendant entre la pièce de préhension et la partie cintrable, suffisamment loin en direction distale pour qu'un bandage s'opposant à l'action du moyen de traction se produise. Ce

tube tendeur peut être immobilisé dans sa position de bandage et en être à
nouveau retiré en cas de besoin.
On va maintenant décrire plus en détails des exemples de réalisation, non limitatifs, de la présente invention, représentés sur les dessins annexés, dans lesquels: 6 la figure 1 est une vue de côté d'un instrument endoscopique dont l'extrémité de la tige est cintrée, la figure 2 est une vue en coupe longitudinale de l'instrument de la figure 1, dont l'extrémité de la tige est droite, la figure 3 est une représentation agrandie de l'extrémité distale cintrable de la tige de l'instrument représenté sur les figures 1 et 2, 12 placée dans une première position angulaire de cintrage, la figure 4 est une représentation, analogue à la figure 3, de l'extrémité de la tige placée dans une deuxième position angulaire de cintrage, la figure 5 est une représentation, à plus grande échelle, du détail V de la figure 4, 18 la figure 6 est une vue en coupe transversale de la tige, dans la partie cintrable de celle-ci, la figure 7 représente, dans une première position, l'extrémité distale cintrable de la tige avec la pince qui s'y trouve montée, la figure 8 représente l'instrument de la figure 7 dans une deuxième position, et 24 les figures 9 à 11 représentent deux segments voisins d'un autre mode de réalisation de la tige, suivant une vue de côté et dans trois
positions différentes.
L'instrument endoscopique représenté comporte une tige 1 qui, du côté proximal, est fixée dans une pièce de préhension 2. La tige 1 est d'une conception rigide, sauf à sa partie d'extrémité distale 3. La partie
d'extrémité distale 3 peut, au contraire, être cintrée.
Pour orienter la partie d'extrémité distale 3 cintrable dans un plan qui, dans les modes de réalisation représentés, est le plan du dessin, il est prévu un fil métallique de traction et de poussée 4, qui est guidé à l'intérieur de la tige 1 et qui est fixé à l'extrémité distale 5 de la tige, d'un côté, ainsi qu'à un curseur 6 monté déplaçable en translation sur la tige 1, à

l'intérieur de la pièce de préhension 2. Le curseur 6, guidé à l'intérieur de la pièce de préhension 2, comporte, sur sa face tournée vers l'extérieur, une denture qui engrène avec le filetage intérieur d'un écrou moleté 7 monté en rotation sur la pièce de préhension 2. Par une rotation de l'écrou moleté 7, le curseur 6 est déplacé en translation longitudinale le long de la tige 1, ce qui 6 permet de modifier la longueur utile du fil métallique 4 et de régler l'angle de cintrage dans la partie d'extrémité distale 3. Plus la longueur utile du fil métallique 4 est courte, plus le cintrage est important. Comme le fil métallique 4 peut transmettre non seulement des forces de traction, mais également des forces de poussée, la tige 1 peut être amenée, sans l'utilisation d'un ressort, non seulement dans sa position droite (figure 2), 12 mais aussi, au-delà de cette position, dans une position de dépassement de
redressement (figure 3).
Un endoscope flexible ou un instrument endoscopique flexible, par exemple une pince 8, peut être guidé à travers la tige creuse 1, comme cela est représenté à titre d'exemple sur les figures 7 et 8. Lorsque l'on met en oeuvre un instrument endoscopique, il est essentiel que la tige 1 18 reste la plus rigide possible, y compris dans sa partie d'extrémité cintrable 3, après cintrage de celle-ci, ceci afin que la pince flexible 8 ou un autre instrument puisse être dirigé avec précision quand, par exemple, un tel cintrage est nécessaire pour contourner un vaisseau sanguin ou un organe du corps humain. Pour en être assuré, on dispose sur la tige 1 un fourreau 9 déplaçable axialement sur cette dernière, qui constitue un tube tendeur. Ce 24 fourreau comporte, du côté proximal, une pièce de préhension 10, au moyen de laquelle il peut être déplacé axialement sur la tige 1. Pour immobiliser la tige 1, et en particulier sa partie d'extrémité cintrée 3, dans cette position angulaire, comme cela est représenté à titre d'exemple sur les figures 1, 3 et 4, on pousse le fourreau 9 aussi loin que possible en direction distale à l'aide de la pièce de préhension 10, grâce à quoi le tronçon proximal de la partie d'extrémité 3 est bandé à l'intérieur du tube tendeur 9, à l'encontre de la force exercée par le fil métallique 4. Grâce à cette disposition, la partie
d'extrémité 3 est pratiquement rigide dans chaque position angulaire choisie.
Plus particulièrement, si l'on dote la tige 1 et le tube tendeur 9 d'un profil approprié, comme celui qui est représenté sur la figure 6 pour la tige 1 et qui sera encore décrit plus en détails ci-après, le tube tendeur tient

de lui-même dans la position immobilisant la tige 1. Toutefois, on peut éventuellement prévoir, à titre d'assistance, un organe de blocage dans la pièce de préhension 10. Un ressort hélicoïdal de compression ou un autre organe élastique peut également être prévu entre la pièce de préhension principale 2 et la pièce de préhension 10, quand on souhaite que la partie 6 d'extrémité 3 soit automatiquement amenée dans une posture rigide. Il convient alors, pour permettre d'ajuster l'angle de cintrage au moyen de l'écrou moleté 7, de reculer préalablement le tube tendeur 9 à l'encontre de
la force du ressort.
En vue de les nettoyer, on peut démonter les composants précédemment mentionnés, en retirant une goupille de blocage 11 12 représentée sur la figure 2. On peut alors désassembler la pièce de préhension 2 pour en séparer les éléments avant et arrière, retirer l'écrou moleté 7 de l'élément avant de la pièce de préhension 2 et extraire la tige 1, à laquelle sont rattachés le fil métallique 4 et le curseur 6. On peut enlever
également le tube de bridage 9 en le tirant au-delà de l'extrémité distale 5.
La tige 1 présente une section transversale sensiblement ovale, 18 comme cela ressort de la figure 6. A un sommet de cette forme ovale, la tige 1 est aplatie, la zone aplatie étant désignée par 12, et elle est conformée en pointe au niveau du sommet opposé, cette partie en pointe étant quant à elle désignée par 13. Entre la zone aplatie 12 et la partie en pointe 13, s'étendent
les parties latérales 14 qui ont une courbure relativement peu prononcée.
Pour pouvoir cintrer la tige 1 dans la partie d'extrémité 3, il est 24 prévu de multiples découpes transversales circonférentielles 15 qui sont disposées l'une à la suite de l'autre dans la direction longitudinale de la tige et qui, dans le présent mode de réalisation, s'étendent sur un secteur angulaire d'environ 300 , c'est-à-dire sur la totalité de la circonférence de la tige à l'exception de la partie aplatie 12. De la sorte, sont formés des segments tubulaires 16 qui sont simplement reliés, dans la région des découpes transversales 15, par une barrette plate 17 subsistant après le découpage. Les découpes transversales 15 s'étendent, d'une façon essentiellement rectiligne, dans une direction transversale à l'axe longitudinal de la tige 1, tout en étant cependant dessinées sensiblement sous la forme d'un crochet dans les parties latérales 14, et plus précisément à proximité de la partie en pointe 13, afin que soient chaque fois formés, sur

des segments voisins 16, deux saillies en forme de crochet 18 et deux
évidements 19 ayant un contour correspondant.
Chaque saillie en forme de crochet constitue avec l'évidement associé 19, dans lequel elle est prise, un couple d'éléments d'accouplement s'engageant l'un dans l'autre. Ces éléments d'accouplement s'engageant l'un 6 dans l'autre assurent que la tige ne puisse pas s'incurver au- delà de son angle de cintrage maximum prescrit ou dépasser sa position droite redressée. Ceci est important pour protéger les barrettes 17, qui forment chacune une articulation pour les segments 16, contre des déformations permanentes et donc contre une rupture prématurée. C'est plus particulièrement dans la direction d'un dépassement de la position droite redressée (voir figure 3 et 12 figure 7) que ces barrettes 17 sont en danger. Dans cette situation, les faces intérieures des saillies en forme de crochet 18 forment, en liaison avec les faces, situées en regard, des évidements 19, une butée. Plus ces couples de surfaces sont placés loin de la partie en pointe 13 de la section transversale du segment, plus les forces, qu'ils peuvent absorber, sont élevées. Dans la direction inverse, c'est-à-dire dans la direction prescrite du cintrage (voir 18 figures 4 et 8), ce sont les bords des découpes 9 dans la partie en pointe 13 qui remplissent la fonction de butée, mais celle-ci peut encore, dans cette direction également, être renforcée par les saillies en forme de crochet 18 en
liaison avec les évidements de contour correspondant 19.
Pour éviter un allongement excessif de la région de la barrette 17, une découpe transversale 15 n'aboutit pas directement sur la partie 24 aplatie 12, mais est au contraire réalisée à cet endroit sous la forme d'un T, de telle sorte que la barrette 17 soit agrandie dans la direction longitudinale de la tige 1. Grâce à cela, l'angle d'inflexion peut être augmenté, car la flexion se répartit sur une zone plus longue. Par ailleurs, dans cette région terminale en forme de T, la découpe 15 est d'une configuration telle, qu'il subsiste, à peu près au point de croisement du T, une région 20 de la tige qui fait saillie en direction de la découpe 15, et que, par conséquent, la découpe est en retrait dans cette région. Une accumulation appropriée de matière est ainsi obtenue au milieu de la barrette 17, ce qui permet de renforcer cette
région qui serait sinon une région particulièrement critique.
Pour stabiliser davantage la tige 1 dans sa partie cintrable, et en particulier pour augmenter sa résistance à la torsion, chaque segment 16

comporte, dans la légion en pointe 13 de sa section transversale, un guide 21 rigidement relié audit segment, guide qui enjambe la découpe transversale voisine 15. Dans le mode de réalisation représenté, les guides 21 sont formés par des tôles, qui épousent la forme du contour intérieur de la région en pointe 13, afin d'autoriser seulement le mouvement de cintrage 6 prescrit dans le plan du dessin. Comme le montre la représentation en coupe transversale de la figure 6, le guide 21 présente une section transversale ayant sensiblement la forme d'un triangle aplati, une découpe étant alors prévue dans la partie en pointe 13, découpe à travers laquelle le fil métallique 4 est guidé. A droite et à gauche de cette découpe et dans des positions voisines de celle-ci, sont prévues deux découpes plus petites, dans 12 chacune desquelles est guidé un câble optique. Les câbles optiques peuvent, par exemple, être des faisceaux de fibres non ordonnés pour la transmission d'une lumière d'éclairement ou également des faisceaux de fibres ordonnés pour la transmission d'images, ou peuvent encore être respectivement un faisceau de fibres optiques pour la lumière d'éclairement et un faisceau de fibres optiques pour la transmission d'images. L'espace libre qui subsiste à 18 l'intérieur de la tige 1 a une forme sensiblement circulaire, si l'on ne tient pas compte de l'aplatissement 12. Les guides 21 constituent donc non seulement un moyen de guidage pour les segments 16, mais en même temps aussi un moyen de guidage pour le fil métallique 4, qui est guidé, soit à
l'intérieur des guides 21, soit entre les guides 21 et la tige 1.
Les figures 9 à 11 montrent une variante de configuration de 24 deux segments 16', qui est représentée sur la figure 9 dans une position de cintrage maximum (correspondant sensiblement à un cintrage tel que celui qui est représenté sur les figures 4 à 8), sur la figure 10 dans la position droite redressée et, sur la figure 11, dans la position de dépassement de redressement (correspondant sensiblement à un dépassement de
redressement tel que celui qui est représenté à l'aide des figures 3 et 7).
Les segments 16' sont définis par des découpes transversales ', qui se distinguent des découpes transversales 15, précédemment décrites, par le fait qu'elles forment, dans la partie latérale, des saillies en forme de crochet 18' dirigées aussi bien vers la partie aplatie 12 que vers la partie en pointe 13. Les évidements 19' ont des contours de forme correspondante. Cette configuration des saillies 18' et des évidements 19', l 2738736 qui ressort de façon précise des figures, offre dans les deux directions (coudage excessif et dépassement de redressement) une protection particulièrement bonne contre les contraintes excessives. Ladite configuration est telle, que lors du cintrage maximum autorisé (figure 9), des segments voisins 16' s'appliquent l'un contre l'autre sur pratiquement la 6 totalité de la partie latérale 14, par leurs faces frontales tournées l'une vers l'autre. Dans l'autre position extrême (figure 11), dans laquelle il existe le risque d'un dépassement de redressement, deux saillies en forme de crochet 18' s'appliquent contre des parties saillantes correspondantes de chaque évidement 19', ce qui fait qu'un transfert de forces uniforme a lieu 12 des deux côtés de cet évidement. Dans toutes les positions intermnédiaires, les segments 16' peuvent par contre se mouvoir librement l'un par rapport à l'autre dans ces régions latérales. Les guides 21 ne sont pas représentés sur
les figures 9 à 11, pour rendre plus clair l'examen de ces figures.
Par ailleurs, on peut nettement voir sur les figures 9 à 11, à quel point la partie saillante 20 de la tige est avantageuse à l'égard de la
18 sollicitation des barrettes.

Claims (14)
1. Instrument endoscopique cintrable, comportant une tige (1) qui présente, dans sa partie cintrable (3), des découpes transversales (15) subdivisant cette dernière en segments (16) et s'étendant sur un secteur angulaire de plus de 270 , ledit instrument comportant, en outre, un moyen de traction (4) s'étendant en direction longitudinale, au- dessus des découpes (15) et transversalement à celles-ci, pour la commande du cintrage depuis l'extrémité proximale de l'instrument, caractérisé - en ce que la tige (1) présente, au moins dans sa partie cintrable (3), une section transversale aplatie d'un côté, et assume, dans cette partie aplatie (12), une fonction d'articulation, et - en ce que le moyen de traction (4) est disposé, sur le côté (13) opposé à la partie aplatie (12), dans des guides (21) qui épousent la forme du contour intérieur de la tige (1) dans cette région (13), qui enjambent chacun une découpe transversale respective (15) et qui sont reliés rigidement à un segment (16), d'un seul côté de chacune
des découpes (15) qu'ils enjambent.
2. Instrument selon la revendication 1, caractérisé en ce que des éléments d'accouplement (18, 19) s'engageant l'un dans l'autre sont disposés dans la tige (1), entre des segments voisins (16),
pour limiter l'angle de pivotement de ces derniers.
3. Instrument selon la revendication 2, caractérisé en ce que les éléments d'accouplement (18, 19) s'engageant l'un dans l'autre
sont définis par les découpes transversales (15).
4. Instrument selon l'une quelconque des revendications
précédentes, caractérisé en ce que la tige (1) présente une section
transversale sensiblement ovale et aplatie d'un côté.
5. Instrument selon l'une quelconque des revendications
précédentes, caractérisé en ce que des éléments d'accouplement (18, 19) s'engageant l'un dans l'autre sont disposés des deux côtés de la tige (1), entre la partie aplatie (12) de cette dernière et les guides du
moyen de traction.
6. Instrument selon la revendication 2 ou 5, caractérisé en ce que l'un des éléments d'accouplement s'engageant l'un dans l'autre

est constitué par une structure en forme de crochet (18) découpée dans la paroi latérale (14) d'un segment (16) et l'autre, par un évidement (19) présentant un contour de forme correspondante, avec

7. Instrument selon l'une quelconque des revendications
précédentes, caractérisé en ce que les découpes transversales (15) se terminent, du côté de la partie aplatie (12) de la tige, sous une forme en T, en projection, et sont de préférence réalisées avec un retrait au point de croisement du T.
8. Instrument selon l'une quelconque des revendications
précédentes, caractérisé en ce que les guides (21) sont formés en tôle
et sont reliés au segment associé (16) par un soudage par points.
9. Instrument selon l'une quelconque des revendications
précédentes, caractérisé en ce que le moyen de traction (4) est fixé, côté distal, à la tige (5) et, côté proximal, à un organe de commande (6), qui est guidé en translation dans une pièce de préhension (2)
reliée à la tige (1).
10. Instrument selon l'une quelconque des revendications
précédentes, caractérisé en ce que le moyen de traction (4) est conçu
pour transmettre également des forces de poussée.
11. Instrument selon l'une quelconque des revendications
précédentes, caractérisé en ce que la tige (1) est pourvue d'un tube tendeur (9) s'étendant de la pièce de préhension (2) jusqu'au voisinage de la partie cintrable (3), ce tube tendeur (9) étant déplaçable longitudinalement sur la tige (1) et pouvant être immobilisé dans sa
position par rapport à cette dernière.
12. Instrument selon l'une quelconque des revendications
précédentes, caractérisé en ce que les guides (21), outre le moyen de
traction (4), reçoivent et guident au moins un câble optique.
