Abstract

La tige présente, au niveau de sa partie proximale, une courbure antérieure (1c) dans le plan sagittal et, dans le plan frontal, une courbure interne (1a) correspondant à la famille des fémurs extra-courbes dits "flute de champagne".

Description

Tige fémorale pour Prothèse de hanche.
L'invention concerne le domaine technique des implants orthopédiques et plus particulièrement des prothèses de hanches.
De nombreuses solutions ont été proposées pour tenter d'obtenir une fixation métaphysaire la plus sûre et la plus efficace possible, étant donné que c'est à ce niveau que la fixation de la tige fémorale doit être particulièrement bien adaptée.
On a également proposé des solutions pour tenir compte du fait qu'il existe en réalité, deux grandes familles de fémur, à savoir les fémurs sensiblement rectiligines ou droits et les fémurs courbes, c'est-àdire à large courbure interne.
La demande de brevet EP 91420329.4 dont le demandeur de la présente est également titulaire définit un ensemble prothétique de la hanche, comprenant deux familles de tiges fémorales, correspondant chacune aux fémurs droits, sensiblement rectilignes et aux fémurs courbes.
Les tiges fémorales de chacune des familles, présentent au niveau de leur partie proximale, d'une part, dans le plan sagittal, une courbure antérieure et, d'autre part, dans le plan frontal, une courbure interne.
De telles caractéristiques s'avèrent particulièrement importantes et efficaces, pour les fémurs droits et courbes, correspondant généralement respectivement à des sujets agés et jeunes. Par contre, cette solution n'aborde pas le problème de tenir compte des fémurs extracourbe que l'on rencontre très généralement chez de très jeunes sujets.
Pour résoudre un tel problème, il a été conçu et mis au point, une tige fémorale qui présente, au niveau de sa partie proximale, une courbure antérieure dans le plan sagittal et, dans le plan frontal, une courbure interne correspondant à la famille des fémurs extra-courbes dits "flute de champagne", selon la classification de NOBLE.
Pour tenir compte de cette forme particulière, qui apparait essentiellement au niveau de la métaphyse, la tige fémorale selon l'invention présente, du côté externe, une partie renflée apte à exercer une force de compression au niveau de la corticale externe du fémur, permettant ainsi d'équilibrer l'évasement généré par la forte courbure interne.
Toujours en ayant pour objectif de tenir compte de la morphologie du fémur, la tige présente un col qui est décalé angulairement dans le plan sagittal par rapport à l'axe de symétrie, pour correspondre à l'angle d'antéversion de la tête du fémur.
Le col est latéralisé en fonction du rayon de courbure interne, pour correspondre à l'angle cervico-diaphysaire souhaité.
A noter également qu'en fonction de l'augmentation de la taille, la longueur du col est augmentée selon la direction de l'angle cervico-diaphysaire.
L'invention est exposée, ci-après plus en détail à l'aide des dessins annexés, dans lesquels:
La figure 1 est une vue de face d'une tige fémorale selon l'invention; I on a superposé une tige fémorale de même taille, correspondant à un fémur courbe (tracé traits mixtes) et à un fémur droit (tracé traits interrompus), correspondant à l'enseignement de la demande de brevet EP 91420329.4.
La figure 2 est une vue de profil de la tige fémorale correspondant à la figure 1.
La tige fémorale désignée dans son ensemble par (1) comprend au niveau de sa partie proximale et dans le plan frontal, une courbure interne (la) correspondant à la famille des fémurs extra-courbe, dont la partie métaphysaire a une forme en flûte de champagne. A l'opposé de la courbure (la), c'est-à-dire du côté externe, la tige présente directement lors de sa fabrication, une partie renflée (1 b).Cette partie (i b) a pour but d'exercer une force de compression au niveau de la corticale externe du fémur après introduction de la tige dans le canal médullaire, permettant d'équilibrer l'évasement généré par la forte courbure interne (la)~
On voit donc que la combinaison de la courbure interne (la) et de la partie (lob) assure un remplissage métaphysaire parfait dans le cas d'un fémur extra courbe.
Il apparait que la courbure interne (la) est beaucoup plus accentuée que dans le cas d'une tige fémorale adaptée pour les fémurs courbes (tracé traits mixtes) (figure 1) et encore plus par rapport au fémur droit (tracé traits interrompus) (figure 1). De même, la partie renflée (lob) est en relation dimentionnelle avec l'évasement interne généré par le rayon de courbure (la).
La tige fémorale, telle que définie pour les fémurs extracourbes, présente dans le plan sagittal, une courbure antérieure (il). Ce rayon de courbure antérieur (1c) est le même, aussi bien pour une tige fémorale de la famille des fémurs droits que pour une tige fémorale de la famille des fémurs courbes. On renvoie à la demande de brevet EP 91420329.4.
La tige présente un col (nid) latéralisé dans le plan frontal et qui est décalé en combinaison avec la courbure interne (1 a). Ce décalage correspond à l'angle cervico diaphysaire prothétique.
En outre, le col présente, dans le plan sagittal, un décalage par rapport à l'axe de symétrie qui correspond à l'antéversion de la tête fémorale.
Bien évidemment, la tige (1), telle que définie selon les caractéristiques de l'invention, est exécutée dans différentes tailles.
En fonction de l'augmentation de la taille, la longueur du col (Id) est augmentée selon la direction (xx') de l'angle cervico-diaphysaire.
Les avantages ressortent bien de la description.

Claims (6)

    REVENDICATIONS
  1. -1- Tige fémorale pour prothèse de hanche, caractérisée en ce qu'elle présente, au niveau de sa partie proximale, une courbure antérieure (1 c) dans le plan sagittal et, dans le plan frontal, une courbure interne (la) correspondant à la famille des fémurs extra-courbes dits "flute de champagne".
  2. -2- Tige selon la revendication 1, caractérisée en ce qu'elle présente, du côté externe, une partie renflée (lob) apte à exercer une force de compression au niveau de la corticale externe du fémur.
  3. -3- Tige selon la revendication 2, caractérisée en ce que la partie renflée (i b) est en relation dimentionnelle avec l'évasement interne généré par le rayon de courbure (la).
  4. -4- Tige selon la revendication 1, caractérisée en ce qu'elle présente un col (nid) qui est latéralisé en fonction du rayon de courbure interne (la), pour correspondre à l'angle cervico-diaphysaire souhaité.
  5. -5- Tige selon la revendication 4, caractérisée en ce qule col latéralisé (nid) est décalé angulairement dans le plan sagittal par rapport à l'axe de symétrie, pour correspondre à l'angle d'antéversion de la tête du fémur.
  6. -6- Tige selon la revendication I, caractérisée en ce qu'en fonction de l'augmentation de la taille, la longueur du col (nid) est augmentée selon la direction de l'angle cervico-diaphysaire.
