Abstract

The length of the pin (10) is determined as a function of the distance (P) between the head (20) and the shaft (2) of the prosthesis. The external diameter of the end surface (EP) of the pin is very slightly larger than the internal diameter of the inner end of a conical hole in the head, into which the pin is inserted. The internal diameter of a hollow portion of the base (EB) of the pin is very slightly less than the external diameter of the base of a tenon of the prosthesis shaft, which fits into the hollow portion. The external surface (12) of the pin is ribbed, and the pin is formed of metal or plastics material. ADVANTAGE - Allows adjustment of dimensions of pin to suit different applications.

Description

DOUILLE DE FIXATION POUR TETE PROTHETIQUE FEMORALE
DESCRIPTION
DOMAINE DE LtINVENTION
FIXING SOCKET FOR FEMALE PROSTHETIC HEAD
DESCRIPTION
FIELD OF THE INVENTION
The invention relates to Surgery of your hip, and more particularly to The placing of a femoral prosthetic head on the main body of the femur, at a determined location to take place in the acetabulum of the coxat bone.

PRIOR ART AND PROBLEM POSED
Hip prostheses are generally made up of three parts: your stem, your head and your cotyLe. The rod is placed inside your femoral cavity, the cotyLe is placed on the iliac bone and the head, in the shape of a ball joint, is placed on your upper part of your rod, ensuring mechanical articulation with the acetabulum.

THEY mainly replace the rotuLe connected to the femur by a narrowing, commonly called "neck of the femur". In the context of the construction and installation of these femoral prostheses, a structure has imposed itself to a large extent.

It includes a femoral stem which must be placed in the femur, and on the Free end of which is fixed The prosthetic head in the shape of a ball joint.

This must cooperate with the natural cup, if it is intact, or with an artificial bowl if it is not. For manufacturing and surgical reasons, we prefer to manufacture
The rod and your head in the form of two separate parts s.

Generally, the femoral stem protrudes from the upper part of the femur by a fixing cone, on which the prosthetic head must be permanently fixed. This method of fixing being imposed, it is easily understood that the machining of the cone of your rod must be very precise, just like the conical hole of the head. In addition, the prosthetic head, unlike the cone of the rod, which is usually metallic, is either metallic or non-metallic (ceramic, crystal, mixed).

The term ceramic must be taken as a whole and not be limited to alumina oxides or zirconium, for example: ruby head, metal head covered with oxide. On the other hand, for the surgeon, it is necessary to have, for the operation, according to the operating technique, the age, the sex, the morphology of the patient, heads of different sizes. In addition, this head must be able to be placed at a variable distance from the top of the rod according to the morphology of the patient and the decisions of the surgeon.

For these reasons, there are also several
Lengths in the possibilities of fixing this head on the rod. The surgeon must therefore choose between several possibilities of fixing lengths for placing the head, in addition to the fact that he must already have several different diameters of heads.

The assembly of the head on your rod is usually done by means of a female cone dug in your head which is placed on a male cone machined at the top of the rod. The variation of the placement distance of the head with respect to the vertex is obtained by machining inside the head a cone with a single opening angle, but with a variable maximum radius. The maximum radius is called the nominal radius. A slight increase in this nominal radius will allow the head to be placed closer to the femur, while a slight decrease in this nominal radius will move the head away from the femur.

Thus, your variation of the head-rod assembly distance is obtained by using heads of slightly different nominal radius. In order to adapt prosthetic heads on rods having a cone with an opening angle or nominal radius different from that of the prosthetic head, there are adapter pieces in the form of conical sockets which are fixed on the cone prosthetic, thus modifying the nominal radius and / or the opening angle of the prosthetic cone. These sockets, having a constant length and thickness, thus make it possible to use prosthetic heads initially not intended for this series for a series of rods.

- pour une seule operation, le chirurgien doit disposer d'un nombre important de têtes differentes qui est Le produit du nombre de diamètres differents de têtes par le nombre de tailles differentes des trous coniques de ces dernières ;
Several disadvantages emerge from this way of doing things, and are the following
- for a single operation, the surgeon must have a large number of different heads which is the product of the number of different diameters of heads by the number of different sizes of the conical holes of the latter;
- the fixing surface of the head on the socket is variable. It is generally observed that the mechanical resistance of the femoral head-coL couple of the femoral stem decreases when the contact surface between the head and the cone decreases.

The invention therefore aims to solve these problems by providing an additional part in the prosthetic head / prosthetic stem assembly.

This piece allows you to vary the distance between the prosthetic head / prosthetic stem without changing the prosthetic head, thus limiting the number of heads that must be made available to the surgeon.

IT also makes it possible to ensure a constant bearing surface on the female cone of the ceramic head regardless of the assembly distance chosen during the intervention. It finally authorizes the machining of the cone of the prosthetic rods intended for the use of non-metallic heads, according to the tolerances necessary for the assembly of a metal head.

SUMMARY OF THE INVENTION
To this end, the main object of the invention is a fixing socket forming part of a femoral prosthesis comprising a prosthetic head to be placed at a determined distance on a femoral stem, the head having a conical fixing hole of dimension determined, the rod having a fixing cone of determined size and centered on an axis. The fixing sleeve has a conical external shape corresponding to the conicity of the conical hole of the head and a conical internal shape corresponding to the conicity of the cone for fixing the rod.

According to the invention, the length of the sleeve is a function of the determined distance separating the head from the rod; the external diameter of the tip of the external surface of the socket is very slightly greater than the bottom diameter of the conical hole of the prosthetic head; the diameter of the base of the cone is very slightly less than the external diameter of the base of the cone of the rod, so that the longitudinal distance along the axis separating the internal cone and the external cone from the socket is a function of the determined distance separating the prosthetic head from the rod, thus making it possible to adjust the distance separating the head from the rod to a determined value.

DESCRIPTION DETAILLEE DE L'INVENTION
DETAILED DESCRIPTION OF THE INVENTION
FIG. 1 shows the implantation, in general as for the invention, of a femoral prosthesis in the upper part of a femur 1.

This femoral prosthesis is shown without the acetabulum of the coxal bone, with which it cooperates. The latter mainly comprises a femoral stem 2, a socket 10, which is preferably blind, and a prosthetic head in the shape of a rotuLe 20.

This barrel 4 can be completed with cement 6 to facilitate its anchoring in the femur 1. The protruding part of the rod 2 is optionally supplemented with a collar 5 applied to the surface of the femur 1 possibly prepared by milling for this purpose.

As summarized above, the sleeve 10 according to the invention makes it possible to fix the head 20 on the rod 2. To simplify its description, it can be said that the sleeve 10 has the shape of a sewing machine.

It must have a conical hole 14 whose conicity corresponds to that of the fixing cone 8 and a conical external surface 12 whose conicity corresponds to that of the conical hole of the head 20. All these conical shapes cooperating in the fixing of the head 20 on the rod 2 are coaxial with respect to a fixing axis 9.

By comparing the upper part of this FIG. 1 with FIG. 2 which takes up the same elements, it is easily understood that the variation in shapes of the socket 10 influences the position of the prosthetic head 20 relative to the rod 2, along the longitudinal axis 9. In fact, if the conical external surface 12 of the bush 10 is longitudinally distant from the conical internal surface 14 of this same bush 10 (FIG. 1), the head 20 is distant from the rod 2. On the contrary, (Figure 2) if this outer surface 12 is brought closer to the lower surface 14, the head 20 is closer to the rod 2. The different shapes of the sleeve 10 are detailed below, by means of the other two Figures 3 and 4. It is noted above that it is preferable that the sleeve 10 which, as in the prior art, has a conical annular structure, has a bottom 15 giving the sleeve 10, here a form of sewing. In fact, this bottom 15 cooperates with the mechanical resistance of this socket 10 by improving the stability of the narrowest part of the socket 10.

It is also noted that it is preferable that the socket 10 is not pressed as far as the bottom surface 22 of the head 20, that is to say that there exists a space 24 at the bottom of the conical hole of the head 20. Otherwise, there would be a risk of not having hermetic contact between the outer surface 12 of the sleeve 10 and the head 20. The fixing of the latter would be disadvantaged.

At the present time, we want to use heads 20 whose conical holes have always
The same dimensions, in order to standardize the manufacture of these heads and reduce the cost price. Therefore, as is the case for the socket according to the invention, there is provided a conical external surface 12 which always has the same dimension, that is to say the same small diameter and the same large diameter. In this way, as shown in Figures 1 and 2, the scope of this outer surface 12 inside the head is always the same. As the surfaces are always the same, it is of course advantageous to provide this maximum range.

Les formes précises qui permettent de réaliser cette fixation sont detaillees par les figures 3 et 4.Consequently, provision is therefore made for the socket 10 to cover almost the entire internal surface of the head hole 20. The precise shapes which make it possible to achieve this fixing are detailed in FIGS. 3 and 4.

Referring to Figure 3, we find the socket of Figure 1 on an enlarged scale.

This sleeve has the shape of a sewing with a relatively large bottom 15, unlike that of the sleeve of Figure 4 which corresponds to that shown in Figure 2. However, between these two Figures 3 and 4, many dimensions of the sockets shown are the same. Taper
CE of the conical external surface 12 must correspond exactly to the conicity of the conical hole of the prosthetic head to be fixed. The manufacturing tolerance of these two conicities is around 2.5 seconds in the case of a metal / ceramic assembly. This tolerance can be plus or minus 4 seconds for the metal / metal assembly.

Par contre, la douille de la figure 3 comporte une partie cylindrique 13 de la surface externe sur une longueur supplémentaire P. Concerning this external surface 12, the length L of the conicity must also be the same. In Figure 4, this length L corresponds to the total length of the sleeve which does not have a cylindrical outer surface. On the other hand, the bush of FIG. 3 has a cylindrical part 13 of the external surface over an additional length P.

The external diameters EP of the tip of the socket and EB of the base of the socket are also fixed according to the dimensions of the conical hole of the head.

Similarly, the base IB and tip IP internal diameters, the internal conicity
CI of taper hole 14 of socket and length
T of this hole are also dimensions fixed as a function of the dimensions of the conical pin 8 of the rod 2 of FIGS. 1 and 2.

Only the total length of the socket varies. It is equal to P + L in the case of Figure 3. It is equal to L in the case of Figure 4.

Dans le cas de la figure 2, la douille 10 est beaucoup plus courte, comme le représente la figure 4, et la tête 20 est placée près de la tige 2.The additional distance P to be added to the socket of FIG. 3 corresponds to the additional distance of the value by which the prosthetic head 20 must be moved away from the rod 2, in the case of FIG. 1. In the case of the FIG. 2, the bush 10 is much shorter, as shown in FIG. 4, and the head 20 is placed near the rod 2.

Consequently, the bottom 15 of the sleeve has a variable thickness according to the cases of positioning of the head relative to the rod.

It is the same for the thickness of the sleeve at the conical hole 14. -Indeed, if the external cones 12 and internal 14 are relatively distant longitudinally, the thickness of the sleeve will tend to be thick (Figure 3 ). On the contrary, (Figure 4) if these external 12 and internal 14 cones are brought together, the thickness will be less. We still take care to be able to guarantee a sufficient level of mechanical resistance.

Figure 4 shows in dashed lines an optional boss 16, which can if necessary strengthen the mechanical strength of the wall of the socket.

The external diameter of the EP tip must be slightly greater than the diameter of the bottom of the conical hole in the head. Similarly, the external diameter of the base EB of the socket must be slightly greater than the exit diameter of the conical hole of the head.

The same dimensional arrangements can be provided between the sleeve and the conical post of the rod. More specifically, the internal diameter of IT must be slightly less than the diameter of the end of the conical post of the rod. The internal diameter IB of the base of the socket must be slightly less than the diameter of the base of the conical post of the rod.

In conclusion, the dimensions of the liai sounds of the socket 10, either with the head 20 or with the rod 2, are always the same. Only the length of the sleeve 10 varies as a function of the positioning to be obtained of the prosthetic head relative to the rod.

The socket according to the invention can of course be made of metal. We also plan to make it plastic.

In order to improve the contact of the connection of the socket 10 with the head 20, the external surface 12 can be striated.

It is thus understood that it is possible to use a single type of conical hole for the head, a single type of conical post for the rod. This simplifies the manufacture of prosthetic heads, rods, and therefore reduces the cost of the material necessary for an operation. It is also recalled that the connections being constant, these are optimized to ensure maximum resistance.
